Three Different Doses of Intrathecal Morphine as Part of a Multimodal Regimen for Post-Cesarean delivery Analgesia: A Randomized Double-Blinded Trial

Brazil is one of the countries that carry out cesarean delivery in the world. The pain after cesarean delivery (CD) is one of the main concerns of parturient. Intrathecal Morphine (ITM) is widely used for the treatment of pain after CD;however, the optimal dose with minimal side effects is to be established. Objectives: To compare the analgesia and side effects resulted by three different doses of ITM as part of a multimodal regimen for post-cesarean delivery analgesia. Methods: One hundred ninety-four patients were sampled in this randomized, double-blinded, three-arm study (group 0.075: ITM 0.75 mg;group 0.1: ITM 0.1 mg and group 0.15: ITM 0.15 mg). All patients received intravenous dexamethasone, dipyrone and ketoprofen. The primary endpoint was pain intensity measured at rest and on movement, while the secondary endpoint was overall satisfaction with pain relief, the incidence of nausea and vomiting, intensity of pruritus, and the use of additional tramadol, antipruritic and antiemetic use within 24 hours after postoperation. Results: The three groups showed similar pain intensity at rest and on movement. The pain intensity on movement was significantly higher than at rest in the groups (p 0.001). There were no significant differences among the groups regarding secondary endpoint. Conclusions: Our findings suggest that in cases of an elective CD, 0.075 mg of ITM produces postoperative analgesia of similar quality that provided by 0.1 or 0.15 mg ITM. The patients should have a multimodal analgesia approach that includes: ITM, anti-inflammatory agents and access to additional systemic analgesic if necessary.

REE Distribution Pattern in Plants and Soils from Pitinga Mine—Amazon,Brazil

The rare earth element contents of plant specimens of the families Rhamnaceae, Ampelozizyphus amazonicus Ducke (local name: Saracura-Mirá) and of Pteridófitas from genus Gleichenia sp. e Adiantum sp. (ferns) were determined and compared to those of the soils, in the Pitinga Mine area, Amazon, Brazil. The Pitinga mine district has large tin reserves genetically related to two granite bodies, Agua Boa e Madeira, both intrusive in volcanic rocks included in the Iricoumé Group. This deposit contains, also, bodies of cryolite and rare metals, such as Zr, Nb, Ta, Y and REE. The REE biogeochemical signatures, shown by the collected plants, reflect the patterns of the respective soils. The Eu and Ce anomalies shown by some plant samples are inherited from soils, as well. The higher contents of REE observed in fern samples confirm they are accumulators and reflect the abundance of REE in the soils of Pitinga Mine region. Additionally, that supports their potential use in geochemical exploration and bioremediation. The results of this study stress the importance of biogeochemical research integrated with geochemistry of soils, rocks and minerals.

Background: Suicide among physicians is a serious public health issue, with an extremely complex and multifactorial behavior. Aim: The aim of this study was to use the theme “Suicide among Physicians” to exemplify the analysis of methodological similarities between the scientific content available at MEDLINE and BVS databases, as scientific research tools. Methods: This is a systematic review with metanalysis. The following combinations of keywords were used for data search in the referred databases: “suicide” AND “physicians” AND “public heath”. Results: Three hundred and thirteen publications were identified, but only 16 studies were chosen. Great association was found between MEDLINE and BVS databases and the Odds Ratio regarding the theme: “Suicide among physicians”. Conclusions: Considering the similarities found in the utilization of the two analyzed databases, it was possible to identify that suicide among physicians is associated with the exercise of an important professional role in the society and in the workplace. Arboviruses and Sexually Transmitted Infections in the Bioceanic Route: Health Indicators in a Municipality

Background: the physical integration of South America through a Bioceanic Corridor has been idealized for a long time by the countries that make up Mercosur. This integration will have impacts on the population residing in the cities where the route will be built. Among them, Porto Murtinho, southern Mato Grosso on the border with Paraguay, stands out, where a bridge will be built over the Paraguay River that will link these two countries. Methods: a retrospective, quantitative and cross-sectional study of a descriptive type of documentary approach that sought to analyze the prevalence of arboviruses, Sexually Transmitted Infections (STIs) and Diseases Related to Inadequate Environmental Sanitation (DRIES) notified and stored in the National System of Notifiable Diseases (SINAN). The sample consisted of new cases of these pathologies in the municipality of Porto Murtinho registered between January 2017 and december 2020. Results: there was an increase of 10% in the total number of diseases within this period. The number of dengue cases increased 36 times;the incidence of Syphilis, HIV and HPV decreased;and, finally, maintenance of cases of Gonorrhea, Leptospirosis, Yellow Fever, Zika, Chikungunya, Hepatitis A and B. Conclusions: actions are needed to prevent the increase of these pathologies together with the construction of the Bioceanic Route, thus avoiding damage population health and increased consumption of government resources.
