Adverse events associated with the gold probe and the injection gold probe devices used for endoscopic hemostasis:A MAUDE database analysis

BACKGROUND Gastrointestinal(GI)bleeding accounts for over half a million admissions annually and is the most common GI diagnosis requiring hospitalization in the United *** electrocoagulation devices are used for the management of gastrointestinal *** is no data on device-related adverse events for gold probe(GP)and injection gold probe(IGP).AIM To analyze this using the Food and Drug Administration(FDA’s)Manufacturer and User Facility Device Experience(MAUDE)database from 2013 to *** We examined post-marketing surveillance data on GP and IGP from the FDA MAUDE database to report devicerelated and patient-related adverse events between *** MAUDE database is a publicly available resource providing over 4 million records relating to medical device *** analyses were performed using IBM SPSS Statistics V.27.0(IBM Corp.,Armonk,NY,United States).RESULTS Our search elicited 140 reports for GP and 202 reports for IGP,respec-tively,during the study period from January 2013 to August *** reportedly occurred in 130 cases for GP,and actual patient injury or event occurred in 10 patients.A total of 149 patients(74%)reported with Injection GP events suffered no significant consequences due to the device failure,but 53 patients(26%)were affected by an *** GP and IGP are critical in managing gastrointestinal *** study of the FDA MAUDE database revealed the type,number,and trends of reported device-related adverse *** endoscopist and support staff must be aware of these device-related events and be equipped to manage them if they occur.

Transaminitis is an indicator of mortality in patients with COVID-19:A retrospective cohort study

BACKGROUND Since its discovery in Wuhan,China in December of 2019,the novel coronavirus has progressed to become one of the worst pandemics seen in the last 100 ***,there has been an increased interest in the hepatic manifestations of coronavirus disease 19(COVID-19).AIM To describe the demographic and clinical characteristics of COVID-19 positive patients and study the association between transaminitis and all-cause *** This is a descriptive retrospective cohort study of 130 consecutive patients with a positive COVID PCR test admitted between March 16,2020 to May 14,2020 at a tertiary care University-based medical *** Wilcoxon-rank sum test and paired t-test were used for comparing non-parametric and parametric continuous variables respectively and a multivariable logistic regression models to study the association between transaminitis and mortality using SAS version 9.4(SAS Institute,Cary,NC,United States).RESULTS Out of the 130 patients,73(56%)patients were found to have transaminitis and 57(44%)did *** compared to patients without transaminitis,the transaminitis group was found to have a higher median body mass index(30.2 kg/m2 vs 27.3 kg/m2,P=0.04).In the multivariate analysis those with transaminitis were found to have 3.4 times higher odds of dying as compared to those without transaminitis adjusting for gender,the Age-adjusted Charlson Comorbidity Index and admission to the intensive care unit(P=0.03).CONCLUSION Our study showed that transaminitis on admission was associated with severe clinical outcomes such as admission to the intensive care unit,need for mechanical ventilation,and mortality.

The aim of the present work is to develop rifampicin loaded phospholipid lipospheres containing sulfphobutyl etherβ-cyclodextrin and Vitamin C for inhalation to test their potential for deep lung *** findings of the solid state characterization revealed the amorphous nature of the *** exhibited a better flowability,an aerodynamic diameter in the range of 1.76 to 3.99μ***,the fine particle fraction and emitted dose was found in the range of 68.84–83.73% and 80–93%,***,lipospheres exhibited enhanced/equivalent efficacy in vitro in H37Rv ***,the results show the potential of lipospheres for pulmonary delivery of rifampicin.

Optimal PV Array Configuration for Extracting Maximum Power Under Partial Shading Conditions by Mitigating Mismatching Power Losses

The grid-connected or standalone PV central inverter architecture is comprised of several PV modules which are connected in different ways to form the PV *** power generation capability of the PV array is primarily affected by partial shading conditions(PSC).Due to PSCs,the power output of the PV array is dramatically reduced,and mismatching losses are induced in the PV *** on the extent of these problems,multiple peaks also appear in the power-voltage(P-V)curve,which makes it very difficult to track the global maximum power point(GMPP).The main objective of this research paper is to model and simulate the series(S),series-parallel(SP),bridge-link(BL),honey-comb(HC),total-cross-tied(TCT)and proposed triple-tied(TT)solar PV array configurations under various partial shading *** performance of all PV configurations is evaluated under a uniform approach,considering eight different shading *** performance of the considered PV configurations is analyzed in terms of their mismatching power losses,fill factors,efficiency,global maximum power points(GMPPs),local maximum power points(LMPPs),voltages and currents at GMPPs,open circuit voltage and short circuit *** above-mentioned PV configurations are modeled and simulated in a Matlab/Simulink environment by considering the KC-200GT module parameters.

Performance Investigation of Hybrid and Conventional PV Array Configurations for Grid-connected/Standalone PV Systems

Demand response based congestion management in a mix of pool and bilateral electricity market model

The independent system operator （ISO） is a key element in the deregulated structure with one of the responsibilities of transmission congestion management （CM）. The ISO opts market based solutions to manage congestion receiving bids from generation companies （GENCOs） as well as distribution companies （DISCOMs） to reschedule their generation and relocate demand. The nodal prices increases during the congestion hours and the demand response to nodal prices will be an effective tool for the control of congestion. In this paper, demand response-based CM has been proposed for a mix of pool and bilateral electricity market model. The linear bid curves have been considered for demand bids to respond to the congestion in the network. The bilateral demand has been obtained with minimum deviations in their preferred schedule. The impact of flexible alternating current transmission system （FACTS） devices viz static var compensator （SVC） and thyristor controlled series compensator （TCSC） has also been considered for demand management during congestion. Multi-line congestion cases have been considered to study the impact on demand response without and with FACTS devices. The proposed approach has been tested on the IEEE 24 bus test system.

Mechanisms of NLRP3 inflammasome-mediated hepatic stellate cell activation: Therapeutic potential for liver fibrosis

The liver injury leads to an inflammatory response, which causes the activation of hepatic stellate cells (HSCs) that further secrete ECM proteins and play an important role in liver fibrosis. Moreover, the inflammatory response is a driving force for fibrogenesis, which is triggered by many types of injuries. Exaggerated inflammatory immune responses are mediated by cytoplasmic protein complexes known as inflammasomes, which are involved in many chronic liver diseases. Inflammasomes are pattern recognition receptors (PRRs) that can sense any microbial motifs known as pathogen-associated molecular patterns (PAMPs), and host- or environmental-derived stress signals known as damage-associated molecular patterns (DAMPs). The inflammasomes cause caspase-mediated proteolytic cleavage of pro-IL-1β and pro-IL-18 into active IL-1β and IL-18. In this review, we provide a comprehensive summary of the important roles of NLRP3 inflammasome in the pathogenesis of liver fibrosis with an emphasis on several direct and indirect pathways responsible for the NLRP3 inflammasome-mediated HSCs activation and fibrogenesis. In addition, we discuss the general pharmacological and genetics strategies for the inhibition of NLRP3 inflammasome activation and its downstream signaling with examples of emerging pharmacotherapeutics, targeting the NLRP3 inflammasome signaling as well as a possible way to develop effective and safer NLRP3 inflammasome inhibitors.

Renewable energy-based solar photovoltaic(PV)generation is the best alternative for conventional energy sources because of its natural abundance and environment friendly *** power extraction from the PV system plays a critical role in increasing the efficiency of the solar power generation during partial shading conditions(PSCs).Therefore,a suitable maximum power point tracking(MPPT)technique to track the maximum power point(MPP)is of high need,even under *** paper presents an organized and concise review of MPPT techniques implemented for the PV systems in literature along with recent publications on various hardware design *** classification is done into four categories,***,intelligent,optimal,and hybrid depending on the tracking algorithm utilized to track MPP under *** uniform insolation,classical methods are highly preferred as there is only one peak in the P-V ***,under PSCs,the F-V curve exhibits multiple peaks,one global maximum power point(GMPP)and remaining are local maximum power points(LMPP’s).Under the PSCs,classical methods fail to operate at GMPP and hence there is a need for more advanced MPPT *** MPPT technique has its advantages and limits,but a streamlined MPPT is drafted in numerous parameters like sensors required,hardware implementation,cost viability,tracking speed and tracking *** study provides the advancement in this area since some parameter comparison is made at the end of every classification,which might be a prominent base-rule for picking the most gainful sort of MPPT for further research.
