Understanding pediatric surgical needs in North Korea: a modeling analysis

background Sixty-five percent of children worldwide lack access to surgical care,the majority of whom live in low-income and middle-income countries(LMiCs).Developing surgical infrastructure requires information on surgical need;however,this information is often limited in *** Korea(Democratic People's Republic of Korea,DPRK)has a low amount of publicly available ***,we analyzed available modeled data to understand the causes of pediatric deaths due to conditions treatable with surgery in *** we used world bank data and models from the Institute for Health Metrics and Evaluation to identify causes of pediatric deaths affecting pediatric patients(agewe compared mortality of disease between DPRK and countries with similar economic *** between 1990 and 2019,the number of overall pediatric deaths has *** 2019,32.2%of allpediatric deaths in DPRK were caused by surgical *** leading categories of surgical conditions were injuries(53.9%),congenital conditions(34.2%),tumors(8.8%),and abdominal conditions(3.2%).DPRK has a lower relative rate of pediatric deaths compared with other LMICs with similar gross domestic product per ***,it has a higher relative rate of pediatric deaths due to conditions requiring treatment with *** injuries contribute significantly to the high rate of pediatric deaths in *** Although DPRK may be allocating overall resources toward pediatric healthcare more efficiently than economic peers,DPRK may benefit from improvement in pediatric surgical *** availability of data and close international collaboration could be potential solutions to bridge this gap.

Extracorporeal life support(ECLS)serves as a rescue therapy for patients with congenital diaphragmatic hernia(CDH)and severe cardiopulmonary failure,and only half of these patients survive to *** costly intervention has a significant complication risk and is reserved for patients with the most severe disease physiology refractory to maximal cardiopulmonary *** contraindications to ECLS do exist such as coagulopathy,lethal chromosomal or congenital anomaly,very preterm birth,or very low birth weight,but many of these limits are being evaluated through further *** guidelines from the past decade vary in recommendations for ECLS use in patients with CDH but this therapy appears to have a survival benefit in the most severe subset of *** outcomes have been observed for patients treated at high-volume *** review details the evolving literature surrounding management paradigms for timing of CDH repair for patients receiving preoperative *** recent data support early repair following cannulation to avoid non-repair which is uniformly fatal in this *** ECLS runs are associated with decreased survival,and patient physiology should guide ECLS weaning and eventual decannulation rather than limiting patients to arbitrary run *** of care across centers is a major focus to limit unnecessary costs and improve short-term and long-term outcomes for these complex patients.

we study how to use the surface states in a bi2Se3 topological insulator ultra-thin film that are affected by finite size effects for the purpose of quantum *** demonstrate that:(i)surface states under the finite size effect can effectively form a two-level system where their energy levels lie in between the bulk energy gap and a logic qubit can be constructed,(ii)the qubit can be initialized and manipulated using electric pulses of simple forms,(iii)two-qubit entanglement is achieved through a√SwAP operation when the two qubits are in a parallel setup,and(iv)alternatively,a Floquet state can be exploited to construct a qubit and two Floquet qubits can be entangled through a Controlled-NOT *** Floquet qubit offers robustness to background noise since there is always an oscillating electric field applied,and the single qubit operations are controlled by amplitude modulation of the oscillating field,which is convenient experimentally.

This study investigates the efficacy of sodium alginate(SA),xanthan gum(XG),guar gum(GG)and chitosan(CS)d each applied at five different solid biopolymer-to-water mass ratios(or dosages)and cured for 7 d and 28 d d on the unconfined compressive strength(UCS)performance of a high plasticity clayey ***,on identifying the optimum biopolymer-treatment scenarios,their performance was compared against conventional stabilization using hydrated *** a given curing time,the UCS for all biopolymers followed a riseefall trend with increasing biopolymer dosage,peaking at an optimum dosage and then subsequently decreasing,such that all biopolymer-stabilized samples mobilized higher UCS values compared to the unamended *** optimum dosage was found to be 1.5%for SA,XG and CS,while a notably lower dosage of 0.5%was deemed optimum for ***,for a given biopolymer type and dosage,increasing the curing time from 7 d to 28 d further enhanced the UCS,with the achieved improvements being generally more pronounced for XG-and CS-treated *** of the investigated biopolymers was able to produce UCS improvements equivalent to those obtained by the 28-d soilelime samples;however,the optimum XG,GG and CS dosages,particularly after 28 d of curing,were easily able to replicate 7-d lime stabilization outcomes achieved with as high as twice the soil’s lime ***,the fundamental principles of clay chemistry,in conjunction with the soil mechanics framework,were employed to identify and discuss the clayebiopolymer stabilization mechanisms.

In this paper, the problem of stability analysis for neural networks with time-varying delays is considered. by constructing a new augmented Lyapunov-Krasovskii＇s functional and some novel analysis techniques, improved delaydependent criteria for checking the stability of the neural networks are established. The proposed criteria are presented in terms of linear matrix inequalities （LMIs） which can be easily solved and checked by various convex optimization algorithms. Two numerical examples are included to show the superiority of our results.

Detrital zircon geochronology and Hf isotope analysis can be used for inferring provenance characteristics,and to evaluate the tectonic evolution of sedimentary basins and their link with regional *** Paleozoic sequences of the Okcheon belt consist of the Lower Paleozoic Joseon and the Upper Paleozoic Pyeongan supergroups with Middle Paleozoic hiatus locally on top of the Neoproterozoic bimodal volcanic rocks,reflecting an intracontinental rift setting between the two basements(*** and Yeongnam massifs)at southern part of the Korean *** detrital zircon U-Pb ages and Lu-Hf isotope results show that all these Paleozoic strata commonly have Paleoproterozoic and Paleozoic zircon ages with rare Meso-to Neoproterozoic *** individual zircon populations display following features,allowing estimation of their sedimentary provenances:(i)The Paleoproterozoic zircons(ca.1.85 Ga and 2.50 Ga)with similar ranges ofεHf(t)values are most common in the basement rocks of the Korean Peninsula,and were sourced from both the Gyeonggi and Yeongnam massifs.(ii)The Meso-to Neoproterozoic zircons,preserved only in the Middle to Late Cambrian clastic sedimentary rocks within the carbonate sequences probably reflect proximal provenance.(iii)The youngest Paleozoic zircons of each formation,almost coincident with their deposition ages,suggest presence of syndepositional magmatism,indicating proximal magmatic sources during their deposition.(iv)The Cambrian-Ordovician zircons,from the Lower Paleozoic sequences,but rare in the successive Upper Paleozoic sequences,suggest a provenance change after the hiatus between the two sedimentary successions.(v)The Permian zircons showing differentεHf(t)values indicate that detrital sources were varied at that *** integrated results in our study suggest provenance variability linked to diverse tectonic environments,reflecting prolonged subduction-related crustal evolution of the proto-Korean Peninsula during the Paleozo

Coating with viscous formulations has been essential in numerous industries as it can be a means for providing functionalization, additional properties, as well as other benefits. However, there have been scarce studies that have investigated and proposed methodologies in literature. Continuous coating of powders with viscous liquids poses as a promising technology, which has been mentioned in some studies, but has not yet been thoroughly investigated. This paper employs the use of image processing and analysis, in combination with statistical analysis of particles to evaluate the effectiveness of foams and liquids as a means of coating powder beds. Two different sizes of twin screw mixers that are working in continuous operation are employed, and a new continuous foaming device is fabricated and used for the experiments of coating. The effect of materials and process parameters (as for example rotational speed, and flowrate) on the quality of coating are investigated. Image analysis is used to assess the coating quality. The results clearly showcase the potential of using twin screw mixers for coating purposes and not only for mixing. The hypothesis that using large bubble foams to improve the coating of viscous liquids on particles is proven correct, as they provide higher quality coatings compared to their equivalent liquids, when used in the twin screw mixer. Surprisingly, using a larger scale twin screw mixer, does not show a substantial effect on the mixing, regarding quality, however there is still a requirement for mix optimization for achieving scale-up of this process. These results provide a new pathway for coating powders with viscous formulations in industrial applications, requiring less energy and effort in this process, and can pave the way towards introducing more sustainable industrial methodologies for coating.
