Profile of Autoimmune Polyendocrinopathies at the medical Clinic II of the Abass Ndao Hospital: About 40 Cases

Introduction: Autoimmune polyendocrinopathies (AP) represent a group of rare concomitant pathologies, making them underdiagnosed. The objective was to study their profile at the medical Clinic II of the Abass Ndao Hospital. Patients and methods: This was an observational, descriptive and analytical study, lasting 24 months, from January 1, 2020 to December 31, 2022. We assessed the epidemiological, clinical and paraclinical characteristics of the patients and classified the APs found. Results: We included 40 patients divided into type III (38 cases) and IV (2 cases). A female predominance was noted with a sex ratio of 0.21. The mean age was 38.6 years. A family history of component diseases of autoimmune polyendocrine syndrome (APS) was found in 62.5%. Goiter (80%) was the main clinical sign present. All 38 patients with ISAP-3 had autoimmune thyroiditis, including 29 cases of Graves’ disease (72.5%) and 9 cases of Hashimoto’s thyroiditis (22.5%). They were associated with either type 1 diabetes (57.9%), Biermer’s disease (21.1%), vitiligo + alopecia (18.4%), lupus (2.6%). The 2 patients with AP-4 had Biermer’s disease associated with either Addison’s disease or type 1 diabetes. management depended on the pathologies present and their possible complications. The immunological phenomena were also controlled. Conclusion: This series is globally similar to the literature. The polymorphous character of the clinical pictures requires a better collaboration between specialists leading to a clinical and holistic synthesis.

Audio communications and computer networking play essential roles in our daily lives,including many domains with different *** in these technologies are *** consequence,there is a dire need to secure these technologies up to *** paper presents an efficient model for secure audio signal transmission over the wireless noisy uncorrelated Rayleigh fading ***,the performance of the utilized multiple secret keys-based audio cryptosystem is analyzed in different transformation *** discrete cosine transform(DCT),the discrete sine transform(DST),and the discrete wavelet transform(DWT)are investigated in the utilizedmultiple secret key-based audio *** results show consistent results with the wireless noisy *** performance of the proposed multiple secret keys-based audio cryptosystem can be ranked concerning the employed domain as DWT,DCT,and DST transform *** simulation experiments proved that the presented multiple secret keysbased audio cryptosystemfor audio signals transmitted over the wireless noisy uncorrelatedRayleigh fading channel achieves reliable and secure wireless link utilizing combined multi security layers.

Barrett's esophagus:Prevalence and risk factors in patients with chronic GERD in Upper Egypt

AIm:To determine the prevalence and possible risk factors of Barrett's esophagus(BE) in patients with chronic gastroesophageal reflux disease(GERD) in el minya and Assuit, Upper ***:One thousand consecutive patients with chronic GERD symptoms were included in the study over 2 *** were subjected to history taking including a questionnaire for GERD symptoms, clinical examination and upper digestive tract *** signs suggestive of columnar-lined esophagus(CLE) were defined as mucosal tongues or an upward shift of the squamocolumnar *** was diagnosed by pathological examination when specialized intestinal metaplasia was detected histologically in suspected *** was monitored in 40 ***:BE was present in 7.3% of patients with chronic GERD symptoms, with a mean age of 48.3 ± 8.2 years, which was signif icantly higher than patients with GERD without BE(37.4 ± 13.6 years).Adenocarcinomawas detected in eight cases(0.8%), six of them in BE *** was no signiflcant difference between patients with BE and GERD regarding sex, smoking, alcohol consumption or symptoms of *** with BE had significantly longer esophageal acid exposure time in the supine position, measured by pH ***:The prevalence of BE in patients with GERD who were referred for endoscopy was 7.3%.BE seems to be associated with older age and more in patients with nocturnal gastroesophageal reflux.

Protective compartments are typically used to protect some specific structures from internal explosions,such as industrial buildings that contain devices that may explode in certain *** research investigates how the response of reinforced concrete(RC)compartment structures subjected to internal blast loads are affected by the following aspects:introduction of material nonlinearity in the analysis,reinforcement ratio,and aspect ratio of the *** achieve this goal,a calibrated and sophisticated FE numerical model is introduced,and a parametric study for the intended aspects is carried out.A discussion of the results and conclusions are offered,which show the role of each aspect in the dynamic performance of the compartment *** main conclusions are as follows:introduction of material nonlinearity in this type of analysis and for these structures is very important and significant in obtaining accurate outputs that are similar to actual behavior;the reinforcement ratio has a significant effect on the response and its effect varies depending on the thickness of the compartment;in general,increasing the reinforcement ratio enhances the behavior and reduces the stresses in the compartment;and the aspect ratio of the compartment does not show a clear pattern on the response of such structures under internal blast loads.

Cervical meningoceles are the least common forms of spina bifida, representing between 4% to 8% of all spinal dysraphisms. They are not accompanied by neurological deficit in childhood but attached cord syndrome can occur with growth. Surgical treatment, in addition to avoiding rupture and correcting the aesthetic damage, will preserve neurological functions. We report 2 cases that we took care of. They are a 5-month-old infant and a 3-year-old girl. The first presented a 7 cm rounded meningocele pedunculated through the C1 lamina defect. The second presented an 8.6 cm polylobed meningocele pedunculated through the C2 lamina defect. They both underwent surgery with good progress.

Giant cerebriform nevus cell nevus of the scalp is an extremely rare form of congenital melanocytic nevus. Giant cerebriform nevus of the scalp has a major psychosocial impact because of its unsightly appearance with fetid maceration. We report the case of a 35-year-old woman who had a painless, malodorous swelling of the cerebriform scalp measuring 20 × 17 cm in diameter with a wide base of insertion at the occipital level adhering to the deep planes. The excision associated with a skin plasty was carried out. Histology concluded that there was a giant cerebriform naevo-cellular nevus of the scalp.

An Efficient Encryption and Compression of Sensed IoT medical Images Using Auto-Encoder

Healthcare systems nowadays depend on IoT sensors for sending data over the internet as a common *** ofmedical images is very important to secure patient *** these images consumes a lot of time onedge computing;therefore,theuse of anauto-encoder for compressionbefore encodingwill solve such a *** this paper,we use an auto-encoder to compress amedical image before encryption,and an encryption output(vector)is sent out over the *** the other hand,a decoder was used to reproduce the original image back after the vector was received and *** convolutional neural networks were conducted to evaluate our proposed approach:The first one is the auto-encoder,which is utilized to compress and encrypt the images,and the other assesses the classification accuracy of the image after decryption and *** hyperparameters of the encoder were tested,followed by the classification of the image to verify that no critical information was lost,to test the encryption and encoding *** this approach,sixteen hyperparameter permutations are utilized,but this research discusses three main cases in *** first case shows that the combination of mean Square Logarithmic Error(mSLE),ADAgrad,two layers for the auto-encoder,and RelU had the best auto-encoder results with a mean Absolute Error(mAE)=0.221 after 50 epochs and 75%classification with the best result for the classification *** second case shows the reflection of auto-encoder results on the classification results which is a combination ofmean Square Error(mSE),RmSprop,three layers for the auto-encoder,and RelU,which had the best classification accuracy of 65%,the auto-encoder gives mAE=0.31 after 50 *** third case is the worst,which is the combination of the hinge,RmSprop,three layers for the auto-encoder,and RelU,providing accuracy of 20%and mAE=0.485.

Single Band EBG Antenna for Wireless Power Transfer Applications

This work demonstrates the design of a single band electromagnetic Band Gap(EBG)antenna by employing an open stub EBG microstrip of a patch antenna for better achievements in terms of its performance to be utilized in a reconﬁgurable harvester to operate over a wide input power *** EBG antenna has been used to gather RF energy and a FET-transistor has been obtain-able to determine and control the powerﬂow with the intention to operate at the same time for a different level of input *** measured data of the EBG antenna shows a directivity of 8.52 dBi,a gain of 7.18 dBi,a return loss of-27 dB with a radiation efﬁciency of 84.3%,showing a clear enhancement in directivity,peak realized gain,and radiation efﬁciency by 41.76%,25.61%,and 17.12%respectively compared with a regular reference antenna;without utilized the EBG ***,the harvester design accomplishes 40%of RF-DC power conversion efﬁciency over a wide dynamic input power range from-15 to 27 dBm,and its peak is around 78%.The harvester is designed to work at the ISm band for 915 mHz and is suitable for Wireless Power Transfer(WPT)uses.

The Internet of Things(IoT)is a modern approach that enables connection with a wide variety of devices *** to the resource constraints and open nature of IoT nodes,the routing protocol for low power and lossy(RPL)networks may be vulnerable to several routing ***’s why a network intrusion detection system(NIDS)is needed to guard against routing assaults on RPL-based IoT *** imbalance between the false and valid attacks in the training set degrades the performance of machine learning employed to detect network ***,we propose in this paper a novel approach to balance the dataset classes based on metaheuristic optimization applied to locality-sensitive hashing and synthetic minority oversampling technique(LSH-SmOTE).The proposed optimization approach is based on a new hybrid between the grey wolf and dipper throated optimization *** prove the effectiveness of the proposed approach,a set of experiments were conducted to evaluate the performance of NIDS for three cases,namely,detection without dataset balancing,detection with SmOTE balancing,and detection with the proposed optimized LSHSOmTE *** results showed that the proposed approach outperforms the other approaches and could boost the detection *** addition,a statistical analysis is performed to study the significance and stability of the proposed *** conducted experiments include seven different types of attack cases in the RPL-NIDS17 *** on the 2696 CmC,2023,vol.74,no.2 proposed approach,the achieved accuracy is(98.1%),sensitivity is(97.8%),and specificity is(98.8%).
