Pre-and-post transplant considerations in patients with nonalcoholic fatty liver disease

Non-alcoholic fatty liver disease(NAFLD) is currently the third most common indication for liver transplantation in the United States. With the growing incidence of obesity, NAFLD is expected to become the most common indication for liver transplantation over the next few decades. As the number of patients who have undergone transplantation for NAFLD increases, unique challenges have emerged in the management and long-term outcomes in patients. Risk factors such as obesity, hypertension, diabetes, and hyperlipidemia continue to play an important role in the pathogenesis of the disease and its recurrence. Patients who undergo liver transplantation for NAFLD have similar long-term survival as patients who undergo liver transplantation for other indications. Research shows that post-transplantation recurrence of NAFLD is commonplace with some patients progressing to recurrent non-alcoholic steatohepatitis and cirrhosis. While treatment of comorbidities is important, there is no consensus on the management of modifiable risk factors or the role of pharmacotherapy and immunosuppression in patients who develop recurrent or de novo NAFLD *** review provides an outline of NAFLD as indication for liver transplantation with a focus on the epidemiology, pathophysiology and risk factors associated with this disease. It also provides a brief review on the pretransplant considerations and post-transplant factors including patient characteristics, role of obesity and metabolic syndrome, recurrence and de novo NAFLD, outcomes post-liver transplantation, choice of medications, and options for immunosuppression.

Objective: Bergenia ciliata(Haw.) Sternb. is used in the Indian traditional system of medicine to treat various ailments including rheumatism and to heal wounds. The objective of the present study was to perform a preclinical characterization of the B. ciliata-based botanical extract ***: ⅢM-160 was chemically standardized and analyzed for heavy metal content, aflatoxins,pesticides and microbial load. The in vitro and in vivo efficacies were determined in suitable models of inflammation, arthritis and nociception. An acute oral toxicity study was performed in Swiss albino mice.A suitable oral formulation was developed and ***: Bergenin was found to be the major component(9.1% w/w) of ⅢM-160. The botanical lead displayed inhibition of lipopolysaccharide-induced production of proinflammatory cytokines in THP-1 cells, with selectivity toward interleukin-6(IL-6) and had an excellent safety-window. It showed anti-inflammatory, anti-arthritic and antinociceptive activity in animal models and was not toxic at oral doses up to 2 g/kg in Swiss-albino mice. The gastroretentive, sustained-release capsule formulation showed sustained-release of the bergenin over the period of 24 h, resulting in improved plasma-exposure of bergenin in Sprague–Dawley ***: The dual-activity of IL-6 inhibition and antinociception marks the suitability of ⅢM-160 for treating rheumatoid arthritis. This study will serve as the benchmark for further research on this botanical formulation.

A Review on Detonation Gun Sprayed Coatings

Materials are precious resources. Different methods are employed to protect the material from degradation. Thermal spraying is one of the most effective method to protect the material from wear, high temperature corrosion, stresses and erosion, thus increasing the life of material in use. Detonation gun spraying is one of the thermal spraying techniques known for providing hard, wear resistant and dense microstructured coatings. This paper summarizes the results of previous research done by various authors on different coatings done by detonation gun spraying technique.

Synthesis and Characterization of Co<sub>3</sub>O<sub>4</sub>Thin Film

Nanosized Co3O4 thin films were prepared on glass substrates by using sol-gel spin coating technique. The effect of annealing temperature (400°C - 700°C) on structural, morphological, electrical and optical properties of Co3O4 thin films were studied by X-ray diffraction (XRD), Scanning Electron Microscopy (SEM), Electrical conductivity and UV-visible Spectroscopy (UV-Vis). XRD measurements show that all the films are nanocrystallized in the cubic spinel structure and present a random orientation. Six prominent peaks, corresponding to the (111) phase (2θ ≈ 18.90°), (220) phase (2θ ≈ 31.29°), (311) phase (2θ ≈ 36.81°), (222) phase (2θ ≈ 38.54°), (400) phase (2θ ≈ 44.80°), (511) phase (2θ ≈ 59.37°) and (440) phase (2θ ≈ 65.27°) appear on the diffractograms. The crystallite size increases with increasing annealing temperature. These modifications influence the optical properties. The morphology of the sol gel derived Co3O4 shows nanocrystalline grains with some overgrown clusters and it varies with annealing temperature. The optical band gap has been determined from the absorption coefficient. We found that the optical band gap energy decreases from 2.58 eV to 2.07 eV with increasing annealing temperature between 400°C - 700°C. These mean that the optical quality of Co3O4 films is improved by annealing. The dc electrical conductivity of Co3O4 thin films were increased from 10–4 to 10–2 (Ω.cm)–1 with increase in annealing temperature. The electron carrier concentration (n) and mobility (μ) of Co3O4 films annealed at 400°C - 700°C were estimated to be of the order of 2.4 to 4.5 × 1019 cm–3 and 5.2 to 7.0 × 10–5 cm2.V–1.s–1 respectively. It is observed that Co3O4 thin film annealing at 700°C after deposition provide a smooth and flat texture suited for optoelectronic applications.

Gossypin: A phytochemical of multispectrum potential

Nature is the source of the various raw chemicals in the form of oils, food supplement, neutraceuticals, and colour pigments. A lot of presently existing drugs available in the market mainly belong to the natural sources including the plants, animal, microorganism, minerals and marine source. Flavonoids, benzo-c-pirone derivates are one of the main secondary flavonoid liable for the different shades of flowers like orange, yellow and red color. More than 4000 flavonoids like flavanols, flavonols, isoflavones, flavanones, flavanonols and flavones are found in the different edible plants which are also found in the regular human diet. Flavonoids are present in vegetables, spices, tea, whole grains, fruits, wine, herbs and seeds. Gossypin, a flavonol glucoside, is present in the flowers of different plants like Gossypium indicum, Hibiscus esculentus and Hibiscus vitifolius and is one of the major chemical of plants belonging to 'Malvaceae' family. In the present review, we have collected all the information of gossypin and found that it has antioxidant, antiinflammatory, anticancer, anticataract, antidiabetic, analgesic and hepatoprotective activities. It also protects against beta-amyloid induced toxicity. The information provided in the present review will be beneficial to the researchers of the various field of science for the development of better alternate for various disorders. On the basis of the presented database of this review we can conclude that gossypin has various beneficial effects and it could be used for the treatment of various disorders.

Computerized tomography based “patient specific blocks” improve postoperative mechanical alignment in primary total knee arthroplasty

AIM: To compare the postoperative mechanical alignment achieved after total knee arthroplasty(TkA) using computer tomography(CT) based patient specific blocks(PSB) to conventional instruments(CI).METHODS: Total 80 knees were included in the study, with 40 knees in both the groups operated using PSB and CI. All the knees were performed by a single surgeon using the same cruciate sacrificing implants. In our study we used CT based PSB to compare with CI. Postoperative mechanical femoro-tibial angle(MFT angle) was measured on long leg x-rays using picture archiving and communication system(PACS). We compared mechanical alignment achieved using PSB and CI in TkA using statistical ***: The PSB group(group 1) included 17 females and seven males while in CI group(group 2) there were 15 females and eight males. The mean age of patients in group 1 was 60.5 years and in group 2 it was 60.2 years. The mean postoperative MFT angle measured on long-leg radiographs in group 1 was 178.23°(SD = 2.67°, range: 171.9° to 182.5°) while in group 2, the mean MFT angle was 175.73°(SD = 3.62°, range: 166.0° to 179.8°). There was significant improvement in postoperative mechanical alignment(P value = 0.001), in PSB group compared to CI. Number of outliers were also found to be less in group operated with PSB(7 knee) compared to those operated with CI(17 knee).CONCLUSION: PSB improve mechanical alignment after total knee arthroplasty, compared to CI. This may lead to lower rates of revision in the PSB based TkA as compared to the conventional instrumentation.

Association between Toll-Like Receptor 4 (Thr399Ile and Asp299Gly), Toll-Like Receptor 2 196-174 Deletion Polymorphisms with cryptosporidiosis

Toll-like receptors (TLRs) 2 and 4 specifically recognize lipopolysaccharide motifs on surfaces of microorganisms. Polymorphims in the TLR genes result in structural variations in the proteins, abnormal host-pathogen interactions and hence, altered immune response. Opportunistic parasite Cryptosporidium spp. infects immuno-compromised patients who present with heterogeneous clinical manifestations ranging from mild infection resolving in a few weeks, to severe form characterized by voluminous diarrhoea, prolonged symptoms and recurrences. The present study investigates the role of TLR2 196_174del, TLR4 Asp299Gly and Thr399Ile polymorphisms in pathogenesis of cryptosporidiosis. Stool samples were collected from 210 immuno-compromised (renal transplant recipients and patients with Human immunodeficiency virus infection) both with and without cryptosporidiosis, and 200 healthy subjects for detection of Cryptosporidium spp. Blood samples were also collected from the patients and healthy subjects for genotyping of Δ22 (TLR2 196_174del) polymorphism, A896G (TLR4 Asp299Gly) and C1196T (Thr399Ile) single nucleotide polymorphisms (SNPs) by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). Cryptosporidium spp. was detected in 70 immunocompromised patients, while it was absent in all the healthy controls. No significant difference was observed in Δ22, A896G and C1196T alleles and genotypes (P > 0.05), between cases and controls. Thus, TLR2 196_174del, TLR4 Asp299Gly and Thr399Ile polymorphisms are not significantly associated with the pathogenesis or progression of cryptosporidiosis, in the Indian population included in the study.

Apart from many advantages,High Strength Concrete(HSC)has disadvantages in terms of brittleness and poor resistance to *** studies suggest that when polypropylene(PP)fibers are uniformly distributed within concrete,they play an active role in improving spalling resistance of concrete when exposed to elevated temperature while having no adverse effect on its mechanical ***,there is a necessity to quantify the effect of the addition of polypropylene fibers in terms of the fiber dosage,the strength of the concrete,and the residual mechanical properties of fiber-reinforced concrete under exposure to high temperature from *** study was carried out on three water/cement(w/c)ratios(0.47,0.36&0.20)using granite aggregate for determining short term mechanical properties of Polypropylene fiber reinforced concrete in comparison to control *** experimental program includes 100×200 mm&150 x 300 mm cylinders with fiber volume of 0.5%,that were subjected to temperatures exposures of 400°C and 600°C for durations of 1 *** the results,it was observed that no significant enhancement in mechanical properties such as modulus of elasticity,Poisson’s ratio,split tensile strength,flexural strength,and compressive strength was observed at room temperature and at elevated temperatures.

Study of Propagation of Ion Acoustic Waves in Argon Plasma

The properties of small amplitude acoustic waves (IAW) in unmagnetised plasma have been discussed in detail. An experimental set up to study the propagation of IAW in argon plasma has been descried. The speed of IAW under different conditions of discharge current and pressure has been measured from the time-of flight technique. From these measurements, electron temperatures have been calculated. The results have been compared with those obtained by single probe method, and were found to be in good agreement with each other so IAW speeds can be used to calculate plasma parameters.
