Timing of Elective Repeat Cesarean Delivery at 38 Weeks versus 39 Weeks: Rate of Spontaneous Onset of Labor before Planned Cesarean Section and Impact on Maternal Outcome: A Retrospective Cohort Study

Background: The timing of elective repeat cesarean delivery at 38 weeks versus 39 weeks is still a debatable subject, both regarding maternal and neonatal outcomes. In the Saudi context, there is lack of local data to aid decision-making regarding the timing of elective repeat cesarean delivery. Objectives: To estimate the rate of spontaneous onset of labor before the planned gestational age for repeat cesarean section in women who were booked at gestational age of (39 0/7 - 39 6/7) weeks (W39) versus (38 0/7 - 38 6/7) weeks (W38) and to compare the rate of maternal composite outcome between these groups. Design: Retrospective cohort. Setting: This study was conducted at King Abdulaziz Medical City, Jeddah, KSA. Method: Delivery registry books were reviewed to identify all deliveries from 1 January 2014 to 31 December 2016 (3 years). All low-risk pregnant women who had 2 or more cesarean deliveries and who met the inclusion criteria were included. Results: A total of 440 women were included of whom 318 (72.3%) were planned for elective cesarean section at W38 gestational age and 122 women at W39 gestational age. Mothers planned at W39 had higher rate of emergency cesarean deliveries versus those planned at W38 (18.0% versus 10.4%, p = 0.030;RR = 13.06), most frequently due to early onset of contractions (16.4% versus 8.2%, p = 0.012;RR = 12.17) or cervical dilatation (11.6% versus 5.4%, p = 0.024, RR = 16.15). No difference in the incidence of individual or composite maternal complications was noted between the two groups. Mother’s age (OR 0.93, p = 0.018) and schedule date at W39 (OR = 1.94, p = 0.028) were independently associated with spontaneous onset of labor before the scheduled gestational age, while no association was found with parity, previous number of spontaneous vaginal deliveries, number of previous cesarean deliveries or interval from last cesarean delivery. Endotracheal intubation and extubation in intensive care unit (ICU) patients are difficult. Extubation will be more worrisome if patient has difficult intubation, sleep apnea, surgical procedure involving airway or surrounding structures. In such extubation airway exchange catheters (AEC) are used, as they are simple to use and able to provide oxygen to the patient. Rarely AEC use can cause potential life threatening complications. We report a case of pneumothorax following use of AEC in post-thyroidectomy patient. Case: A 32 years old male patient was admitted to our ICU, with difficult intubation after thyroidectomy. He was a known case of obtructive sleep apnea, hypertension and large goiter. In ICU for proper visualization of vocal cords and resecuring the airway, AEC was used, but patient had hypoxia with bradycardia. He was recovered with Ambu bag ventilation and required brief cardiopulmonary resuscitation. Post resuscitation he had left impending tension pneumothorax and lung laceration requiring chest drain, which was removed after tracheostomy and weaned from the ventilator. After 9 days supra glottic edema subsided and vocal cords were moving;His trachea was decanulated and he was discharged home. Conclusion: Rarely AEC use can cause life threatening injuries. Pneumothorax following the use of AEC is not always due to tracheobronchial injury;it can also occur as a result of alveolar injury.

Validation of a Food Frequency Questionnaire for Vitamin D and Calcium Intake in Healthy Female College Students

Objective: The objective of this study was to examine the reproducibility and validity of a Food Frequency Questionnaire (FFQ) and assess calcium and vitamin D intake in health female college students. Methods: Thirty-five healthy female students were conveniently selected to participate in the study. None of the subjects were taking any supplements. The FFQ was validated against intakes from a three-day diet food record report (FR). Results: Positive correlations were observed of daily vitamin D (r = 0.82, p –8, 9, p < 0.676 and 43 mg/d (95% CI: 20, 65, p < 0.01). Conclusions: The FFQ used in this study shows promising validation evidence to be used in the future for assessing vitamin D and calcium intakes in female students.

Tri-trophic interactions between fertilizer applications, cotton aphid (Aphis gossypii Golver) and associated beneficial insects were studied to investigate direct and indirect effects of fertilizers (types and ratios) on potato plants under field and greenhouse conditions, A. gossypii and associated beneficial insects. Fertilizers regimes showed direct impacts on the potato plant phenology and indirect effects on both A. gossypii population and the associated beneficial insects. Our data indicated that potato plants had been influenced by fertilizer elements used within tri-trophic system comprising potato plants, cotton aphid, and certain associated beneficial insects. This demonstrates that a bottom-up interaction is robust and has a particular value in the attraction of beneficial insects towards the potato plant signals due to used fertilizers which can also have a function when plants are attacked by A. gossypii. Yet, flexibility in the use of fertilizers (as chemical cues) is conserved, and that may help beneficial insects to specifically focus on the odor of plants that carry potential plant hosts and avoid plants that are only attacked by non-hosts. These results support the still controversial notion that fertilizer elements, at least in part, help plants to serve as functional signals to attract the enemies of the harmful insects. These observations declare the benefits of the tri-trophic interactions as an ecological phenomenon in particular and the food chain in general. Additionally, this study may be useful to be used as a predictable model with the associated beneficial insects which may have key roles in overall aphid suppression or regulating its population. Background: Myelodysplastic syndromes (MDS) are subtypes of hematological disorders which are known to have partial bone marrow dysplasia, peripheral cytopenia, and later on an increased risk to develop acute myeloid leukemia. Chromosomal aberrations are detected in ~50% of cases of de novo MDS cases and the most common chromosomal abnormalities of this entity include complete or partial monosomy of chromosomes 5 and 7, partial deletion of 20q and 12p, trisomy 8, and 11q23 aberrations. A few primary and/or secondary MDS cases combined with other cancer have been reported. Case Presentation: We report here an adult MDS associated with squamous cell carcinoma (SCC). G-banding and array-proven multicolor banding (aMCB) revealed an unbalanced translocation der(7)t(1;7)(q21;q21), which led to 1q partial trisomy and 7q partial monosomy. Immunophenotype of this case was consistent with refractory anemia with excess of blasts (RAEB-2) according to World Health Organization (WHO) classification. Conclusions: As far as we know, this is the first adult MDS case associated with SCC and an unbalanced translocation t(1;7). Our patient received first cycle of azacitidine treatment and he showed bilateral pleural effusion as a secondary event. This toxicity is not limited to the first cycle as in previous MDS cases;our case is the first one to shown this toxicity as a secondary event of azacitidine treatment. As less than 10 cytogenetcially comparable cases without SCC were reported before in male MDS, we carefully conclude that this cytogenetic aberration may be a hint on a new gender associated MDS subgroup.

Background: Post-menopausal bleeding is a warning sign that accounts for about 5% of all outpatient gynaecologic visits and is a common indication for referral to rapid access clinics because of the fear of underlying malignancy. Endometrial malignancies differ from other malignancies in that early symptomization is common, allowing early cure. Patients and Methods: During the study period, 100 women with post-menopausal bleeding having inclusion criteria were evaluated in Al Hussein University Hospital. For each patient full history, general, abdominal and pelvic examination was performed. Routine pre-operative investigations were done. Patients were divided into four groups: Group 1 included 29 patients with endometrial polyp. Group 2 included 34 patients with endometrial hyperplasia. Group 3 included 21 patients with atrophic endometrium. Group 4 included 16 patients with endometrial carcinoma. Results: As regards the predictive value of BMI, in the study there was a high statistical significance in comparison between the endometrial carcinoma group and all other benign groups. When discussing the predictive value of blood glucose level, in the study there was a high statistical significance in comparison between the endometrial carcinoma group and all other benign groups. It is worth to mention that the predictive value of endometrial thickness, in the study, was with high statistical significance in comparison between the endometrial carcinoma group and all other benign groups providing the highest specificity and sensitivity. At the last the predictive value of uterine artery velocimetry, in the study, was with high statistical significance in comparison between the endometrial carcinoma group and all other benign groups. Conclusion: BMI, blood glucose level, endometrial thickness and uterine artery velocimetry indices, improve the prediction of endometrial carcinoma in women with post-menopausal bleeding.

Transthoracic echocardiographic and clinical predictors in first versus recurrent ischemic strokes

Recurrent strokes are more likely to be disabling or fatal than first ever strokes. The aim of the study is to evaluate transthoracic echocardiographic (TTE) and clinical predictors in patients with first versus those with recurrent ischemic strokes. A prospective observational comparative study of 217 patients admitted with ischemic strokes who were in sinus rhythm. Two groups of patients were simultaneously enrolled. The first group was 110 patients with first ischemic stroke and the second was 107 patients with recurrent ischemic stroke. TTE was done for all patients. Both echocardiographic and clinical risk factors were compared between both groups. Logistic regression analysis identified predictors for recurrent strokes. Among clinical risk factors hypertension, hyperlipidemia, family history of atherosclerotic vascular disease, prior coronary artery disease, peripheral vascular disease, and chronic kidney disease were significantly higher in recurrent stroke group. Multivariate logistic analysis identified age (OR, 1.03;95% CI, 1.01 - 1.07), hypertension (OR, 2.25;95% CI, 1.03 - 4.92), and hyperlipidemia (OR, 2.73;95% CI, 1.40 - 5.35), as predictors for recurrent ischemic strokes. Left ventricular diastolic dysfunction, left ventricular hypertrophy (LVH) and aortic sclerosis were significantly more common in the recurrent stroke group compared to the first stroke group. However, in multivariate logistic analysis only LVH (OR, 3.50;95% CI, 1.69 - 7.23), was identified as a predictor for recurrent strokes. Older age, hypertension, hyperlipidemia and left ventricular hypertrophy are significant predictors of recurrent ischemic strokes in patients with sinus rhythm. Those patients need more aggressive lipid lowering therapy and optimal blood pressure control.

Objective:This study examined the relationship between structural empowerment and nurses’experience and attitudes toward computer ***:This study was conducted using a cross-sectional quantitative design.A total of 184 registered nurses from four hospitals in Jordan participated in the current *** were collected using a demographics questionnaire,the Conditions for Work Effectiveness Questionnaire-II(CWEQ-II),and the Pretest for Attitudes toward Computers in Healthcare(PATCH).Results:The median of experience in years among nurses was 5.0,ranging from one to 26 *** mean score for the attitudes toward computer use was 61.90±*** half of the participants,45.11%,were in the category of“feel comfortable using user-friendly computers.”The participants’mean average of the total structural empowerment was 12.40±2.43,and the values for its four subscales were:opportunity 3.57±0.87,resources 2.83±0.85,information 3.06±0.79,and support 2.95±*** frequencies analysis revealed that most participants had a moderate level of empowerment(n¼127,69.02%).The bivariate correlation between nurses’experience and attitudes toward computer use was significant(r¼0.17,P<0.05).The relationship between the total structural empowerment score and attitudes toward computer use was positive but weak(r¼0.20,P<0.01).Conclusion:The results indicated that more experienced nurses are more reluctant toward computer ***,creating an empowering work environment can facilitate nurses’attitudes toward computer use.
