Comparative Performance of Microscopy and Nested PCR for the Detection of Cryptosporidium Species in Patients Living with HIV/AIDS in Abidjan (Côte d’Ivoire)

Cryptosporidium spp. infection is one of the causes of diarrhea in people living with HIV/AIDS. The objective of this study is to compare the sensitivity of microscopy and molecular biology to determine the prevalence of Cryptosporidium spp. in Patients Living with HIV (PLwH). This is a descriptive cross-sectional study conducted in three care centers for people living with HIV/AIDS in Abidjan. It took place from November 2018 to March 2020. Sociodemographic data were obtained via a questionnaire. Stool and blood samples were collected and analyzed for microscopy and Nested PCR detection of Cryptosporidium spp. Blood samples were analyzed for CD4+ count. A total of 363 stool samples were collected from the three sites. Individuals aged 40 - 50 years (36.52%) were most likely to participate in the study. HIV Type 1 accounted for 86.22% of the study population. The samples collected consisted of 47.65% diarrheal stool. Microscopic examination of the stool yielded a prevalence of 3.86% for Cryptosporidium spp. while the prevalence was 3.96% with molecular identification. No statistically significant difference was observed between these two prevalences (χ2 = 0.26;p = 0.609). CD4+ count was the factor associated with Cryptosporidium spp. infection for both microscopy (OR = 0.887, p = 0.001) and PCR (OR = 0.896, p = 0.001). This study demonstrated that Nested PCR improves the detection of Cryptosporidium spp. in patient diagnosis.

The Bronchiolitis in Paediatric Emergencies at the University Teaching Hospital of Gabriel Touré

Introduction: Bronchiolitis is a generally benign condition characterised by acute inflammation, oedema and necrosis of the epithelial cells lining the small airways, and increased mucus production. Symptoms usually begin with rhinitis and cough, which may progress to tachypnoea, wheezing, and rales. The most common etiology is a respiratory syncytial virus (RSV). Bronchiolitis occupies an important place among Acutes Respiratory Infections (ARI) and represented the fifth cause of hospitalisation in the paediatric emergency department of the Gabriel Touré University Hospital in 2008 with a frequency of 10% and a mortality rate of 3.2%. This shows that we are constantly confronted with the management of this pathology. The objective of our study was to study the epidemiological, clinical, and therapeutic aspects of bronchiolitis in the paediatric emergency room of the Gabriel Touré University Hospital. Materials and Method: This was a prospective cross-sectional and descriptive study from April 1, 2018, to March 31, 2020, i.e. 2 years in infants aged 1 to 23 months. Data were collected using an individual survey form by questioning parents and physically examining patients. Results: During the study period, we were able to collect 342 patients meeting our inclusion criteria out of 4207 hospitalized patients, or a frequency of 8.1%. The age range of 1 to 5 months represented 64.9%. The sex ratio was 1.2. The most common reason for consultation was respiratory discomfort (86.8%). Most patients (46%) were hospitalised during the period from September to November. In the majority of cases (74%), patients consulted within five days of the onset of symptoms. The physical examination was mainly dominated by signs of respiratory struggle, fever (51.8%), and sibilants (37%) on auscultation. Complications associated with bronchiolitis were mainly respiratory infections with 57.6% of cases. The main management steps were: nasopharyngeal decontamination (78.9%), oxygen therapy (

Introduction: Cervical cancer, caused by persistent high-risk human papillomavirus (HPV) infection, remains a global public health problem. The cellular transformation and maintenance of the malignant phenotype of these HPVs are attributed to the viral oncoproteins E6 and E7. Objective: This study aims to detect the presence of human papillomavirus DNA and E6/E7 oncoprotein mRNA of HPV genotypes 16, 18, 31 and 33 in cases of cervical cancer and precancerous lesions, histologically confirmed in Burkina Faso. Methods: This descriptive cross-sectional study focused on cases of cervical cancer and high-grade intraepithelial neoplasia (CIN) and was conducted from June to December 2022. One hundred (100) samples of fixed and paraffin-embedded tissues were collected from the pathological anatomy and cytology laboratories of hospitals in the capital of Burkina Faso. High-risk human papillomavirus (HR-HPV) DNA was detected using multiplex real-time PCR, while the presence of E6 and E7 mRNA in cervical cancer and high-grade CIN samples was determined using real-time Reverse Transcriptase-PCR (RT-PCR) with TaqMan probes. Results: The mean age of women diagnosed with cervical cancer and high-grade CIN was 50.81 ± 13.65 years, ranging from 22 to 82 years. Cervical cancer and high-grade CIN were positive for at least one high-risk human papillomavirus (HR-HPV) in 80% of cases. The most prevalent genotypes observed were HPV16, 18, 31, and 33, collectively accounting for 70.08% of cases. Of the 89 samples that tested positive for HR-HPV genotypes 16, 18, 31, and 33, 88 (98.88%;95% CI: [94.58 - 99.94]) were also positive for the presence of mRNA encoding the E6 and E7 oncoproteins of HPV16, 18, 31, and 33. Conclusion: In the presence of HPV DNA, testing for E6 and E7 oncoprotein mRNA could serve as a promising biomarker and valuable tool for improved assessment of the progression to cervical cancer.

Objective: Provide the place of ENT diseases at the Reference Health Center of the Commune V of Bamako. Materials and Method: A cross-sectional study using ENT medical assessment was carried out from January 2017 to December 2017 at the Reference Health Center of Commune V of Bamako’s District. Results: 1911 patients were included and that number represented 15.46% of all non-obstetrics medical consultations in the health center. The sex ratio was 0.83 for women (54.7%). The average age was 22 years old. Otologic diseases were 66.4% of the cases followed by nasal sinus cases (16.6%). Infectious and inflammatory diseases were 74.1% and were the main diagnosed diseases. The difficulties were mainly from technical platform. Conclusion: The diversity of these pathologies required a strong development or reinforcement of human and material resources capacities in of these structures for proximity care management.

Objective: The work aimed at describing an epidemioclinical, therapeutic and evolutionary characteristics of patients hospitalized for aortic dissection in the cardiology department in Point “G” Hospital University Center in Bamako—Mali. Methodology: It was a descriptive cross-sectional study from January 2010 to February 2017 in the CHU Point G cardiology department, including all patients hospitalized during this period. Results: Of 6912 hospitalized patients, 23 patients were concerned by aortic dissection. The prevalence of aortic dissection was 0.33%. The most affected age group was 50 - 69 (43.5%) of patients. The predominance was male with a sex ratio of 4.75. The cardiovascular risk factors were high blood pressure (73.9%) and smoking (60.9%). The major functional signs were chest pain (65.2%) and dyspnea (65.2%). Asphygmy (56.5%) and breath of aortic insufficiency (60.9%) were the dominant physical signs. The electrocardiogram recorded sinus tachycardia with 86.9% of patients. The radiographic of the frontal thorax showed mediastina widening (73.9%). At echocardiography, dilatation of the ascending aorta was described with 73.9% and the intimal veil (47.8%). Pericardial effusion was observed with 26.1% of patients. In the thoracic angioscan, the aortic dissection gave 43.5% for type A and 56.5% for type B. The aneurysm of the aorta was abdominal with 21.7%, ascending portion (13.0%) and descending with 8.7%. Complications were dominated by heart failure (47.8%) and aneurysm of the aorta (34.8%). The lethality was 52.2%. Conclusion: Aortic dissection is a medical and surgical emergency with poor prediction.

【研究背景】中国冬小麦（Triticum aestivum L.）生产高度依赖氮肥施用,大多数小农户无法根据具体情况准确施用氮肥,这给冬小麦生产带来了挑战。假设估测不同地点的小麦植株氮吸收量可以潜在地改善基于理论施氮量方法（TNR）的特定地点的氮素管理。基于现有论文数据,整合品种、产量、气候、地理参数的小麦氮吸收估算模型有望构建高度可靠的小麦植株氮吸收量预测模型。【材料与方法】为了解决这个问题,研究提出了一种经济有效且用户友好的方法来准确预测小麦植株的氮吸收,以改进氮管理决策。研究采用Meta分析和基于数据的建模方法,整合了在中国五个主要小麦生产省份进行的试验的数据集。然后使用根据已发表的论文数据集构建的九种不同的氮输入和产量水平情景来评估模型的性能。【结果与分析】结合线性混合效应模型和随机森林模型建立了小麦在特定目标产量下的氮肥推荐模型,其中随机森林模型表现出优越的性能。该模型将地理、品种和气候特征作为模型输入变量,预测植物的氮需求指数（NDI）。在适合模型的情景下,模型减少施氮量17.91～110.48 kg ha N,经济效益提高14.15～137.59$ha,减少NO排放15.23%～53.7%,提高NEEB 0.18%～1.78%。值得注意的是,中等水平的氮输入和低产量产出情景与小农生产情况非常相似,在此情景下模型也有较好地表现,说明模型可以应用于中国大部分小农户的实际生产中。分析影响NDI的输入变量发现,改善小麦籽粒蛋白质含量和株高特征,可以改善小麦植株全生育过程的氮素需求量。对模型在九个不同情景应用下的氮肥推荐量、经济效益、NO排放和净生态系统经济效益（NEEB）进行了评估,发现模型在大部分情景下都有良好地表现,在为小农户带来经济效益的同时也有较好的环境效益,可以大幅度减少NO排放并提高NEEB。根据模型输入变量的相关性分析和随机森林重要性分析,确定了影响NDI的因素,并为小麦育种改进提供了见解。【结论】模型适用性评估表明,在典型的小农生产情景下,该模型在减少污染的同时提供了经济效益。通过实施这种方法,小农可以实现优化的氮肥管理,改善其经济效益并为环境可持续性做出贡献。本文的模型可以作为政策制定者的策略,并在农业集约化程度较低的地区推广。

以石头扁桃和桃扁桃为材料,采用盆栽方法,研究了盐胁迫对2种扁桃幼苗根部K+、Na+和Cl-含量的影响和超微结构的变化,以揭示其耐盐性差异机制。结果表明:NaCl胁迫下,2种扁桃根部Na+、Cl-含量均随NaCl浓度的升高和胁迫时间延长而增加,桃扁桃根部Na+、Cl-相对增长量大于石头扁桃;K+含量均随盐浓度的增加而降低,桃扁桃根中K+含量大于石头扁桃。2种扁桃根中Na+/K+随盐浓度的增加而升高,在高盐浓度胁迫下,桃扁桃根中Na+/K+值较石头扁桃低。盐胁迫后,2种扁桃根细胞核膜不清晰,细胞中均出现很多大的液泡,根细胞导管中出现大量淀粉粒。扁桃根细胞中Na+、Cl-含量明显增加,根部细胞核膜发生解离,导管扭曲严重,说明离子毒害起到破环作用,但是没有发生明显的质壁分离,不能明确表明渗透胁迫的同时发生。桃扁桃根部根组织维持较低的Na+/K+值,其受危害的程度小于石头扁桃,表现出较强的抗盐能力。

Unresectable stage Ⅲ non-small-cell lung cancer: Have we made any progress?

Lung cancer is responsible for the most cancer deaths worldwide with an incidence that is still rising. One third of patients have unresectable stage ⅢA or stage ⅢB disease. The standard of care for locally advanceddisease in patients with good performance status consists of combined modality therapy in particular concurrent chemoradiotherapy. But despite a lot of efforts done in the past, local control and survival of patients with unresectable stage Ⅲ non-small-cell lung cancer(NSCLC) remains poor. Improving outcomes for patients with unresectable stage Ⅲ NSCLC has therefore been an area of ongoing research. Research has focused on improving systemic therapy, improving radiation therapy or adding a maintenance therapy to consolidate the initial therapy. Also implementation of newer targeted therapies and immunotherapy has been investigated as well as the option of prophylactic cranial irradiation. This article reviews the latest literature on improving local control and preventing distant metastases. It seems that we have reached a plateau with conventional chemotherapy. Radiotherapy dose escalation did not improve outcome although increasing radiation dose-intensity with new radiotherapy techniques and the use of newer agents, e.g., immunotherapy might be promising. In the future well-designed clinical trials are necessary to prove those promising results.
