Hypertrophic cardiomyopathy (HcM), the most common single-gene cardiovascular disease, is associated with increased risk for arrhythmias, heart failure, and sudden cardiac death. The hemodynamic changes known to occur during pregnancy can exacerbate heart failure and arrhythmias in women with HcM. We present a 30-year-old woman with HcM to illustrate the benefits of multidisciplinary team management of severe left ventricular outflow tract obstruction (peak gradient >100 mmHg) for optimal maternal and fetal outcomes.

climate variability as occasioned by conditions such as extreme rainfall and temperature, rainfall cessation, and irregular temperatures has considerable impact on crop yield and food security. This study develops a predictive model for cassava yield (Manihot esculenta crantz) amidst climate variability in rainfed zone of Enugu State, Nigeria. This study utilized data of climate variables and tonnage of cassava yield spanning from 1971 to 2012;as well as information from a questionnaire and focus group discussion from farmers across two seasons in 2023 respectively. Regression analysis was employed to develop the predictive model equation for seasonal climate variability and cassava yield. The rainfall and temperature anomalies, decadal change in trend of cassava yield and opinion of farmers on changes in rainfall season were also computed in the study. The result shows the following relationship between cassava and all the climatic variables: R2 = 0.939;P = 0.00514;cassava and key climatic variables: R2 = 0.560;P = 0.007. The result implies that seasonal rainfall, temperature, relative humidity, sunshine hours and radiation parameters are key climatic variables in cassava production. This is supported by computed rainfall and temperature anomalies which range from −478.5 to 517.8 mm as well as −1.2˚c to 2.3˚c over the years. The questionnaire and focus group identified that farmers experienced at one time or another, late onset of rain, early onset of rain or rainfall cessation over the years. The farmers are not particularly sure of rainfall and temperature characteristics at any point in time. The implication of the result of this study is that rainfall and temperature parameters determine the farming season and quantity of productivity. Hence, there is urgent need to address the situation through effective and quality weather forecasting network which will help stem food insecurity in the study area and Nigeria at large. Pancreatic cystic lesions(PcLs) have been increasingly recognized in clinical practice. Although inflammatory cysts(pseudocysts) are the most common PcLs detected by cross-sectional imaging modalities in symptomatic patients in a setting of acute or chronic pancreatitis, incidental pancreatic cysts with no symptoms or history of pancreatitis are usually neoplastic cysts. For these lesions,it is imperative to identify mucinous cysts(intraductal papillary mucinous neoplasms and mucinous cystic neoplasms) due to the risk of their progression to malignancy. However, no single imaging modality alone is sufficient for a definitive diagnosis of all PcLs. The cyst fluid obtained by endoscopic ultrasound-guided fine needle aspiration provides additional information for the differential diagnosis of PcLs. current recommendations suggest sending cyst fluid for cytology evaluation and measurement of carcinoembryonic antigen(cEA) levels. Unfortunately, the sensitivity of cytology is greatly limited, and cyst fluid cEA has demonstrated insufficient accuracy as a predictor of mucinous cysts. More recently, cyst fluid glucose has emerged as an alternative to cEA for distinguishing between mucinous and nonmucinous lesions. Herein, the clinical utility of cyst fluid glucose and cEA for the differential diagnosis of PcLs was evaluated.

There are five vital signs that healthcare providers assess: temperature, pulse, respiration, blood pressure, and pain. Normal levels for the five vital signs are published by the American Heart Association, and other specialty organizations, however, the sixth vital sign (resilience) which adopts the measure of immune resilience is suggested in this paper. Resilience is the ability of the immune system to respond to attacks and defend effectively against infections and inflammatory stressors, and psychological resilience is the capacity to resist, adapt, recover, thrive, and grow from a challenge or a stressor. Individuals with better optimal immune resilience had better health outcomes than those with minimal immune resilience. The purpose of this paper is to conceptualize, contextualize, and operationalize all six vital signs. We suggest measuring resilience subjectively and objectively. Subjectively, use a 5-item guided interview revised from the connor-Davidson Resilience Scale (cDRc), a scale of 10 items. The revised cDRc scale is a 5-item scale. The scale is rated on a 5-point Likert scale from 0 (not true) to 4 (true all the time). The total score ranges from 0 to 20, with higher total scores indicating greater resilience. The scale demonstrated good construct validity and internal consistency (α = 0.85) during the development of the scale. The cD-RISc had a good cronbach’s alpha level of 0.85. The Revised cD-RISc can be completed in 2 - 4 minutes. To measure resilience objectively, we suggest using Immune Resilience (IR) levels, the level of resilience to preserve and/or rapidly restore immune resilience functions that promote disease resistance and control inflammation and other inflammatory stress. IR levels are gauged with two peripheral blood metrics that quantify the balance between cD8 and cD4 T-cell levels and gene expression signatures tracking longevity-associated immunocompetence and mortality- or entropy-associated inflammation. Generation of construction wastes constitutes a major impact to the environment. Studies have been directed toward reducing wastes of the various construction processes. However, contractors in Hong Kong are rather conservative and lack motivation to develop new technology for waste minimization. This paper examines the benefits of using prefabrication in reducing material wastage levels for some traditional in-situ trades such as concreting, rebar fixing, bricklaying, drywall, plastering, screeding and tiling in Hong Kong. By measuring thirty construction projects, it is been found that waste levels of all major construction trades can be effectively reduced. This is particularly significant in the trade of plastering where 100% saving can be achieved after adopting prefabrication. The standardized design of the building can also be useful in the adoption of prefabrication for private housing projects.

Let G be a finite connected graph. The eccentric connectivity index ξ^c(G) of G is defined as ξ^c(G)=∑v∈V(G)ec(υ)deg(υ), where ec(v) and deg(υ) denote the eccentricity and degree of a vertex v in G, respectively. In this paper, we give an asymptotically sharp upper bound on the eccentric connectivity index in terms of order and vertex-connectivity and in terms of order and edge-connectivity. We also improve the bounds for triangle-free graphs.

Background:Despite reports that associate donor specific antibody(DSA)with rejection after liver transplantation,grafts are still allocated according to blood group(ABO)but not human leukocyte antigen(HLA)compatibility,possibly due to the absence of an easily discernible clinical association between adverse recipient outcome and ***-transplantation provides a test environment where the presence of preformed DSA is prevalent and its effect on outcome should be ***:All patients undergoing a second liver transplantation with available pre-operative serum were included with the exception of ABO incompatible or multiple organ *** sera were tested for anti-HLA antibodies with Luminex-based solid phase ***-HLA antibodies to the second donor(D2SA)were determined using antibodies specificity and HLA typing of 2nd liver ***:Preformed HLA antibodies directed to second liver transplantation(D2SA)were found in 31(39%)of the 79 patients that were included in the *** and re-transplantation characteristics were similar in both subgroups except first graft survival which was significantly shorter in recipients who are negative for *** survival of the second graft was similar in D2SA+and D2SA?cohorts[8.55(range,0.01–24.74)vs.7.56(range,0–23.53)years respectively,P=0.574].Mean patient survival after 2nd liver transplantation was similar in D2SA+and D2SA?cohorts[9.11(range,0.01–24.74)vs.8.10(range,0–23.53)years respectively,P=0.504].Subgroup univariate analysis demonstrated no detrimental effect of class,locus,or strength of D2SA on survival of the second liver *** multivariate cox regression model,neither class I D2DSA(HR=1.101,P=0.92)nor class II D2SA(HR=1.74,P=0.359)were significant risks of graft ***:Presence of D2SA was not found to be associated with inferior outcomes in this retrospective cohort study of liver re-transplantation suggesting that changes to the allocation system are not requ
