Therapeutic mammoplasty in management of Breast Cancer: A Prospective Clinical Study

Background: The introduction of oncoplasticbreast surgery, with a concomitant contralateral symmetrization procedure, allows the surgical oncologist the freedom to perform wider excisions, with more favorable cosmetic outcomes. Objective: To assess the feasibility of bilateral breast reduction for the management of breast cancer, and to determine the most suitable surgical technique of therapeutic mammoplasty for each patient. And to evaluate the cosmetic outcome during the postoperative period. methods: This study was from June 2014 until June 2016, and included 86 female patients with breast cancer;divided in 3 groups;group A;had bilateral superior pedicle reduction mammoplasty, group B;had bilateral inferior pedicle reduction mammoplasty, group C;had bilateral batwing mastopexy. 1 month postoperative, patients were asked to answer a 5-scale questionnaire evaluating their own cosmetic outcome. Criteria they were asked to evaluate were: symmetry, shape and volume, projection, correction of ptosis, visibility of the scars and overall satisfaction. A similar questionnaire was answered by a panel made up of a surgeon and breast-care nurse after seeing the pre- and post-operative photos of the patients in question. Results: Group A had a mean tumor size of 2.7 ± 0.69 cm, least resected margins 1.2 - 4.3 cm, Group B had a mean tumor size of 3.2 ± 1.19 cm, least resected margins 0.7 - 3.8 cm, Group C had a mean tumor size of 3.4 ± 1.12 cm, least resected margins 1.7 - 5.2 cm. Cosmetic results fared high above average. Group C fared better in overall satisfaction but Group B fared better in ptosis correction, projection and symmetry. The panel answers showed difference regarding shape and volume;the only two criteria to be almost of the same result among the three groups. Nutrient Fluxes and Sediments Composition in El mex Bay and Surround Drains, Alexandria, Egypt

The regeneration of pore water (PW) nutrients was investigated and the contribution of benthic nutrient fluxes to the overlying bottom water (BW) was examined. Dissolved inorganic nutrients (NO-2, NO-3, PO3-4 and SIO4-4) were measured in PW and BW in El mex Bay and surround drains during spring 2010. Nutrient concentrations gradiance in PW with overlying seawater were evaluated according to Fick’s Law. Average inorganic nutrient fluxes were estimated using the pore water gradient concentration across the sediment-water interface. Calculated nutrient fluxes had averages of ﹣7.24, ﹣1.36, ﹣7.86 and ﹣1.33 in El mex Bay. Additionally, the fluxes in the drains were ﹣34.39, ﹣32.28, ﹣53.20 and ﹣117.6 mg·m﹣2·day﹣1 for NO-3, NO-2, PO3-4 and SIO4-4, respectively. mineralogical studies of sediment samples by using IR, X-ray analysis were carried out to identify the chemical structure of sediments. The results revealed that calcite, aragonite and quartz are the dominant minerals. On the other hand, differential thermal analysis (DTA) was used to evaluate and discuss different kinetic parameters such as Ea#, ΔG#, ΔH#, ΔS#, Z and Tm support the view of the extra stability of these sediments.

Impact of Quebracho Tannins Supplementation on Productive and Reproductive Efficiency of Dairy Cows

This study was conducted to investigate effects of supplementing two doses of quebracho tannins (QT;100 or 200 g/cow/d;QT100 or QT200) pre and post parturition to thirty Holstein dairy cows on Dry matter Intake (DmI), milk yield and composition, blood metabolites and reproductive performance for 12 weeks. There were no significant changes in DmI due to QT at transition period of dairy cows. QT supplementation at either level decreased (P 0.05) the total number of ovarian follicles, number of large follicles, diameter of largest follicle, number of small follicles, number and diameter of corpus luteum, and progesterone concentration. Role of Vascular Endothelial Growth Factor-C during Stem Cell Therapy Using Autologous Bone marrow mononuclear Cells in Patients with Lower Limb Lymphedema

Introduction: Vascular endothelial growth factor-C (VEGF-C) is the primary lymphangiogenic factor that stimulates lymphangiogenesis by signaling via specific receptor, vascular endothelial growth factor receptor 3 (VEGFR3). This study was conducted to evaluate the change in the level of VEGF-C before and after autologous bone marrow mononuclear cell transplantation for treatment of Lower limb lymphedema. Patient and methods: Forty patients with lower limb lymphedema were divided into two groups. Group I included 20 patients with chronic lower limb lymphedema who underwent autologous bone marrow mononuclear cell transplantation. Group II included 20 patients with chronic lower limb lymphedema who were exposed only to compression therapy as a control group. VEGF-C level in the diseased limbs was measured in both groups at the beginning of the study then 3 and 6 months respectively. Results: Group I included 20 patients, 8 patients were male (40%) and 12 patients were females (60%) with mean age 29.5 ± 12.15 while group II included 20, 10 patients were male (50%) and 10 patients were females (50%) with mean age 39.5 ± 11.5. In group I, the specimens were taken at 3 and 6 months after transplantation showed a marked decrease in the VEGF-C level with statistically significant p value, 0.02 and 0.001 respectively. In group II the level of VEGF-C after compression therapy alone at 3 and 6 months interval showed fluctuation with statistically non-significant p value, 0.64 and 0.55 respectively. Conclusion: VEGF-C is essential for regulation of lymphangiogenesis. The level of VEGF-C was found elevated in patients with lymphedema and decrease after autologous mononuclear bone marrow cells, however these results were statically non-significant.

The outbreaks of invasive plant species can cause great ecological and agronomic problems through aggressively competing for environmental resources that could be otherwise utilized by other desirable species. Thus, it is crucial for detecting small infestations before they reach a significant extent that can cause ecological and economic damages over a large geological area. Remote sensing is a proven method for mapping invasion extent and pattern based on geospatial imagery and indicated great repeatability, large coverage area, and lower cost compared with traditional ground-based methods before. We investigated the feasibility and performances of adopting multispectral satellite imagery analyses for mapping infestation of musk thistle (Carduus nutans) on native grassland, crop field, and residential areas in early June using spectral angle mapper classifier. Our results showed an overall classification accuracy of 94.5%, indicating great potential of using moderate resolution multispectral satellite-based remote sensing techniques for musk thistle detection over a large spatial scale.

Cost Optimization modeling of Renewable Energy Sources in Smart-Grid Using SCADA

In recent times, renewable energy production from renewable energy sources is an alternative way to fulfill the increased energy demands. However, the increasing energy demand rate places more pressure, leading to the termination of conventional energy resources. However, the cost of power generation from coal-fired plants is higher than the power generation’s price from renewable energy sources. This experiment is focused on cost optimization during power generation through pumped storage power plant and wind power plant. The entire modeling of cost optimization has been conducted in two parts. The mathematical modeling was done using mATLAB simulation while the hydro and wind power plant’s emulation was performed using SCADA (Supervisory control and data acquisition) designer implementation. The experiment was conducted using ranges of generated power from both power sources. The optimum combination of output power and cost from both generators is determined via mATLAB simulation within the assumed generated output power range. Secondly, the hydro-generator and wind generator’s emulation were executed individually through synchronizing the grid to determine each generator’s specification using SCADA designer, which provided the optimum power generation from both generators with the specific speed, aligning with results generated through mATLAB. Finally, the operational power cost (with no losses consideration) from mATLAB was compared with the local energy provider to determine the cost-efficiency. This experiment has provided the operational cost optimization of the hydro-wind combined power system with stable wind power generation using SCADA, which will ultimately assist in operations of large-scale power systems, remotely minimizing multi-area dynamic issues while maximizing the system efficiency.

L-Arginine Supplementation Alters maternal Blood Biochemical Attributes and milk Composition Relative to Neonatal Traits of <i>Najdi</i>Ewes

In a 2 × 3 factorial arrangement, 3 levels of L-arginine (0, 75 and 150 mg/kg BW per day) were tested at two stages of pregnancy (Early 60 days vs. Late 60 days) on the maternal blood metabolites and postnatal milk composition relative to the newborn traits of Najdi ewes. Thirty adult pregnant Najdi ewes were randomly assigned into 6 treatments (n = 5). Control early (CE) ewes were orally administered 50 mL normal saline (0.9% NaCl) daily during the first 60 days of pregnancy;control late (CL) ewes were similar to CE except saline was given at the last 60 days of pregnancy;low dose of L-arginine at early pregnancy (LE);low dose of L-arginine at late pregnancy (LL);high dose of L-arginine at early (HE) and high dose of L-arginine at late pregnancy (HL). Blood and milk samples were collected on a weekly basis. Data of lambing and newborn traits were considered. There were non-significant differences in litter size due to L-arginine;however lamb birth and weaning weights increased by 35% in LE-ewes. Arginine did not change blood protein and albumin. Low arginine decreased lipid fractions, creatinine, urea, and milk fat;however, it increased AST, ALT, milk lactose and milk ash. Administration of L-arginine at 75 mg/Kg BW per day for 60 days at an early gestation in ewes improved the maternal body health and milk composition for the welfare of the neonates.

Diagnostic accuracy of biochemical markers of fibrosis in black African patients with chronic hepatitis B

Contradictory results of the accuracy of bio-chemical markers to predict the stage of fibrosis in black African patients with chronic hepatitis B (CHB) were previously published. We con-ducted a prospective cohort study to determine the diagnostic accuracy of aspartate ami-notransferase to platelet ratio (APRI), aspartate aminotransferase to alanine aminotransferase ratio (AAR), platelet count, age-platelet (AP) in-dex, and Fib-4 index for the prediction of sig-nificant fibrosis or cirrhosis in 117 black African patients (median age: 38 years, males: 73 %) with CHB not previously treated. Among them, 45 had significant fibrosis and 18 had cirrhosis using the mETAVIR score system. Factors as-sociated either with significant fibrosis or cir-rhosis were determined in logistic multivariate analysis. Areas under receiver operating curve were assessed and compared for APRI, AAR, AP index, Fib-4 index and platelet count. Sensitivity, specificity, positive and negative predictive values were determined for each biochemical markers. multivariate analysis showed that as-partate aminotransferase (p 1.1) and Fib-4 index (cut-off > 2.1) ruled out significant fibrosis with high specific-ity of 84.7 % and 86.1 % respectively and nega-tive predictive values of 78.2 % and 72.9 % re-spectively. more accurately, APRI (cut-off > 0.63) or Fib-4 index (cut-off > 1.26) ruled out cirrhosis with high sensitivity of 94.4% and 88.9% and high negative predictive values of 98.1% and 96.3% respectively. In conclusion, APRI and Fib-4 index are simple readily available markers to exclude significant fibrosis or more accu-rately cirrhosis in black African patients with CHB.

Surgical management of primary hyperparathyroidism guided by double-phase Tc-99m-mibI scintigraphy

Objective:The aim of this work was to study the clinicopathological features of cases with primary hyperparathyroidism (PHPT) referred to National Cancer Institute (NCI), Cairo University in the last six years and to study the role of nuclear medicine techniques in the initial diagnosis and preoperative localization directing surgical management of these cases. methods: This study included 27 patients with PHPT properly diagnosed and treated in the NCI, Cairo University from January 2005 to December 2010. Preoperative neck U/S and Tc-99m-mibI scintigraphy were done in all cases. If preoperative localization detected a single lesion, unilateral exploration was done. Bilateral exploration was done if multiple foci of active parathyroid glands or no lesions were detected. Results: This study included 27 patients (8 males and 19 females). The median age was 43 years (range from 19 to 68 years). All cases presented with bone disease in the form of bony pain in 23 patients, bony swellings in 15 patients and pathological fractures in 6 cases. Twenty one patients (77.8%) had single adenoma, 5 cases (18.5%) had parathyroid hyperplasia, and only one case had double adenomas. The sensitivity of neck U/S in detecting single adenoma was 61.9% (13/21), with 81% positive predictive value (PPV) while the sensitivity of Tc-99m-mibI scintigraphy was 90.5% (19/21) with 100% PPV. Based on preoperative localization tests, unilateral exploration was done in 19 patients with solitary adenomas. All cases in this group were cured with no reported case of persistent or recurrent hypercalcemia. Conclusion: Presentation of PHPT may mimic malignant bone tumors but fortunately these patients were correctly diagnosed with bone scan which confirm the presence of metabolic bone disease rather than bone metastases.
