Background: Spontaneous gallbladder perforation(GBp) is an uncommon diagnosis. This study presented the experience of managing spontaneous GBp over nine years at a large, tertiary care university hospital in north India and investigated the outcomes and treatment strategies. Methods: A retrospective review of prospectively maintained digital database of consecutive patients was performed. All patients received medical and/or surgical treatment for spontaneous GBp in our department between January 2010 and June 2018. Results: We identifed 151 patients(81 females and 70 males) with mean age of 53 years. Most common presenting features were pain(96.7%), fever(54.3%) and jaundice(31.1%). Most common cause was gallbladder stones(84.8%) followed by common bile duct stones(30.5%), xanthogranulomatous cholecystitis(17.9%) and malignancy(11.9%). As per Niemeier classifcation, 8.6% had type 1 GBp(free perforation in peritoneal cavity), 76.2% had type 2 GBp(localized perforation) and 13.2% had type 3 GBp(cholecystoenteric fstula). About 60% of the perforations were diagnosed preoperatively. Type 1 was more common in patients with diabetes and also had the worst prognosis. Surgery was performed in 109 patients(72.2%). Seven patients(4.6%) had a postoperative morbidity of Clavien-Dindo Ⅲ or higher. There were three mortalities in patients who underwent surgery. Conclusions: High index of suspicion is required for preoperative diagnosis of GBp, especially in types 2 and 3. Laparoscopic cholecystectomy can be difficult in these patients and patients may require open or partial cholecystectomy. Early diagnosis and step-up approach for the treatment of GBp is critical.

The radiant systems consume less energy and provide better thermal comfort as compared to the conventional system,but the main drawback of radiant technology is inability to cater the latent load and *** problem has engaged researchers to push their emphasis on personalized Conditioning System(pCS).It forms a micro-climate region near the *** study is aimed at pCS combined with a radiant conditioning system,which evaluates the performance of the pCS and compares it with different radiant cooling systems.A radiant cubicle was used to investigate the performance of the pCS for both heating&cooling *** heating mode,the Standard Effective Temperature(SET)was calculated with the help of thermal comfort surveys which were in a range of 23.4℃ to 27.3℃.For cooling mode,four different cases compared the performance of the system which are:Conventional Air Conditioning System(CACS),Radiant Cooled Ceiling System(RCCS),personalized Radiant Conditioning System(pRCS),and both pRCS-RCCS *** was observed that pRCS alone reaches the thermal comfort criteria with low energy *** energy-saving of RCCS-pRCS was achieved 38%when compared with *** can be concluded that pRCS is a new step in the field of thermal comfort.

On-table hepatopancreatobiliary surgical consults for difficult cholecystectomies:A 7-year audit

Background: Cholecystectomy is considered a general surgical operation. However, general surgeons are not trained to manage severe complications such as bile duct injury(BDI) and should refer to hepatopancreatobiliary(HpB) surgeons when difficulty arises. This study aimed to investigate the outcomes of patients who had on-table HpB consults during cholecystectomy. Methods: This is an audit of 50 patients who required on-table HpB consult during cholecystectomy from 2011 to 2017. Consultations were classified as “proactive” and “reactive”, where consults were made before or after surgical incision, respectively. patient demographics and perioperative details were collected. Results: The median age of the patients was 62.5 years [interquartile range(IQR) 50.8–71.3 years]. Eight(16%) patients had underlying HpB co-morbidity. Gallbladder wall was thickened in all patients(median 5 mm, IQR 4–7 mm), and common bile duct was of normal caliber in all patients(median 5 mm, IQR 4–6 mm). Median length of operation and length of stay were 165 min(IQR 124–209 min) and five days(IQR 3–7 days), respectively. Subtotal cholecystectomy was performed in 18(36%) patients. Forty-eight patients were initially managed by laparoscopic approach, 15(31%) required open conversion;majority(9/15, 60%) were initiated before on-table consult. Majority of referrals(98%) were reactive. Common reasons for referral included unclear anatomy or anatomical variations(30%), presence of dense adhesions and/or contracted gallbladder(18%) and impacted stones in Hartmann’s pouch(16%). Three(6%) patients were referred for BDI(2 Strasberg D and 1 Strasberg E1), and two(4%) were referred for torrential bleeding from arterial injury(1 cystic artery and 1 right hepatic artery). Any morbidity and 30-day readmission were 22% and 6%, respectively. There was no 90-day mortality. Conclusions: Calling for help in BDI is obligatory, but in other instances is a personal choice. Wind turbines（WTs） are quite expensive pieces of equipment in power industry. Maintenance and repair is a critical activity which also consumes lots of time and effort, hence making it a costly affair. Carefully planning the maintenance based upon condition of the equipment would make the process reasonable. Mostly the WTs are equipped with some kind of condition monitoring device/system, which provides the information about the device to the central data base i.e., supervisory control and data acquisition（SCADA） data base. These devices/systems make use of data processing techniques/methods in order to detect and predict faults. The information provided by condition monitoring equipments keeps on recoding in the SCADA data base. This paper dwells upon the techniques/methods/algorithms developed, to carry out diagnosis and prognosis of the faults, based upon SCADA *** data driven approaching for SCADA data interpretation has been reviewed and an artificial intelligence（AI） based framework for fault diagnosis and prognosis of WTs using SCADA data is proposed.

Biophysical quantification of biodiversity and ecosystems services of forest ecosystems in the Western Ghats: a case study of Uttara Kannada District, India

Biodiversity and ecosystem services play key roles in future economic strategies seeking to promote development and prosperity. This study assesses the status of biodiversity and flow of ecosystem services from selected forest types in the Western Ghats. At the sampling sites, the number of tree species ranged from 16 to 79 per hectare depending on the forest type. The estimates for Shannon–Wiener index for trees in the evergreen, moist deciduous and dry deciduous forest plots were 3.02, 2.9 and 1.54, respectively. The total biomass carbon stocks in evergreen, moist deciduous and dry deciduous forests in the study area was 229, 221 and 189 t C/ha, *** of dependency patterns of local communities on forest resources indicated a high dependency on provisional services such as fuelwood, manure and fodder. The study records the highest dependency rates for fuelwood,ranging between 72 and 100% for the three forest types and indicates that forest ecosystems underpin the well-being of the population dependent directly and indirectly on them.

The synthesis of visual information from multiple medical imaging inputs to a single fused image without any loss of detail and distortion is known as multimodal medical image *** improves the quality of biomedical images by preserving detailed features to advance the clinical utility of medical imaging meant for the analysis and treatment of medical *** study develops a novel approach to fuse multimodal medical images utilizing anisotropic diffusion(AD)and non-subsampled contourlet transform(NSCT).First,the method employs anisotropic diffusion for decomposing input images to their base and detail layers to coarsely split two features of input images such as structural and textural *** detail and base layers are further combined utilizing a sum-based fusion rule which maximizes noise filtering contrast level by effectively preserving most of the structural and textural *** is utilized to further decompose these images into their low and high-frequency *** coefficients are then combined utilizing the principal component analysis/Karhunen-Loeve(pCA/KL)based fusion rule independently by substantiating eigenfeature reinforcement in the fusion *** NSCT-based multiresolution analysis is performed on the combined salient feature information and the contrast-enhanced fusion ***,an inverse NSCT is applied to each coef-ficient to produce the final fusion *** results demonstrate an advantage of the proposed technique using a publicly accessible dataset and conducted comparative studies on three pairs of medical images from different modalities and *** approach offers better visual and robust performance with better objective measurements for research development since it excellently preserves significant salient features and precision without producing abnormal information in the case of qualitative and quantitative analysis.

Obesity causes extracellular matrix (ECM) remodelling which can develop into serious pathology and fibrosis, having metabolic effects in insulin-sensitive tissues. The ECM components may be increased in response to overnutrition. This review will focus on specific obesity-associated molecular and pathophysiological mechanisms of ECM remodelling and the impact of specific interactions on tissue metabolism. In obesity, a complex network of signalling molecules such as cytokines and growth factors has been implicated in fibrosis. Increased ECM deposition contributes to the pathogenesis of insulin resistance at least in part through the activation of cell surface integrin receptors and CD44 signalling cascades. These cell surface receptors transmit signals to the cell adhesome which orchestrates an intracellular response that adapts to the extracellular environment. Matrix proteins, glycoproteins, and polysaccharides interact through ligand-specific cell surface receptors that interact with the cytosolic adhesion proteins to elicit specific actions. Cell adhesion proteins may have catalytic activity or serve as scaffolds. The vast number of cell surface receptors and the complexity of the cell adhesome have made study of their roles challenging in health and disease. Further complicating the role of ECM-cell receptor interactions is the variation between cell types. This review will focus on recent insights gained from studies of two highly conserved, ubiquitous axes and how they contribute to insulin resistance and metabolic dysfunction in obesity. These are the collagen-integrin receptor-Ipp (ILK-pINCH-parvin) axis and the hyaluronan-CD44 interaction. We speculate that targeting ECM components or their receptor-mediated cell signalling may provide novel insights into the treatment of obesity-associated cardiometabolic complications.
