Cord complications: associated risk factors and perinatal outcome

Objective: To assess the perinatal outcome in patients with cord presentation and cord prolapse over a period of 2 years and to assess the obstetric risk factors associated with these cord complications. Design: Retrospective case study. Setting: A tertiary health care centre of India. Sample: All cases of cord presentation and cord prolapse diagnosed in the department over a period of 2 years (2009-2010). Methods: The information regarding maternal presentation at the time of admission, mode of delivery, and neonatal outcome was retrieved from case sheets of patients available in Medical Record Department. Maternal factors in terms of age, parity, gestational age, mode of presentation, mode of delivery were analysed. Foetal outcome was assessed by Diagnosis to Delivery Interval, APGAR score at 0 minutes and 5 minutes, foetal weight and need for NICU admission. Results: There were 53 cases of cord complications, out of which 7 (13.20%) had cord presentation and 46 (86.80%) had cord prolapse. Among these 46 patients, 11 patients developed cord prolapse in labour room (3 after artificial rupture of membranes and 8 after spontaneous rupture of membranes), rest 34 patients presented in emergency with cord prolapse. There were 79.24% multiparous patients, 16 patients had malpresentations and breech was commonest malpresentation. Forty one patients had positive cord pulsations at the time of admission, all these patients underwent emergency caesarean section and all had live babies. Twelve patients reported to us with absent cord pulsation, 3 among these underwent section for obstructed labour with hand and cord prolapse, rest 9 delivered vaginally. The average DDI was 26.00 minutes, DDI in patients developed cord prolapse in hospital was 18.90 minutes ± 5.48 minutes, whereas in patients admitted with cord prolapse as emergency, the DDI was 29.34 minutes ± 6.37 minutes (p < 0.05). Abdominal wall hernia in a rural population in India—Is spectrum changing?

Hernia is a common word that most surgeons are familiar with. A retrospective study was planned to analyse the spectrum of abdominal wall hernias in a rural population inIndia. Majority of the patients were of 40 - 70 yrs. Male to female ratio was 7:1. Incidence of groin hernias showed an increasing trend with advancing age. Out of total 320 cases, inguinal hernias were predominating in 77.81% cases. Ventral hernias were present in about 18% cases. However, femoral hernias were rare. We concluded that spectrum of abdominal wall hernias is almost the same all over the globe despite having differences in their socioeconomic and educational status.

A prospective randomized, placebo-controlled trial comparing mifepristone and vaginal misoprostol to vaginal misoprostol alone for elective termination of early pregnancy

Author: Dr Roopa Malik, Assistant Professor, Obstetrics and gynaecology department Pt BDS PGIMS Rohtak BACKGROUND: Vaginal misoprostol has been shown to be an effective single agent for medical agent for medical abortion. This randomized, placebo controlled trial compared a regimen of mefipristone and misoprostol with misoprostol alone for termination of early pregnancy. METHODS: 200 women with gestation <56 days were randomized by a random number table to receive either 200 mg mifepristone orally or placebo followed 48 h later by 800 ug vaginal misoprostol. Abortion success was defined as complete abortion without the use of surgical aspiration. RESULTS: Successful medical abortions occurred in 96 out of 100 subjects (96%) after mifepristone followed by vaginal misoprostol. In all, 79 out of 100 subjects (79%) successfully aborted after placebo and vaginal misoprostol. The higher success rate of complete abortion with mifepristone and misoprostol regimen was statistically significant compared with the placebo and misoprostol regimen (p < 0.05). CONCLUSION: A regimen of mifepristone and misoprostol was significantly more effective for termination of pregnancies <56 days than misoprostol alone. The misoprostol alone regimen for termination of early pregnancy is not a very good method for medical abortion but 79% efficacy obtained with vaginal misoprostol alone may clinically acceptable when mifepristone is not available.

SPICE compatible analytical electron mobility model for biaxial strained-Si-MOSFETs

This paper describes an analytical model for bulk electron mobility in strained-Si layers as a function of strain. Phonon scattering, columbic scattering and surface roughness scattering are included to analyze the full mobility model. Analytical explicit calculations of all of the parameters to accurately estimate the electron mobility have been made. The results predict an increase in the electron mobility with the application of biaxial strain as also predicted from the basic theory of strain physics of metal oxide semiconductor （MOS） devices. The results have also been compared with numerically reported results and show good agreement.

In the present paper,we consider a problem of reflection and transmission of plane waves at an interface between two different transversely isotropic micropolar piezoelectric *** plane wave solution of governing equations for micropolar piezoelectric medium indicates the propagation of three coupled plane *** incident plane wave at an interface between two dissimilar half-spaces generates three reflected and three transmitted *** relations between amplitude ratios of reflected and transmitted waves are derived which,in general,depend on material parameters and angle of *** explicit expressions for energy ratios of reflected and transmitted waves are also *** relevant material parameters,the results are illustrated graphically to show the micropolar piezoelectric effects on variations of amplitude ratios and the square root of energy ratios against the angle of incidence.

In cutting-edge optical technologies,polarization is a key for encoding and transmitting vast information,highlighting the importance of selectively switching and modulating polarized ***,anisotropic two-dimensional materials have emerged for ultrafast switching of polarization-multiplexed optical signals,but face challenges with low polarization ratios and limited spectral ***,we apply strain to quasi-one-dimensional layered ZrSe_(3)to enhance polarization selectivity and tune operational energies in ultrafast all-optical ***,transient absorption on unstrained ZrSe_(3)reveals a sub-picosecond switching response in polarization along a specific crystal axis,attributed to shifting-recovery dynamics of an anisotropic ***,its polarization selectivity is weakened by a slow non-excitonic response in the perpendicular *** overcome this limitation,we apply strain to ZrSe_(3)by bending its flexible *** compressive strain spectrally decouples the excitonic and nonexcitonic components,doubling the polarization selectivity of the sub-picosecond switching and tripling it compared to that in the tensile-strained ZrSe_(3).It also effectively tunes the switching energy at a shift rate of~93 meV%-*** strain-tunable switching is repeatable,reversible,and robustly maintains the sub-picosecond ***-principles calculations reveal that the strain control is enabled by momentum-and band-dependent modulations of the electronic band structure,causing opposite shifts in the excitonic and non-excitonic *** findings offer a novel approach for high-performance,wavelength-tunable,polarization-selective ultrafast optical switching.
