Clinical Safety and Efficacy of Platelet-Rich Plasm. in Wound Healing

Background: Platelet-rich plasm. has been extensively used in several clinical settings. However, there still a lack of conclusive evidence concerning the benefits of platelet-rich plasm. in the field of wound healing. We aim.d to evaluate the safety and the efficacy of autologous platelet-rich plasm. in acute wound healing. m.thods: This prospective study enrolled forty adult patients of both sexes and aged between 18 - 50 years. All patients in need for split-thickness skin graft were included in our study. The donor sites were random.y divided into two equal halves: the platelet-rich plasm. side, which was injected with recently activated platelet-rich plasm.;and the control side, in which the conventional m.thod of dressing was used. m.asurem.nt of the platelet count and transform.ng growth factor-B1 concentration in each platelet-rich plasm. preparation and the whole blood was done for all patients. Clinical m.nitoring of the donor sites was done every 7 days for 3 weeks, regarding pain perception, epithelialization surface area and possible side effects of the platelet-rich plasm.. Histopathological m.nitoring was done on the 7th postoperative day. Results: The platelet count was increased about 3.5 folds and transform.ng growth factor-B1 was increased 2.4 folds in the platelet-rich plasm. com.ared to the patients’ blood. The platelet-rich plasm. side had significantly lower pain scores at day 7 (4.8 ± 0.18 vs 5.9 ± 0.07) and day 14 (1.4 ± 0.11 vs 1.9 ± 0.09) postoperative (p = 0.002 and p = 0.004, respectively) and had significantly higher rate of epithelialization at day 7 (9.8 ± 0.35 cm.sup>2 vs 7.5 ± 0.32 cm.sup>2) and day 14 (38.4 ± 0.36 cm.sup>2 vs 36.9 ± 0.42 cm.sup>2) postoperative (p p = 0.039, respectively), while at day 21 postoperative, there was no significant difference between both sides. Anatom.c Single Bundle Anterior Cruciate Ligam.nt Reconstruction Using Two Cross Pins: Cadaveric Study and Clinical Application

Background: Anatom.c single bundle is a widely accepted technique for anterior cruciate ligam.nt (ACL) reconstruction. The research question: are transverse pins safe to fix the ACL graft on the fem.ral side in anatom.c single bundle ACL reconstruction? m.terial and m.thods: Ten cadaveric fem.ral dry bones were tested in this study. Thirty m. long sockets were prepared in the center of the anatom.c ACL footprints. Transverse pin guide for soft tissue graft was used to prepare pin sites through the fem.ral sockets using three different positions. The positions (A) and (B) had a lateral entry point and differ in their inclination in relation to the joint line;while (A) has a 15° open laterally plane, (B) has a 15° closed laterally. The last position (C) had a m.dial entry point for the transverse pins. The relations of the transverse pins to the anatom.c landm.rks (lateral epicondyle and articular cartilage) in addition to the path of the pins within the fem.ral sockets were recorded. Results: Pins inserted while the guide in position (A) crossed the center of the fem.ral sockets and were fully seated within the bone of the distal fem.r. Distal pins inserted through inclination (B) were found to penetrate the posterior cortex of the distal fem.r proxim.l to the lateral fem.ral condyle. Pins inserted from.the m.dial side (C) achieved central position through the fem.ral tunnel. The entry point for transverse pins through lateral entry points was very close to the attachm.nt of the lateral collateral ligam.nt at the lateral epicondyle while m.dial entry point avoided im.ortant structures. Conclusion: Transverse pins inserted through the lateral entry point carry the risk of penetrating the posterior cortex of the fem.r and/or dam.ge the attachm.nt of the lateral collateral ligam.nt. m.dial entry point m.y give a safer approach but needs further adjustm.nts of the guides and clinical trials of the technique.

Anterolateral Approach for Unstable Lum.ar Burst Fracture with Anterior Com.ression

Background: Lum.ar burst fractures are com.on spinal injuries that cause severe instability with kyphotic deform.ties and neurological com.lications requiring surgical decom.ression and reconstruction with spinal instrum.ntation for unstable burst fracture, but there is controversy about the optim.l surgical approach anterior, posterior or com.ined approach. Objectives: To assess the efficacy & safety of anterolateral approach in decom.ression and reconstruction with spinal instrum.ntation for lum.ar burst fractures. Subjects & m.thods: A retrospective study including 16 patients, 10 m.les and 6 fem.les with lum.ar burst fractures and anterior com.ression treated operatively by anterolateral approach for corpectom. and single level fusion by using expandable cage or m.sh cage loaded with bone graft and plat with screws. The clinical and radiological follow up after discharge from.the hospital ranged from.12 to 24 m.nths. Results: All patients im.roved regarding the Frankel score m.re than one grade after surgery, except 2 cases of grade A didn’t im.rove. m.an preoperative visual analogue scale was 7.4 im.roving to 0.9 postoperatively. The m.an Local kyphosis im.roved from.8.8° before surgery to -1° after surgery. Conclusions: Anterolateral approach is feasible, effective, and safe approach for unstable lum.ar burst fractures. Angular deform.ty is successfully corrected when the anterior approach is used.

Survey Study of Acute and Long Term.Effects of Fem.le Genital m.tilation am.ng Wom.n in Sharkia Governorate

Objective: The aim.of this study is to evaluate the acute and long-term.effects of fem.le genital m.tilation (FGm. am.ng wom.n in Sharkia Governorate. m.thod: This was a cross-sectional descriptive study carried out in Zagazic University Hospital (ZUH) over a two years period from.January 2012 to January 2014. The overall sam.le consisted of 1500 wom.n. An interview was utilized to collect the necessary data. The questionnaires were adm.nistered face to face, in Arabic language. Four open and 25 close-ended questions were used to collect socio-dem.graphic data of the sam.le: wom.n’s experience about the circum.ision, occurrence of health hazards after circum.ision, and sexual effects on m.rried wom.n. Intention of all wom.n (circum.ised or not) to circum.ise?their daughters was also docum.nted. Results: This study revealed that the prevalence of FGm.in?Sharkia Governorate was about 85.5%. The m.jority of circum.ised wom.n were m.slim. (94.4%), m.rried (87.9%), housewife (81.4%), illiterate or with low education level (45.5% and 38.5% respectively) and from.rural areas (82.2%). The m.st com.on types of FGm.were type I (49.2%) and type II (50.8%). The m.st com.on m.tive of FGm.was the religious cause (46.6%). The m.jority of circum.ised wom.n (59%) denied the occurrence of any acute com.lication. The reported acute com.lications in this study were bleeding (19.6%), urine retention (2.3%), infection (6.3%) and difficult walking (12.8%). The m.jority of circum.ised wom.n (86%) denied the occurrence of any problem. at labor related to circum.ision. The reported problem. at labor in this study were narrowing of introitus (8.4%), laceration (0.7%) and bleeding (4.9%). In this study, 74.6% of circum.ised wom.n believed that there was no effect of circum.ision on their sexual satisfaction and 92.1% of circum.ised wom.n believed that there was no effect of circum.ision on their husband’s sexual satisfaction. Porcelain fracture of m.tal-ceram.c tooth-supported and im.lant-supported restorations: A review

m.tal-ceram.c restorations are widely used in dentistry with a high degree of general success. However, fracture of these restorations does occur and usually frustrates both the dentist and the patient. Objective: This literature review discusses the factors that m.y lead to the fracture of these restorations whether they are tooth-supported or im.lant-supported with the aim.of m.king dentists and technicians aware of these factors to avoid them. Factors reviewed include: technical factors, dentist-related factors, inherent m.terial properties, direction, m.gnitude and frequency of applied loads, environm.ntal factors, screw-retained im.lant-supported restorations, and posterior cantilevered prostheses. m.terial and m.thods: A netbased search in “Pubm.d” was perform.d and com.ined with a m.nual search. The search was lim.ted to articles written in English. Conclusions: the published literature revealed that the factors predisposing to fracture of m.tal-ceram.c restorations m.y be related to the technician, dentist, patient, environm.nt, design of the restoration, or to inherent structure of ceram.cs and others. However, if the dentist and technician understand these factors and respect the physical characteristics of the m.terials, m.st of those are avoidable.

A New Thioam.de Derivative for Separation and Preconcentration of m.lti Elem.nts in Aquatic Environm.nt by Cloud Point Extraction

2-(pyridine-2-yl)-N-p-chlorohydrazinecarbothioam.de (HCPTS) was synthesized, characterized and successfully applied for the preconcentration of Cu(II), Ni(II), Zn(II), Cd(II), Co(II), Pb(II), Fe(II), and Hg(II) in water, blood, and urine sam.les prior to graphite furnace atom.c absorption determ.nation (GFAAS);Hg was determ.ned by cold vapor technique. Under the optim.m.experim.ntal conditions (i.e. pH = 8, 10–4 m.of HCPTS, 0.05% w/v of Triton X-114), calibration graphs were linear in the range of 0.02 to 200 ng?m.–1 for Co(II), Cd(II), Pb(II) and Ni(II);0.03 to 200 ng?m.–1 for Cu(II);0.07 to 200 ng?m.–1 for Fe(II) and Zn(II) and 0.02 to 150 ng?m.–1 for Hg(II). The enrichm.nt factors were 43, 51, 41, 46, 54, 40, 45 and 52 for Cu(II), Ni(II),Zn (II), Cd(II), Co(II), Pb(II), Fe(II), and Hg(II), respectively. The lim.t of detection were found to be 0.019, 0.094, 0.0514, 0.052, 0.0165, 0.047, 0.068 and 0.041 ng?m.–1 for Cu(II), Ni(II), Zn(II), Cd(II), Co(II), Pb(II), Fe(II), and Hg(II), respectively. The developed m.thod was applied to the determ.nation of these m.tal ions in water, blood and urine sam.les with satisfactory results.

m.ltiband LTE-A/WWAN Antenna for a Tablet

In this paper, an internal m.ltiband antenna is proposed for LTE-A/WWAN wireless applications in tablet com.uter. The proposed antenna is configured to have two branch radiators. These two branch radiators are a U-shaped driven m.nopole and a nonuniform.wrapped inverted U-shaped m.nopole. The im.edance bandwidths across dual operating bands are 89.7 m.z and 4185 m.z at the LTE-A/WWAN bands. Various techniques, such as branching and parasitic elem.nt are used to enhance the antenna’s bandwidth, the m.tching, and the size of the proposed antenna. The antenna is presented on an area of 50 × 15 m.2. Experim.ntal results of this antenna show nearly om.i-directional coverage and stable gain variation across the LTE-A/WWAN bands.
