The financial viability of a solar STES （seasonal thermal energy store） installed in a mixed commercial and residential multiunit development of low-energy buildings located in Lysekil, Sweden, a maritime Scandinavian Climate has been investigated. Using recorded figures for the installation costs and performance, a financial life cycle analysis has been undertaken to determine the cost effectiveness of the system. The time value of money is considered and an LCC （life cycle cost） analysis undertaken to identify the cost-effectiveness of the solution. It shows that while a direct heating and hot water system incorporating STES can be economically viable in a Swedish maritime climate in the long term, assistance such as that provided by government incentives is required to assist with the high capital cost of the initial investment.

目的通过对教科书和教育理念发展的研究,针对中国的全科医学教科书的质量和发展方向提出建议。方法文献研究和政策分析。结果教科书是给特定学科的学生开发的媒介、工具和资源。国外的全科医学教科书发展比较早,优秀的教科书能够反映全科医学的理念和操作技能,是全科医生重要的学习资源。结论中国的全科医学教科书的发展与学科本身的发展步伐相一致,需要进一步完善教科书,综合考虑教科书的质量,为全科医学培训提供重要的学习资源。

目的本文从全科医学教育的阶层结构的角度,重点分析全科医学导师/精英的特征和能力,并详细讨论全科医学师资的遴选、培养和支持标准,提出优先发展全科医学精英和导师,从而带动全科医学师资队伍发展,保证全科医学培训质量的建议。方法文献研究和政策分析。结果现代全科医学站在巨人的肩膀上,大师们给予我们的不仅仅是学术上的睿智,更重要的是他们的人品和精神。全科医学导师是传承和发扬全科医学的中坚力量,除了他们自身的条件和努力外,也需要环境的支持。全科医学师资队伍是由合格的医生和优秀的全科医生组成的。结论应该根据中国实际,制定符合全科医学教育规律的师资入门条件和培训计划,以保证培训的质量。

目的通过对澳大利亚全科医学课程计划的研究和分析,归纳课程计划特点和制定策略,为中国进一步提高全科医学课程计划质量提供科学依据。方法文献综述和政策文件分析。结果澳大利亚的全科医学课程计划是全科医生学会制定的,体现了该学科的发展过程和现状。该课程计划贯穿医学本科、职业教育前、职业教育、持续职业发展等四个阶段,并确定沟通与医患关系、实用全科医学知识和技能、人群健康与全科医学背景、职业角色和伦理角色、组织与法律问题等五个维度。课程计划强调从实践中学习,秉承学徒式全科医学教育理念,强调全科医生从常见患者表现或症状中提出初步诊断和初步管理措施,并与国家卫生工作重点和疾病防治重点相结合。结论澳大利亚的全科医学课程计划地制定过程,以及原则和框架值得中国借鉴。

Objective:This review paper explores the concept,spirit,principles,and core skills of moti-vational interviewing(MI)as a psychological approach applied to CHD ***:Literature reviews based on English and Chinese ***:The English and Chinese literature provided strong evidence for the effectiveness of MI in terms of behavior changes,blood pressure control,hyperglycemia and hyperlipidemia control,compliance improvement,and improving quality of life amongst chronic heart failure *** Happy Life Club(HLC)is the only randomized controlled trial(RCT)that examined the effects of MI on behavior changes and self-management improvements in Chinese patients with type 2 diabetes *** HLC pilot revealed positive outcomes,and RCTs will provide further ***:MI has been expanded in its application to a wider range of lifestyle-related chronic diseases,including *** the time of this review,MI represents a new concept and ap-proach in China for improving physical and mental health(body and mind)in patients with *** order to integrate MI into medical practice,the authors suggest improving the medical education curriculum and strengthening vocational training,while conducting further research initiatives to assess the innovation and inform practice management.

Human induced pluripotent stem cell(hiPSC)-derived kidney organoids have prospective applications ranging from basic disease modelling to personalised ***,there remains a necessity to refine the bio-physical and biochemical parameters that govern kidney organoid *** within fully-controllable and physiologically relevant 3D growth environments will be critical to improving organoid reproducibility and ***,we matured hiPSC-derived kidney organoids within fully synthetic self-assembling peptide hydrogels(SAPHs)of variable stiffness(storage modulus,G′).The resulting organoids con-tained complex structures comparable to those differentiated within the animal-derived matrix,***-cell RNA sequencing(scRNA-seq)was then used to compare organoids matured within SAPHs to those grown within Matrigel or at the air-liquid interface.A total of 13,179 cells were analysed,revealing 14 distinct *** compositional analysis revealed a larger proportion of nephron cell types within Transwell-derived organoids,while SAPH-derived organoids were enriched for stromal-associated cell ***,dif-ferentiation within a higher G’SAPH generated podocytes with more mature gene expression ***-tionally,maturation within a 3D microenvironment significantly reduced the derivation of off-target cell types,which are a known limitation of current kidney organoid *** work demonstrates the utility of syn-thetic peptide-based hydrogels with a defined stiffness,as a minimally complex microenvironment for the selected differentiation of kidney organoids.

BACKGROUND Bowel perforation from biliary stent migration is a serious potential complication of biliary stents,but fortunately has an incidence of less than 1%.CASE SUMMARY we report a case of a 54-year-old Caucasian woman with a history of Human Immunodeficiency virus with acquired immunodeficiency syndrome,chronic obstructive pulmonary disease,alcoholic liver cirrhosis,portal vein thrombosis and extensive past surgical history who presented with acute abdominal pain and local *** further evaluation she was diagnosed with small bowel perforation secondary to migrated biliary stents and underwent exploratory laparotomy with therapeutic *** This case presentation reports on the unusual finding of two migrated biliary stents,with one causing *** addition,we review the relevant literature on migrated stents.
