Outcome of distal end clavicle fractures treated with locking plates

Purpose： Fractures of the lateral end of the clavicle are relatively uncommon. These fractures are unstable due to the various deforming forces which act on the fragments as well as the small distal fracture fragment. At most times the deforming forces are not taken into consideration, and the fracture is not fixed securely. In this study, we assessed a fixation technique using the precontoured locking plates to find out whether it provided a stable fixation with good functional outcome. Methods： Totally, 32 patients with lateral end clavicle fracture （Neer＇s Type ll） were included in the study. After the informed consent and preoperative investigations were obtained, open reduction and intemal fixation was done using a 3.5 mm precontoured superior locking plate with lateral extension under general anesthesia. Postoperative X-rays were done on day I and every 6 weeks after operation, until radiological union was achieved. The postoperative pain was assessed using Visual Analogue Scale （VAS） on postoperative days 1, 2 and 10. Postoperatively arm pouch sling was given for 2 weeks followed by active mobilization. Patients were asked to do their daily routine work and avoid lifting heavy weights. The functional outcome was assessed at the end of 2nd and 6th months with the help of Disabilities of the Arm, Shoulder and Hand （DASH} scoring. Results： There were no intraoperative complications in the procedure. The mean VAS score on post- operative day 1 was found to be 5 which decreased to 3 on day 2 and 0 on day 10. The mean DASH score was calculated as 11.63 at the end of postoperative month 2 and then 4.6 at the end of month 6. There was one case of malunion in whom the overhead abduction was restricted but was not painful and was managed conservatively. Conclusion： The precontoured locking plates with lateral extension may be a good method to fix the fractures of the lateral end clavicle, which provide a stable fixation with good functional outcome with very few instan

BACKGROUND Ventriculoperitoneal(VP)shunt placement has become a standard of care procedure in managing hydrocephalus for drainage and absorption of cerebrospinal fluid(CSF)into the *** pseudocysts containing CSF are the common long-term complication of this frequently performed procedure,mainly because VP shunts have significantly prolonged *** these,liver CSF pseudocysts are rare entities that may cause shunt dysfunction,affect normal organ function,and therefore pose therapeutic *** SUMMARY A 49-year-old man with history of congenital hydrocephalus status post bilateral VP shunt placement presented with progressively worsening dyspnea on exertion,abdominal discomfort/*** computed tomography(CT)scan revealed a large CSF pseudocyst in the right hepatic lobe with the tip of VP shunt catheter into the hepatic cyst *** underwent robotic laparoscopic cyst fenestration with a partial hepatectomy,and repositioning of VP shunt catheter to the right lower quadrant of the ***-up CT demonstrated a significant reduction in hepatic CSF *** A high index of clinical suspicion is required for early detection of liver CSF pseudocysts since their presentation is often asymptomatic and cunning early in the ***-stage liver CSF pseudocysts could have adverse outcomes on the treatment course of hydrocephalus as well as on hepatobiliary *** is paucity of data to define the management of liver CSF pseudocyst in current guidelines due to rare nature of this *** reported occurrences have been managed by laparotomy with debridement,paracentesis,radiological imaging guided fluid aspiration and laparoscopic-associated cyst *** surgery is an additional minimally invasive option in the management of hepatic CSF pseudocyst;however,its use is limited by lack of widespread availability and cost of surgery.

Objective:To measure seasonal effects on semen quality profiles,seminal biochemical and oxidative stress attributes in fresh and liquid stored semen in monsoon and dry ***:A total of 10 Teressa bucks(3-4 years)were selected from breeding farm,ICAR-Central Island Agricultural Research Institute,Port Blair,Andaman and Nicobar Islands,*** samples(n=25 per season)were collected through artificial vagina method and preserved at refrigerated temperature(5℃)for 48 h using Tris citrate glucose based *** detected semen quality parameters[volume,mass activity,pH,sperm concentration,total motility,viability,total sperm abnormality,and plasma membrane,acrosomal and nuclear integrities],biochemical profiles[aspartate amino transferase(AST),alanine amino transferase(ALT)and total cholesterol],and oxidative stress markers[total antioxidant capacity(TAC)and malondialdehyde(MDA)]during monsoon and dry ***:Semen quality parameters significantly differed between seasons(P<0.05)and among storage periods(P<0.05).Volume,pH,mass activity,motility,viability,acrosomal,plasma membrane and nuclear integrities,and TAC were significantly higher(P<0.05).Sperm concentration,sperm abnormalities,MDA,AST,ALT and total cholesterol were significantly lower in fresh semen of monsoon than dry season(P<0.05).Motility,viability,acrosomal,plasma membrane and nuclear integrities,and TAC were significantly decreased(P<0.05)while sperm abnormality,AST,ALT,total cholesterol and MDA were significantly increased as liquid semen storage period advanced(P<0.05).Conclusions:Monsoon season has higher beneficial effects on semen quality profiles and liquid stored semen remained usable for upto 48 *** quality ejaculates with higher TAC and lower MDA can be cryopreserved and will be used for artificial insemination.

We consider comparison of Mutual Information （MI）-Energy tradeoff in correlated wireless sensor network （WSN） for cooperative multiple input and multiple output （MIMO） and non-cooperative （referred to as single input and single output （SISO）） transmission *** numerical results indicate that for short distances SISO schemes achieve better MI-Energy tradeoff than the cooperative MIMO schemes,while for long distances the cooperative MIMO schemes achieve better MI-Energy *** next consider the issue of sensor placement in a correlated ***,we prove that the spacing between nodes to maximize MI is in general an increasing function of observation *** this structural result,we conclude that at high SNR,the various stages of the WSN can be separated by relatively large distances than at low SNR.

Microbial diversity has been an important facet of scientific research, since microbes promise a plethora of biomolecules which are otherwise not found in nature. Microbes are subjected to high level of competition for survival in the environment, and hence develop mechanisms of defense. The biomolecules produced by these microbes as part of their defense or survival mechanism, are of importance for human and animal drugs and many other industrial and environmental applications. The marine counterparts of these terrestrial microbes have yet higher potential, since the marine environment has higher biotic and abiotic stresses, leading to new molecule discovery. In the current study, a bacterial diversity study of the culturable bacteria of the mangrove rhizosphere of Avicennia marina has been undertaken, to understand the flora diversity. Mangroves are unique ecosystems which are under a combination of marine and terrestrial influence. Mangroves are seaward, inland and also found in creek areas. This diversity in their habitat, leads them to produce variable root exudates, which support the growth of different types of organisms. This study has revealed that certain species are dominant in these ecosystems irrespective of the biotic and abiotic stresses, whereas certain species appear only at neutral pH. The study will help select organisms for further biomolecule discovery programs, based on their environment of isolation and other growth parameters.

AIM To determine the prevalence,characteristics and clinical course of pancreatic cystic neoplasms(PCNs) in liver transplantation(LT) *** We retrospectively studied consecutive patients who underwent LT between January 1998 to April 2016. Clinical and laboratory data were obtained from patient medical records. Imaging findings on computed tomography and magnetic resonance cholangiopancreatography were reviewed by two *** During the study period,872 patients underwent cadaveric LT. Pancreatic cysts were identified in 53/872(6.1%) and 31/53(58.5%) were PCNs [28 intraductal papillary mucinous neoplasm(IPMN),2 mucinous cystic neoplasm(MCN),1 serous cystadenoma]. Patients with PCNs exhibited less male predominance(55% vs 73%,P = 0.03) compared to patients without pancreatic cysts. Thirteen patients(42%) were diagnosed with PCN pre-LT while 18 patients(58%) developed PCN post-LT. The median size of PCNs was 13 mm [interquartile range(IQR) 10-20 mm]. All IPMNs were side-branch type. Most PCNs were found in the head and body of pancreas(37% each),followed by the tail(25%). Five patients underwent further evaluation with endoscopic ultrasound. Progress imaging was performed on 81% of patients. PCNs remained stable in size and number in all but 2 patients. During a median follow up of 39 mo(IQR 26-58 mo),the 2(6%) patients with MCN underwent pancreatectomy. No PCN patient developed pancreatic adenocarcinoma,while 5 died from illnesses unrelated to the PCN. Among patients without PCN,1/841(0.1%) developed pancreatic *** The prevalence of PCNs in LT recipients was similar to the general population(3.6%,31/872). Side-branch IPMNs do not appear to have accelerated malignant potential in post-LT patients,indicating the current surveillance guidelines are applicable to this group.

Genetic profiling of environmentally important organisms is very essential for easy identification of biodegrading bacteria. In the previous study, we have reported the perchlorate biodegrading bacteria and characterized them by biochemical analysis and 16 S sequencing. We have observed a very similar isolates of Arthrobacter (Actinobacteria) degrading 4.1 mM and 4.7 mM of ammonium perchlorate [1] 08D0C9EA79F9BACE118C8200AA004BA90B02000000080000000E0000005F005200650066003300390038003100390037003300340037000000 . In this study, we report PCR based DNA fingerprinting technique to generate the genomic signature of these closely related group of Arthrobacter species. This study also effectively generates unique genomic signature for each of these isolates that has potential for use in molecular monitoring as well as for tracking genomic variation and rearrangements.
