Short-term effects of supplementary feeding with enteral nutrition via jejunostomy catheter on post-gastrectomy gastric cancer patients

Background Most gastric cancer patients who undergo gastrectomy develop malnutrition. It is, therefore, crucial to establish an effective means to provide nutrition for these patients. To perform home enteral nutrition （EN） to ensure adequate nutritional intake in gastric cancer patients, we placed a jejunostomy catheter during gastric surgery. Most patients showed improved nutritional status. Methods Twenty-nine inpatients at our hospital underwent radical gastrectomy and jejunostomy from December 2002 to December 2007 and were designated as the jejunostomy group, and 32 matched patients without a jejunostomy tube were designated as the tube-free group. The jejunostomy group was treated with EN from 72 hours to 3 months postoperatively. The tube-free group did not receive EN. Data including preoperative and postoperative body weight, body mass index （BMI）, nutrition risk screening （NRS） score, Karnofsky performance score （KPS）, and laboratory biochemical indicators were documented respectively and compared. Results Compared with preoperative week 1, both groups showed decreased body weight and BMI at 3 months postoperatively. The weight loss in the jejunostomy group （（7.1±3.3） kg） was significantly less than that in the tube-free group （（9.9±3.1） kg）. Similarly, BMI decreased by （2.4±1.0） kg/m2 in the jejunostomy group, which was significantly less than in the tube-free group （（3.2±0.9） kg/m2）. The number of patients with postoperative NRS 〉3 was decreased in the jejunostomy group, but was increased in the tube-free group, and this difference was significant. There were no significant differences between the two groups in total lymphocyte count, hemoglobin, albumin and prealbumin, and adverse drug effects. Conclusions Short-term （3 months） EN supplementation via jejunostomy tube can reduce the risk of malnutrition and weight loss, and improve tolerance of chemotherapy. We operated a p-type point contact high purity germanium(PPCGe)detector(CDEX-1B,1.008 kg)in the China Jinping Underground Laboratory(CJPL)for 500.3 days to search for neutrinoless double beta(0νββ)decay of^(76)Ge.A total of 504.3 kg⋅day effective exposure data was *** anti-coincidence and the multi/single-site event(MSE/SSE)discrimination methods were used to suppress the background in the energy region of interest(ROI,1989–2089 keV for this work)with a factor of 23.A background level of 0.33 counts/(keV⋅kg⋅yr)was *** lower limit on the half life of^(76)Ge 0νββdecay was constrained as T_(1/2)^(0ν)>1.0×10^(23)yr(90%C.L.),corresponding to the upper limits on the effective majorana neutrino mass:<3.2–7.5 eV.

Laparoscopic-endoscopic cooperative surgery for gastric submucosal tumors

碘是人体必需的微量元素,在自然界中分布广泛但极不均衡,因此生活于不同地区人群的碘摄入量存在很大差别。碘摄入过低可引起地方性甲状腺肿、呆小症和甲状腺功能减退,碘摄入过量可导致高碘性甲状腺肿、慢性淋巴细胞性甲状腺炎、碘性甲状腺功能亢进和甲状腺功能减退,碘缺乏及碘过量对甲状腺癌的影响均主要表现为组织学类型的变化。我国自1996年实施食盐碘化以来已在国家水平上基本消除碘缺乏病,同时也出现了其他甲状腺疾病谱带的变化,包括碘性甲状腺功能亢进、自身免疫性甲状腺疾病及甲状腺癌中乳头状甲状腺癌的比例增多。对人群,尤其是甲状腺疾病患者进行个体碘营养状况评测具有重大意义。

目的探讨原发性甲状腺功能亢进合并隐匿性甲状腺癌的外科诊断和治疗方法。方法对北京协和医院2004年1月至2014年11月收治的51例患者的临床资料进行回顾性分析。结果原发性甲状腺功能亢进合并隐匿性甲状腺癌占同期甲状腺功能亢进手术患者的5.03%,占甲状腺癌手术患者的0.67%。女性患者占92.16%。51例患者术前均诊断为原发性甲状腺功能亢进,其中11例术前同时诊断甲状腺癌可能。所有患者中,25例术中冰冻病理提示恶性,26例术后石蜡病理诊断甲状腺癌。26例行甲状腺大部或次全切除术,4例行甲状腺全切除术,21例行甲状腺全切除＋区域性淋巴结清扫术。肿瘤大小0.1~1.0 cm,平均（0.63±0.35）cm;其中直径≤0.5 cm的有28例（54.9%）。获得随访43例,随访时间1~121个月,平均（28.6±22.7）个月,随访患者均生存至本文发稿时。结论原发性甲状腺功能亢进合并隐匿性甲状腺癌女性患者多见,术前诊断困难,超声是主要检查方法,重视冰冻检查可以提高术中诊断率,手术治疗效果满意,预后良好。

目的研究北京协和医院27年间甲状腺疾病手术患者构成的变化。方法收集1986～2012年北京协和医院住院行甲状腺手术并取得病理诊断的患者的基本临床资料,建立数据库,分析不同特征人群、不同年份甲状腺疾病构成比和甲状腺恶性肿瘤构成比变化。结果北京协和医院27年间甲状腺疾病住院手术患者明显增加,结节性甲状腺肿、甲状腺癌增高明显。1996年后结节性甲状腺肿、甲状腺癌构成比增加明显,恶性肿瘤中甲状腺乳头状癌增多明显。2008年后微小乳头状癌的检出率不断增高。结论北京协和医院27年间甲状腺住院患者病种及恶性肿瘤病理类型构成发生了显著变化。

mammalian target of rapamycin(m TOR) inhibitor as an attractive drug target with promising antitumor effects has been widely investigated. High quality clinical trial has been conducted in liver transplant(LT) recipients in Western countries. However, the pertinent studies in Eastern world are paucity. Therefore, we designed a clinical trial to test whether sirolimus can improve recurrence-free survival(RFS) in hepatocellular carcinoma(HCC) patients beyond the Milan criteria after LT. This is an open-labeled, single-arm, prospective, multicenter, and real-world study aiming to evaluate the clinical outcomes of early switch to sirolimus-based regimens in HCC patients after LT. Patients with a histologically proven HCC and beyond the Milan criteria will be enrolled. The initial immunosuppressant regimens are center-specifc for the frst 4-6 weeks. The following regimens integrated sirolimus into the regimens as a combination therapy with reduced calcineurin inhibitors based on the condition of patients and centers. The study is planned for 4 years in total with a 2-year enrollment period and a 2-year follow-up. We predict that sirolimus conversion regimen will provide survival benefts for patients particular in the key indicator RFS as well as better quality of life. If the trial is conducted successfully, we will have a continued monitoring over a longer follow-up time to estimate indicator of overall survival. We hope that the outcome will provide better evidence for clinical decision-making and revising treatment guidelines based on Chinese population data.

At present,natural orifice specimen extraction surgery(NOSES)has attracted more and more attention worldwide,because of its great advantages including minimal cutaneous trauma and post-operative pain,fast post-operative recovery,short hospital stay,and positive psychological ***,NOSES for the treatment of gastric cancer(GC)is still in its infancy,and there is great potential to improve its theoretical system and clinical ***,several key points including oncological outcomes,bacteriological concerns,indication selection,and standardized surgical procedures are raised with this innovative ***,it is necessary to achieve an international consensus to regulate the implementation of GC-NOSES,which is of great significance for healthy and orderly development of NOSES worldwide.

目的明确胃、结直肠恶性肿瘤患者手术前后肿瘤性贫血、缺铁性贫血的发生率及其治疗现状.方法前瞻性收集2009年8月至12月在北京市8家综合医院明确诊断为胃、结直肠恶性肿瘤的患者共262例,检测术前、术后7 d空腹静脉血血红蛋白、血清铁及铁蛋白水平,分析患者手术前后肿瘤性贫血和缺铁性贫血的发生率.结果在131例胃癌患者中,手术前肿瘤相关性贫血发生率为36.6%,缺铁性贫血发生率为52.1%,其中贫血患者平均年龄高于无贫血患者年龄[（62±11）岁比（57±12）岁,P＜0.05];手术后缺铁性贫血发生率升至72.6%,明显高于术前（P＜0.05）.131例结直肠癌患者手术前肿瘤相关性贫血发生率为37.4%,缺铁性贫血发生率为61.2%,其中45%贫血患者结肠肿瘤部位为右半结肠癌;手术后缺铁性贫血发生率明显高于术前（76.7%,P＜0.05）.消化道肿瘤性贫血的术前治疗率为10.3%,术后治疗率为22.7%,半数以上的肿瘤性贫血患者接受了输血治疗.结论胃、结直肠恶性肿瘤患者中缺铁性贫血及肿瘤相关性贫血发生率高,主要病因为缺铁性贫血.老年患者、右半结肠癌患者中贫血发生率高.贫血患者接受治疗比例较低,需要建立系统的肿瘤性贫血治疗规范.
