Non-alcoholic fatty liver disease (NAFLD), a further expression of metabolic syndrome, strictly linked to obesity and diabetes mellitus, is characterized by insulin resistance (IR), elevated serum levels of free fatty acids and fatty infi ltration of the liver, which is known as hepatic steatosis. Hepatocyte apoptosis is a key feature of this disease and correlates with its severity. Free-fatty-acidinduced toxicity represents one of mechanisms for the pathogenesis of NAFLD and hormones, growth factors and adipokines influence also play a key role. This review highlights the various pathways that contribute to the development of hepatic steatosis. Circulating concentrations of inflammatory cytokines are reckoned to be the most important factor in causing and maintaining IR. Low-grade chronic inflammation is fundamental in the progression of NAFLD toward higher risk cirrhotic states.

AIM： To shed some light on the relationship between anti-apoptotic serum Bcl-2 concentrations and metabolic status, anthropometric parameters, inflammation indi- ces, and non-alcoholic fatty liver disease severity were investigated in 43 young individuals with fatty liver （FL） and 41 with nonalcoholic steatohepatitis （NASH）. METHODS： Circulating levels of Bcl-2 were detected in 84 patients with ultrasonographic findings of ＂bright liver＂ and/or hyper-transaminasemia of unknown origin and/or increase in T-glutamyl-transpeptidase （T-GT） strictly in the absence of other acute or chronic liver disease, whose age was not advanced, who gave consent to liver biopsy and were then divided on the basis of the histological results into two groups （43 with FL and 41 with NASH）. Twenty lean subjects, apparently healthy and young, were chosen as ***： Serum Bcl-2 concentrations were significantly higher in the FL group than in the NASH group. Insulin resistance and γ-GT activity were significantly higher in NASH subjects. Apoptotic hepatocytes were significantly more numerous in NASH patients. NASH patients presented with larger spleens and augmented C-reactive protein （CRP） concentrations than healthy subjects. Steatosis grade at histology was similar in both NASH and FL populations. The number of apoptotic cells was significantly related to anti-apoptotic Bcl-2 protein values in FL patients. Bcl-2 serum levels positively correlated to body mass index （BMI） values （P ~ 0.0001） but not to age of the population. Triglycerides/HDL ratio correlated well to waist circumference in males （P = 0.0008）. γ-GT activity was associated with homeostatic metabolic assessment （HOMA）（P = 0.0003） and with serum ferritin （P = 0.02）. Bcl-2 concentrations were not related to either spleen size or CRP values. NASH patients pre- sented a weak negative correlation between Iobular inflammation and Bcl-2 levels. Cabergoline(CAB)是一种长效多巴胺促效剂，已在临床用于治疗催乳素瘤。本研究目的是了解长期用CAB治疗巨催乳素腺瘤的疗效是否会受以往用过溴隐亭(BRC)或诺果利(CV)治疗的影响。对象与方法将110名患者分为4组：①新发病组：26名，平均血清催乳素(PRL)水平为(1 013.4±277.7)μg/L(±SEM；范围 185.5～5 611 μg/L)。②不耐受组：19名以往对BRC不耐受，平均血清PRL水平(539.4±172.2)μg/L(范围 174～3 564 μg/L)。③抵抗组：37名，对BRC、CV或两者治疗均抵抗或低反应，平均血清PRL水平(602.6±136.8)μg/L(范围 148～3 511 μg/L)。④有反应组：28名，以往用BRC或CV治疗1～5 a，可使血清PRL水平降至正常并能恢复性腺功能，但因依赖性差或缺药而未再治疗，停药15～30 d后该组患者血清PRL水平为(397±43.1)μg/L(范围 140～978 μg/L)。该研究中CAB的治疗剂量为每周0.25～3.5 mg，治疗时间为1～3 a。在CAB治疗前及治疗后12、24和36个月几个时间点行磁共振成像(MRI)检查，评估肿瘤大小(以治疗后肿瘤较原先缩小80%以上为有显著性)。用线性相关分析估计各变量的相关性，多步骤线性回归分析估计各变量与肿瘤缩小之间的相关性。结果新发病组中21人(80.7%)经0.25～2 mg/周CAB治疗1～6月后血PRL正常，另5人用0.5～3 mg/周剂量治疗24个月后血PRL降至正常。肿瘤体积平均缩小(92.1±2.9)%，92.3%的患者肿瘤显著缩小，16名患者(61.5%)肿瘤影像完全消失；不耐受组中18人(94.7%)经0.25～1 mg/周CAB治疗1～6个月后，血PRL达正常，1人仍维持轻度高催乳素血症。肿瘤体积平均缩小(66.2±6.4)%，42.1%的患者肿瘤显著缩小，其中4人(21%)肿瘤影像完全消失；抵抗组19人(51.3%)经1～2 mg/周CAB治疗6～12个月，另18人经3～3.5 mg/周剂量治疗18～24个月后，血PRL达正常。肿瘤体积平均缩小(58.4±4.9)%，33名肿瘤缩小患者中10人(30.3%)缩小显著，但无一人肿瘤影像完全消失；有反应组患者中23人(82.1%)经1～2 mg/周CAB治疗1～6个月，另5人经3 mg/周剂量治疗12个月后，血PRL达正常。肿瘤体积平均缩小(59.2±6.2)%，26名肿瘤缩小患者中10人(38.4%)缩小显著，4人(15.4%)肿瘤影像完全消失。用CAB治疗后，新发病组患者在所有患者中获得了最低的血PRL水平(P<0.001)和最高的肿瘤缩小百分率(P<0.001)，在4组患者组每周平均使用CAB剂量最多(P<0.001)。肿瘤缩小和以往的治疗之间有显著相关性(X2＝27.1；P<0.0001)。多步骤相关分析表明PRL的谷值水平是肿瘤缩小的较强的预测指标(r2＝0.556；P<0.0001)，其次是CAB剂量(r2＝0.577；P<0.0001)。结论用标准剂量CAB治疗1～3 a后，新发病组巨催乳素腺瘤的缩小率(92.3%)高于不耐受组(42.1%)、抵抗组(30.3%)和有反应组(38.4%)，因此CAB可作为治疗巨催乳素腺瘤的一线用药，PRL水平控制得越低则肿瘤缩小得越显著。

Hepatic steatosis in overweight/obese females: New screening method for those at risk

AIM: To identify which parameters could help to distinguish the "metabolically benign obesity", which is not accompanied by insulin resistance (IR) and early ***: Eighty two of 124 overweight/obese females formed the study population, which was divided into two groups (52 and 30 subjects, respectively) with and without IR according to a HO meostatic Metabolic Assessment (HOMA) cut-off of 2, and were studied in a cross-sectional manner. The main outcome measures were waist circumference, serum uric acid, high-density lipoprotein-cholesterol and triglycerides, alanine amino-transferase, blood pressure and the two imaging para-meters, hepatic steatosis and longitudinal diameter of the spleen, which were measured in relation to the presence/absence of IR. RESULTS: A variable grade of visceral obesity was observed in all subjects with the exception of *** of a severe grade was represented more in the group of IR individuals (P = 0.01). Hepatic steatosis, revealed at ultrasound, was more pronounced in IR than in non-IR subjects (P = 0.005). The two groups also demonstrated a clear difference in longitudinal spleen diameter and blood pressure, with raised and signif icant values in the IR group. Metabolic syndrome was frequent in the IR group, and was not modified when adjusted for menopause (P = 0.001). At linear regression, the β values of waist circumference and body mass index predicting HOMA were 0.295, P = 0.007 and 0.41, P = 0.0001, respectively. Measures of spleen longitudinal diameter were well predicted by body mass index (BMI) values, β = 0.35, P = 0.01, and by HOMA, β = 0.41, P = 0.0001. Blood pressure was predicted by HOMA values, β = 0.39, P = 0.0001). HOMA and hepatic steatosis were highly associated (rho = 0.34, P = 0.002). Interestingly, IR patients were almost twice as likely to have hepatic steatosis as non-IR patients. Gitelman's syndrome(GS) is a salt-losing tubulopathy with an autosomal recessive inheritance caused by mutations of SLC12A3, which encodes for the thiazidesensitive Na Cl cotransporter. In this study we report a new mutation of SLC12A3 found in two brothers affected by GS. Hypokalemia, hypocalciuria and hyperreninemia were present in both patients while hypomagnesemia was detected only in one. Both patients are compound heterozygotes carrying one well known GS associated mutation(c.2581 C > T) and a new one(c.283 del C) in SLC12A3 gene. The new mutation results in a possible frame-shift with a premature stopcodon(pG ln95 Argfs X19). The parents of the patients, heterozygous carriers of the mutations found in SLC12A3, have no disease associated phenotype. Therefore, the new mutation is causative of GS.

Background and Objectives: The aim of this study is to evaluate the usefulness of Magnetic Resonance Imaging (MRI) for planning surgery and postoperative control of complex cases of abdominal wall hernia. Methods: Included in a prospective data base with more than 350 patients treated in two centers, a prospective study over ten patients with complex incisional hernias was designed. In all cases, preoperative diagnosis study and postoperative imaging control by MR Imaging were established. In the early and late follow-up controls, local and general complications were specifically noted and studied by MRI in each case. Results: The preoperative MRI was effective method for an adequate understanding of the defects and hernia sack content. In the postoperative control, the MRI was useful in the follow-up of the mesh integration, detection of complications and diagnosis of the relapsing. Conclusion: In our experience the MRI test is specially recommended in the preoperative workup of complex cases of incisional hernia.
