The Solar wind Magnetosphere Ionosphere Link Explorer(SMILE)is a joint mission of the European Space Agency(ESA)and the Chinese Academy of Sciences(CAS).Primary goals are investigating the dynamic response of the Earth's magnetosphere to the solar wind(Sw)impact via simultaneous in situ magnetosheath plasma and magnetic field measurements,X-Ray images of the magnetosheath and magnetic cusps,and UV images of global auroral *** deformations associated with magnetosheath high speed jets(HSJs)under a quasi-parallel interplanetary magnetic field condition are studied using a threedimensional(3-d)global hybrid *** X-ray intensity calculated based on both physical quantities of solar wind proton and oxygen ions is *** obtain key findings concerning deformations at the magnetopause:(1)Magnetopause deformations are highly coherent with the magnetosheath HSJs generated at the quasi-parallel region of the bow shock,(2)X-ray intensities estimated using solar wind h+and self-consistentO7+ions are consistent with each other,(3)Visual spacecraft are employed to check the discrimination ability for capturing magnetopause deformations on Lunar and polar orbits,*** SMILE spacecraft on the polar orbit could be expected to provide opportunities for capturing the global geometry of the magnetopause in the equatorial plane.A striking point is that SMILE has the potential to capture small-scale magnetopause deformations and magnetosheath transients,such as HSJs,at medium altitudes on its *** results also demonstrate that a lunar based imager(e.g.,Lunar Environment heliospheric X-ray Imager,LEXI)is expected to observe a localized brightening of the magnetosheath during HSJ events in the meridian *** preliminary results might contribute to the pre-studies for the SMILE and LEXI missions by providing qualitative and quantitative soft X-ray estimates of dayside kinetic processes.

Bilhemia after trans-jugular intra-hepatic porto-systemic shunt and its management with biliary decompression

Bilhemia or bile mixing with blood is a rare clinical problem. The clinical presentation is usually transient self-resolving hyperbilirubinemia, progressive and rapidly rising conjugated hyperbilirubinemia, or recurrent cholangitis. Endoscopic retrograde cholangiopancreatography (ERCP) plays an important role in diagnosis and management. Biliary decompression with endoscopic sphincterotomy is useful in treating these patients. If not recognized and treated in time, the condition can be fatal in a significant proportion of patients. This usually occurs after blunt or penetrating hepatic trauma due to a fistulous connection between the biliary radicle and portal or hepatic venous radical. Cases have been described due to iatrogenic trauma such as liver biopsy and percutaneous biliary drainage. However, the occurrence after trans-jugular intra-hepatic porto-systemic shunt (TIPS) is very rare. we report a case of bilhemia presenting as rapidly rising bilirubin after TIPS. The patient was managed successfully with ERCP and removal of a blood clot from the common bile duct.

Risk factors for long COVId in children and adolescents:a systematic review and meta-analysis

Background The long-term sequelae of COVId-19 in children and adolescents remain poorly understood and *** systematic review and meta-analysis sought to summarize the risk factors for long COVId in the pediatric *** we searched six databases from January 2020 to May 2023 for observational studies reporting on risk factors for long COVId or persistent symptoms those were present 12 or more weeks post-infection using multivariable regression *** registries,reference lists of included studies,and preprint servers were hand-searched for relevant ***-effects meta-analyses were conducted to pool odds ratios for each risk *** study risk of bias was rated using QUIPS,and the GRAdE framework was used to assess the certainty of evidence for each unique *** Sixteen observational studies(N=46,262)were included,and 19 risk factors were amenable to *** moderate certainty in the evidence,age(per 2-year increase),allergic rhinitis,obesity,previous respiratory diseases,hospitalization,severe acute COVId-19,and symptomatic acute COVId-19 are probably associated with an increased risk of long *** sex,asthma,comorbidity,and heart diseases may be associated with an increased risk of long COVId,and Asian and Black races may be associated with a decreased risk of long *** did not observe any credible subgroup effects for any risk *** The current body of literature presents several compelling risk factors for the development of long COVId in the pediatric *** research is necessary to elucidate the pathophysiology of long COVId.

despite a significant increase in utilization over the past decade,the number of donation after circulatory death(dCd)organs that are procured and transplanted in the United States(US)remains well below its *** is still room for expansion,as utilizing dCd organs to the fullest extent is currently the most viable solution to the persistent mismatch between supply and demand in *** convened a multidisciplinary transplantation summit to examine various aspects of dCd,with faculty members from around the world with clinical and academic interest in dCd donation and transplantation,including abdominal and cardiothoracic surgeons,organ procurement organization directors,hepatologists,and *** conference focused on identifying barriers to dCd organ utilization and strategies to overcome these *** divide the barriers to dCd utilization into three mains categories:(Ⅰ)policy and process variation;(II)logistical and transportation challenges;and(Ⅲ)higher risk perceptions related to dCd *** each barrier,we proposed a variety of solutions,providing an overview of the status of dCd donation in the US and suggestions on how to increase the use of *** is a specific focus on ex situ machine perfusion,normothermic regional perfusion,and other opportunities to expand dCd utilization without negatively impacting recipient outcomes.

Long-term temporal-spatial dynamics of marine coastal water quality in the Tolo Harbor,Hong kong,China

The long-term temporal and spatial dynamics of marine coastal water quality in Tolo Harbor, Hong kong were explored. The Harbor is divided into three zones represented as Harbor, Buffer, and Channel Subzones. The time range for the study covers the period from the 1970s to the 1990s. The selected indicators for the comprehensive assessment of water quality consist of physical, chemical and biological aspects, including suspended solids(SS), Secchi disk depth(Sd), 5-day biochemical oxygen demand(BOd\-5), total nitrogen(TN), total phosphorus(TP), faecal coliform, chlorophyll-a(Chl-a), and the number of red tide occurrences. The results indicated the presence of obvious temporal and spatial trends with regard to changes in water quality. Spatially, water quality in the Channel Subzone is the best, while that in the Harbor Subzone is the worst. On a temporal basis, the average trend from bad to good was 1980s>1990s>1970s as indicated by most of the selected water quality indicators. water quality during the late 1980s reached its worst level with the lowest Sd, the highest BOd\-5, TN, TP, Chl-a concentrations, and the number of red tide occurrences. These long-term temporal-spatial water quality trends were also found in other studies of the Tolo Harbor. The large quantity of pollutants produced as a result of increasing population, industrial and commercial actives, and urbanization and industrialization trends in both Shatin and Tai Po seem to be primarily responsible for the changes in marine coastal water quality.
