Autologous bone marrow derived stem cell therapy in patients with type 2 diabetes mellitus-defining adequate administration methods

AIM To carry out randomized trial for evaluating effects of autologous bone marrow derived stem cell therapy(ABMSCT) through different *** Bone marrow aspirate was taken from the iliac crest of patients. Bone marrow mononuclear cells were separatedand purified using centrifugation. These cells were then infused in a total of 21 patients comprising three groups of 7 patients each. Cells were infused into the superior pancreaticoduodenal artery(Group Ⅰ), splenic artery(Group Ⅱ) and through the peripheral intravenous route(Group Ⅲ). Another group of 7 patients acted as controls and a sham procedure was carried out on them(Group Ⅳ). The cells were labelled with the PET tracer F18-FDG to see their homing and in vivo distribution. Data for clinical outcome was expressed as mean ± SE. All other data was expressed as mean ± SD. Baseline and post treatment data was compared at the end of six months, using paired t-test. Cases and controls data were analyzed using independent t-test. A probability(P) value of < 0.05 was regarded as statistically significant. Measures of clinical outcome were taken as the change or improvement in the following parameters:(1) C-peptide assay;(2) HOMA-IR and HOMA-B;(3) reduction in Insulin dose; subjects who showed reduction of insulin requirement of more than 50% from baseline requirement were regarded as responders; and(4) reduction in HbA 1c. RESULTS All the patients, after being advised for healthy lifestyle changes, were evaluated at periodical intervals and at the end of 6 mo. The changes in body weight, body mass index, waist circumference and percentage of body fat in all groups were not significantly different at the end of this period. The results of intra-group comparison before and after ABMSCT at the end of six months duration was as follows:(1) the area under C-peptide response curve was increased at the end of 6 mo however the difference remained statistically non-significant(P values for fasting C-peptide were 0.973, 0

We obtain an exact solution for spherically symmetric Letelier AdS black holes immersed in perfect fluid dark matter(PFDM).Considering the cosmological constant as the positive pressure of the system and volume as its conjugate variable,we analyze the thermodynamics of our black holes in the extended phase *** to the background clouds of strings parameter(a)and the parameter endowed with PFDM(β),we analyze the Hawking temperature,entropy,and specific ***,we investigate the relationship between the photon sphere radius and phase transition for the Letelier AdS black holes immersed in *** the analysis,with a particular condition,non-monotonic behaviors are found between the photon sphere radius,impact parameter,PFDM parameter,temperature,and *** can regard the changes in both the photon sphere radius and impact parameter before and after phase transition as the order parameter;their critical exponents near the critical point are equal to the same value,1/2,similar to that in ordinary thermal *** indicates that a universal relation of gravity may exist near the critical point for a black hole thermodynamic system.

A randomized controlled trial on triamcinolone versus saline impregnated merocel post endoscopic sinus surgery: Our experience in a tertiary care centre

Purpose: To assess the potential benefit of impregnating Merocel (a non-absorbable nasal dressing) with a topical steroid solution, for use as a direct and slow local delivery system of steroids after sinus surgery to improve postoperative wound ***: In this randomized controlled trial, 40 patients with bilateral chronic rhinosinusitis with nasal polyposis were subjected to functional endoscopic sinus surgery. Following the completion of the surgery, Merocel packs were inserted in the bilateral nasal cavities and infiltrated with 4 mL triamcinolone (40 mg/mL) in one nasal cavity (treatment group) and 4 mL normal saline in the other (control group). Nasal packs were removed on the third postoperative day and postoperative healing assessment was done on postoperative Weeks 1, 2, 4, and 12. The findings were noted as per Lund Kennedy (LKES) and perioperative sinus endoscopy (POSE) scores and compared on both ***: Significant (P < 0.05) improvement was noted in Lund Kennedy score for crusting and polyp at Week 12, for edema at Week 1, and nasal discharge at Weeks 1 and 12, but there was no significant improvement in scarring at any week. Overall, the difference between the treatment and control arms was statistically significant at all postoperative visits except at Week 4. Also, there was a significant improvement in POSE scores at Weeks 1, 2, and 12 but not at Week ***: This study positively concludes that the nasal cavity packed with drug-soaked packs had less scarring and edema in the postoperative period and the overall wound healing was much better as compared to saline-soaked packs.

GUARDIAN: A Multi-Tiered Defense Architecture for Thwarting Prompt Injection Attacks on LLMs

This paper introduces a novel multi-tiered defense architecture to protect language models from adversarial prompt attacks. We construct adversarial prompts using strategies like role emulation and manipulative assistance to simulate real threats. We introduce a comprehensive, multi-tiered defense framework named GUARDIAN (Guardrails for Upholding Ethics in Language Models) comprising a system prompt filter, pre-processing filter leveraging a toxic classifier and ethical prompt generator, and pre-display filter using the model itself for output screening. Extensive testing on Meta’s Llama-2 model demonstrates the capability to block 100% of attack prompts. The approach also auto-suggests safer prompt alternatives, thereby bolstering language model security. Quantitatively evaluated defense layers and an ethical substitution mechanism represent key innovations to counter sophisticated attacks. The integrated methodology not only fortifies smaller LLMs against emerging cyber threats but also guides the broader application of LLMs in a secure and ethical manner.

Meigs syndrome(MS)is characterized by a benign ovarian tumor(fibroma),hydrothorax on the right side,and ascites;which can be resolved permanently after *** literature reveals that most MS surgeries were performed under general anesthesia(GA)[1].However,GA poses major risks to the *** the high risk of gastric content regurgitation,poor general condition or dyselectrolytemia may lead to delayed arousal,and associated multi-organ *** mechanical ventilation is also difficult due to ascites and hydrothorax(reduced cardiac output,impaired ventilation-perfusion in lungs causing hypoxia and hypercapnia)[1,2].In this letter,we report a case of MS tumor resection under the subarachnoid block(SAB)to mitigate these issues and also review the complications associated with both techniques.

In this study,we measured the58Ni（n,p）58Co reaction cross section with neutron energies of 1.06,1.86,and 2.85 *** cross section was measured using neutron activation techniques and γ-ray spectroscopy,and it was compared with cross section data available in the ***,we calculated the covariance matrix of the measured cross section for the aforementioned nuclear *** uncertainties of the theoretical calculation for58Ni（n,p）58Co reaction cross section were calculated via Monte Carlo *** this study,we used uncertainties in the optical model and level density parameters to calculate uncertainties in the theoretical cross *** theoretical calculations were performed by using *** this study,we aim to analyze the effect of uncertainties of the nuclear model input as well as different experimental variables used to obtain the values of reaction cross section.

Evaluating Privacy Leakage and Memorization Attacks on Large Language Models (LLMs) in Generative AI Applications

The recent interest in the deployment of Generative AI applications that use large language models (LLMs) has brought to the forefront significant privacy concerns, notably the leakage of Personally Identifiable Information (PII) and other confidential or protected information that may have been memorized during training, specifically during a fine-tuning or customization process. We describe different black-box attacks from potential adversaries and study their impact on the amount and type of information that may be recovered from commonly used and deployed LLMs. Our research investigates the relationship between PII leakage, memorization, and factors such as model size, architecture, and the nature of attacks employed. The study utilizes two broad categories of attacks: PII leakage-focused attacks (auto-completion and extraction attacks) and memorization-focused attacks (various membership inference attacks). The findings from these investigations are quantified using an array of evaluative metrics, providing a detailed understanding of LLM vulnerabilities and the effectiveness of different attacks.

Advancing frontiers in anaesthesiology with laparoscopy

The introduction of laparoscopy in the surgeon’s armamentarium was in fact a “revolution in the history of surgery”. Since this technique involves insufflation of carbon dioxide it produces several pathophysiological changes which have to be understood by the anaesthesiologist who can modify the anaesthesia technique accordingly. Advantages of laparoscopy include reduced pain, small scars and early return to work. Certain complications specific to laparoscopic surgery are due to carboperitoneum and increased intra-abdominal pressure. Venous air embolism, although very rare, can be lethal if not managed promptly. Other complications include subcutaneous emphysema, haemodynamic compromise and arrhythmias. Although associated with minimal postoperative morbidity, postoperative pain, nausea and vomiting can be quite problematic. The limitations of laparoscopy have been overcome by the introduction of robotic surgery. There are important implications for the anaesthesiologist during robotic surgeries which have to be practiced accordingly. Robotic surgery has a learning curve for both the surgeon and the anaesthesiologist. The robot is bulky, and cannot be disengaged after docking. Therefore it is important that the anaesthetized patient remains immobile throughout surgery and anaesthesia is reversed only after the robot has been disengaged at the end of surgery. Advances in laparoscopy and robotic surgery have modified anaesthetic techniques too.

Tribological response of waste tire rubber as micro-fillers in automotive brake lining materials

Elastomeric materials show promise as potential micro-fillers in brake *** can provide vibration damping and acoustic advantages in intermittent and abrupt impact applications such as *** elastomeric material can be salvaged from non-biodegradable automotive tires,thereby providing an opportunity to reuse materials that will otherwise be discarded in *** tribological and thermomechanical performances of the waste tire rubber were assessed to determine their potential for use as micro-fillers in the brake linings of commercial vehicles with a gross weight less than 16 ***,the brake lining materials were fabricated with fine waste tire rubber particulates(WTRPs)as the micro-fillers,phenolic-R resin as the binder,graphite as the dry lubricant,laterite as the co-filler,and coconut coir for natural fiber *** effects of increasing the WTRP weight fraction on the brake response of the linings were analyzed,and the different compositions were benchmarked against a commercial brake *** characterization comprising compressive strength,hardness,density,and porosity studies were carried *** and wear characteristics of the linings were analyzed using a rotary tribometer with simultaneous thermal *** manufactured lining with 15 wt%WTRPs exhibited a mean friction coefficient of^0.38,a specific volumetric loss rate of 1,662μm3/(N?m),and improved thermal *** optical microscopy and scanning electron microscopy(SEM)with energy dispersive spectroscopy(EDS),wear surface morphology studies compared the relative development of primary and secondary plateaus and revealed the redistribution of wear debris,leading to the stability of the coefficient of friction.

Long term results of use of azathioprine in patients with ulcerative colitis in India

AIM： To evaluate the role of azathioprine （AZA） in Indian patients with ulcerative colitis over longer duration of time. METHODS： One hundred fifty six patients with ulcerative colitis who were treated with AZA from January 1995 to December 2003 were reviewed. The indications for its use were as follows：（1） steroid dependent and steroid refractory disease; （2） Azathioprine monotherapy for na＇fve patients with severe disease; and （3） combination therapy （AZA ＋ sulfasalazine or 5-aminosalicylates） for naive patients with severe disease. The data included patient and disease demographics, efficacy and toxicity profile of AZA. Patients with a minimum duration of 6 mo use of AZA were included in this report. RESULTS： Of a total of 156 patients treated with AZA, 45 were excluded from analysis for the following reasons- （follow up less than 6 mo, n = 9; poor follow up, n = 18; adverse affects, n = 18）. In steroid refractory/dependent group the mean number of relapses prior to and post initiation of AZA therapy were 3.28 （± 0.81） and 0.94 （± 0.29） respectively. Discontinuation of steroids could be accomplished in 12 of the 15 steroid dependent patients. The proportion of patients with sustained remission of 1, 2, 3, 4 and 5 years duration were calculated. Eighteen patients experienced adverse effects necessitating withdrawal of AZA （pancreatitis, n = 7; hepatitis, n = 3; gastrointestinal intolerance, n = 2; alopecia, n = 2; and hematological, n = 4） while 13 patients needed dose reduction or temporary withdrawal of the drug. CONCLUSION： Azathioprine is well tolerated and has therapeutic benefits lasting as long as 4 years. Adverse effects such as pancreatitis, hepatitis, cytopenias and gastrointestinal symptoms do occur but are controlled by drug withdrawal only.
