Evidence-based medicine: An update on treatments for peritoneal dialysis-related peritonitis

Peritonitis continues to be a major complication of peritoneal dialysis （PD）, and adequate treatment is crucial for a favorable outcome. There is no consensus regarding the optimal therapeutic regimen, and few prospective controlled studies have been published. The objectiveof this manuscript is to review the results of PD peritonitis treatment reported in narrative reviews, systematic reviews, and proportional meta-analyses. Two narrative reviews, the only existing systematic review and its update published between 1991 and 2014 were included. In addition, we reported the results of a proportional meta-analysis published by our group. Results from systematic reviews of randomized control trials （RCT） and quasi-RCT were not able to identify any optimal antimicrobial treatment, but glycopeptide regimens were more likely to achieve a complete cure than a first generationcephalosporin. Compared to urokinase, simultaneous catheter removal and replacement resulted in better outcomes. Continuous and intermittent IP antibiotic use had similar outcomes. Intraperitoneal antibiotics were superior to intravenous antibiotics in reducing treatment failure. In the proportional meta-analysis of RCTs and the case series, the resolution rate （86%） of ceftazidime plus glycopeptide as initial treatment was signifcantly higher than frst generation cephalosporin plus aminoglycosides （66%） and glycopeptides plus aminoglycosides （75%）. Other comparisons of regimens used for either initial treatment or treatment of gram-positive rods or gram-negative rods did not show statistically significant differences. The superiority of a combination of a glycopeptide and a third generation cephalosporin was also reported by a narrative review study published in 1991, which reported an 88% resolution rate.

Analysis of Deformation in Ni-Cr Structures with New and Recast Alloys

The present study analyzed new and recast Ni-Cr alloys, regarding the relationship between the applied force and the deformation in cantilevered bar segments, with dimensions of 4.0 mm, 3.5 mm, 3.0 mm in thickness, 4.5 mm width and 15 mm length, on a universal testing machine “EMIC”. The bars in the tests were initially obtained acrylic resin by 4.8 mm wide × 4.3 mm thick × 4 cm long. We obtained 30 bars divided into two groups, with 15 to test new alloys and 15 with alloys recast. The alloy used was Tilite. For the application of the load, the bars were attached to “EMIC” where the active tip of 200 kgf load cell was at a specific point of the bar (15 mm) with a speed of 0.5 mm per minute. The data showed statistically significant differences in relation to alloys and thickness among the bars, and all thicknesses evaluated were different. Thus, it was concluded that there was statistically significant difference between the groups and their variables, and that the alloys recast could be reused at least 1 time, without loss of properties.

Preliminary Tests of a Kerma-Area Product (KAP) Meter to Be Used as a Monitor Chamber in a Conventional Diagnostic X Ray System

This study presents the results of the preliminary tests performed using a kerma-area product (KAP) meter in order to verify its usefulness as a monitor chamber in a clinical X ray system, which will be used in ionization chambers’ quality control program. The tests performed were repeatability, surface measurement uniformity, linearity with tube current, measurement variation changing the distance and field size, KAP chamber radiation attenuation, and energy dependence. The KAP meter has presented a good repeatability (a maximum variation of 0.4%);its surface has a homogeneous response (a maximum variation of 1.0% among the different irradiated regions);the response is linear with the tube current and the variation of the PKA along the distance is less than 3.43%. On the other hand, the results obtained for chamber attenuation and energy dependence show that some corrections must be applied when the measurements are made. The results have shown a good performance of the KAP meter, confirming that it is possible to use this instrument as a monitor chamber. However, it is important to highlight that some correction factors for energy dependence and chamber radiation attenuation must be considered.

Almost 110 years after the first studies by Dr. Carlos Chagas describing an infectious disease that was named for him, Chagas disease remains a neglected illness and a death sentence for infected people in poor countries. This short review highlights the enormous need for new studies aimed at the development of novel and more specific drugs to treat chagasic patients. The primary tool for facing this challenge is deep knowledge about the similarities and differences between the parasite and its human host.

Endoscopic ultrasound-guided vs endoscopic retrograde cholangiopancreatography biliary drainage for obstructed distal malignant biliary strictures: A systematic review and meta-analysis

BACKGROUND For palliation of malignant biliary obstruction(MBO), the gold-standard method of biliary drainage is endoscopic retrograde cholangiopancreatography(ERCP)with the placement of metallic stents. Endoscopic ultrasound(EUS)-guided drainage is an alternative that is typically reserved for cases of ERCP ***, however, there have been robust randomized clinical trials(RCTs)comparing EUS-guided drainage and ERCP as primary approaches to *** To compare EUS guidance and ERCP in terms of their effectiveness and safety in palliative biliary drainage for *** This was a systematic review and meta-analysis, in which we searched the MEDLINE, Excerpta Medica, and Cochrane Central Register of Controlled Trials databases. Only RCTs comparing EUS and ERCP for primary drainage of MBO were eligible. All of the studies selected provided data regarding the rates of technical and clinical success, as well as the duration of the procedure, adverse events, and stent patency. We assessed the risk of biases using the Jadad score and the quality of evidence using the Grading of Recommendations Assessment,Development and Evaluation *** The database searches yielded 5920 records, from which we selected 3 RCTs involving a total of 222 patients(112 submitted to EUS and 110 submitted to ERCP). In the EUS and ERCP groups, the rate of technical success was 91.96%n and 91.81%, respectively, with a risk difference(RD) of 0.00%(95%CI:-0.07, 0.07;P = 0.97; I^2 = 0%). The clinical success was 84.81% and 85.53% in the EUS and ERCP groups, respectively, with an RD of-0.01%(95%CI:-0.12, 0.10; P = 0.90; I^2 =0%). The mean difference(MD) for the duration of the procedure was-0.12%(95%CI:-8.20, 7.97; P = 0.98; I^2 = 84%). In the EUS and ERCP groups, there were14 and 25 adverse events, respectively, with an RD of-0.06%(95%CI:-0.23, 0.12; P= 0.54; I^2 = 77%). Simultaneous Determination of Ten Endocrine Hormone Disrupters in Water Using SPE/LC-(ESI)MS-MS

The aim of this study was developed and validated an analytical method based on liquid chromatography and tandem mass spectrometry after solid phase extraction to monitorizing ten endocrine hormone disrupters in Lisbon drinking water system. Natural and synthetic hormones (17-β-estradiol, ethinylestradiol, estriol, estrone, progesterone, mestranol and diethylstilbestrol) and some industrial products (4-n-nonylphenol, 4-tert-octylphenol and bisphenol A) were studied. Mass spectrometer detection parameters were optimized, such as the best conditions for the precursor ion formation, namely cone voltage, when applying negative and positive electrospray ionization, and also collision energy for MRM1 and MRM2 transitions. The best conditions of the solid phase extraction (SPE) using Waters Oasis HLB (6 mL, 200 mg) and Isolute C18 (EC) (6 ml, 1000 mg) were also optimized. The method was validated through the application of several statistical tests and the uncertainty estimation of the analytical assay. This method showed a very good linear range for all the studied analytes with determination coefficients (r2) between 0.9962 and 0.9999 and coefficients of variation lower than 4%. There were no significant differences between recoveries obtained with the studied matrices, like groundwater, surface water and water for human consumption. In these matrices, the recovery values varied between 32 and 95%. The limits of method detection were between 0.28 and 22 ng/L. The validated method was applied for the analysis of water samples from the EPAL (Empresa Portuguesa das águas Livres, S.A.) water supply system including tap water, spring water, groundwater, and river water. Some target compounds (bisphenol A, progesterone, 4-tert-octylphenol, and 4-n-nonylphenol) were found in trace amounts in analysed waters.

Evaluating Nitrogen Behavior in Sugarcane after Fertilization Using Leaf and Sap Extract Analyzes

Nitrogen (N) fertilization is important for sugarcane crops, for both agronomic and environmental reasons. Better use of N from fertilizer is required for crop management, and in order to achieve it, adequate N nutrition monitoring is necessary. Sap extract analysis is a sensitive technique that has been adopted in other crops to tackle this objective, and can be used for sugarcane. The aim of this study was to evaluate the behavior of N and the response of sugarcane after N fertilization, using sap and leaf tissue analysis. The study was undertaken in two experimental areas in 2012/2013 season, using ratoon cane, following application of 0, 50, 100, and 150 kg·ha-1 of N. The youngest visible leaves were sampled from sugarcane at five time points between 0 and 150 days after fertilization, to evaluate the nitrate, ammonium, and total N content of sap extract, and total N content of leaf tissue. At the first sampling point (30 days after N fertilizer application), mineral N in sap increased as a result of N fertilization in medium-textured soil, and remained high until 120 days after fertilization in clayey soil. The total N content of sap followed a similar behavior. Stalk yield was only observed to increase in sugarcane grown in the clayey soil. Sap analysis proved to be an efficient method for monitoring N status in sugarcane grown in different soil types.

In order to further improve the impact of the continuously evolving neurointerventional techniques, the interventional neuroradiologist needs to have at his disposal more powerful techniques to image the central nervous system. With the recent development of diagnostics techniques that are computed tomography and magnetic resonance based we are now able to assess not just morphology, but also physiology, physiopathology and function. This review discusses the place of diagnostic techniques in the evaluation that the interventional neuroradiologist hast to make when confronted with patients. We provide an overview of current techniques for the brain and spine.
