Review of evolution of clinical, training and educational services and research program for autism spectrum disorders in Hong Kong

The evolution of a local fragmented model of services for children with autism in Hong Kong emerged gradually over the past three decades with lack of government funding or support. This had been due to increasing number of children with autism being detected and referred for earlier assessment. With increasing pressure from parents due to long waiting time for assessment and training services and the increasing polarization by mass media there had been a gradual increasing public awareness over the past five years. Though still highly fragmented in the availability of services, there is a growing "business model" available in the community due to increasing need and lack of public funding for support. There is a lack of strategic planning for medical diagnostic and management issues in Hong Kong. Our University of Hong Kong based Autism Research Program was pioneered in 1985 based on the increasing load of autism cases referred for assessment for other developmental problems and diagnosed as Autism in the Duchess of Kent Children's Hospital. As the first author has been the staff of the University of Hong Kong, this program flourished as a research based program. The benefits of early identification and intervention of autism spectrum disorder(ASD) had been increasingly recognized, and with the increased public awareness and increasing trend of earlier diagnosis, there has been a continuously high demand from parents for earlier assessment and training for children suspected to have ASD. This model had not received extra funding for this integrated program for research, teaching and training in autism. We had to apply for various donations and grants to support the development of this pioneer program. The research output and organization of forums for public education and awareness are reviewed. This paper presents data from a 21 cm H I emission drift scan observation of a field partially covering the NGC 4636 galaxy group with the Five-hundred meter Aperture Radio Telescope(FAST). We construct a pipeline to reduce the data, and use So Fi A for source finding. When not contaminated by Radio Frequency Interference(RFI), the FAST observations are capable of detecting all of the galaxies previously detected by the Arecibo Legacy Fast ALFA(ALFALFA) survey in the same region. Comparing to ALFALFA for the detections in common, the FAST data show consistent integrated spectra when the H I disks are spatially unresolved, and capture more flux when the H I disks are resolved. The FAST data further reveal 10 new detections in the region mutually covered with ALFALFA, and 18 new detections beyond the footprint of ALFALFA. All of the new detections have the matching optical counterparts. For the member galaxies of the NGC 4636 group, the detection limit of FAST is deeper by 0.4 dex in H I mass than that of the ALFALFA data. After correcting for the incompleteness caused by RFI contamination, we show that the H I detection rate of galaxies rises steeply with radius out to the virial radius of the group, and flattens beyond that. We also examine four spatially resolved galaxy systems with potential tidal interaction features in detail. Considering that the data have been taken during the“shared-risk” period before a major source of local RFI was eliminated, the results highlight the power of FAST in detecting extragalactic H I.

Dear Editor,Given the systemic and substantial health consequences of smoking and the significant cancer risk from smoking[1,2],it is expected that cancer patients would quit smoking after cancer ***,the smoking rate among cancer survivors is only slightly lower than that among the general population[3,4],and 64%of smokers diagnosed with cancer continued to smoke even after they learned they had cancer[5].In addition,some former smokers may resume smoking after surviving cancer[6].A number of previous studies have investigated the association between smoking and clinical outcomes in cancer patients[1,7–10].Nevertheless,most studies were conducted in smoking-related cancers,and evidence for non-smoking-related cancers is *** is an urgent need for more convincing evidence showing the harms of smoking and the benefits of smoking cessation to promote smoking cessation in cancer patients and *** systematically studied the associations of smoking status,smoking intensity,age at initiation,and smoking cessation at cancer diagnosis with all-cause mortality in a prospective cohort of 128,423 cancer patients across 23 cancer types from the MD Anderson Cancer Patients and Survivors Cohort.

目的介绍人用药品技术要求国际协调理事会(Technical Requirements for Pharmaceuticals for Human Use,ICH)主办下编制的国际医学术语集—监管活动医学词典(Medical Dictionary for Regulatory Activities,MedDRA)及其在监管领域的应用。方法描述20世纪90年代全球药品监管领域对国际标准术语集的需求,阐释MedDRA的结构特征和维护模式,历数MedDRA成长和发展的里程碑,包括近期面对COVID-19疫情挑战所采取的行动,并展望MedDRA未来的发展。结果和结论由于药品研发领域的监管活动亟需一个国际标准术语集,ICH发起创办了MedDRA。MedDRA用于医学产品的注册、文件记录、安全监测与监督,从临床试验到上市后监测,贯穿于产品开发周期中的各个阶段。MedDRA的结构特征和维护模式是按照其使用目的设计开发的。自启动以来,MedDRA作为一个用户驱动的术语集,在维护和支持服务组织(Maintenance and Support Services Organization,MSSO)的精心管理下扩充了术语体量和范围,以满足用户的需求以及不断发展的监管报告要求,并跟上医学进展的步伐。

Nutritional stimulation by in-ovo feeding modulates cellular proliferation and differentiation in the small intestinal epithelium of chicks

Nutritional stimulation of the developing small intestine of chick embryos can be conducted by in-ovo feeding(IOF).We hypothesized that IOF of glutamine and leucine can enhance small intestinal development by promoting proliferation and differentiation of multipotent small intestinal epithelial *** embryos(n=128)were subject to IOF of glutamine(IOF-Gln),leucine(IOF-Leu),NaCl(IOF-NaCl)or no injection(control)at embryonic d 17(E 17).Multipotent,progenitor and differentiated cells were located and quantified in the small intestinal epithelium between E 17 and d 7 after hatch(D 7)in all treatment groups by immunofluorescence of SRY-box transcription factor 9(Sox9)and proliferating cell nuclear antigen(PCNA),in-situ hybridization of leucine-rich repeat containing G-protein coupled receptor 5(Lgr5)and peptide transporter 1(PepT1)and histochemical goblet cell *** effects of IOF treatments at E 19(48 h post-IOF),in comparison to control embryos,were as follows:total cell counts increased by 40%,33%and 19%,and multipotent cell counts increased by 52%,50%and 38%,in IOF-Gln,IOF-Leu and IOF-NaCl embryos,*** IOF-Gln embryos exhibited a significance,36%increase in progenitor cell *** IOF treatments shifted Lgr5+ stem cell localizations to villus *** differentiated,PepT1+region of the villi was 1.9 and 1.3-fold longer in IOF-Gln and IOF-Leu embryos,respectively,while goblet cell densities decreased by 20%in IOF-Gln ***,crypt and villi epithelial cell counts were significantly higher IOF-Gln chicks,compared to control chicks(P<0.05).We conclude IOF of glutamine stimulates small intestinal maturation and functionality during the peri-hatch period by promoting multipotent cell proliferation and differentiation,resulting in enhanced compartmentalization of multipotent and differentiated cell niches and expansions of the absorptive surface area.

Fatty liver in hepatitis C patients post-sustained virological response with direct-acting antivirals
