Comparison of Coblation Tonsillectomy vs Dissection Tonsillectomy

Background: Tonsillectomy is one of the most commonly performed surgical operations in the recent years. It is mainly done for chronic tonsillitis and obstructive symptoms. Aims & Objectives: To compare the operating time, intraoperative blood loss, postoperative pain, time needed to return to normal diet and activity in coblation and dissection tonsillectomy. Methods: This is a prospective study done at Pushpagiri Medical College for a period of one and a half years from March 2015 to September 2016. Out of the 50 patients, 25 underwent coblation tonsillectomy and 25 underwent conventional dissection tonsillectomy. Method of surgery depended on patients’ or parents’ choice. Intra operative measures like operating time, blood loss and post-operative morbidity were measured and compared. Results: In this study, there was significant difference in intraoperative time in coblation tonsillectomy compared to cold dissection tonsillectomy (mean operative time—13.4 minutes for coblation and 20.4 minutes for cold dissection with p value less than 0.05). Intraoperative blood loss was significantly less for coblation (18.9 ml) compared to traditional (43.0 ml) with p value 0.002. Average postoperative pain score 6 hours after operation was 7.6 for coblation and 8.5 for cold dissection with a significant p value of 0.002. Average time taken to return to normal diet among coblation was 6.4 days and dissection was 7.0 days with p value of 0.078 which is not statistically significant. However, average time taken to return to normal activity among coblation was 6.3 days and dissection was 7.1 days with a significant p value of 0.024. Conclusion: Coblation tonsillectomy significantly reduces the operation time, intraoperative blood loss, immediate postoperative pain, and patient returns early to regular routine, but with a higher cost.

Hearing Outcome in Stapes Surgery Using Various Biomaterial Prostheses

Aims: To assess the hearing outcome in patients who have undergone stapes surgery using polytetrafluoroethylene (PTFE) and Titanium prosthesis at 1 and 3 months post operatively. Material & Methods: Research design: Prospective study. Setting: ENT department, Pushpagiri Medical College. Study population: All age groups with clinical diagnosis of otosclerosis and air-bone gap more than 15 dB irrespective of sex difference. Patients with Meniere’s disease are excluded. Method: A prospective study was conducted for a period of one and a half years from January 2014 to July 2015 to find out the hearing outcome of stapes surgery in patients with otosclerosis using polytetrafluoroethylene (PTFE) and Titanium prostheses, attending our outpatient department. Results: In this study, 84.21% of ears with polytetrafluoroethylene (PTFE) and 81.81% of ears with titanium were able to achieve good results in terms of Air-Bone gap (ABG) closure (ABG Conclusion: Stapes surgery is the surgical procedure of choice for otosclerosis. In our study, a significant hearing improvement was seen in majority of cases. None of the factors considered relevant for the success of stapes surgery like age, sex, type of prosthesis appeared to affect the post-operative hearing significantly. Hence stapes surgery can be effectively used to improve hearing in otosclerosis.

Improved properties and thermal stability of a titanium-stainless steel solid-state weld with a niobium interlayer

Stainless steel(SS)and titanium alloys can be welded in the solid-state by high speed oblique ***,the effect of a niobium(Nb)interlayer on weld strength and thermal stability is *** Ti/SS and Ti/Nb/SS welds were subjected to thermal exposure ranging from 300℃ to 950℃ for 1.5 *** exposure monotonically decreases the strength and toughness of the Ti/SS pair with a dramatic falloff in strength and change in failure mode from partial pullout failure to full interfacial fracture at 600℃.With the interlayer,toughness was increased versus baseline up to 700℃ thermal exposure and then intermetallic formation again caused falloff in *** for the production,properties and applications of these classes of welds are provided.

The maximum flyer impact velocity based on a dynamic solidification cracking mechanism is proposed to describe the upper limit of collision welding process ***,the upper limit of the weld window is governed by the evolution of dynamic stresses and temperatures at the weld *** formulations for the upper limit of the collision weld window assume that both the flyer and target are made of the same material and approximate stress propagation velocities using the acoustic velocity or the shear wave velocity of the weld ***,collision welding fundamentally depends on the impacts that generate shockwaves in weld members,which can dominate the stress propagation velocities in thin weld ***,this study proposes an alternative weld window upper limit that approximates stress propagation using shock velocities calculated from modified 1-D Rankine-Hugoniot *** shock upper limit is validated against the experimental and simulation data in the collision welding literature,and offers a design tool to rapidly predict more accurate optimal collision weld process limits for similar and dissimilar weld couples compared to existing models without the cost or complexity of high-fidelity simulations.

In this article, we focus to study about modified objective function approach for multiobjective optimization problem with vanishing constraints. An equivalent η-approximated multiobjective optimization problem is constructed by a modification of the objective function in the original considered optimization problem. Furthermore, we discuss saddle point criteria for the aforesaid problem. Moreover, we present some examples to verify the established results.

Characterization of 3D microstructure,thermal conductivity,and heat flow of cement-based foam using imaging technique

This study presents the results of the 3D microstructure,thermal conductivity,and heat flow in cement-based foams and examines their changes with a range of *** were captured using X-ray micro computed tomography(micro-CT)imaging technique on cement-based foam samples prepared with densities of 400,600,and 800 kg/m^(3).These images were later simulated and quantified using 3D data visualization and analysis *** on the analysis,the pore volume of 11000µm^(3)was determined across the three densities,leading to optimal ***,distinct pore diameters of 15µm for 800 kg/m^(3),and 20µm for 600 and 400 kg/m^(3)were found to be *** of the pores were spherical,with only 10%appearing elongated or *** addition,a difference of 15%was observed between the 2D and 3D porosity ***,a difference of 5%was noticed between the experimentally measured thermal conductivity and the numerically predicted value and this variation was constant across the three cast *** 3D model showed that heat flows through the cement paste solids and with an increase in porosity this flow reduces.

Detection of QTL(quantitative trait loci) associated with wood density by evaluating genetic structure and linkage disequilibrium of teak

To find the quantitative trait loci associated with wood density in teak(Tectona grandis L.f.), 21 co-dominant markers including 13 site specific recombinase and 8 EST-based co-dominant markers designed from lignin biosynthesis genes were applied to 174 teak plus tree clones at the National Germplasm Bank, Chandrapur,India. The germplasm bank exhibited 10.6% coefficient of variation for wood densities with 84.5 ± 31.3 genetic polymorphism(%). The highly panmictic set of genotypes(FST= 0.035 ± 0.004) harbored 96.47 ± 0.40 genetic variability(%). The average allelic frequency of the 21 codominant markers was 0.65 ± 0.11 with 12.9% pairs of loci in significant LD(p\0.05, R^2 values [ 0.1), confirming their suitability for a strong marker-trait association study. The marker CCoAMT-1 was significantly(p\0.01) associated with wood density showing stability by both GLM and MLM models and explained 4.3% of the phenotypic effect. The marker from the EST representing CCoAMT can be further developed for gene-assisted selection of elite genotypes of teak with greater wood density. Therefore, we believe that the report will help accelerate the genetic improvement and advance the breeding program of the species.
