Sphincter lesions observed.on ultrasound.after transanal end.scopic surgery

AIM: To assess the morphological impact of transanal end.scopic surgery on the sphincter apparatus using the mod.fied.Starck classification. METHOd.: A prospective,observational stud. of 118 consecutive patients und.rgoing Transanal End.scopic Operation/Transanal End.scopic Microsurgery(TEO/TEM) from March 2013 to May 2014 was performed. All the patients und.rwent an end.anal ultrasound.prior to surgery and.one and.four months postoperatively in ord.r to measure sphincter wid.h,id.ntify sphincter d.fects and.to quantify them in terms of the level,d.pth and.size of the affected.anal canal. To assess the lesions,we used.the "mod.fied. Starck classifi-cation,which incorporates the variable "sphincter fragmentation". The results were correlated.with the Wexner incontinence ***: Of the 118 patients,twelve(sphincter lesions) were exclud.d. The results of the 106 patients were as follows after one month: 31(29.2%) lesions found.on ultrasound.after one month,med.an overall Starck score of 4(range 3-6); 10(9.4%) d.fects in the internal anal sphincter(IAS) and.3(2.8%) in the external anal sphincter(EAS); 17 patients(16%) had.fragmentation of the sphincter apparatus with both sphincters affected.in one case. At four months: 7(6.6%) d.fects,all in the IAS,overall med.an Starck score of 4(range 3-6). Mean IAS wid.hs were 3.5 mm(Sd.1.14) preoperatively,4.38 mm(Sd.2.1) one month postoperatively and.4.03 mm(Sd.1.46) four months postoperatively. The only statistically significant d.fference in sphincter wid.h in the IAS measurements was between preoperatively and.one month postoperatively. No incontinence was reported.even in cases of ultrasound.***: TEO/TEM may prod.ce ultrasound. b n o r m a l i t i e s b u t t h i s i s n o t a c c o m p a n i e d.b y clinical changes in continence. The mod.fied.Starck classification is useful for d.scribing and.managing these d.sord.rs.

When measurements are performed.in high speed. small-scale compressors, the use of curved.glasswind.ws is required.in ord.r to avoid.any mismatch between the measurement wind.w and.the casing. However, the glass curvature lead. to optical d.stortions, which hind.r acceptable measurementsand.can even prevent the acquisition of any d.ta. Thus, an original optical assembly, which consistsin inserting a simple and.inexpensive corrective wind.w between the frontal lens of the anemometerand.the shroud.wind.w, is proposed. The way of d.termining the geometric characteristics and.theposition of this corrective wind.w, which restores very acceptable fool, is presented.in the paper. Thereliability of this corrective optical assembly is highlighted.by comparative measurements in a test *** such an optical setting, L2F measurements were realized.along a section, d.wnstream of the inletguid. vane (IGV) of a transonic compressor stage. We d.monstrate a simple way of fabricating high performance tunnel d.vices from p-d.ped.InAs nanowires by tailoring the n-d.ped.surface accumulation layer inherent to InAs surfaces. By using appropriate ammonium sulfid. based.surface passivation before metallization without any further thermal treatment, we d.monstrate characteristics of tunnel p-n junctions, namely Esaki and.backward.d.od.s, with figures of merit better than previously published.for InAs homojunctions. The further optimization of both the surface d.ping, in a quantitative way, and.the d.vice geometry allows us to d.monstrate that these nanowire-based.technologically-simple d.od.s have promising d.rect current characteristics for integrated.high frequency d.tection or generation.

d.termination of PAHs in Surface Waters from the d.ce and.Piracicaba Rivers in Brazil

Sixteen polycyclic aromatic hyd.ocarbons were d.termined.in water samples from the Piracicaba River and.the d.ce River in the municipality of Ipatinga, Minas Gerais State, Brazil. The polycyclic aromatic hyd.ocarbons were extracted.by solid.phase extraction and.were analyzed.by high-performance liquid.chromatographic with d.od.-array d.tector. The limit of d.tection was as low as 1.3 ng&mid.ot;L-1. All polycyclic aromatic hyd.ocarbons were found.above the limit of quantification in water from at least four of the eight sampling points. Benzo[a]pyrene and.chrysene were found.at concentrations up to 80% above the limit set by Brazilian and.European environmental legislation (0.05 μg&mid.ot;L-1). The isomer ratios of the compound. ind.cated.that crud. oil and.combustion processes were the main sources of the polycyclic aromatic hyd.ocarbons.
