Prevalence and Factors Associated with Diabetes among City Hall Workers, Bohicon, Benin

Introduction: Living and working conditions are increasingly conducive to a sedentary lifestyle. Many administrative workers are exposed daily to long hours of work in a prolonged sitting position, which can favour the onset of diabetes. The aim of the study was to determine the prevalence and factors associated with diabetes among workers at a town hall in Benin in 2019. Methods: This was a descriptive cross-sectional study conducted from December 2018 to January 2019. Exhaustive recruitment was carried out. Data were collected during a face-to-face interview, using a standardized questionnaire including socio-professional factors, behavioral factors, anthropometric measurements, and blood glucose. Frequencies were calculated. Frequencies were compared using the chi-square and Fisher tests. The significance threshold was 5%. Results: A total of 85 workers were included. Their mean age was 41.55 ± 9.82 years, with a sex ratio of 6.1. = The prevalence of diabetes was estimated at 8.2% [95% CI: 3.38 - 16.23%] and 3 of the workers knew they had diabetes. A family history of diabetes was associated with diabetes (p = 0.032). Occupationally, 10.77% of permanent employees had diabetes, compared with 0.0% of non-permanent employees;12.50% of employees with at least 10 years’ seniority had diabetes, compared with 7.25% of those with less seniority;19.05% of employees who spent at least 6 hours a day sitting at work had diabetes, compared with 4.69% of those who spent less time sitting. Conclusion: Preventing chronic disease in the workplace is part of promoting workers’ health, and must take into account occupational factors such as prolonged sitting.

Introduction: Sleep disorders among professional drivers are a major road safety problem. They cause fatigue and drowsiness at the wheel, which can lead to road traffic accidents and even accidents at work. The aim of this study was to assess sleep disorders among professional drivers of oil tankers in Ouagadougou, Burkina Faso. Methods: This was a descriptive and analytical cross-sectional study carried out from 11 April 2020 to 11 September 2020 in the national hydrocarbon company’s fuel depot. All the drivers present at the time were included and accomplished a questionnaire incorporating the commonly scales using to assess sleep disorders, sleep quality and sleep apnoea. Data analysis using R 3.6.1 software enabled to perform univariate and multivariate analyses to identify associated factors. Adjusted odd ratios were used to measure the strength of association. The significance level chosen was p ≤ 0.05. Results: All the 339 respondents were male. A proportion of 33.6% suffered from insomnia, including 12.7% with moderate to severe insomnia, and 18.58% were at risk of sleep apnoea. On the Epworth scale, 26.6% of drivers showed sleep debt and 4.1% excessive daytime sleepiness. The risk factors associated with insomnia were the use of psychostimulants. A history of diabetes and insomnia were risk factors associated with sleep apnoea syndrome. Conclusion: The prevalence of sleep disturbance and insomnia among the respondents were 52.2% and 33.6% respectively. Use of psychostimulants was the identified risk factor. The prevalence of SAS was 4.72%, with diabetes and insomnia as associated factors.

Introduction: Non-communicable diseases (NCDs) are on the increase in several sectors of activity. The aim of the study was to determine the frequencies of cardiovascular risk factors (CVRF) and factors associated with hypertension among hotel workers in the city of Bohicon. Methods: This was a descriptive and analytical cross-sectional study conducted in the period from December 2018 to January 2019. The variables studied were sociodemographic, occupational, anthropometric and behavioral. Participant recruitment was exhaustive. A standardized questionnaire inspired by the STEPS-WHO questionnaire was used for data collection. After the descriptive analysis, a bivariate analysis was performed using Pearson’s Chi-square test or Fisher’s test at the 5% threshold. Results: A total of 101 workers, 53 (52.47%) of them male, participated in the study. The mean age was 30.60 ± 8.20 years, with extremes of 18 and 55 years. The frequencies of the main risk factors were: 68.32% sitting position > 6 hours/day;38.61% shift work;21.78% high psychological demand, 82.18% low decision latitude, 24.75% low social support, 21.78% occupational stress;7 (6.93%) harmful alcohol consumption;6.93% tobacco consumption;48.51% insufficient physical activity;98.02% insufficient fruit and vegetable consumption;59.41% overweight;14.85% HTA. Factors associated with hypertension were: sex, age and marital status. Conclusion: Occupational factors need to be taken into account when assessing CVRFs. NCD control programs are needed in all sectors of activity. A larger study is needed in the hotel sector.

Epidemiological Profile of Dermatosis among Automobile Repair and Maintenance Garages in the City of Parakou in 2020

Background: Occupational dermatoses, also called occupational dermatitis or occupational dermatitis, are very common in the workplace and are still a topical subject. All over the world, especially in developed countries. They handicap the professional activities of the workers. Purpose: To determine the epidemiological profile of dermatosis among automobile repair and maintenance garages in Parakou. Methods: It was a cross-sectional and descriptive study carried out from 20 April to 17 May 2020. The sampling method was non-probabilistic. Data collected from a questionnaire, clinical examination and worksite observation were analyzed with Epi Info 7.***.2.1 software. Results: The participation rate was 98.08%. The sex ratio (male/female) was 178. The mean age of the respondents was 25.80 ± 10.22 years. Respondents with less than 5 years of experience in the field were 61.22%. Mechanics were the most numerous. In the sample, 95.81% had frequent skin contact with fuels, 84.08% had contact with motor oils and 15.92% had contact with dilute battery electrolyte solutions. The frequency of dermatosis was 73.46%. Hands and wrists were the most affected seats (47.35%). Some (33.84%) wore them before the start of the activity in the garage but in 66.22% of them, the dermatosis had occurred because of their arrival in the garage. Among these skin diseases, keratoderma predominated at 80.01%. Conclusion: Dermatosis is very common among garage owners in Parakou.

Hearing Loss among Grain Millers in the Informal Sector in Benin

Introduction: Noise-induced hearing loss is a preventable health problem worldwide. However, it continues to affect workers especially in the informal sector, due to the lack of medical and environmental monitoring. In Benin, millers are highly exposed. The objective of the study was to assess the hearing health situation of grain millers in the Dantokpa market in 2020. Methods: This was a cross-sectional study that included by exhaustive recruitment 57 millers. The data were collected using a standardized questionnaire followed by blood pressure measurements, noise levels and the performance of audiometries. Descriptive and univariate analysis was performed. Results: The sample consisted only of men. The median age was 25 years with extremes of 18 and 50 years. Noise levels at the workstation ≥ 85 dB (A) were obtained for 94.74% of the workers. No worker was wearing hearing protection equipment. The prevalence of hearing loss was 87.72% (95% CI = [76.32%;94.92%]) and that of occupational deafness was 29.82% (95% CI = [18.43% - 43.40%]) which was associated with age over 28 and high blood pressure. An awareness campaign with donation of hearing pads was carried out. Conclusion: Hearing loss is important among millers who do not benefit from any supervision because they are in the informal sector. A restructuring of this sector is necessary for the hearing health of workers.

Introduction: The use of biomass for cooking is a very common practice in Africa and Benin, and is associated with exposure to organic pollutants. The aim of this study was to assess carbon monoxide exposure and respiratory disorders in women at artisanal fish-smoking sites. Methods: This was a cross-sectional study of women fish smokers at two artisanal fish-smoking sites in Benin. Recruitment was exhaustive. An interview using a respiratory questionnaire inspired by the Union questionnaire, spirometry and carbon monoxide (CO) measurement were carried out. Descriptive analysis and comparison of means using Student’s t-test with a threshold of p = 0.05 were performed. Results: The median age of the 81 people surveyed was 40, with an interquartile range (IQT) of [25 to 75]. The median duration of daily exposure to smoke was 6 h IQT [4, 7]. Of those surveyed, 64 (79.01%) were illiterate and 39 (48.24%) had been working for 20 years. CO levels at the start of activity varied from 89 to 145 ppm in one case and from 40 to 89 ppm in the other. Respiratory symptoms were present in 19 (23.46%);18 (22.22%) had mild airway impairment on spirometry. There was a statistically significant decrease between the mean values of peak expiratory flow (PEF), mean expiratory volume in seconds (FEV1) and forced vital capacity (FVC) before and after exposure to smoke. Conclusion: CO levels are high on fish smoking sites and respiratory problems are common. Improved working conditions are important for these women.

Introduction: One form of child labor is early learning, which is a less worrying phenomenon in our communities in Benin. The objective of this study was to assess the practice of early learning for children in rural areas. Methods: This was a cross-sectional study combined with a qualitative component conducted in the Kissamey district of Benin with four targets: child apprentices (52), master craftsmen (41), parents and guardians (34), local authorities (9). The collection tools were a questionnaire and an interview guide. Results: The frequency of early learning among children was 32.07% with difficult socioeconomic conditions: polygamy (75%), strong siblings (79%), out of school (33%), unmet food needs (96%). The reasons for early learning according to parents were: refusal of the child to go to school (44%), financial difficulties (31%), school failure (22%), but 38% of these children did not know the reason for their learning. The actors had little knowledge of the regulatory texts. Conclusion: Early learning remains a societal problem related to out-of-school and difficult socioeconomic conditions.

Results of the Evaluation of the Respiratory Condition of Permanent Workers in an Oil Mill in Benin

Introduction: The process of extracting oil from cotton seeds can create dusty work atmospheres that can cause respiratory problems. The main objective of this study was to determine the prevalence of respiratory problems among permanent workers in an oil mill in Benin. Methods: This cross-sectional study of 52 workers in an oil mill took place in January 2017 as part of the annual medical check-ups of workers. A questionnaire was administered and spirometry using Spirobank II and chest radiography were performed. The spirometry results were interpreted by an occupational physician and a pulmonologist. Data were entered and analyzed using Epidata software. Results: The mean age was 42.7 ± 6.4 years, and 43 of the 52 workers were men. Of these, 58% were in technical production positions and 42% in administrative positions. Most of them had more than 10 years of seniority. The prevalence of respiratory symptoms among production workers was 4 (13%) versus 2 (9%) among administrative workers. A total of 8 (15.4%) abnormal spirometry was identified with 4 obstructive syndrome, 3 restrictive syndrome, 1 a mixed pattern. There were 5 (16.6%) workers in production versus 3 (13.6%) in administration who had abnormal spirometry. The means 25/75 forced expiratory flow (FEF) value of production workers was significantly lower than that of administration workers. Abnormal chest radiographs were 5 (17%) in production workers compared to 3 (14%) in administration workers. Conclusion: Oil mill workers had few respiratory symptoms. However, production workers had more ventilatory disorders than administrative workers. A spirometric follow-up of this group of workers is therefore necessary.

Introduction: In the general population as well as in the workplace, hypertension is one of the main causes of morbidity and mortality in the world. The particularities of hypertension in the world of work remain little studied. The objective of this study was to determine the prevalence and factors associated with hypertension among workers in a town hall in Benin in 2019. Methods: This was a descriptive, analytical cross-sectional study conducted from December 2018 to January 2019. A comprehensive recruitment was conducted. Data were collected during a face-to-face interview, supplemented by measurement of anthropometric parameters and blood pressure as well as a work stress assessment. Frequencies were compared using the chi-square test and Fisher’s test. The significance level was 5%. Results: A total of 86 workers were included. Their mean age was 41.55 ± 9.82 years with a sex ratio of 6.1. The prevalence of hypertension was estimated to be 18.6% [95% CI: 11.96% - 29.75%]. Hypertension was higher in workers under work stress (41.18%) as opposed to non-stressed workers (14.49%) (p = 0.032);in those who were overweight (30.77) as opposed to non-overweight (2.94%) (p Conclusion: Work stress and obesity are modifiable factors associated with hypertension in workers at the Bohicon city hall. These factors deserve to be corrected by occupational health promotion and prevention actions.

Prevalence and Factors Associated with Vitamin D Deficiency in Patients with COVID-19 in Parakou in 2021

Introduction: Coronavirus 19 disease (COVID-19) often presents several serious complications in patients. 25-hydroxyvitamin D [25(OH)D] deficiency is considered a risk factor for severe forms of the disease. Objective: The objective of this work was to determine the prevalence and factors associated with vitamin D deficiency in patients with COVID-19 in Parakou in 2021. Methods: The method used was a descriptive and analytical cross-sectional study with prospective data collection that covered the period of August 1, 2021, to December 30, 2021. It concerned patients with COVID-19, symptomatic or not, in the commune of Parakou, selected by non-probability sampling. The 25(OH)D assay was performed by an enzyme immunoassay using the heterogeneous phase competition technique. Logistic regression was used to determine the factors associated with 25(OH)D deficiency at the 5% threshold. Results: A total of 197 patients with COVID-19 were included in the study with a mean age of 35.4 ± 15.2 years;with a female predominance (52.3%). The overall prevalence of 25(OH)D deficiency was 31.5% (95% CI [25.1 - 38.5]);it was 32.0% in females and 30.8% in males. 25(OH)D deficiency was more observed in patients ≥ 60 years than in patients < 60 years with no significant difference (p = 0.121), and in females (32.0%) than in males (30.8%) with no significant difference either (p = 0.857). Diabetes (p = 0.036), overweight or obesity (p = 0.032), severe disease forms (p = 0.003) and rhinitis (p = 0.009) were significantly associated with 25(OH)D deficiency. Conclusion: One-third of patients with COVID-19 in Parakou in 2021 were 25(OH)D deficient. 25(OH)D deficiency is associated with the severe form of the disease and with comorbidities justifying supplementation of this vitamin to patients with COVID-19.
