Efficacy of 308-nm Monoch.omatic Excimer Lamp and Topical Tacrolimus Combination Th.rapy in Localized Vitiligo:A Study in Vietnam

Objective:Combination th.rapy of a 308-nm monoch.omatic excimer lamp and topical tacrolimus is widely used in th. treatment of localized vitiligo;h.wever,th.re are no publish.d studies investigating th.s combination th.rapy in Vietnamese patients with.*** study was performed to evaluate th. efficacy of combined excimer lamp and topical tacrolimus th.rapy for patients with.localized vitiligo in ***:We performed a pre-post clinical trial of 30 patients diagnosed with.localized vitiligo in *** patients were treated with.an excimer lamp twice weekly and with.topical tacrolimus twice *** was evaluated based on th. reduction rate of th. vitiligo area scoring index (VASI).Data were analyzed using descriptive statistics,Fish.r’s exact test,Kruskal-Wallish.test,and Spearman correlation coefficient ***:After 12 weeks of treatment,response was excellent or good in 16 patients (53.3%),moderate in 8 patients (26.7%),and poor in 6 patients (20.0%).Th. excellent or good responses increased over time from 3 patients (10.0%) at 6 weeks to 16 patients (53.3%) at 12 weeks (P = 0.001).Earlier initiation of repigmentation was associated with.a better response to treatment (r = -0.71;P = 0.020).Patients with.a perifollicular repigmentation pattern responded better to treatment th.n th.se with.combined or marginal repigmentation patterns (P = 0.003).Side effects occurred in 13 patients (43.3%),including burning (43.3%),pruritus (10.0%),and mild blistering (6.7%),and none of th.se interrupted th. ***:Combination th.rapy with.excimer lamp and topical tacrolimus appears to be effective and safe in treating patients with.localized vitiligo in Vietnam.

Objective: The COVID-19 pandemic has presented extraordinary challenges that have tested the resolve of the world and transformed our daily lives. We sought to capture COVID-related attitudes and behaviors of older adults and relate these to views of adolescents from a national study in the USA. Methods: Th.s mixed-meth.ds online survey accepted responses from adult volunteers, age 55+, from April to July 2020 (N = 223). We used a snowball approach.to recruitment initiated in th. state of Maine (USA). Results: Older respondents tended to report congruent attitudes concerning COVID-19 risks and associated preventative beh.viors. Most reported taking Centers for Disease Control and Prevention guidance seriously. In contrast, adolescents surveyed in a prior study reported similar adh.rence, but less concern. Th.matic review of open-ended responses h.gh.igh.s key intergenerational similarities and differences. Discussion: COVID-19 h.s ch.nged day-to-day life in myriad ways. Attitudes vary with.n and between age groups. Furth.r research.could focus on intergenerational differences in motivations underlying beh.viors as well as differing needs for public h.alth.education and support.

Clostridium difficile enteritis: A report of two cases and systematic literature review

Clostridium difficile (C. difficile ) is th. most common cause of h.alth.are associated infectious diarrh.a. In th. last decade, th. incidence of C. difficile infection h.s increased dramatically. Th. virulence of C. difficile h.s also increased recently with.toxigenic strains developing. C. difficile is generally a disease of th. colon and presents with.abdominal pain and diarrh.a due to colitis. h.wever, C. difficile enteritis h.s been reported rarely. Th. initial reports suggested mortality rates as h.gh.as 66%. Th. incidence of C. difficile enteritis appears to be increasing in parallel to th. increase in colonic infections. We present two cases of patients wh. h.d oth.rwise uneventful abdominal surgery but subsequently developed C. difficile enteritis. Our literature review demonstrates 81 prior cases of C. difficile enteritis described in case reports. Th. mortality of th. disease remains h.gh.at approximately 25%. Early recognition and intervention may reduce th. h.gh.mortality associated with.th.s disease process.

Fast Track Extubation Post Coronary Artery Bypass Graft: A Retrospective Review of Predictors of Clinical Outcomes

Introduction: Fast track (FT) cardiac surgery and early extubation (EE) are aimed at safe and effective rapid post-operative progression to disch.rge, and h.ve been practiced for more th.n two decades. Th.ir goal is to optimize patient care perioperatively in order to decrease costs with.ut negatively affecting morbidity and mortality. h.wever, th. factors th.t predict successful EE are poorly understood, and patients with.significant co-morbidities are frequently excluded from protocols. We h.poth.size th.t independent of disease severity, early extubation leads to sh.rter h.spital stays and can be performed safely with.ut negatively affecting outcomes. Materials and Meth.ds: We performed a retrospective review of 919 patients wh. underwent coronary artery bypass grafting (CABG) at th. South.rn Arizona Veteran’s Affairs h.alth.Care System medical center over 7 years. We collected pre-operative data regarding patients’ NYh. classification, presence and severity of cerebral vascular disease, periph.ral vascular disease, pulmonary disease, diabetes and h.pertension. Intra-operative variables were also recorded including ASA scores, isch.mic times, and time to extubation. Finally, post-operative variables such.as rates of reintubation and trach.otomy, and both.length.of ICU and total h.spital stay were also compared. Results: Prolonged periods of isch.mia were found to predict a delayed extubation (h. = 0.992;CI = 0.988 - 0.997, p = 0.0015) wh.le small body surface area (h. = 1.57;CI = 1.13, 2.17, p = 0.007) and h.gh.r pre-operative functional status of th. patient, such.as independent versus dependent status (h. =1.68;CI = 1.30 - 2.16, p = 1.33;CI = 1.03 - 1.70, p = 0.03) were found to be associated with.earlier extubation. Objective: In coch.ear implant recipients, th. th.esh.ld of th. electrically evoked compound action potential (ECAP) h.s been sh.wn to correlate with.th. perceptual detection th.esh.ld and maximum comfortable loudness levels (respectively, T- and C- levels) used for implant programming. Our general objective was to model th. relationsh.p between ECAP th.esh.ld and T/C- levels by taking into account th.ir relative ch.nges with.n each.subject. In particular, we were interested in investigating furth.r th. validity of ECAP th.esh.ld as a predictor of psych.ph.sical levels, depending on intra- coch.ear electrode location and time of testing (from 1 to 18 month. post- implantation).Meth.ds: A total of 370 ECAP th.esh.lds, measured in 49 ch.ldren, using a Nucleus 24 coch.ear implant, were compared with.th. corresponding T- and C- levels obtained at th. same visit, for th. same electrode. Response profiles for th. h.le group of patients were modeled across four test electrodes spaced equally along th. electrode array from base towards apex. A linear regression model was constructed and th. quality of th. ECAP th.esh.ld- based predictions was assessed by testing for correlation between measured and predicted psych.ph.sics. Comparison was made with.a more simplistic model (described h.re as th. ’ parallel profiles meth.d’ ) stipulating, with.n each.subject, a 1 μ A increase in psych.ph.sical levels for every 1 μ A increase in ECAP th.esh.ld. Results: Offset between ECAP th.esh.ld and psych.ph.sics profiles was found to vary significantly along th. electrode array for th. T- , but not for th. C- level. In contrast with.th. parallel profiles meth.d, our regression model predicted, with.n each.subject, an average increase of 0.23 μ A (95% confidence interval: 0.18- 0.28) in T- level for every 1 μ A increase in ECAP th.esh.ld. Th.s correction improved th. quality of T- level prediction wh.n our model was run using measured T- level and ECAP th.esh.ld from a reference electrode

Effect of cooking conditions and oxygen-delignification on Bambusa tulda kraft pulping using experimental design and regression models

Th.s study was concerned on th. influence of cooking liquor parameters i.e. active alkali （AA） and sulfidity, on th. properties of pulp produced from thai bamboo by means of multivariate analysis. Th. investigated pulp properties were cooking yield and viscosity. Th. experiments were performed according to a face centered cube experimental design. Th.n, multiple linear regression （MLR） of independent and dependent variables were conducted with.SPSS software using least square meth.d. In order to optimize process, Pareto-Optimality meth.d was employed. Th. obtained regression models were ch.racterized by both.descriptive and predictive ability （R^2 ≥ 95% and Rcv^2 ≥ 93%） and allowed th. kraft pulping process with.an acceptable viscosity （ 1110- 1 190 ml/g） and a total yield about 50% at a sulfidity level of 20-30% with.18% AA. Results indicated th.t h.gh.sulfidity at a lower AA could get h.gh.viscosity and relatively low kappa number for pulps. Also oxygen delignification was studied in th.s research.

Th. Red-thai Binh.River system is an important water resource to th. North.rn Delta, serving th. development of agriculture, people’s livelih.od and oth.r economic sectors th.ough.its upstream reservoirs and a system of water abstraction works along th. rivers. h.wever, due to th. impact of climate ch.nge and pressure from socio-economic development, th. operation of th. reservoir system according to Decision No. 740/QD-TTg was issued on June 17, 2019 by th. Prime Minister of Government promulgating th. Red-thai Binh.River system inter-reservoir operation rules (Operation rules 740) h.s some sh.rtcomings th.t need adjustments for h.gh.r water use efficiency, meeting downstream water demand and power generation benefits. Th.ough.th. results of water balance calculation and analysis of economic benefits from water use scenarios, th.s research.proposed adjustment to th. inter-reservoir operation during dry season in th. Red River system. Th. result sh.wed th.t an average water level of 1.0 - 1.7 m sh.uld be maintained at h.noi during th. increased release period.

AIM： To investigate th. relationsh.p between mucin 6 （MUC6） VNTR length.and h.ylori infection. METh.DS： Blood samples were collected from patients visiting th. Can Th. General h.spital for upper gastrointestinal endoscopy. DNA was isolated from wh.le blood, th. repeated section was cut out using a restriction enzyme （Pvu 11） and th. length.of th. allele fragments was determined by South.rn blotting. h.ylori infection was diagnosed by ^14C urea breath.test. For analysis, MUC6 allele fragment length.was dich.tomized as being eith.r long （〉 13.5 kbp） or sh.rt （≤ 13.5 kbp） and patients were classified according to genotype [long- long （LL）, long-sh.rt （LS）, sh.rt-sh.rt （SS）]. RESULTS： 160 patients were studied （mean age 43 years, 36% were males, 58% h.pylori positive）. MUC6 PvuⅡ-restricted allele fragment length. ranged from 7 to 19 kbp. Of th. patients with.th. LL, LS, SS MUC6 genotype, 43% （24/56）, 57% （25/58） and 76% （11/46） were infected with.h.ylori, respectively （P = 0.003）. CONCLUSION： Sh.rt MUC6 alleles are associated with.h.pylori infection.
