Paraneoplastic retinopathies: an update on pathogenesis, diagnosis and management

The paraneoplastic retinopathies are a heterogeneous group of disorders with significant visual consequences occuring in the setting of a systemic *** conditions may be characterized by the presence of antiretinal antibodies and may predate or follow the diagnosis of an underlying *** I review the clinical findings,pathophysiology,laboratory testing and management of the paraneoplastic retinopathies:cancer-associated retinopathy(CAR),melanoma-associated retinopathy(MAR),bilateral diffuse uveal melanocytic proliferation(BDUMP)and paraneoplastic vitelliform maculopathy(PVM).The pathophysiology of the paraneoplastic retinopathies varies from molecular mimicry resulting in anti-retinal antibody production(CAR,MAR)to relases of soluble factors either by the primary tumor(BDUMP)and/or immune system in response to the primary tumor(PVM)which result in retinal and/or retinal pigment epithelium *** each condition,structural and functional multimodal retinal testing can be helpful in establishing the *** for the paraneoplastic retinopathies involves a combination of treating the underlying malignancy plus additional local and/or systemic immunosuppressive agents though no systemic therapeutic protocols *** these interventions,the retinopathy may be progressive or the retinopathy may be a harbinger of poor ***,prompt diagnosis may help identify an underlying malignancy earlier and may thus improve cancer-related survival.

In this research paper,a solar air heater with triangular fins has been experimentally analysed and ***,an experimental set-up of a solar air heater having triangular fins has been developed at the location of 28.10°N,78.23°*** heat transfer rate through fins and fins efficiency has been determined by the Finite Difference Method model *** experimental data and modeled data of response parameters have been optimized in MINITAB-17 software by the Response surface Methodology *** creating the response surface design,three input parameters have been selected namely solar intensity,Reynolds number,and fin base-to-height *** range of solar intensity,Reynolds number,and fin base-to-height ratio is 600 to 1000W/m^(2),4000 to 6000,and 0.4 to 0.8 *** response surface design has been analyzed by calculating the outlet temperature,friction factor,Nusselt number,fin efficiency,thermal performance factor,and exergy *** optimum settings of input parameters:solar intensity is 1000 W/m^(2);Reynolds number is 4969.7,and the fin base to height ratio is 0.6060,on which these response:namely outlet temperature of 92.531℃,friction factor of 0.2350,Nusselt number of 127.761,thermal efficiency of 50.836%,thermal performance factor of 1.4947,and exergy efficiency of 8.762%.

We aim to evaluate the incidence of incontinence following laser endoscopic enucleation of the prostate(EEP)comparing en-bloc(Group 1)versus 2-lobe/3-lobe techniques(Group 2).We performed a retrospective review of patients undergoing EEP for benign prostaticenlargement in 12 centers between January 2020 and January *** were presented as median and interquartile range(IQR).Univariable and multivariable logistic regression analysis was performed to evaluate factors associated with stress urinary incontinence(sUI)and mixed urinary incontinence(MUI).There were 1711 patients in Group 1 and 3357 patients in Group *** in Group 2were significantly younger(68[62–73]years vs 69[63–74]years,P=0.002).Median(interquartile range)prostate volume(PV)wassimilar between the groups(70[52–92]ml in Group 1 vs 70[54–90]ml in Group 2,P=0.774).There was no difference in preoperativeInternational Prostate symptom score,quality of life,or maximum flow ***,morcellation,and total surgical time weresignificantly shorter in Group *** 1 month,overall incontinence rate was 6.3%in Group 1 versus 5.3%in Group 2(P=0.12),and urge incontinence was significantly higher in Group 1(55.1%vs 37.3%in Group 2,Ps,the overall rate ofincontinence was 1.7%in Group 1 versus 2.3%in Group 2(P=0.06),and sUI was significantly higher in Group 2(55.6%vs 24.1%in Group 1,P=0.002).At multivariable analysis,PV and IPss were factors significantly associated with higher odds of transient sUI/***,surgical time,and no early apical release technique were factors associated with higher odds of persistent sUI/MUI.

Piezoelectric resonant de-icing systems are attracting great *** paper aims to assess the implementation of these systems at the aircraft *** article begins with the model to compute the power requirement of a piezoelectric resonant de-icing system sized from the prototype detailed in Part 1/2 of this *** the mass,drag,and fuel consumption of this system and the subcomponents needed for its implementation are *** features of a piezoelectric resonant de-icing system are finally computed for aircraft similar to Airbus A320 aircraft and aircraft of different categories(Boeing 787,ATR 72 and TBM 900)and compared with the existing thermal and mechanical ice protection systems.A sensitivity analysis of the main key sizing parameters of the piezoelectric de-icing system is also performed to identify the main axes of improvement for this *** study shows the potential of such ice protection *** particular,for the realistic input parameters chosen in this work,the electro-mechanical solution can provide a 54% reduction in terms of mass and a 92% reduction in terms of power consumption for an A320 aircraft architecture,leading to a 74% decrease in the associated fuel consumption compared to the actual air bleed system.

Background:The incidence of prostate cancer is increasing worldwide.A significant proportion of patients developmetastatic disease and are initially prescribed androgen deprivation therapy(ADT).However,subsequent sequences of treatments in real-world settings that may improve overall survival remain an area of active *** and methods:Data were collected from 384 patients presenting with de novo metastatic prostate cancer from 2011 to 2015 at a tertiary cancer *** were categorized into surviving(n=232)and deceased(n=152)groups at the end of 3 *** sequence pattern mining techniques(Generalized sequential Pattern Mining and sequential Pattern Discovery using Equivalence Classes)were applied to determine the exact order of the most frequent sets of treatments in each ***:Degarelix,as the initial form of ADT,was uniquely in the surviving *** sequence of ADT followed by abiraterone and docetaxel was uniquely associated with a higher 3-year overall *** followed by fosfestrol was found to have a unique niche among surviving patients with a long duration of response to the initial *** who received chemotherapy followed by radiotherapy and those who received radiotherapy followed by chemotherapy were found more frequently in the deceased ***:We identified unique treatment sequences among surviving and deceased patients at the end of 3 *** should be the preferred formof *** who received ADT followed by abiraterone and chemotherapy showed better *** requiring palliative radiation and chemotherapy in any sequence were significantlymore frequent in the deceased group,identifying the need to offer such patients the most efficacious agents and to target them in clinical trial design.

Injecting carbon dioxide(CO_(2))into coal seams may unlock substantial carbon sequestration *** the coal acts like a carbon filter,it can preferentially absorb significant amounts of CO_(2).To explore this further,desorption of the adsorbed gas due to pressure drop is investigated in this paper,to achieve an improved understanding of the long-term fate of injected CO_(2) during post-injection *** paper presents a dual porosity model coupling gas flow,adsorption and geomechanics for studying coupled processes and effectiveness of CO_(2) sequestration in coals.A new adsorption?desorption model derived based on thermodynamics is incorporated,particularly,the desorption hysteresis is *** reliability of the proposed adsorption-desorption isotherm is examined via validation *** is indicated that occurrence of desorption hysteresis is attributed to the adsorption-induced pore *** injection ceases,the injected gas continues to propagate further from the injection well,while the pressure in the vicinity of the injection well experiences a significant *** the adsorbed gas near the well also decreases,this decrease is less compared to that in pressure because of desorption *** unceasing spread of CO_(2) and drops of pressure and adsorbed gas depend on the degree of desorption hysteresis and heterogeneity of coals,which should be considered when designing CO_(2) sequestration into coal seams.

When water, light or sound waves impinge on objects smaller than the wavelength, a scattering process takes place[1]. The scattering of waves plays an essential role in a myriad of natural phenomena. In ocean engineering, enhancing the scattering of water waves allows capturing energy from larger areas, with clear implications for energy harvesting and coastal protection.
