Trends in the Annual Incidence of Carbapenem Resistant among Gram Negative Bacilli in a Large Teaching Hospital in Makah City, Saudi Arabia

Objective: To detect the prevalence of carbapenem resistance among Gram negative bacilli at the Al-Noor Specialist Hospital in the western part of Saudi Arabia. Materials and Methods: This study was conducted in the Microbiology department, Al-Noor Specialist Hospital, Makah city, Saudi Arabia. The clinical samples were collected from admitted patients in the wards, Intensive Care Units (ICUs), urology unit and surgery unit, these samples included blood, urine, sputum, pus and wound. The identification and antibiotic susceptibility testing of all bacterial isolates were performed via the Vitek 2 Compact System. Results: Out of 4803 Gram negative bacterial bacilli were isolated, the prevalence rate of carbapenem resistance among these isolates was 2797/4803 (58.23%), and sensitivity rate was 2006/4803 (41.77%) of the total isolates. The rate of resistance to carbapenem was among Acinetobacter baumannii 1710 (99.13%), followed by Pseudomonas aeruginosa 575 (62.4%), Klebsiella pneumoniae 459 (38%) and Escherichia coli 56 (5.59%). The trend of carbapenem resistance among Gram negative bacterial isolates from 2013-2015 was 53.99%, 59.88% and 61.43% respectively. Conclusion: A. baumannii rec-orded, the highest rate of carbapenem resistance through three year, 98.19%, 99.48% and 99.81% respectively and the lowest rate of resistance recorded with E. coli, 4.63%, 5.79% and 6.31% respectively.

Traffic congestion has become a serious problem that arises in our cities; this is due to rapid population growth, the rapid increase in the number of cars. Therefore, there is an infernal traffic jams. It is in this light, it is imperative to eliminate or at least reduce traffic congestion through the adoption of the policy of ITS (intelligent transport systems). The objective of this paper is to propose a study on the problem of congestion in cities, by reducing traffic congestion.

Establishing a quality indicator format for endoscopic ultrasound

AIM: To perform a quality control(QC) review of endoscopic ultrasound(EUS) with emphasis on current consensus established quality indicators. METHODS: A national quality control study of EUS was performed with expanded international comparison. Ten different healthcare institutions in Israel participated in coordination with University of Chicago Medical Center. Each Israeli center provided ten patient reports, compared with twenty reports from University of Chicago Medical Center. Quality indicator forms were prepared with sections to be completed before, during, and after EUS. Physician compliance to all listed indicators wasevaluated. Quality indicators were evaluated prior to, during, and after performing EUS. RESULTS: One hundred different EUS procedural reports were analyzed. The mean patient age was 59 years old. Indications for referral were mostly for pancreatic or biliary reasons. QC showed several strongly reported areas, including indications for EUS(97%), anesthesia given(94%), periprocedural pancreatic evaluation(87%), and an overall summary of the EUS examination(82%). Intermediately reported areas included patients' pertinent past medical history(71.7%), evaluation of the biliary tree(63%), and providing medical guidance about potential procedural adverse events, including pancreatitis and bleeding(52%). Half of the reports(50%) did not include a systemic organ evaluation. Other areas, including systematic reporting of screened organs(36%), description of fine needle aspiration(15%), tumor description via tumor-nodemetastasis(5%), and listing of adverse events(0%) were largely lacking from procedural documentation. CONCLUSION: Documenting specific EUS quality indicators including listing post-procedural recommendations may improve the quality and efficiency of future EUS examinations and subsequent patient follow-up.

Clinical asthma phenotyping:A trial for bridging gaps in asthma management

Asthma is a common disease affecting millions of people worldwide and exerting an enormous strain on health resources in many countries. Evidence is increasing that asthma is unlikely to be a single disease but rather a series of complex, overlapping individual diseases or phenotypes, each defined by its unique interaction between genetic and environmental factors. Asthma phenotypes were initially focused on combinations ofclinical characteristics, but they are now evolving to link pathophysiological mechanism to subtypes of asthma. Better characterization of those phenotypes is expected to be most useful for allocating asthma therapies. This article reviews different published researches in terms of unbiased approaches to phenotype asthma and emphasizes how the phenotyping exercise is an important step towards proper asthma treatment. It is structured into three sections; the heterogeneity of asthma, the impact of asthma heterogeneity on asthma management and different trials for phenotyping asthma.
