Effectiveness of Bilateral Pulmonary Artery Banding in Patients with Hypoplastic Left Heart Syndrome and Congenital Heart Defects with a Functional Single Ventricle: A Single-Center Retrospective Study

Background:Bilateral banding of the branches of the pulmonary artery in patients with hypoplastic left heart syndrome(HLHS)and other duct dependent critical neonatal heart malformations can significantly reduce the incidence of severe complications in the postoperative period,especially in severely unstable patients.In our study we compared different surgical techniques of bilateral pulmonary artery banding(PAB)in respect to their success in balancing systemic and pulmonary blood flow.Methods:We included 44 neonates with a HLHS and congenital heart diseases(CHD)with a functional single ventricle underwent a hybrid operation:bilateral PAB and patent ductus arteriosus stenting.The hybrid surgery for method No.1 is performed as a one-stage procedure,together with patent ductus arteriosus(PDA)stenting.After median sternotomy,two Gore-Tex 1–2 mm wide bands with a diameter of 3–3.5 mm are put.When we apply method No.2 then the thread is used to create bands.Method No.3 is distinguished by intraoperative assessment of blood flow at the site of narrowing of the branches of the pulmonary artery and optional stenting of the PDA.The cuff for banding is made of Gore-Tex tubing.Effectiveness when applying method Nos.1 and 2 is assessed by the change in invasive blood pressure and oxygen saturation after narrowing of the branches of the pulmonary artery.Also,with these techniques PDA stenting by inserting the introducer via pulmonary artery trunk is performed.Results:HLHS with mitral or aortic valve atresia or both was present in 19 patients(43.1%),with severe left heart obstruction resulting in PDA dependent systemic circulation in 16 babies(36.4%).CHD with single ventricle physiology occurred in 9 patients(20.5%).14 babies(31.8%)undergo the procedure following the method No.1,8 patients(18.2%)method No.2 and 22 patients(50%)method No.3.Qp/Qs=1/1 was achieved in 30 patients(30/44,68.1%):as a result of the method No.1 was achieved in 5 patients(5/14,35.7%),method No.2 in 4 patients(4/8,50%),method No.3 in 21 patients(21/22,95.

A New Three-Dimensional(3D)Printing Prepress Algorithm for Simulation of Planned Surgery for Congenital Heart Disease

Background:Three-dimensional printing technology may become a key factor in transforming clinical practice and in significant improvement of treatment outcomes.The introduction of this technique into pediatric cardiac surgery will allow us to study features of the anatomy and spatial relations of a defect and to simulate the optimal surgical repair on a printed model in every individual case.Methods:We performed the prospective cohort study which included 29 children with congenital heart defects.The hearts and the great vessels were modeled and printed out.Measurements of the same cardiac areas were taken in the same planes and points at multislice computed tomography images(group 1)and on printed 3D models of the hearts(group 2).Pre-printing treatment of the multislice computed tomography data and 3D model preparation were performed according to a newly developed algorithm.Results:The measurements taken on the 3D-printed cardiac models and the tomographic images did not differ significantly,which allowed us to conclude that the models were highly accurate and informative.The new algorithm greatly simplifies and speeds up the preparation of a 3D model for printing,while maintaining high accuracy and level of detail.Conclusions:The 3D-printed models provide an accurate preoperative assessment of the anatomy of a defect in each case.The new algorithm has several important advantages over other available programs.They enable the development of customized preliminary plans for surgical repair of each specific complex congenital heart disease,predict possible issues,determine the optimal surgical tactics,and significantly improve surgical outcomes.

Critical aortic valve stenosis in newborns is the cause of a severe clinical condition with the onset of symptoms during first hours after birth.We present a clinical case of a successful surgical correction of a critical aortic stenosis using a hybrid method applied in a newborn during the first day of life.The infant was diagnosed with a hypoplastic left heart complex with an intact atrial septum(aortic and mitral valves stenosis variant),that led to the cardiogenic shock and acute pulmonary edema.The procedure included bilateral banding of the pulmonary artery branches and atrioseptostomy with stenting of the interatrial septum.The surgery was performed through a median sternotomy.

Probiotics and autoprobiotics for treatment of Helicobacter pylori infection

The article discusses various approaches for probiotic treatment of Helicobacter pylori(H.pylori)infection:Probiotics as an adjuvant treatment in the standard eradication therapy;probiotic strains as a monotherapy;and autoprobiotics as a monotherapy.Autoprobiotics refer to indigenous bifidobacteria,lactobacilli,or enterococci isolated from a specific individual,intended to restore his/her microbiota and improve his/her health.The potential mechanisms of probiotic action against H.pylori include correction of the gut microbiota,immunological effects(enhancement of humoral and cellular immunity,and reduction of oxidative stress),direct antagonistic effects against H.pylori(such as colonization resistance and bacteriocin synthesis),and stimulation of local immunological protection(strengthening of the mucous protective barrier and reduction of gastric mucosa inflammation).The incorporation of probiotics into comprehensive eradication therapy shows promise in optimizing the treatment of H.pylori infection.Probiotics can enhance the eradication rates of H.pylori,reduce the occurrence and severity of side effects,and improve patient compliance.Probiotic or autoprobiotic monotherapy can be considered as an alternative treatment approach in cases of allergic reactions and insufficient effectiveness of antibiotics.We recommend including probiotics as adjunctive medications in anti-H.pylori regimens.However,further randomized multicenter studies are necessary to investigate the effects of probiotics and autoprobiotics against H.pylori,in order to gain a better understanding of their mechanisms of action.

Seismicity,structure and tectonics in the Arctic region

The "Arctic" region,where the North Pole occupies the center of the Arctic Ocean,has been affecting the environmental variation of the Earth from geological time to the present.However,the seismic activities in the area are not adequately monitored.Therefore,by conducting long term monitoring of seismic phenomenon as sustainable parameters,our understanding of both the tectonic evolution of the Earth and the dynamic interaction between the cryosphere and geosphere in surface layers of the Earth will increase.In this paper,the association of the seismicity and structure of the Arctic region,particularly focused on Eurasian continent and surrounding oceans,and its relationship with regional evolution during the Earth’s history is studied.The target areas cover representative tectonic provinces in the Eurasian Arctic,such as the wide area of Siberia,Baikal Rift Zone.Far East Russia,Arctic Ocean together with Greenland and Northern Canada.Based on discussion including characteristics of seismicity,heterogeneous structure of the crust and upper mantle,tectonic history and recent dynamic features of the Earth’s surface in the Arctic are summarized.

The normal cell proliferation and wound healing effect of polysaccharides from Ganoderma amboinense

To study the cell proliferation and wound healing activity of polysaccharides from Ganoderma amboinense(GAMPS),the polysaccharide was extracted by water extraction and alcohol precipitation method,and its monosaccharide composition and molecular weight were analyzed.The effects of different concentrations of GAMPS on the cell proliferation were determined by cell survival rate test,and the wound healing ability of GAMPS to NIH/3T3 cells was detected.The preliminary evaluation of the antioxidant ability of GAMPS was conducted by the oxygen radical absorbance capacity(ORAC).The results showed that the GAMPS was composed of glucose,mannose,and galactose at a molar ratio of 67.62:14.07:7.50,and the weightaverage molecular weights were 5.439×10^(6) and 1.704×10^(5) g/mol by using high-performance gel-permeation chromatography-multiple angle laser scatter(HPGPC-MALS)analysis.GAMPS(0.2μg/μL)showed the strongest proliferation ability to THP-1 cells,with cell survival rate of 178.7%.The wound healing effect of GAMPS(0.1μg/μL)was obvious on NIH/3T3 and 3.75μg/μL of GAMPS showed the strongest total antioxidant ability.All the results indicate that GAMPS promotes cell proliferation,and has cell wound healing effect and strong antioxidant activity.The results provide theoretical foundation for the development and utilization of GAMPS.

The perspectives of the means of metabolic syndrome correction in Xinjiang-Uyghur region of China

There has been proved the link between the intestinal microbiota composition characteristics and the development of diseases,in particular colorectal cancer,kidney failure,irritable bowel syndrome,intestinal infections,obesity,type I diabetes and metabolic syndrome( MS). Metagenomic studies demonstrated the presence of significant differences in the structure of the intestinal microbiota in populations of Europeans and Asians. In this aspect,the Xinjiang Uygur Autonomous region of China is the most affected district,where substantially the indigenous Uighur,Kazakh and also Han populations is affected. There is an objective need to ascertain the microecological and metabolic status of persons of the indigenous population in Western China,which are in the risk zone. The test for microalbuminuria is reliable method of kidney failure primary stages diagnosis,reflecting the initial stages of vessel pathology and the development of MS. It invariably correlates with an increase of cardiovascular morbidity and mortality. We introduce this screening test for early detection of predisposition to the development of the MS in the example of Xinjiang population for use also in other regions of China.There is a need to carry out the search,selection and analysis of the new strains of probiotics in the Xinjiang from national milk products with lactic acid or other fermented products with the purpose of studying their clinical characteristics. This will have an impact on increasing life expectancy and improving the quality of life of the population of Xinjiang.
