Vidangadi churna is a popular Ayurvedic formulation described in the chapter Krimicikitsa of the Ayurvedic literature Cakradatta for the treatment of Krimiroga. The preparation is a composite mixture of the fine powder of fruits of Vidang （Embelia ribs）, glandular trichomes of the fruits of Kamala （Mallotus philippensis）, mature fruits of Harde （Terminalia chehula）, Saindhava and Yavakshara. The use of reversed phase C18 column eluted with gradient mobile phase of acetonitrile and water enabled the efficient separation of the chemical markers in 22 rain. Validation of the method was performed in order to demonstrate its selectivity, accuracy, precision, repeatability and recovery. All calibration curves showed good linear correlation coefficients （r2〉0.995） within the tested ranges. Three markers in Vidangadi churna were quantitied with respect to Embelin （0.647%, w/w）, Rottlerin （4.419%, w/w）, and Ellagic acid （0.459%, w/w）. lntraand inter-day RSDs of retention times and peak areas were less than 3.12%. The recoveries were between 99.66% and 102.33%. In conclusion, a method has been developed for the simultaneous quantification of three markers in Vidangadi churna. The RP-HPLC method was simple, precise and accurate and can be used for the quality control of the raw materials as well as formulations.

Fractures of the talus are rare in children.A high index of suspicion is needed to avoid missing such an injury,which is not an uncommon occurrence especially with undisplaced *** present an unusual case of an undisplaced talar neck fracture in a five-year-old child leading to a delayed presentation of a symptomatic osteochondral loose body in the ankle *** our knowledge there are no reports in the literature of osteochondral loose bodies occurring in conjunction with an associated undisplaced talar neck fracture in either children or *** loose body was removed using anterior ankle *** child had an uneventful post operative recovery and regained full range of movement and function of his ankle joint and was discharged at one year *** aim to highlight the need to have a low threshold to further evaluate symptomatic children after fracture healing of an undisplaced talar neck fracture for a possible associated loose bodyin the ankle joint.

BACKGROUND Understanding how patients choose a provider may improve the overall experience by identifying ways to tailor a clinical *** To identify factors that patients consider important when choosing an orthopedic *** New patients presenting to an orthopedic oncology clinic within a tertiary academic medical center from January 2019 to August 2020 were invited to complete an anonymous *** questionnaire consisted of 27 items including a Likert-type assessment of the importance of selection *** A total of 101 new patients with a median age of 66 years(range,14 years to 91 years)*** were referred by another doctor(n=63,62.4%),and of the referring providers,the most frequent specialty was orthopedic surgery(n=32,51%).Using a Likert-type scale with 1 representing‘least important’and 5 representing‘most important’,the most important factor was the hospital reputation(mean,4.65;SD,0.85).Additional factors of importance were the number of years in practice(3.87±1.3)and a primary care provider referral(3.71±1.6).Patients younger than 40 years old found social media(P=0.016)and internet presence(P=0.035)of their surgeon to be more important than older *** contrast,older patients considered care within an academic center to be of greater importance than younger patients(P=0.014).CONCLUSION This investigation suggests a primary care referral,as well as hospital and physician reputation,are among the most important factors when selecting an orthopedic ***,social media utilization appears to be more important for younger patients.

Coagonist of glucagon-like peptide-1 and glucagon receptors ameliorates kidney injury in murine models of obesity and diabetes mellitus

AIM To investigate the role of glucagon-like peptide-1(GLP-1)/glucagon receptors coagonist on renal dysfunction associated with diabetes and obesity. METHODS Chronic high-fat diet fed C57 BL/6 J mice, streptozotocintreated high-fat diet fed C57 BL/6 J mice and diabeticC57 BLKS/J db/db mice were used as models of diabetes-induced renal dysfunction. The streptozotocintreated high-fat diet fed mice and db/db mice were treated with the GLP-1 and glucagon receptors coagonist(Aib2 C24 Chimera2, 150 μg/kg, sc) for twelve weeks, while in chronic high-fat diet fed mice, coagonist(Aib2 C24 Chimera2, 150 μg/kg, sc) treatment was continued for forty weeks. Kidney function, histology, fibrosis, inflammation, and plasma biochemistry were assessed at the end of the treatment. RESULTS Coagonist treatment decreased body weight, plasma lipids, insulin resistance, creatinine, blood urea nitrogen, urinary albumin excretion rate and renal lipids. In kidney, expression of lipogenic genes(SREBP-1 C, FAS, and SCD-1) was decreased, and expression of genes involved in β-oxidation(CPT-1 and PPAR-α) was increased due to coagonist treatment. In plasma, coagonist treatment increased adiponectin and FGF21 and decreased IL-6 and TNF-?. Coagonist treatment reduced expression of inflammatory(TNF-α, MCP-1, and MMP-9) and pro-fibrotic(TGF-β, COL1 A1, and α-SMA) genes and also improved histological derangement in renal *** Coagonist of GLP-1 and glucagon receptors alleviated diabetes and obesity-induced renal dysfunction by reducing glucose intolerance, obesity, and hyperlipidemia.

Jejunal diverticulosis (JD) is a rare clinical entity. The potential complications of this condition are discussed here through a series of cases presented to our centre. A retrospective analysis of four cases, which were diagnosed and treated, was performed. These included two cases of gastrointestinal haemorrhage, one case of perforation and one case of enterolith obstruction. All of these cases were secondary to jejunal diverticulosis and treated surgically. This was accompanied by a literature search to identify the different modalities for diagnosis and treatment of this condition. JD is rare and may lead to a diagnostic delay. Awareness of the wide spectrum of potential complications can prevent this delay.

Co(II), Ni(II), Cu(II) and Mn(II) complexes synthesized by reflux of 6-bromo-3-(3-(4-chlorophenyl)acryloyl)-2H- chromen-2-one, Ciprofloxacin and various transition metal. 1H, 13C, IR and ESI Mass confirm the formation of ligand. The metal complexes were characterized on the basis of various spectroscopic techniques like IR studies and elemental analysis while the geometry of complexes was octahedral which is confirmed by electronic spectra and thermogravimetric analysis. The compounds were subjected to antimicrobial, antioxidant and anti-tubercular activity screening using serial broth dilution method and Minimum Inhibitory Concentration (MIC) is determined. Mn(II) complex has shown significant antifungal activity with an MIC of 6.25 μg/ml while Cu(II) complex is noticeable for antibacterial activity at the same concentration. Anti-TB activity of the ligand has enhanced on complexation with Ni(II) and Co(II) ions. While Ni(II) complex shows superior antioxidant activity than other complexes.

A case of a 50-year-old pedestrian who was hit by a bike and suffered fractures of both bones of his right leg was presented. Complete clinical and radiographic assessment showed double segmental fractures of the tibia and multisegmental fractures of the fibula. Review of the literature revealed that this fracture pattern was unique and only a single case was reported so far. Moreover, we discussed the possible mechanisms which can lead to such an injury. We also discussed the management of segmental tibial fracture and the difficulties encountered with them. This case was managed by modern osteosynthesis technique with a pleasing outcome.

Gas flow in shales follows a number of physical mechanisms that include Knudsen diffusion,Darcy flow,and adsorption in the matrix and micro *** aim of the study is to resolve the interplay of gas transport in these media at increased effective stress as well as net pore *** this research,we investigated the nature of gas transport in the matrix of shale by sending He,CH_(4)and CO_(2)gases through a transient upstream pressure pulse decay instrument.A series of experiments were conducted at constant pore pressures and a gradually increasing confining *** same study was done in three different scenarios,injecting He,CO_(2)and CH_(4).At a constant pore pressure,gas permeability appears to decrease with an increasing confining pressure and effective *** increasing effective stress,the slip factor also decreases along with the *** decrease in slip could be attributed to prestressing,that is likely to create new *** the three purged gases,permeability of shale to CH_(4)is the highest,and that to CO_(2)is the lowest owing to its high *** permeability of CH_(4)against He,could be attributed to the dual transport mechanism.

Despite the promise of immunotherapy such as the immune checkpoint inhibitors(ICIs)anti-PD-1 and anti-CTLA-4 for advanced melanoma,only 26%–52%of patients respond,and many experience grade III/IV immune-related adverse *** by the need for an effective therapy for patients non-responsive to clinically approved ICIs,we have developed a novel nanoimmunotherapy that combines locally administered Prussian blue nanoparticle-based photothermal therapy(PBNP-PTT)with systemically administered agonistic anti-CD137 monoclonal antibody therapy(aCD137).PBNP-PTT was administered at various thermal doses to melanoma cells in vitro,and was combined with aCD137 in vivo to test treatment effects on melanoma tumor progression,animal survival,immunological protection against tumor rechallenge,and *** administered at a melanoma-specific thermal dose,PBNP-PTT elicits immunogenic cell death(ICD)in melanoma cells and upregulates markers associated with antigen presentation and immune cell co-stimulation in ***,PBNP-PTT eliminates primary melanoma tumors in vivo,yielding long-term tumor-free ***,the antitumor immune effects generated by PBNP-PTT cannot eliminate secondary tumors,despite significantly slowing their *** addition of aCD137 enables significant abscopal efficacy and improvement of survival,functioning through activated dendritic cells and tumor-infiltrating CD8^(+)T cells,and generates CD4^(+)and CD8^(+)T cell memory that manifests in the rejection of tumor rechallenge,with no long-term *** study describes for the first time a novel and effective nanoimmunotherapy combination of PBNP-PTT with aCD137 mAb therapy for melanoma.

We report a case of heat stroke induced acute cerebellar dysfunction, a rare neurological disease characterized by gross cerebellar dysfunction with no acute radiographic changes, in a 61 years old ship captain presenting with slurred speech and gait ataxia. A systematic review of the literature on heat stroke induced cerebellar dysfunction was performed, with a focus on investigations, treatment and outcomes. After review of the literature and detailed patient investigation it was concluded that this patient suffered heat stroke at a temperature less than that quoted in the literature.
