Management of Acute Myeloblastic Leukaemia (AML) Treated with Intensive Chemotherapy: Experience in a Single Centre

Introduction: Acute myeloblastic leukaemia (AML) is a haematological malignancy with a poor prognosis, despite significant therapeutic progress. This study presents the results of AML management in Mali according to GFAOP recommendations. Methodology: This was a retrospective, cross-sectional study. It included patients aged 0 - 15 years treated in the paediatric oncology unit for AML and followed up between January 2016 and December 2020. Results: During the study period, 85 cases of acute leukaemia were diagnosed in the paediatric oncology unit (including 51 cases of ALL), of which 34 cases of AML were included in this study. The majority were boys (59%). The mean age was 8 years, with extremes of 18 months and 15 years. The mean time to diagnosis was 68 days. In 79% of cases, patients were referred by 1st or 2nd level hospitals. Anaemia was observed in 91% of cases, an infectious syndrome in 68%, haemorrhage in 56% and a tumour syndrome in 85%. The haemogram showed hyperleukocytosis in 15% of cases, thrombocytosis in 22% and severe anaemia in 73%. Death occurred in 85% of cases, most often in the context of sepsis or haemorrhage. Conclusion: AML is probably underestimated in Mali and diagnosis delayed, which may be explained by patient-related factors (lack of knowledge, financial constraints) and a cumbersome referral system. These results suggest the need to implement an appropriate diagnostic and therapeutic strategy, with strong involvement of the political authorities.

The emergence of single-cell RNA-sequencing(scRNA-seq)technology has introduced new information about the structure of cells,diseases,and their associated biological *** of the main uses of scRNA-seq is identifying cell populations,which sometimes leads to the detection of rare cell ***,the new method is still in its infancy and with its advantages comes computational challenges that are just beginning to *** important tool in the analysis is dimensionality reduction,which transforms high dimensional data into a meaningful reduced *** technique allows noise removal,visualization and compression of high-dimensional *** paper presents a new dimensionality reduction approach where,during an unsupervised multistage process,a feature set including high valuable markers is created which can facilitate the isolation of cell *** proposed method,called fusion of the Spearman and Pearson affinity matrices(FSPAM),is based on a graph-based Gaussian *** of the graph theory can be effective to overcome the challenge of the nonlinear relations between cellular markers in scRNA-seq ***,with a proper fusion of the Pearson and Spearman correlation coefficient criteria,it extracts a set of the most important features in a new *** fact,the FSPAM aggregates the various aspects of cell-to-cell similarity derived from the Pearson and Spearman metrics,and reveals new aspects of cell-to-cell similarity,which can be used to extract new *** results of the identification of cell populations via k-means++clustering method based on the features extracted from the FSPAM and different datasets of scRNA-seq suggested that the proposed method,regardless of the characteristics that govern each dataset,enjoys greater accuracy and better quality compared to previous methods.

Resilient Control for Networked Control Systems Subject to Cyber/Physical Attacks

In this paper, we investigate a resilient control strategy for networked control systems（NCSs） subject to zero dynamic attacks which are stealthy false-data injection attacks that are designed so that they cannot be detected based on control input and measurement data. Cyber resilience represents the ability of systems or network architectures to continue providing their intended behavior during attack and recovery. when a cyber attack on the control signal of a networked control system is computed to remain undetectable from passive model-based fault detection and isolation schemes, we show that the consequence of a zero dynamic attack on the state variable of the plant is undetectable during attack but it becomes apparent after the end of the attack. A resilient linear quadratic Gaussian controller, having the ability to quickly recover the nominal behavior of the closed-loop system after the attack end, is designed by updating online the Kalman filter from information given by an active version of the generalized likelihood ratio detector.

Digestive Surgical Emergencies at the “Mother Child”Hospital Center Luxembourg in Mali

Digestive surgical emergencies concern all patients admitted urgently, for whom a decision for surgical intervention may be necessary within 24 hours. They are on guard duty day and night. To carry out this work, we set ourselves the objectives of: 1) Studying digestive surgical emergencies in the general surgery department of the “Mother Child” Le Luxembourg hospital in Bamako, Mali;2) Determine the frequency of digestive surgical emergencies, 3) Describe the clinical and therapeutic aspects, and 4) Analyze the results of treatment. From November 1, 2022 to October 31, 2023, the general surgery department of the “Mother Child” Luxembourg Hospital Center in Bamako, Mali, carried out 139 digestive surgical emergencies whose files were usable;75 men and 64 women, a sex ratio of 1.2. The 20 - 40 years old age group was the most represented, at 40.29%. The average age was 39 years;the extremes 16 years and 93 years with a standard deviation of 21.65 years. The reference concerned 51.08% of our patients. Abdominal pain was the main reason for consultation (100% of cases). In the majority of cases, the physical examination made it possible to make the diagnosis. Faced with certain clinical cases, we requested ultrasound (109/139), ASP (46/139) and CT (15/139). The main etiology was acute appendicitis with 42.45% of cases. The frequency of digestive surgical emergencies was 10.71% of all activities in the general surgery department of the “Mère Enfant” Le Luxembourg hospital center in Bamako. All our patients were seen in consultation by an anesthesiologist before entering the operating room. The surgical consequences were complicated in 11.51% of cases with 7.91% deaths. Surgical site infections accounted for 12.5% of postoperative complications. Eleven deaths were noted, representing 68.75% of complications and 7.91% of our sample. Acute peritonitis was the cause of death in 100% of cases. The average cost of care was 329,000 FCFA.

Self-centring segmental retaining walls—A new construction system for retaining walls

Introduction: Uterine fibroid is a mixed mesenchymal tumor, developing from smooth muscle cells of the myometrium, separated by connective tissue. The majority of fibroids are asymptomatic and do not require any intervention or other exploratory measures. However, in some cases fibroids are symptomatic, their treatment should aim to improve symptoms and quality of life. Objectives: Describe the epidemiological-clinical aspects of uterine fibroids in the gynecology and obstetrics department of the reference health center of commune VI of the Bamako district. Methodology: This was a retrospective descriptive study carried out over a period of one year. It concerns all patients seen in consultation in the gynecology and obstetrics department of the reference health center of commune VI. Results: during the study period, the uterine fibroid frequency was 1.80%. During this same period, fibroids represented 5.59% of gyneco-obstetric pathologies operated on in the department. The 30 - 45 year old age group was the most represented with a frequency of 75.63%. The average age was 36.87 years ± 6.2 years with extremes of 25 and 63 years. Married women were the most represented 97.48%. The vast majority of our patients 95.8% were not postmenopausal. The multigravidas were the most represented, i.e. 37.50% with a large part of the pauciparous 41.29%. In our patients, 96.64% had a clinical symptom on their fibroid with the main reason for consultation being the sensation of a pelvic mass in 97.48%. we recorded three cases of infertility as associated factors. Ultrasound was performed in all patients. The location of the myxomatous nuclei was subserosal in the majority in 42.86% and with multiple nodules in 84.85% of cases. The main indication for myomectomy was the failure of medical treatment in 86.49% of cases. Surgical treatment was mainly a myomectomy 93.30%, a hysterectomy was indicated in 6.70%. Delayed Grafting as a Valid Technique in Burn Management: Experience from a Burn Unit in Oman

Background: Early excision and grafting has been the preferred method of managing major burns around the world since 1970. Considering the advances in health care and the development of new antibiotics over the past 50 years, delayed grafting as a technique for the management of burns over 15%-20% of total body surface area (TBSA) could have comparable results to that of early excision. This study aims to highlight the outcomes of practicing delayed grafting in burn patients. Methods: A case series analysis was performed of 51 patients who were admitted to the burns unit in Sultan Qaboos Hospital Salalah with over 20% TBSA between January 2014 and December 2019. The patients received prophylactic antibiotics and silver sulphadiazine dressing until the burn eschar had completely separated, followed by grafting. Results: Two patients were lost during the entire duration of the study. The mortality rate was comparable to that of early excision, while the rate of hypertrophic scarring was lower than the range reported by other studies. Conclusion: In the management of patients with over 20% TBSA, delayed grafting after complete separation of eschar is still a valid technique.

The present study goes into the search for the safety domain of civil engineering structures. The objective is to show how a reliability-evaluation brought by a mechanical sizing can be obtained. For that purpose, it is necessary to have a mechanical model and a reliability model representing correctly the behavior of this type of structure. It is a question on one hand, to propose a formulation for the nonlinear calculation (mechanical nonlinearity) of the spatial structures in trusses, and on the other hand, to propose or to adapt a formulation and a modeling of the reliability. The principle of Hasofer-Lind can be applied, in first approach, for the reliability index estimation, scenarios and the probability of failure. The made check concerned metallic in truss structures. Finally, some structures are calculated using the method adapted by Hasofer-Lind to validate the probability approach of the reliability analysis.

Behaviour of self-centring shear walls——A state of the art review

The application of unbonded post-tensioning (PT) in structural walls has led to the development of advanced self-centring (rocking) shear wall systems that has significant advantages, including accelerated construction due to the incorporation of prefabricated elements and segmental construction for different materials (e.g., concrete, masonry, and timber), reduced residual drifts, and little damage upon extreme seismic and wind loads. Concrete, masonry, and timber are often used for the construction of unbonded PT structural wall systems. Despite extensive research since the 1980s, there are no well-established design guidelines available on the shear wall configuration with the required energy dissipation system, joint’s locations and acceptance criteria for shear sliding, confinement, seismic performance factors, PT loss, PT force range and residual drifts of shear walls subjected to lateral loads. In this research a comprehensive state-of-the-art literature review was performed on self-centring shear wall system. An extensive study was carried out to collect a database of 100 concrete, masonry, and self-centring shear wall tests from the literature. The established database was then used to review shear walls’ configurations, material, and components to benchmark requirements applicable for design purposes. The behaviour of concrete, masonry and timber shear walls were compared and critically analysed. The general behaviour, force-displacement performance of the walls, ductility, and seismic response factors, were critically reviewed and analysed for different self-centring wall systems to understand the effect of different parameters including configurations of the walls, material used for construction of the wall (concrete, masonry, timber) and axial stress ratio. The outcome of this research can be used to better understand the behaviour of self-centring wall system in order to develop design guidelines for such walls.
