目的分析部队某大型综合医院集中收治模式日间治疗(除日间手术以外的其他日间医疗)服务的开展现状,为推行日间治疗集中收治模式的管理及运营等方面提供实践支持。方法回顾性收集2021年2月7日-2023年12月31日解放军总医院第一医学中心日间诊疗科集中收治模式日间治疗患者的资料,对患者情况、日间治疗运行指标及质控指标进行统计分析。结果日间治疗患者共14 903例,其中女8 548例,男6 355例;年龄18~74岁;住院次数≥2次者9 757例(65.47%);支付方式以医保为主(83.16%)。以乳腺癌为主要诊断的患者占比最高(26.45%);患者的治疗方式以靶向、免疫治疗为主(68.03%)。患者共出现1 146例药物相关不良反应,主要涉及抗肿瘤药物,其中15例为治疗当日处理的药物相关不良反应,另外1 131例为患者在院外发生的药物相关不良反应。2023年1月优化日间治疗住院全流程后(分时段诊疗),与2022年相比患者入院后平均等候时间缩短了26.89%,床位使用率提高了9.09%,患者满意度提高到98.98%。2023年全年的日间治疗爽约率1.15%,日间治疗当日取消率4.39%,出院随访率达100%。结论解放军总医院第一医学中心日间诊疗科集中收治模式日间治疗以乳腺癌患者为主,主要治疗方式为靶向、免疫治疗。分时段诊疗有利于缩短患者入院后平均等候时间,提高床位使用率和患者满意度。

Various neuromodulation techniques have been developed to modulate the peak activity of neurons, thereby regulating brain function and alleviating neurological disorders. Additionally, neuronal stimulation and imaging have significantly contributed to the understanding and treatment of these diseases. Here, we propose utilizing photonic nanojets for optical stimulation and imaging of neural cells. The application of resin microspheres as microlenses enhances fluorescence imaging of neural lysosomes, mitochondria, and actin filaments by generating photonic nanojets. Moreover, optical tweezers can precisely manipulate the microlenses to locate specific targets within the cell for real-time stimulation and imaging. The focusing capabilities of these microlenses enable subcellular-level spatial precision in stimulation, allowing highly accurate targeting of neural cells while minimizing off-target effects. Furthermore, fluorescent signals during neural cell stimulation can be detected in real-time using these microlenses. The proposed method facilitates investigation into intercellular signal transmission among neural cells, providing new insights into the underlying mechanisms of neuronal cell activities at a subcellular level.

动态贝叶斯网络在描述具有多个通道的复杂随机过程方面具有优异的性能.基于动态贝叶斯网络进行音视频双模态说话人识别的工作.分析了音视频联合建模的层级结构,利用动态贝叶斯网络对不同层级的音视频关联关系建立模型,并基于该模型进行音视频说话人识别的实验.通过对不同层级的建模过程及说话人识别实验的结果进行分析,结果表明,动态贝叶斯网络为描述音视频间的时序相关性和特征相关性提供了有效的建模方法,在不同语音信噪比的情况下均能提高说话人识别的性能.

Axillary lymph node status is one of the most important prognostic indicator of survival for breast cancer, especially in ductal carcinoma in situ （DCIS）. The purpose of this study was to investigate whether sentinel lymph node biopsy （SLNB） should be performed in patients with an initial diagnosis of DCIS. Methods： A retrospective study was performed of 124 patients with an initial diagnosis of DCIS between March 2000 and June 2014. The patients were treated with either SLNB or axillary node dissection during the surgery, and we compared the clinicopathologic characteristics, image features, and immunohistochemical results. Results： Eighty-two patients （66.1%） had pure DCIS and 25 （20.2%） had DCIS with microinvasion （DCISM）, 17 （13.7%） updated to invasive breast cancer （IBC）. 115 patients （92.7%） underwent SLNB, among them, 70 patients （56.5%） underwent axillary node dissection. 3 of 115 patients （2.6%） had a positive sentinel lymph node, only 1 （1.4%） of 70 patients had axillary lymph node metastasis, in 84 patients （66.7%） who were diagnosed DCIS by core needle biopsy （CNB） and vacuum-assisted biopsy （VAB）. 26 patients （31.0%） were upstaged into IBC or DCISM in the final histological diagnosis. The statistically significant factors predictive of underestimation were large tumor size, microcalcifications, comedo necrosis, positive Her-2 status, negative estrogen receptor status. Conclusion： The metastasis of sentinel lymph nodes in pure DCIS is very low, but the underestimation of invasive carcinoma in patients with an initial diagnosis of DCIS is an usual incident, especially in the cases when DCIS is diagnosed by CNB or VAB. Our findings suggest patients presenting with a preoperative diagnosis of DCIS associated with large tumor sizes, microcalcifications, comedo necrosis, positive Her-2 status, negative ER status are more likely to be DCISM and IBC in final diagnosis. SLNB should be performed in this part of patients.

Hydrogen peroxide(H_(2)O_(2))photoproduction in seawater with metal-free photocatalysts derived from biomass materials is a green,sustainable,and ultra environmentally friendly ***,most photocatalysts are always corroded or poisoned in seawater,resulting in a significantly reduced catalytic ***,we report the metal-free photocatalysts(RUT-1 to RUT-5)with in-situ generated carbon dots(CDs)from biomass materials(Rutin)by a simple microwave-assisted pyrolysis *** visible light(λ≥420 nm,81.6 mW/cm^(2)),the optimized catalyst of RUT-4 is stable and can achieve a high H_(2)O_(2)yield of 330.36μmol/L in seawater,1.78 times higher than that in normal *** transient potential scanning(TPS)tests are developed and operated to in-situ study the H_(2)O_(2)photoproduction of RUT-4 under operation ***-4 has strong oxygen(O_(2))absorption capacity,and the O_(2)reduction rate in seawater is higher than that in *** cations in seawater further promote the photo-charge separation and facilitate the photo-reduction *** RUT-4,the conduction band level under operating conditions only satisfies the requirement of O_(2)reduction but not for hydrogen(H2)*** work provides new insights for the in-situ study of photocatalyst under operation condition,and gives a green and sustainable path for the H_(2)O_(2)photoproduction with metal-free catalysts in seawater.

Elucidating the flow features around piles in local scouring processes is crucial for studies of local scouring mechanisms and scour depth *** study details the flow turbulence characteristics of two submerged piles that are determined by solving the Navier-Stokes equations with the improved delayed detached eddy simulation *** model is verified by comparing experimental and numerical results for hydrodynamic parameters with the literature for both square-crossing piles(SCPs)and circular-crossing piles(CCPs).Original topographies of flat and scoured beds(i.e.,the initial and equilibrium scouring stages)are based on experimental results obtained by the authors in the present *** and CCP flow features in the scouring process are *** results indicate that during the scouring process,the time-averaged drag coefficient and root mean square(rms)of the lift coefficient increase linearly in the CCP test,while the rms of the lift coefficient in the SCP test decreases ***,the minimum pressure coefficient is always located in the upstream corners in the SCP case but moves from 72.5°to 79.5°when the scour hole is completely developed in the CCP *** flow behind the pile,which is generated by separated boundary layers above the top face of the pile,can reach the sand bed and turn the separated shear layers into patches of small vortices in the near-wake ***,the high shear stress zones are mainly at the scour edges under scoured-bed conditions.
