In this paper we prove in a new way, the well known result, that fermat’s equation asup>4sup> + bsup>4sup> = csup>4sup>, is not solvable in ℕ , when abc≠0 . to show this result, it suffices to prove that: ( f 0 ): a 1 4 + ( 2 s b 1 ) 4 = c 1 4 , is not solvable in ℕ , (where a 1 , b 1 , c 1 ∈2ℕ+1 , pairwise primes, with necessarly 2≤s∈ℕ ). the key idea of our proof is to show that if (fsub>0sub>) holds, then there exist α 2 , β 2 , γ 2 ∈2ℕ+1 , such that ( f 1 ): α 2 4 + ( 2 s−1 β 2 ) 4 = γ 2 4 , holds too. from where, one conclude that it is not possible, because if we choose the quantity 2 ≤ s, as minimal in value among all the solutions of ( f 0 ) , then ( α 2 ,2 s−1 β 2 , γ 2 ) is also a solution of fermat’s type, but with 2≤s−1s , witch is absurd. to reach such a result, we suppose first that (fsub>0sub>) is solvable in ( a 1 ,2 s b 1 , c 1 ) , s ≥ 2 like above;afterwards, proceeding with “Pythagorician divisors”, we creat the notions of “fermat’s b-absolute divisors”: ( d b , d ′ b ) which it uses hereafter. then to conclude our proof, we establish the following main theorem: there is an equivalence between (i) and (ii): (i) (fsub>0sub>): a 1 4 + ( 2 s b 1 ) 4 = c 1 4 , is solvable in ℕ , with 2≤s∈ℕ , ( a 1 , b 1 , c 1 )∈ ( 2ℕ+1 ) 3 , coprime in pairs. Epidemiological, Diagnostic, therapeutic, and Prognostic Profile of Non-tuberculous Community-Acquired Purulent Pleurisy in Children at the Bouaké University Hospital Center, 2017-2021

Introduction: In Côte d’Ivoire, there is a scarcity of data on children’s purulent pleurisies. Objective: this study aims to elucidate the epidemiological, diagnostic, therapeutic, and evolutionary facets of non-tuberculous purulent pleurisies in pediatric patients. Methods: A retrospective analysis was conducted using the medical records of children aged one month to fifteen years with purulent pleurisies at Bouaké University Hospital Center from January 2017 to December 2021. Results: the study identified 124 cases of purulent pleurisies, constituting 18% of lower respiratory tract infections and 0.8% of all hospitalizations. the majority of these cases (69%) were in children between 1 and 24 months of age. Prominent symptoms included dyspnea (85.5%), O2 saturation below 95% in room air (76.6%), respiratory distress (68.5%), cutaneous-mucosal pallor (63.7%), and fever (43.5%). Radiological findings predominantly showed right-sided pleurisy (62.1%). the pleurisy was often extensive (78.2%), accompanied by pneumothorax (37.1%), alveolo-interstitial opacities (8.1% of cases), and abscess formations (1.6%). Pleural fluid cultures were positive in 46.9% of cases, with staphylococcus aureus (75%, methi-s) identified among 32 bacteria. Initial antibiotic treatment was empirical, favoring oxacillin (53.2%) or amoxicillin-clavulanic acid (53.2%) in dual (42%) or triple therapy (33%) with gentamicin (64.1%) and/or metronidazole (21.8%). treatments also included pleural drainage (68.5%) or repeated evacuation punctures (33.1%), and blood transfusion (39%). the mortality rate was 18.8%. Conclusion: Non-tuberculous purulent pleurisy remains a significant concern in pediatric hospitalizations at the CHU of Bouaké, marked by high mortality rates.

Adjuvant Rectal Diclofenac for Post Operative Analgesia after Caesarean section—A Randomized Controlled study

BACKGROUND: Pain management following caesarean section still remains a challenge in our environment. Most potent analgesics are either not readily available or expensive. Diclofenac suppository is an NsAID that can be used for postoperative analgesia. It is available and affordable. OBJECtIVE: to compare the efficacy and safety of combined rectal diclofenac and intramuscular pentazocine with intramuscular pentazocine alone for post operative pain control following lower segment caesarean section. MEtHODOLOGY: A total of 120 women who met the selection criteria scheduled for caesarean section under spinal anaesthesia with bupivacaine were randomized into two equal groups to receive either 75 mg diclofenac suppository 12 hourly for 24 hours or one anusol suppository (the placebo) 12 hourly for 24 hours. Both groups received pentazocine as primary analgesia. REsULt: the primary outcome measure is the proportion of patients with severe pain at 24 hours using the visual analogue rating scale. secondary outcome measures are the time from surgery to ambulation, Passage of flatus, maternal satisfaction and presence of complications. statistical analysis was done using spss version 22 and graph pad statistical package. student t-test was used for continuous variables whereas chi square was used for categorical variables P CONCLUsION: Adjuvant rectal diclofenac is superior to pentazocine alone in the management of pain after caesarean section. Less number of patients had moderate to severe pain at 24 hours post operation. Maternal satisfaction in relation to pain management is better with diclofenac suppository. the levels of complications were comparable in both groups.

Introduction: the practice of cardiac surgery (Cs) in sub-saharan Africa faces a number of challenges. We report on the surgical activities of the first 17 months of the program launched at the Yaoundé General Hospital (YGH), describing the types of interventions and the early results. Patients and Methods: this was a descriptive cross-sectional study including patients who underwent Cs from september 2022 to January 2024 at the YGH. Clinical, operative and postoperative data were collected and analysed retrospectively by R software. Results: A total of 37 patients underwent Cs during the study period, divided into 23 (62.2%) open-heart and 14 (37.8%) closed-heart cases. Males predominated (sex ratio 1.17). the median age and interquartile range (IQR) were 36 years (9 - 51). Isolated valve surgery, in 15 (40.5%) cases, was the most common procedure in open-heart surgery cases. Closed-heart surgery involved closure of the patent ductus arteriosus in 9 (24.3%) cases, coronary artery bypass grafting in 3 (8.1%) cases, and pericardectomy in 2 (5.4%) cases. the median duration of extracorporeal circulation and aortic cross-clamping was 101 min (IQR 84 - 143) and 74 min (IQR 54 - 112) respectively. the median duration of mechanical ventilation was 2 hours (IQR 2 - 3). the median intensive care unit stay and median hospital stay were 2 days (IQR 2 - 3) and 7 days (IQR 5 - 8) respectively. Operative mortality was 2 (5.4%). Conclusion: Cardiac surgery remains a major challenge for African populations. the early results achieved by the local team are satisfactory. An increase in surgical activity should be strongly encouraged to facilitate local training and ensure the sustainability of the activity.

Development of a Web-Based GIs of flood Zones in the Municipality of Ouagadougou in Burkina faso

this document describes the creation of an informative Web GIs aimed at mitigating the impacts of flooding in the municipality of Ouagadougou, in Burkina faso, a region that is highly sensitive to climate change. Burkina faso, which is undergoing rapid urbanization, faces major natural threats, particularly flooding, as demonstrated by the severe floods of 2009 that caused loss of life, injury, structural damage and economic losses in Ouagadougou. the aim of this research is to develop a web map highlighting the municipality’s flood-prone areas, with a view to informing and raising awareness of flood risk reduction. Using the Leaflet Javascript mapping library, the study uses HtML, Css and Javascript to implement web mapping technology. Data on Ouagadougou’s flood zones is generated by a multi-criteria analysis combining saaty’s AHP method and GIs in QGIs, integrating seven (7) parameters including hydrography, altitude, slope, rainfall, soil types, land use and soil moisture index. QGIs processes and maps the themes, PostgresQL with PostGIs serves as the DBMs and Geoserver functions as the map server. the Web GIs platform allows users to visualize the different flood risks, from very low to very high, or the high-risk areas specific to Ouagadougou. the AHP calculations classify the municipality into five flood vulnerability zones: very low (24.48%), low (27.93%), medium (23.01%), high (17.11%) and very high (7.47%). Effective risk management requires communication and awareness-raising. this online mapping application serves as a tool for communication, management and flood prevention in Ouagadougou, helping to mitigate flood-related natural disasters.

In Africa, the prevalence of diabetes is escalating and remains a concern due to the numerous complications it causes. Vascular damage associated with diabetes leads to a prothrombotic state observed in diabetic individuals. Diabetes is a complex and multifactorial disease involving genetic components. With the aim of preventing complications and contributing to an efficient management of diabetes, we investigated genes likely to lead to a risk of thrombosis, in particular the C677t of MtHfR, G20210A of prothrombin, and R506Q of factor V Leiden in type 2 diabetics in Abidjan receiving ambulatory care. A descriptive cross-sectional study was carried out on consenting type 2 diabetic patients. Mutation detection was carried out using the PCR-RfLP method employing restriction enzymes. Hemostasis tests (fibrinogen, D-dimers, fibrin monomers, and von Willebrand factor) were performed using citrate tubes on the stage? star Max automated system. Plasminogen activator inhibitor was assayed by ELIsA method, and biochemical parameters were determined using the COBAs C311. the study population consisted of 45 diabetic patients, 51.1% of whom presented vascular complications, mainly neuropathy. Disturbances in hemostasis parameters were observed, with 15.5% of patients showing an increase in fibrin monomers. Mutation analysis revealed an absence of factor V mutation (factor V Leiden) and of G20210A mutation of the prothrombin gene. However, 15.6% of subjects had a heterozygous C677t mutation of MtHfR, with 57% of them being anemic. the exploration of biological and genetic factors associated with thrombotic risk is of significant interest in the optimal management of African type 2 diabetics.
