Heterologous Bovine Tunica Albuginea Graft Conserved in Honey as Abdominal Wall Reinforcement in Rats

Complex abdominal wall defects might be challenging for human and veterinary surgeons worldwide. Defects from trauma or congenital causes may lead to hernias development. The introduction of meshes to reinforce hernia repairs has improved surgical outcomes and several synthetic and biologic materials have been used. In this context, biomaterial prosthesis seems to be a satisfactory solution when managing great abdominal wall defects. The aim of the current study is to evaluate the bovine tunica albuginea (BTA) preserved in honey as graft material for rats’ abdominal wall reinforcement in incisional herniorrhaphy surgery as well as its viability, cicatrization and integration into the host tissue. Wistar rats were assigned to two main groups: 1) animals (n = 20) underwent median longitudinal laparotomy followed by laparorraphy with suture of the bovine tunica albuginea (BTA) graft as abdominal wall reinforcement;and 2) animals (n = 20) underwent only laparotomy and subsequent laparorrhaphy. Rats were clinically evaluated until euthanasia at post-surgical day 7, 14, 21 and 28. Necropsy and histopathological analysis of abdominal wall fragments were performed to compare groups and subgroups findings. BTA promoted abundant fibrosis, providing resistance and low postoperative complication rates. Besides, animals did not show rejection signs to the implant. In conclusion, BTA preserved in honey is an affordable, easy collection and handling biomaterial for graft, demanding simple surgical implantation technique for abdominal wall repair in rats.

Acinetobacter baumannii Isolated from Clinical Material of Patients from a University Hospital

Model Study: The genus Acinetobacter is composed of opportunistic pathogens that mainly affects immunocompromised and patients during a prolonged hospitalization period. A. baumannii can survive in the hospital setting at various locations such as mechanical ventilators, dialysis machines, ventilation systems, water sources, skin and mucous membranes of health professionals and patients, drug preparations and disinfectants among others. Antimicrobials are essential for the treatment of infections, so their use has become common, and often inappropriate and abusive. This pathogen has presented multi-resistance to several classes of antimicrobials and has become a major cause of death in hospitals. Objective: To evaluate the occurrence of Acinetobacter baumannii in clinical samples from the Laboratory of Clinical Analyzes, HCSL of Pouso Alegre, MG. Methods: All the samples referring to the patients of the Samuel Libanio Clinical Hospital including most of the specialties were evaluated. From the positive samples for Acinetobacter baumannii the medical records of hospitalized patients were evaluated. From the medical records, data were obtained as gender and age of the patient, place of hospitalization, together with month and year of the laboratory procedure. Result: A total of 14,859 samples of diverse materials were evaluated, being 80 (0.5%) samples positive for Acinetobacter baumannii. Of these positive samples, the largest occurrences were identified in male patients 59 (73.7%), aged over 60 years 37 (46.3%). The most affected hospital sector was Adult ICU with 44 (55.0%) of the cases. In May 2015, there were 5 (6.3%) cases registered and in May 2016, 16 (20.0%) cases. The clinical material with the highest occurrence of Acinetobacter baumannii was Mini Bal 29 (36.3%). Self-Care Practice of Patients with Mechanical Heart Valve Prosthesis Accompanied in Nursing Consultation

The objective was to evaluate the practice of self-care of patients with mechanical heart valve prosthesis. A descriptive, cross-sectional study developed in two outpatient valvopathies of teaching hospitals of Fortaleza/CE, from October 2013 to January 2014. The sample consisted of 127 patients with mechanical heart valve prosthesis. It was held an interview using instrument based on the Theory of Orem’s Self-Care and Brazilian Guidelines for Valvular Heart Diseases. The data were presented in tables and charts. Results: Universal self-care practices of larger adhesion: body hygiene (97.6% washed-haired, daily-bath 92.1%);oral hygiene (brushing teeth before sleeping, 87.4%);fluid intake (drinking-water 95.2%);food intake (salt intake ≤ 2 g/day, 92.1%, fruit and vegetable consumption 79.5%);intestinal eliminations (without blood or mucus—96%, non-parasitic 94.4%);urinary elimination (no blood nor pus-96.8%, urination 4 to 6 times a day, 96%). As self-care developmental requirements predominated: never used illegal drugs, tobacco, alcohol, or stopped at the discovery of the disease (70%). As self-care health deviation requirements we have: making use of certain medication at the right dose (95.2%);attending medical appointments (cardiologist—92.1%;nursing—84.2%);INR control (identifies signs of bleeding— 85.8%). We conclude that patients did not perform all recommended self-care practices, being necessary to establish strategies to reduce the self-care deficit.

Characterization of Pegmatoid from Dimension Stones Tailings for Application as Ceramic Raw Material

Exploitation and processing of pegmatoids at Brazil＇s northeast （Serido region-RN State） aiming further use as dimension stone produces mass losses ranging between 30%-90%. The tailings are predominantly composed by feldspar, quartz, muscovite and other Fe-bearing silicates such as biotite and amphiboles, suitable for ceramic production. The recovery of these elements depends on mica＇s remotion to levels under 2.0%, preferentially by dry methods due the scarce amount of water prevailing at Serido region. This paper presents results from pegmatite tailings characterization from Serido region showing the potentially application as raw material for ceramic production. To do it, a new technological route must be developed to fit it as main component to white ceramic industry, considering the properties required by ceramic industries.

Photodynamic Therapy with Phthalomethyl D: Perspectives against SARS-CoV-2

Background: Photodynamic therapy (PDT) is the generation of cytotoxic agents through the dynamic interaction between a photosensitizer excited by light at a specific wavelength. When associated with phthalocyanines, they are efficient in incorporating target cells and exhibiting high rates of triplet generation. This study aimed to characterize PDT associated with the phototherapeutic agent Phthalomethyl D, developed by the authors, in the process of repair, healing and immune improvement for possible application against SARS-CoV-2. Methods: Sixty-nine mice were used, divided into 2 groups: GI, treated with ILIB laser, without a phototherapeutic agent, and subjected to surgery for viral induction;GII, same as GI plus association of Phthalomethyl D. They were divided into subgroups and reevaluated at 7, 14, and 21 days, and then divided into 3 subgroups of 6 animals each, subjected to treatment at 24 h, 48 h, 72 h, 5 and 7 days. Results: Both groups had a high rate of partial incision closure and acute inflammatory control. Microscopically, there was a greater amount of amorphous fundamental substance, fibrocytes, fibroblasts, and giant cells and reduction in the number of keratinocytes, in the amount of keratin, and epidermal thickness in GII than in GI. Conclusions: PDT with Phthalomethyl D stimulates the processes of healing/repair and immunomodulation during viral infection, initially favoring the inflammatory response and, after 21 days, contributing to the anti-inflammatory response profile, making this approach possible in the treatment of individuals infected with SARS-CoV-2.

Metabolic surgery and intestinal gene expression:Digestive tract and diabetes evolution considerations

AIM: To investigate the effects of bariatric surgery on metabolic parameters, incretin hormone secretion, and duodenal and ileal mucosal gene ***: Nine patients with type 2 diabetes mellitus(T2DM), chronic serum hyperglycemia for more than2 years, and a body mass index(BMI) of 30-35 kg/m2 underwent metabolic surgery sleeve gastrectomy with transit bipartition between May 2011 and December2011. Blood samples were collected pre and 3, 6 and12 mo postsurgery. Duodenal and ileal mucosa samples were collected pre- and 3 mo postsurgery. Pre- and postoperative blood samples were collected in the fasting state before ingestion of a standard meal(520kcal) and again 30, 60, 90, and 120 min after the mealto determine hemoglobin A1c(HbA1c) levels and the lipid profile, which consisted of triglyceride and total cholesterol levels. Intestinal gene expression of p53 and transforming growth factor(TGF)-β was analyzed using quantitative reverse-transcription PCR. Gastric inhibitory polypeptide(GIP) and glucagon-like peptide-1(GLP-1)were quantified using the enzyme-linked immunoassay method and analyzed pre- and ***'s t test or repeated measurements analysis of variance with Bonferroni corrections were performed as ***: BMI values decreased by 15.7% within the initial 3 mo after surgery(31.29 ± 0.73 vs 26.398± 0.68, P < 0.05) and then stabilized at 22% at 6mo postoperative, resulting in similar values 12 mo postoperatively(20-25 kg/m2). All of the patients experienced improved T2 DM, with 7 patients(78%)achieving complete remission(HbA1c < 6.5%), and 2patients(22%) achieving improved diabetes(HbA1c< 7.0% with or without the use of oral hypoglycemic agents). At 3 mo postoperatively, fasting plasma glucose had also decreased(59%)(269.55 ±18.24 mg/dL vs 100.77 ± 3.13 mg/dL, P < 0.05)with no further significant changes at 6 or 12 mo postoperatively. Wounds are a serious public health problem in our country. Access to effective, low-cost treatment is still far from the reality for many patients leading to a chronic and serious condition. They interfere in the quality of life of these individuals who isolate themselves socially constrained by the appearance of their wounds. The report shows the case of a patient with type II diabetes mellitus, with chronic wounds in lymphedema in the lower limbs who was treated with the Scar Acceleration Methodology associated with Complex Decongestive Therapy, leading to an improvement in her social participation and quality of life.

Diversity and Structure of the Prokaryotic Communities Indigenous to Two Volcanic Lakes: Nyos and Monoun in Cameroon

This study explores the diversity and structure of prokaryotic communities (Archaea and Bacteria) of 2 tropical volcanic lakes (Nyos and Monoun) in Cameroon, using 16SrRNA sequences. Metagenomics analysis of sequences showed that most OTUs (Operational Taxonomic Units) were associated with 26 phyla (23 for Bacteria and 3 for Archaea) in Nyos and 36 phyla (33 for Bacteria and 3 for Archaea) in Monoun. In both lakes, Proteobacteria for Bacteria and Crenarchaea for Archaea were predominant and present at all depths but in different proportions. Bacterial community compositions were generally dominated by members of Proteobacteria, Firmicutes, Actinobacteria, Chloroflexi and Bacteroidetes covering about 98% of the sequences. Crenarchaea, Thaumarchaea and Euryarchaea were the three main phyla of Archaea common to both lakes. The amount of virus and total bacteria was determined by flow cytometry technic and the evaluated ratio ranged from 0.2 to 1.2 at Nyos and from 0.6 to 2.6 at Monoun. For both lakes, the correlation was very significant between viruses and total bacteria. The depth-dependent variability is discussed with chemical and physical environmental parameters. These could significantly influence virus-mediated bacterial lysis and abundance and vertical stratification of the prokaryotic community.

The Lipid Accumulation Product (LAP) is a clinical marker of visceral obesity and has been proposed as a simple, inexpensive, and accurate tool to estimate cardiovascular risk and mortality. The aim of this study was to verify the association of LAP with anthropometric, biochemical, visceral adiposity index and IR in adults and the elderly. This single cross-section center clinical study, with experimental, analytical, primary, and observational design, included 210 participants. Anthropometric (Body Mass Index (BMI), Waist Circumference (WC), and Neck Circumference (NC)), LAP, Visceral Adipose Index (VAI), and biochemical parameters (fasting glycemia, insulinemia (to calculate the Homa-IR index), total cholesterol, LDL-c, HDL-c, and triglycerides) were evaluated. The results showed that by separating the sample into three groups (adequate BMI and WC, adequate BMI and elevated WC, and elevated BMI and WC), the group with high BMI and WC showed a high value of LAP and VAI compared to the other groups, with a significant difference. Still, the data show a positive and significant correlation when relating the LAP with VAI, HOMA-IR, BMI, WC, NC, total cholesterol, triglycerides, and Diastolic Blood Pressure. Healthcare Professionals’ Adherence to Contact Precautions at a Maternal and Child Teaching Hospital

Background: Healthcare-associated infections affect hundreds of millions of patients worldwide. Children have greater susceptibility to healthcare-associated infections due to the immaturity of their immune system. Contact precautions aim to promote safety, protection and prevention of contamination. Thus, the objective of this study was to verify adherence to contact precaution measures, as well as compliance to the use of personal protective equipment. Methods: This cross-sectional observational study was carried out from July to October 2019 using a checklist to evaluate health professionals’ adherence to hand hygiene procedures and the use of gloves and surgical gowns when assisting children on contact precautions. Results: A total of 941 observations were carried out in a total of 300.532 hours. Hand hygiene was performed before and after contact with the patient in 58.84% and 75.09% of the cases, respectively and a surgical gown was used in 86.40% of the cases. The use of gloves was the variable most adhered to by professionals (87.57%). Intensive care unit professionals were the care workers who most complied with the regulation regarding hand hygiene after contact with the patient (p = 0.009) and the use of the surgical gown (p < 0.001). The correct hand hygiene technique was the recommendation with least adherence. Non-compliance to the hand hygiene technique was statistically significant among intensive care unit professionals (p = 0.002). Conclusions: Adherence to hand hygiene before contact with the patient and compliance with the hand hygiene technique were neglected by most professionals. However, there was good adherence to the use of surgical gloves and gowns, as well as high compliance to the techniques of removing these items. .
