Minimalistic approach to left atrial appendage occlusion guided by cardiac computed tomography angiography

OBJECTIVE To assess the feasibility and safety of the minimalistic approach to left atrial appendage occlusion(LAAO) guided by cardiac computed tomography angiography(CCTA).METHODS Ninety consecutive patients who underwent LAAO, with or without CCTA-guided, were matched(1.2). Each step of the LAAO procedure in the computed tomography(CT) guidance group(CT group) was directed by preprocedural CT planning. In the control group, LAAO was performed using the standard method. All patients were followed up for 1. months, and device surveillance was conducted using *** A total of 90 patients were included in the analysis, with 30 patients in the CT group and 60 matched patients in the control group. All patients were successfully implanted with Watchman devices. The mean ages for the CT group and the control group were 70.0 ± 9.4 years and 68.4 ± 1..9 years(P = 0.52), respectively. The procedure duration(45.6 ± 1..7 min vs. 58.8 ± 1..0 min,P 1. and hospital stay(7.5 ± 2.4 day vs. 9.6 ± 2.8 day, P = 0.001. in the CT group was significantly shorter compared to the control group. However, the total radiation dose was higher in the CT group compared to the control group(904.9 ± 348.0 m Gy vs.71..9 ± 21..2 m Gy, P = 0.002). There were no significant differences in periprocedural pericardial effusion(3.3% vs. 6.3%, P = 0.8) between the two groups. The rate of postprocedural adverse events(1..3% vs. 1..3%, P = 0.55) were comparable between both groups at 1. months *** CCTA is capable of detailed LAAO procedure planning. Minimalistic LAAO with preprocedural CCTA planning was feasible and safe, with shortened procedure time and acceptable increased radiation and contras consumption. For patients with contraindications to general anesthesia and/or transesophageal echocardiography, this promising method may be an alternative to conventional LAAO.

CD3-specific monoclonal antibody was the first one used for clinical practice in field of transplantation. Recently, renewed interests have elicited in its capacity to prevent autoimmune diabetes by inducing immune tolerance. In this study, we tested whether this antibody can also be used to treat another kind of autoimmune disease myasthenia gravis （MG） and explored the possible mechanisms. MG is caused by an autoimmune damage mediated by antibody- and complement-mediated destruction of AChR at the neuromuscular junction. We found that administration of CD3-specific antibody （Fab）2 to an animal model with experimental autoimmune myasthenia gravis （EAMG）（B6 mice received 3 times of AChR/CFA immunization） could not significantly improve the clinical signs and clinical score. When the possible mechanisms were tested, we found that CD3 antibody treatment slightly down-regulated the T-cell response to AChR, modestly up-regulation the muscle strength. And no significant difference in the titers of IgG2b was found between CD3 antibody treated and control groups. These data indicated that CD3-specific antibody was not suitable for treating MG, an antibody- and complementmediated autoimmune disease, after this disease has been established. Four polycarboxylate- type （ PC ） superplasticizers of different functional groups were used ; their dispersing and retaining behaviors were analyzed through ZETA potential measurement, mini-slump test of cement paste and performance test of concrete. The experimental results show that the dispersing and flow-retaining ability of PC was determined by two factors of anionic groups and nonionic groups ： the density of anionic groups - COO^- or - SO3^- acted on the electronic repulsive force. and the length and proportion of nonionic graft groups of PEO in PC chemical stracture affected the steric effect. The compatibility between PC superplasticizers and cement mainly depended on the type of PEO groups ; furtlwrmore, adding mineral powders is good to the compatibility for high performance concretes （ HPCs ）.

To increase the processability and plasticity of the selective laser melting(SLM)fabricated Al-Mn-Mg-Er-Zr alloys,a novel TiB_(2)-modified Al-Mn-Mg-Er-Zr alloy with a mixture of Al-Mn-Mg-Er-Zr and nano-TiB_(2) powders was fabricated by *** pro-cessability,microstructure,and mechanical properties of the alloy were systematically investigated by density measurement,microstruc-ture characterization,and mechanical properties *** alloys fabricated at 250 W displayed higher relative densities due to a uni-formly smooth top surface and appropriate laser energy *** maximum relative density value of the alloy reached(99.7±0.1.%,demonstrating good *** alloy exhibited a duplex grain microstructure consisting of columnar regions primarily and equiaxed regions with TiB_(2),Al6Mn,and Al3Er phases distributed along the grain *** directly aging treatment at a high tem-perature of 400℃,the strength of the SLM-fabricated TiB_(2)/Al-Mn-Mg-Er-Zr alloy increased due to the precipitation of the secondary Al6Mn *** maximum yield strength and ultimate tensile strength of the aging alloy were measured to be(374±1.and(51.±1.)MPa,*** SLM-fabricated TiB_(2)/Al-Mn-Mg-Er-Zr alloy demonstrates exceptional strength and thermal stability due to the synergistic effects of the inhibition of grain growth,the incorporation of TiB_(2) nanoparticles,and the precipitation of secondary Al6Mn nanoparticles.
