干扰素对轮状病毒的作用

本文首次报道了人白细胞干扰素(le IFN)对人轮状病毒的作用。体外实验表明,mA104细胞对人le IFN敏感,人le IFN能抑制轮状病毒wa株对mA104细胞的致病作用。临床观察说明低剂量的粗制白细胞干扰素(5万IU/ml)对轮状病毒胃肠炎有明显的疗效,与常规对症治疗比较,肌注组不论在全病程或用药后治愈天数上均有极明显的缩短(P<0.001),但有发热的付作用。灌肠组亦有显著的疗效(P<0.01),虽逊于肌注组,但无明显付作用。本文建议用部分纯化IFN肌注可能会减少付反应或加大粗IFN的灌肠剂量可望提高疗效。

感染性心内膜炎（IE）是一种具有高发病率的疾病，即使得到适当的诊断和治疗，其死亡率仍可达9％～30％。由IE所致的脓毒栓可随动脉血供累及到全身任一器官或组织，可在12％～40％的IE病人中发生。最常受累的心外部位是中枢神经系统。其他较常受累的部位为肺（尤其是右心IE）、脾、肾、肝和骨骼肌肉系统。此外，动脉系统本身也容易发生潜在的致命的真菌感染性动脉瘤。由于IE的心外并发症常在临床作出IE诊断前发生，因此影像学发现有利于提示IE诊断的特征性征象非常重要。此外。影像学还常用于明确已确诊的IE病人心外并发症的范围。

In a prospective cohort study of 142 outpatients with advanced heart failure followed for a mean of 3 years, 29%of subjects with a depression diagnosis at baseline were significantly more likely to experience the combined end point of death or transplantation(hazard ratio 2.54, 95%confidence interval 1.16 to 5.55). After adjustment for a range of sociodemographic and clinical characteristics, patients with depressive disorders were still significantly more likely to reach the combined end point(hazard ratio 2.41, 95%confidence interval 1.24 to 4.68). Depressed patients also had more heart failure related hospitalizations(1.5±1.8 vs 0.6±1.4, p=0.04) and clinic visits(2.4±1.7 vs 1.7±1.8, p=0.04)-over the first year of follow-up.

脐带过短的危险因素及预后

OBJECTIVE: To identify risk factors and outcomes associated with a short umbilical cord. mETHODs:We conducted a population- based case- control study using linked washington state birth certificate- hospital discharge data for singleton live births from 1987 to 1998 to assess the association between maternal, pregnancy, delivery, and infant characteristics and short umbilical cord. Cases (n = 3,565) were infants diagnosed with a short umbilical cord. Controls (n = 14,260) were randomly selected from among births without a diagnosis of short umbilical cord. REsUlTs: Case mothers were less likely to be overweight (body mass index 25 or more, odds ratio 0.7; 95% confi- dence interval 0.6, 0.8) and more likely to be primiparous (OR 1.4; 95% CI 1.3, 1.6). Case infants were more likely to be female (OR 1.3; 95% CI 1.2, 1.4), have a congenitalmalformation (OR1.6; 95% CI 1.4, 1.8), and be small for their gestational age (risk ratio 1.6; 95% CI 1.4, 1.9). A short cord was associated with increased risk for maternal labor and delivery complications, including retained placenta (RR 1.6; 95% CI 1.2, 2.3) and operative vaginal delivery (RR 1.4; 95% CI 1.3, 1.5). Adverse fetal and infant outcomes in cases included fetal distress (RR 1.8; 95% CI 1.6, 2.1) and death within the first year of life among term infants (RR 2.4; 95% CI 1.2, 4.6). CONClUsION: modifiable risk factors associated with the development of a short cord were not identified. Case mothers and infants are more likely to experience labor and delivery complications. Term case infants had a 2- fold increased risk of death, which suggests closer postpartum monitoring of these infants.

Background and Purpose- magnetic resonance imaging (mRI)- scans in the elderly commonly show white matter findings that may raise concerns. We sought to document incidence, manifestations, and predictors of worsening white matter grade on serial imaging. methods - The Cardiovascular Health study is a population- based, longitudinal study of 5888 people aged 65 years and older, of whom 1919 have had extensive initial and follow- up evaluations, including 2 mRI scans separated by 5 years. scans were read without clinical information in standard side- by- side fashion to determine worsening white matter grade. Results - Worsening was evident in 538 participants (28% ), mostly (85% ) by 1 grade. Although similar at initial scan, participants with worsening white matter grade, compared with those without, experienced greater decline on modified mini- mental state examination and Digit- symbol substitution test (both P≤ 0.001) after controlling for potential confounding factors, including occurrence of transient ischemic attack or stroke between scans. Independent predictors of worsening white matter grade included cigarette smoking before initial scan and infarct on initial scan. Otherwise, predictors differed according to white matter grade on initial scan. For low initial grade, increased age, increased diastolic blood pressure, increased high- density lipoprotein cholesterol, and decreased low- density lipoprotein cholesterol were associated with increased risk of worsening. For high initial grade, any cardiovascular disease and low ankle- arm index were associated with decreased risk of worsening, whereas use of diuretics and statins were associated with increased risk. Conclusion - Worsening white matter grade on serial mRI scans in elderly is common, is associated with cognitive decline, and has complex relations with cardiovascular risk factors.

Although genome-wide association studies have identified more than eighty genetic variants associated with non-small cell lung cancer(NsClC)risk,biological mechanisms of these variants remain largely *** integrating a large-scale genotype data of 15581 lung adenocarcinoma(AD)cases,8350 squamous cell carcinoma(sqCC)cases,and 27355 controls,as well as multiple transcriptome and epigenomic databases,we conducted histology-specific meta-analyses and functional annotations of both reported and novel susceptibility *** identified 3064 credible risk variants for NsClC,which were overrepresented in enhancer-like and promoter-like histone modification peaks as well as DNase I hypersensitive *** factor enrichment analysis revealed that UsF1 was AD-specific while CREB1 was *** annotation and genebased analysis implicated 894 target genes,including 274 specifics for AD and 123 for sqCC,which were overrepresented in somatic driver genes(ER=1.95,P=0.005).Pathway enrichment analysis and Gene-set Enrichment Analysis revealed that AD genes were primarily involved in immune-related pathways,while sqCC genes were homologous recombination deficiency *** results illustrate the molecular basis of both wellstudied and new susceptibility loci of NsClC,providing not only novel insights into the genetic heterogeneity between AD and sqCC but also a set of plausible gene targets for post-Gwas functional experiments.
