Treatment of dyslipidemia in chronic kidney disease: Effectiveness and safety of statins

Several cardiovascular(CV)risk factors may explain the high rate of CV death among patients with chronic kidney disease(CKD).Among them both traditional and uremia-related risk factors are implicated and,moreover,the presence of kidney disease represents"per se"a multiplier of CV risk.Plasma lipid and lipoprotein profiles are changed in quantitative,but above all in qualitative,structural,and functional ways,and lipoprotein metabolism is influenced by the progressive loss of renal function.Statin therapy significantly reduces cholesterol synthesis and both CV morbidity and mortality either directly,by reducing the lipid profile,or via pleiotropic effects;it is supposed to be able to reduce both the progression of CKD and also proteinuria.These observations derive from a post-hoc analysis of large trials conducted in the general population,but not in CKD patients.However,the recently published SHARP trial,including over 9200 patients,either on dialysis or pre-dialysis,showed that simvastatin plus ezetimibe,compared with placebo,was associated with a significant low-density lipoprotein cholesterol reduction and a 17%reduction in major atherosclerotic events.However,no benefit was observed in overall survival nor in preserving renal function in patients treated.These re-cent data reinforce the conviction among nephrologists to consider their patients at high CV risk and that lipid lowering drugs such as statins may represent an important tool in reducing atheromatous coronary disease which,however,represents only a third of CV deaths in patients with CKD.Therefore,statins have no protective effect among the remaining two-thirds of patients who suffer from sudden cardiac death due to arrhythmia or heart failure,prevalent among CKD patients.The safety of statins is demonstrated in CKD by several trials and recently confirmed by the largest SHARP trial,in terms of no increase in cancer incidence,muscle pain,creatine kinase levels,severe rhabdomyolysis,hepatitis,gallstones and pancreatitis;thus confirming the handiness

Advancements are reported in computer-generated holography proofing RGB 4K display through a new strategy based on diffraction model-driven deep networks. In the new 4K-DMDNet, the network is not a “black box” anymore. Rather, the input-output relation must obey to the physics of wavefront propagation, which is embedded here as a constraint. Thus, a labelled dataset is not required, and the model shows superior generalization capabilities with respect to data-driven approaches. The method is promising for the new generation of RGB 4K holographic display, as well as augmented and virtual reality systems.

In this editorial,we have analyzed the historical evolution of rectal and breast cancer surgery,focusing on the progressive reduction of demolitive approaches and the increasing use of more conservative strategies,accompanied by a growing emphasis on perioperative treatments aimed at enhancing surgical outcomes.All of these changes have been made possible due to an increased awareness and understanding of oncological diseases and improved perioperative treatments.

Primary or secondary clear cell sarcoma of the pancreas is an exceedingly rare and aggressive disease.In addition to pathology,molecular analysis is pivotal in differential diagnosis,especially with malignant melanoma.A key aspect in identifying clear cell sarcoma is specific genetic alterations,notably the translocation of t(12;22)(q13;q13),a diagnostic hallmark of this sarcoma subtype,which is absent in malignant melanoma.Treatment of primary clear cell sarcoma of the pancreas is the same as that for adenocarcinoma.

Starting from the importance of preserving our information and protecting our devices against attacks on their vulnerabilities, this article aims to establish the level of knowledge in computer security and problem-solving of students at a university in northwest Mexico. This research has a quantitative approach where the European Framework of Digital Competencies for Citizenship was used as a reference to identify and establish the level of the competencies that were evaluated. The IKANOS Test was used as a data collection tool. The results show that students know the importance of keeping their devices safe and how valuable the information found on them is. On the other hand, the results also show a considerable percentage of students who do not have the knowledge and are at a basic level of knowledge to solve technical problems with their devices.

We investigated herein the morphological,structural,and electrochemical features of electrodes using a sulfur(S)-super P carbon(SPC)composite(i.e.,S@SPC-73),and including few-layer graphene(FLG),multiwalled carbon nanotubes(MWCNTs),or a mixture of them within the current collector design.Furthermore,we studied the effect of two different electron-conducting agents,that is,SPC and FLG,used in the slurry for the electrode preparation.The supports have high structural crystallinity,while their morphologies are dependent on the type of material used.Cyclic voltammetry(CV)shows a reversible and stable conversion reaction between Li and S with an activation process upon the first cycle leading to the decrease of cell polarization.This activation process is verified by electrochemical impedance spectroscopy(EIS)with a decrease of the resistance after the first CV scan.Furthermore,CV at increasing scan rates indicates a Li+diffusion coefficient(D)ranging between 10^(−9) and 10^(−7) cm^(2)·s^(−1)in the various states of charge of the cell,and the highest D value for the electrodes using FLG as electron-conducting agent.Galvanostatic tests performed at constant current of C/5(1 C=1675 mA·g_(s)^(−1))show high initial specific capacity values,which decrease during the initial cycles due to a partial loss of the active material,and subsequently increase due to the activation process.All the electrodes show a Coulombic efficiency higher than 97%upon the initial cycles,and a retention strongly dependent on the electrode formulation.Therefore,this study suggests a careful control of the electrode in terms of current collector design and slurry composition to achieve good electrode morphology,mechanical stability,and promising electrochemical performance in practical Li-S cells.

Blockchain is a technology that creates trust among non-trusting parties without relying on any intermediaries.Consequently,it has attracted the interest of companies operating in a multitude of sectors.However,due to the number of different blockchain solutions that have emerged in the last few years and their rapid changes,it is challenging for such companies to orient their technological decisions.This paper presents a comparative analysis of the key dimensions—namely,governance,maturity,support,latency,privacy,interoperability,flexibility,efficiency,resiliency,and scalability—of some of the most-used permissioned blockchain platforms.Moreover,we present the results of a performance evaluation considering the following frameworks:Hyperledger Fabric 2.2,Hyperledger Sawtooth 1.2,and ConsenSys Quorum 21.1(with both the GoQuorum client and the Hyperledger Besu client).The platforms were tested under similar conditions,and official releases were used,such that our findings provide a reference for companies establishing their technological orientation.

In the last three decades, bridge stock seismic retrofitting prioritization has become one of the cult topics for scientific discussions in the bridge management strategies. More recent methods are focusing on the evaluation of the generalized failure cost, of a specific bridge derived from direct and indirect costs induced to the users/residents of the area exposed to the seismic hazard as a consequence of bridge collapse. However, when these approaches have to be applied to large transport networks, appear still very complex and computational demanding, and therefore simplified methods to evaluate the impact in terms of social cost related to the reduced efficiency of a transportation network due to potential bridge failure, are required.In this work, a simplified method for seismic retrofitting prioritization on a bridge stock is proposed, which is based on a “blended” approach considering specific fragility curves according to several bridge features and condition state, seismic inputs and generalized failure costs related to the transportation network. The effectiveness of the method has been showed on a case study of a local bridge stock placed in central Italy and the obtained results have been compared with those provided by more refined transport simulation models, on one hand, and by more traditional prioritization approaches, on the other. It is highlighted that this method can be very useful for transportation network managers with in a limited budget scenario, in case of lack of information about possible earthquakeinduced impacts on a transportation network efficiency.

The widespread diffusion of renewable energy sources calls for the development of high-capacity energy storage systems as the A-CAES(Adiabatic Compressed Air Energy Storage)systems.In this framework,low temperature(100℃–200℃)A-CAES(LT-ACAES)systems can assume a key role,avoiding some critical issues connected to the operation of high temperature ones.In this paper,two different LT-ACAES configurations are proposed.The two configurations are characterized by the same turbomachines and compressed air storage section,while differ in the TES section and its integration with the turbomachinery.In particular,the first configuration includes two separated cycles:the working fluid(air)cycle and the heat transfer fluid(HTF)cycle.Several heat exchangers connect the two cycles allowing to recover thermal energy from the compressors and to heat the compressed air at the turbine inlet.Two different HTFs were considered:air(case A)and thermal oil(case B).The second configuration is composed of only one cycle,where the operating fluid and the HTF are the same(air)and the TES section is composed of three different packed-bed thermal storage tanks(case C).The tanks directly recover the heat from the compressors and heat the air at each turbine inlet,avoiding the use of heat exchangers.The LT-ACAES systems were modelled and simulated using the ASPEN-Plus and the MATLAB-Simulink environments.The main aim of this study was the detailed analysis of the reciprocal influence between the turbomachinery and the TES system;furthermore,the performance evaluation of each plant was carried out assuming both on-design and off-design operating conditions.Finally,the different configurations were compared through the main performance parameters,such as the round-trip efficiency.A total power output of around 10 MW was set,leading to a TES tank volume ranging between 500 and 700 m^(3).The second configuration with three TES systems appears to be the most promising in terms of round-trip efficiency since the energy produced during the discharging
