Thanks to their biocompatibility,biodegradability,injectability and self-setting properties,calcium phosphate cements(CPCs)have been the most economical and effective biomaterials of choice for use as bone void *** have also been extensively used as drug delivery carriers owing to their ability to provide for a steady release of various organic molecules aiding the regeneration of defective bone,including primarily antibiotics and growth *** review provides a systematic compilation of studies that reported on the controlled release of drugs from CPCs in the last 25 *** chemical,compositional and microstructural characteristics of these systems through which the control of the release rates and mechanisms could be achieved have been *** doing so,the effects of(i)the chemistry of the matrix,(ii)porosity,(iii)additives,(iv)drug types,(v)drug concentrations,(vi)drug loading methods and(vii)release media have been distinguished and discussed *** specificities of in vivo release of drugs from CPCs have been reviewed,*** the kinetic and mechanistic correlations between the CPC properties and the drug release is a prerequisite for the design of bone void fillers with drug release profiles precisely tailored to the application area and the clinical *** goal of this review has been to shed light on these fundamental correlations.

Human dental enamel forms over a period of 2 4 years by substituting the enamel matrix, a protein gel mostly composed of a single protein, amelogenin with fibrous apatite nanocrystals. Self-assembly of a dense amelogenin matrix is presumed to direct the growth of apatite fibers and their organization into bundles that eventually comprise the mature enamel, the hardest tissue in the mammalian body. This work aims to establish the physicochemical and biochemical conditions for the synthesis of fibrous apatite crystals under the control of a recombinant full-length human amelogenin rnatrix in combination with a pro- grammable titration system. The growth of apatite substrates was initiated from supersaturated calcium phosphate solutions in the presence of dispersed amelogenin assemblies. It was shown earlier and confirmed in this study that binding of amelogenin onto apatite surfaces presents the first step that leads to substrate-specific crystal growth. In this work, we report enhanced nucleation and growth under conditions at which amelogenin and apatite carry opposite charges and adsorption of the protein onto the apatite seeds is even more favored. Experiments at pH below the isoelectric point of amelogenin showed increased protein binding to apatite and at low Ca/P molar ratios resulted in a change in crystal morphology from plate-like to fibrous and rod-shaped. Concentrations of calcium and phosphate ions in the supernatant did not show drastic decreases throughout the titration period, indicating controlled precipitation from the protein suspension metastable with respect to calcium phosphate. It is argued that amelobtasts in the developing enamel may vary the density of the protein matrix at the nano scale by varying local pH, and thus control the interaction between the mineral and protein phases. Critical infrastructures (CI) are difficult to handle due to their complexity, size and the number of stakeholders involved. During emergency situations (e.g. fire or terrorist attack), a CI operator in the control room is faced with a flood of information coming from different sensors and legacy monitoring systems. Since in these situations, time is critical and the operators are under a great deal of pressure, a holistic management of all the technical systems and actors involved is needed, reinforced by the Recommendation and Decision Support System (RDSS) that helps emergency managers to take correct and timely decisions. One way to provide an adequate RDSS support to the operator is proposed in this paper which is based on an intelligent, event driven layer that sits on top of the legacy CI monitoring system. Powered by the complex event processing capabilities and facility data model implemented in the form of CI ontology, this layer processes events originating from different sources, conducts the situation and risk assessment, and reacts accordingly, either automatically or via recommendations proposed to emergency personnel. To validate the proposed approach, an event-driven RDSS was deployed on an airport use case (Nikola Tesla airport in Belgrade), as one of the most complex CIs.

Plasma and platelet serotonin levels in patients with liver cirrhosis

AIM: To analyze the relationship between plasma and platelet serotonin levels and the degree of liver insufficiency. METHODS: The prospective study included 30 patients with liver cirrhosis and 30 healthy controls. The degree of liver failure was assessed according to the Child-Pugh classification. Platelet and platelet poor plasma serotonin levels were determined. RESULTS: The mean plasma serotonin level was higher in liver cirrhosis patients than in healthy subjects (215.0 ± 26.1 vs 63.1 ± 18.1 nmol/L; P 0.05). Plasma serotonin levels were significantly higher in Child- Pugh grade A/B than in grade C patients (246.8 ± 35.0 vs 132.3 ± 30.7 nmol/L; P 0.05). CONCLUSION: Plasma serotonin levels are significantly higher in patients with cirrhosis than in the controls and represent the degree of liver insufficiency. In addition, platelet poor plasma serotonin estimation is a better marker for liver insufficiency than platelet serotonin content.

Spontaneous gastrojejunal fistula formation is an extremely rare complication of gastric ulcer disease. We report a 77-year old woman who presented with diffuse abdominal pain, weight loss, malaise, nausea, and occasional dark stools. Laboratory tests showed extreme hyposideremic anemia with inflammatory syndrome. In addition, biochemical parameters of malnourishment were presented. Upper endoscopy revealed the patent esophagus along the full length without any pathological changes. Large and deep ulceration with perforation in the small intestine was detected in the posterior gastric wall. The small intestine loop was reached by endoscope through spontaneously developed gastrojejunal fistula. Polytopic biopsies of described ulcerative change were carried out. Histopathologically reepithelialized ulcerous zone was seen in the gastric mucosa. Also, gastrojejunal fistula was visualized after wide opening of hepatogastric and gastrocolic ligament. Jejunal loop 25 cm from ligament of Treitz was attached to mesocolon and posterior gastric wall because of ulcer penetration. Postoperative course was uneventful, Per oral intake started on the 4^th postoperative day, and the patient was discharged on the 8^th postoperative day. In summary, this case indicates that persistent symptoms of peptic ulcer disease associated with nutritional disturbances may be caused by gastrojejunal fistula.
