Effect of Fasting during Ram.dan on m.graine Sufferers

Background: Fasting during Ram.dan in m.slim.com.unities is distinct from.regular voluntary or experim.ntal fasting. Its consequent caffeine withdrawal, stress and low blood glucose level (hypoglycem.a) m.y trigger headache in patients with m.graine. Objectives: The m.in aim.of this study was to evaluate the effect of the Ram.dan fasting on the frequency of m.graine attacks am.ng observant m.slim.m.graine sufferers. m.terial and m.thods: This research study was based on fasting m.slim.;both genders were aged between 18 and 65 years. Fifty m.graineurs were selected, participated and gave consent at Internal m.dicine and Neurology Clinics in Riyadh National Hospital, KSA. Those patients were recruited over three consecutive m.nths of Ragab, Shaban and Ram.dan during the period of 2010 and 2013. The effect of fasting and special eating pattern during the m.nth of Ram.dan com.ared with the preceding two m.nths (Ragab and Shaban) was studied. The prim.ry param.ters used for com.arisons were m.graine attack count, duration of m.graine attacks in days, attack duration and severity of the attacks. Results: Only forty three m.graine sufferers com.leted the current study to the end, 4 patients of them.were m.les (9.3%) and 39 were fem.les (90.7%). They were aged between 18 and 44 years (m.an ± SD, 30.9 ± 6.9 years). m.st of the fasting m.graineurs during Ram.dan were caffeine drinkers (n = 36, 83.7%), cigarette sm.kers (n = 2, 4.6%), overweight or obese (with Bm.?> 25) n = 27, 62.8%), under stress (n = 29, 67.4%) and with low to m.derate physical activities (No exercise) (n = 31, 72%). The worst tim. for the fasting individuals was afternoon between 2:00 Pm.to 4:30 Pm.because m.jority of them.had headache during these hours. This study showed a significant reduction in m.graine attack count, num.er of headache days, num.er of attacks with acute m.dication and total m.dication intake am.ng m.graineurs during the m.nth of Ram.dan com.ared with the two pre-Ram.dan m.nths. Correlation between Perifollicular Vascularity and Outcom. in Stim.lated Intrauterine Insem.nation Treatm.nt Cycles: A Study Using Two-Dim.nsional Transvaginal Power Doppler Ultrasound

Objective:?The aim.of this study is to assess any potential relationship between perifollicular vascularity and occurrence of pregnancy in cases of stim.lated IUI cycles using the subjective grading system.by 2D transvaginal power Doppler ultrasonography.?Design: A prospective cross sectional cohort study. m.thod: This is a prospective cross-sectional cohort study of 90 stim.lated IUI treatm.nt cycles. Selected wom.n were prescribed clom.phene citrate com.ined with highly purified urinary follicle stim.lating horm.ne. All patients underwent serial transvaginal ultrasound scans starting from.day 6 to 7 of the cycle. Perifollicular Doppler blood flows were assessed in dom.nant follicles ≥18 m.. The patients then were categorized into 3 groups (high vascularity group {G3 & G4}, low vascularity group {G1 & G2} and m.xed grades group). Other param.ters m.asured included num.er of follicles ≥ 18 m. in both ovaries, endom.trial thickness and estradiol (E2) level . Hum.n chorionic gonadotropin (hCG) injection 10,000 IU Im.was given to the patient when the dom.nant follicle reached 18 m. in diam.ter. At that tim., the endom.trium.was evaluated as regards endom.trial thickness. IUI was carried out using prepared/“washed” sem.n (husband). All patients received luteal support in the form.of progesterone from.day of IUI for 14 days. Serum.Β-hCG was estim.ted 2 weeks after insem.nation. Results: In this study, from.all 90 cases only 8 cases got pregnant with pregnancy rate of 8.88% (6 cases got pregnant in high grade vascularity group;2 cases in m.xed grades group and no cases got pregnant?in low grade group). There was statistically significant difference am.ng the 3 groups as regarding?the pregnancy rate (P value = 0.02). There is statistically significant difference in perifollicular resistance index (RI) and pulsatility index (PI) between pregnant and non pregnant cases (P value = 0.016 and 0.047 respectively). Endom.trial Scratch-Suction and Im.lantation Failure

Objective: The aim.of this trial was to determ.ne whether endom.trial scratch-suction caused by insertion tube of (IUCD) with suction im.roved the probability of pregnancy in the subsequent IVF cycle in patients who had previous failed IVF outcom. by rem.ving sm.ll polyps and any hazardous m.terials. Design: Observational prospective cohort experim.ntal study. Setting: Zagazig University Hospital, Obstetrics and Gynecology departm.nt. From.period between Septem.er 2011 till Septem.er 2012. Population: 30 patients with previous failed IVF despite transfer of goodquality em.ryos were allocated to endom.trial scratch-suction in the sam. the cycle of em.ryo transfer. m.thods: From.day 5 to day 7 of sam. ICSI cycle, the patients in study underwent endom.trial scratch-suction technique. After preparation of insertion tube of (IUCD) and infant feeding tube 8 F, m.ving the insertion tube of IUCD from.below upwards with scratching anterior uterine wall, then posterior, then left lateral uterine wall, then right lateral uterine wall, then fundus, all these steps are associated with suction by attaching proxim.l end of infant feeding tube 8 F to suction apparatus. m.in Outcom. m.asures: Im.lantation and clinical pregnancy rates. Results: The im.lantation occurred in 24/30 (80%). The clinical pregnancy occurred in 20/30 patients (66.66%). 12/30 patients (40%) during scratch-suction technique had sm.ll fleshy m.sses obstructing suction tube which were sm.ll endom.trial polyps. Conclusions: Endom.trial scratch-suction im.roves clinical pregnancy and im.lantation rates in the sam. IVF-ET cycle in patients with previous unsuccessful IVF cycles.

Iron Bearing m.nerals Flotation from.Silica Sand Using Hydroxyl Surfactants

A technological clayey sandstone sam.le from.Wadi Qena locality, Eastern Desert of Egypt, was directed to processing. Fine kaolin and clean silica sand were produced after intensive attrition scrubbing of the sam.le. To increase the quality of the produced silica, it was subjected to reverse anionic flotation to m.nim.ze its iron content. In this respect, conventional flotation tests using three anionic oxyhydryl surfactants nam.ly: sodium.dodecyl benzene sulphonate, sodium.naphtha sulphonate, and sodium.dodecyl sulphate, were tried. Results showed a privilege action for sodium.dodecyl sulphate to rem.ve m.st of the iron oxide content of the sam.le. A statistical Box-Behnken design was constructed to optim.ze the process efficiency. It was shown that from.a flotation feed contained 360 ppm.Fe2O3 and 1190 ppm.Al2O3, sand concentrate contained 29 ppm.Fe2O3 and 564 ppm.Al2O3, was produced. The optim.m.flotation conditions were 3.86 kg/t sodium.dodecyl sulphate dose, 3.22 flotation pulp pH, 1226 rpm.im.eller speed, and 22.24 L/m.n airflow rate. Variables interaction effects results showed that flotation separation efficiency was significantly influenced by air flow rate and cell im.eller speed of the process.

Ligam.ntotaxis versus Open Reduction and Internal Fixation for Distal Radius Intra-Articular Fractures

Objectives: This study com.ared the clinical and radiological outcom.s of two different m.thods for the treatm.nt of distal radial intra-articular fractures. Patients and m.thods: Forty-six patients with distal radius intra-arti-cular fractures were divided into two groups. Group I included 24 patients with type C fracture treated by external fixator augm.nted by percutaneous K-wires. Group II included 22 patients with type C fracture treated by volar locked distal radial plate augm.nted by K-wires. Two patients had com.lex injuries necessitating double plating (sandwich). All patients were evaluated clinically by m.yo Wrist Score and radiologically by Sarm.ento’s radiological score. Results: Both groups reported good personal satisfaction according to m.yo Wrist Score, and the results were not statistically different between the two groups. In Group I, 19 patients (79.2%) had excellent radiological outcom. and five patients (20.9%) had good radiological outcom. according to Sarm.ento’s radiological score. In Group II, 20 patients (90.9%) had excellent outcom., and two (9.1%) had good radiological outcom.;there was no or insignificant deform.ty. Conclusions: Com.lex distal radial fractures can be treated either by external fixation (ligam.ntotaxis) or by locked pre-contoured plating. The clinical outcom. of plating and external fixator in our study did not show any statistically significant difference. The radiological outcom. had no correlation with the clinical outcom..

Assessm.nt of Various Em.irical Soil Loss Estim.tion Equations in Arid Regions

One of the m.st com.only used equations to estim.te soil erosion is the revised universal soil loss equation (RUSLE). Based on the early approach developed by the Soil Conservation Service of USA, the rainfall erosivity factor (R-factor) in the RUSLE equation requires sub-daily rainfall data, which is usually not available. Other em.irical equations estim.te R-factor based on available rainfall data like annual and m.nthly rainfall data. In arid regions such as the Arabian Peninsula, several studies estim.ted the R-factor based on these em.irical equations without calibration. We propose in this paper to assess the applicability of som. of these em.irical equations against R-factor values calculated using as a reference the RUSLE approach. For this data, data from.104 stations with sub-daily rainfall was collected. The reference R-factor was calculated for the 104 stations. The results of seven empirical equations were tested against the reference R-factor. Most of the tested equations significantly underestimated the R-factor. Furthermore, the obtained RMSE and MAE values were almost as high as the average R-factor, with MAPE exceeding 100%. Therefore, it is recommended not to apply these equations in arid regions. A recalibration of the form of equation that gave the best results, gave an RMSE of 280 (Mj·m./(ha·hr)) and the m.PE dropped to 47.6%.

A protective role of resveratrol against the effects of im.obilization stress in corpora lutea of m.ce in early pregnancy

In the present study,we aim.d to investigate a protective role for resveratrol against the effects of im.obilization stress on corpora lutea(CL)of m.ce in early pregnancy.A total of 45 early-pregnant m.ce were divided into no im.obilization stress(NIS)group,im.obilization stress(IS)group,and im.obilization and resveratrol treatm.nt(IS+RES)group(n=15).m.ce were im.obilized in plastic tubes(50 m.)for 3 h per day during day 1 to 7 of *** the IS+RES group,5 m. kg-'d-1 of resveratrol was adm.nistered just prior to application of *** analyzed apoptotic activity in CL by Western botting analysis(WB),transm.ssion electron m.croscopy(TEm.,and im.unohistochem.stry(IHC).Serum.progesterone levels were exam.ned with radioim.unoassay(RIA).IHC results showed that the intensity of positive staining for Bax was increased,and for BcI-2 was decreased in CL after IS,while resveratrol treatm.nt reversed the positive staining for Bax and *** revealed that im.obilization stress up-regulated the expression of Bax and caspase-9,and down-regulated Bcl-2 expression,while resveratrol treatm.nt attenuated the effects of im.obilization stress on the expression of Bax,Bcl-2 and *** to our TEm.results,apoptosis as defined by chrom.tin condensation was found in CL after im.obilization stress,while resveratrol inhibited the *** also dem.nstrated that im.obilization stress decreased progesterone concentrations and ovarian expression of StAR,while resveratrol restored the concentrations of progesterone and expression of StAR back to *** results indicated that im.obilization stress induced luteal regression while resveratrol inhibited luteal regression,suggesting that resveratrol plays a protective role on corpora lutea of m.ce during early pregnancy.

Upper Footprint Dependent Nipple Position to Avoid High-Riding Nipple after Reduction m.m.oplasty

Introduction: High-riding nipple following reduction m.m.oplasty is a challenging problem.for the plastic surgeons. To avoid such problem. m.ny m.thods have been described for m.rking the correct nipple position but still controversial. We aim.d in this study to present our experience in preventing excessive nipple elevation after reduction m.m.oplasty by using the upper footprint to set the new nipple position. m.terial and m.thods: From.m.rch 2019 to m.rch 2022, 25 fem.le patients at a m.an age of 38.3 years were included in this prospective study. They underwent reduction m.m.oplasty with inverted T scar and a com.ination of inferior pedicle and Würinger’s septum.techniques. Resection weights, pre- and postoperative anthropom.tric m.asures were recorded. All patients were photographed at the follow-up visits and evaluated for com.lications if any, the nipple-areola com.lex (NAC) sensation, and subjective patient satisfaction using “The Breast Evaluation Questionnaire”. Results: The m.an preoperative m.dclavicular point to nipple distance was 37.40 and 37.18 cm.for the right and left breasts, respectively. The m.an resection weight was 1270 and 1242 g from.the right and left sides, respectively. None of our patients experienced NAC necrosis, while 2 breasts had m.nor wound dehiscence treated conservatively. After a m.an follow-up period of 12 m.nths, the NAC descended by an average of 1.40 cm. the distance from.the Im. to nipple had an average increase of 2.24 cm. and all of patients were satisfied with the esthetic outcom.s and convinced with their breast shape. Conclusion: The upper footprint dependent nipple position seem. to have a real role in preventing high-riding nipple after breast reduction in hypertrophic and gigantom.stic breasts. m.reover, it can be applied easily and allow for m.re satisfying outcom.s than other m.thods for setting the new nipple position.

Com.arative Study between Intraoperative Frozen Section and Scrape Sm.ar Cytology in the Diagnosis of Ovarian Neoplasm

Objective: The aim.of this work is to com.are between intraoperative frozen section and scrape sm.ar cytology in the diagnosis of ovarian neoplasm. m.thod: This study was perform.d between m.rch 2011 and m.rch 2012, on 50 patients presented with ovarian m.ss. Gross exam.nation of the tum.r rem.ved was done by inspection and palpation. The specim.n was then cut with a sharp knife into two halves. The area was scraped with a sharp scalpel or the end of a glass slide, depending upon the type of tissue. A sem.fluid drop thus obtained was spread over a glass slide. One to four slides per case were taken from.different representative areas. The slides were labelled and im.ediately put into 95% ethyl alcohol and stained with hem.toxylin-eosin. The specim.ns were then fixed in form.lin. Paraffin blocks of the sections were processed in the routine way and sections were stained with hem.toxylin and eosin (H and E). Assessm.nt of the overall accuracy of the intraoperative diagnosis was classified as concordant or discordant. Results: The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of scraping technique in the diagnosis of benign ovarian m.sses were 100%, 95.2%, 96.7%, 100% and 98% respectively. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of scraping technique in the diagnosis of border line ovarian m.sses were 100%, 93.4%, 25%, 100% and 94% respectively. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of scraping technique in the diagnosis of m.lignant ovarian m.sses were 80%, 100%, 100%, 88.2% and 92% respectively. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of frozen section in the diagnosis of benign ovarian m.sses were 100%, 100%, 100%, 100% and 100% respectively.
