A mixture of new ceramides (1, 2, 3, 4 and 5) together with a binary mixture of ceramides with long chain alkyl (6 and 7), triterpenoid (10) and steroids (11 and 12) have been isolated from bark of the fruits and of the stems of Luffa operculata (Cucurbitaceae). The structures were elucidated by comprehensive spectro- scopic analysis including 1H and 13C NMR, dePT (distortionless enhancement by polarization transfer), COSY (correlated spectroscopy), HMQC (heteronuclear multiple quantum coherence), HMBC (heteronu- clear multiple bond connectivity), IR (infrared), HR-ESI-MS (electrospray ionization-high resolution mass spectra) and LR-MS (low resolution electron ionization mass spectra) experiments. All the ceramides are reported for the first time in Cucurbitaceae and this is the first report of the rare triterpene 10 isolated from Luffa operculata. The ceramides 6 and 7 showed a high acetylcholine esterase inhibitory effect.

A numerical simulation study of the performance of encoding and decoding of short optical pulse in OCDMA (Optical Code Division Multiple Access) systems under non-linear optical effects is presented. The performance of encoding and decoding short pulses is obtained through use of device FBG (fiber Bragg grating) where the codes are inserted through discrete jumps in the optical phase (±π). The multiuser interference is also the object of the present study, where a figure of merit (interference figure) is used to quantify the interference in multiuser auto and cross correlation. We evaluate the dependence of the multiuser interference for the coupling constant of FBG. Finally, the interference inserted in the autocorrelation due to nonlinear effects generated in the nonlinear switching of the coded pulse in a dual core nonlinear directional coupler is examined, where temporal broadening of pulses decoded is observed.

Difficult intubation, inadequate ventilation and esophageal intubation are the principal causes of death or brain damage related to airway manipulation. The objective of this cross-sectional study was to correlate a preanesthetic evaluation that may be capable of predicting a difficult intubation with the conditions encountered at laryngoscopy and endotracheal intubation. Eighty-one patients submitted to general anesthesia were evaluated at a preanesthetic consultation according to the modified Mallampati classification, the Wilson score and the American Society of Anesthesiologists (ASA) difficult airway algorithm. Findings were then correlated with the Cormack-Lehane classification and with the number of attempts at endotracheal intubation. No statistically significant correlations were found between the patients’ Mallampati classification and their Cormack-Lehane grade or between the Mallampati classification and the number of attempts required to achieve endotracheal intubation. Laryngoscopy proved difficult in four patients and in all of these cases the Wilson score had been indicative of a possibly difficult airway, highlighting its good predicting sensitivity. However, the specificity of this test was low, since another 24 patients had the same Wilson score but were classified as Cormack-Lehane I/II. Moreover, two patients who had a Wilson score ≥ 4 were also classified as Cormack-Lehane grade I/II. The study concluded that the Wilson score, although seldom used in clinical practice, is a highly sensitive predictor of a difficult airway;its specificity, however, is low.

Cardiovascular Risk Assessment in the Nursing Team of a Cardiology Hospital

Objective: Cardiovascular diseases are the most common causes of morbidity and the leading cause of mortality in the world scenario, accounting for about 20% of all deaths in individuals over 30 years. It has attributed this to the increase in the company’s exposure to risk factors. It identifies cardiovascular risk factors in the nursing team and compares the cardiovascular risk by Framingham score among professionals of middle and upper working in a referral hospital in cardiology. Method: Cross-sectional quantitative study was conducted in 2014, in a reference hospital in cardiology located in Recife/Pernambuco, Brazil. Data of cardiologic risk factors were collected from 82 nursing team members, comprised of technical, auxiliary nurses, and nurses between 30 and 74 years of age. The cardiovascular risk level was evaluated by the Framingham Score. Data were organized and analyzed by means of the SPSS, with descriptive statistics and Student-t test for the continuous variables. Results: Family history, stress and sedentary life style were the prevalent risk factors in more than half of the professionals. Only 5.23% of the technical and auxiliary nurses had a high risk score for cardiovascular event. Conclusion: The data indicates a low cardiovascular risk for these professionals and the presence of risk factors that can be modified. Health surveillance of these professionals is necessary in order to avoid a change to a risk of greater vulnerability.

In this paper we report for the first time the presence of bistability in an acoustic-optic tunable filter (AOTF) operating with ultrashort (2 ps) optical light pulses. The results for the study of bistability has shown the dependence of the hysteresis curve with the product of the coupling constant (κ) by the length of the device (ξL) and the conversion power-coupling constant factor (G). The range of bistability varies significantly with both G and with κξL parameters. The variation of κξL directly increases the size of the range of bistability hysteresis while the increase in G causes the bistability to occur at low powers. The phenomenon of optical bistability (OB) is the object of increasing interest due to its possibilities for important device applications. A bistable device is a device with a capability to generate two different outputs for a given input and the physical requirements for this are an intensity-dependence refractive index and an optical feedback mechanism.

Stimulation of the P6 Acupuncture Point for Prophylaxis of Nausea and Vomiting in Pregnant Women Submitted to Cesarean Section: A Blinded Clinical Trial

In pregnant women subjected to spinal anesthesia for a Cesarean section, episodes of nausea and vomiting are common both during and following surgery. Acupuncture for the prophylaxis and treatment of these complications has been gaining in popularity due to its low cost, simplicity, absence of side effects and confirmed efficacy. This study investigated the efficacy of stimulating the P6 acupoint in conjunction with the use of dexamethasone as prophylaxis for nausea and vomiting in pregnant women submitted to spinal anesthesia for a Cesarean section. The patients (n = 100) were randomly distributed into two groups. In the first group (n = 50), a site located one centimeter laterally from P6 was stimulated. This is not a true acupuncture point (sham acupuncture). In the second group (n = 50), P6 was stimulated. In both groups, 4 mg of dexamethasone were administered intravenously. A questionnaire was used to obtain information on the occurrence of nausea and vomiting during surgery and in the first 12 hours postpartum. The chi-square test and Fisher’s exact test were used to assess differences between the groups. Age and physical status were similar in both groups. The incidence of nausea during surgery was 32% (n = 16) in the control group and 22% (n = 11) in the P6 group (p > 0.05). In the first 12 hours following surgery, nausea occurred in 16% of the women in the control group (n = 6) and in 4% in the P6 group (n = 4) (p = 0.045). The incidence of vomiting in the control group was 12% (n = 6) during surgery and 10% (n = 5) in the postoperative period compared to 8% (n = 4) and 4% (n = 2), respectively, in the P6 group (p > 0.05). Although these differences were not statistically significant with the exception of the incidence of nausea in the first 12 hours postpartum, a reduction occurred in the incidence of all the outcomes evaluated in the P6 group.

Corynebacterium pseudotuberculosis is an infectious agent that occurs in small ruminants causing caseous lymphadenitis, and more rarely in humans causing lymphadenitis and pneumonia. The breeding small ruminants have great economic importance in Brazil. Rural farm workers and veterinary students who acquired this disease suffered from weakening symptoms for weeks, and the identification of the etiological agent was time-consuming and complex. Due to the low prevalence of case records, there is probably no available commercial diagnostic kit for C. pseudotuberculosis infection in humans. This study aimed to describe human seroreactivity to secreted antigens from C. pseudotuberculosis. Reactivity of serum from farm workers (n = 14), individuals who work with the bacillus at laboratory (n = 8) or individuals without contact (n = 25) was tested with secreted proteins from PAT10 strain of C. pseudotuberculosis by Western blotting. Samples of all (100%) farm workers showed reactivity to 31 kDa, 71 kDa and 164 kDa proteins, while laboratory workers showed 87.5%, 62.5 % and 37.5%, and no-contact 20%, 0% and 16%, respectively. All sera recognized the 275 kDa protein. Our data suggest that C. pseudotuberculosis secreted proteins are antigenic in humans and the recognition profiles allowed the identification of individuals with and without prior contact with this bacillus. This is the first paper which describes human reactivity to C. pseudotuberculosis in serum samples of workers in Brazil.
