BACKGROUnD The burden of carcinoid syndrome(CS)among patients with neuroendocrine tumors is substantial and has been shown to result in increased healthcare resource use and *** incremental burden of CS diarrhea(CSD)is less well understood,particularly among working age adults who make up a large proportion of the population of patients with *** To estimate the direct medical costs of CSD to a self-insured employer in the United *** CS patients with and without CSD were identified in the IBM®MarketScan®Database,including the Medicare Supplemental Coordination of Benefits *** patients had≥1 medical claim for CS with continuous health plan enrollment for≥12 mo prior to their first CS diagnosis and for≥30 d after,no claims for acromegaly,and no clinical trial participation during the study period(2014-2016).Baseline demographic and clinical characteristics,including comorbidities and treatment,were analyzed using descriptive *** of healthcare resource use and costs were compared between patients with and without CSD,including Emergency Department(ED)visits,hospital admissions and length of stay,physician office visits,outpatient services,and prescription claims,using univariate and multivariate analyses to evaluate associations of CSD with healthcare resource use and costs,controlling for baseline *** Overall,6855 patients with CS were identified of which 4,043 were eligible for the analysis(1352 with CSD,2691 with CS only).Baseline demographic and clinical characteristics were similar between groups with the exception of age,underlying tumor type,and health insurance *** with CSD were older,had more comorbidities,and received more somatostatin analog therapy at *** with CSD required greater use of healthcare resources and incurred higher costs than their peers without CSD,including hospitalizations(44%vs 25%)and ED visits(55%vs 31%).The total adjusted annual healthcare costs

Undesired heat transfer during droplet impact on cold surfaces can lead to ice formation and damage to renewable infrastructure,among *** address this,superhydrophobic surfaces aim to minimize the droplet surface interaction thereby,holding promise to greatly limit ***,the dropletimpact on such surfaces spans only a few milliseconds making it difficult to quantify the heat exchange at the droplet–solid ***,we employ high-speed infrared thermography and a three-dimensionaltransient heat conductionCOMSOL modelto map the dynamic heat flux distribution during droplet impact on a cold superhydrophobic *** comprehensive droplet impact experiments for varying surface temperature,droplet size,and impacting height reveal that the heat transfer effectiveness(Q′)scales with the dimensionless maximum spreading radius as Q′∼(Rmax∕Ri)1.6,deviating from previous semi-infinite ***,despite shorter contact times,droplets impacting from higher heights demonstrate increased heat transfer effectiveness due to expanded contact *** results suggest that reducing droplet spreading time,as opposed to contact time alone,can be a more effective strategy for minimizing heat *** results presented here highlight the importance of both contact area and contact time on the heat exchange between a droplet and a cold superhydrophobic surface.

Clinical-DnA Correlates of Anxiety in Patients with Ehlers-Danlos Syndrome

Introduction: Anxiety disorders have a lifetime prevalence of 34% with a similar level of heritability (31%) yet lack objective markers that could differentiate patients with underlying conditions. Up to 60%-70% of patients with Ehlers-Danlos syndrome have anxiety that meets criteria of generalized anxiety disorder, their clinical-DnA findings worth examining as biomarkers for patients with generalized anxiety. Method: Of the 1899 patients diagnosed with Ehlers-Danlos syndrome, 1261 were systematically evaluated for 80 history and 40 physical findings and separated into 826 who reported anxiety and 435 who did not. The most consistently reported or management-directing 60 of these clinical findings were, along with variations in genes relevant to these disorders, examined for association with anxiety. Results: Among the 30 anxiety- associated findings judged most predictive of Ehlers-Danlos syndrome in patients with anxiety were expected ones of adrenergic stimulation (difficulty concentrating-87% frequency and 1.26 anxiety/no anxiety ratio;chronic fatigue-84%, 1.17;sleep issues 69%, 1.52 that are criteria for generalized anxiety disorder) or of cholinergic suppression (e.g., frequent nausea 64%, 1.26). Less associated but more discriminating for underlying disease were those reflective of neuromuscular impact (e.g., chronic daily headaches 76%, 1.12);hypermobility (e.g., awareness of flexibility 72%, 1.03), or skin changes (e.g., elasticity around jaw 71%, 1.06). Anxiety-associated DnA variants included 54 of 88 in collagen type I/V/VII/IX genes, 14 of 16 in sodium channel SCn9A/10A/ 11A genes, 59 of 85 in POLG/MT-DnA genes, and 21 of 28 in profilaggrin- FLG genes that respectively impacted tissue laxity, sensory neural, autonomic-mitochondrial, and autonomic-inflammatory functions. Conclusion: Analysis of pathogenetic mechanisms in Ehlers-Danlos syndrome selected some 50 clinical-DnA findings useful for its diagnosis in those with generalized anxiety disorders.

背景Dupilumab是一种抗IL-4受体α亚基的抗体,可以阻断IL-4/IL-13的信号转导。IL-4/IL-13是2型/Th2型免疫疾病(如特应性/过敏性疾病)的关键驱动因子。一项重要的2b期临床研究(nCT01854047)显示,在使用了中高剂量吸入激素联合长效β2受体激动剂(ICS/LABA)后仍控制不佳的持续性哮喘患者中,dupilumab可减少哮喘重度急性发作,改善患者的肺功能和生活质量,且耐受性良好。目的探讨在哮喘并发常年性变应性鼻炎(PAR)患者中,dupilumab对SnOT-22的总分及其中变应性鼻炎(AR)相关症状评分的影响。方法对2b临床研究中每2周(q2w) 200 mg和300 mg dupilumab治疗后的数据进行析因分析。这两种剂量的dupilumab目前正在3期临床研究(nCT02414854)中进行验证。PAR定义为:在研究开始时,患者出现对典型的常年性抗原产生特异性应答(Ig E≥0. 35 Ku/L)。结果总共241例(61%)患者患有PAR。与安慰剂相比,每两周300 mg dupilumab显著改善了在哮喘并发PAR的患者中SnOT-22的总分(最小二乘平均差,-5. 98; 95%CI,-10. 45～-1. 5; P=0. 009)和4项AR相关症状的评分(鼻塞,-0. 60; 95%CI,-0. 96～-0. 25;流鼻涕,-0. 67; 95%CI,-1. 04～-0. 31;打喷嚏,-0. 55; 95%CI,-0. 89～-0. 21;鼻后分泌物,-0. 49;95%CI,-0. 83～-0. 16; 4项评分都P nOT-22总分(-1. 82; 95%CI,-6. 46～2. 83; P=0. 443)和AR相关的症状评分,但与安慰剂相比无统计学差异。在无PAR的哮喘患者中,dupilumab与安慰剂治疗这些评分无差异。结论每两周300 mg dupilumab治疗显著改善了在控制不佳的持续性哮喘并发PAR患者中的AR相关的鼻部症状。

On Ideal Convergence of Double Sequences in Probabilistic normed Spaces

The notion of ideal convergence is a generalization of statistical convecgence which has been intensively investigated in last few years. For an admissible ideal ∮ C n × n, the aim of the present paper is to introduce the concepts of ∮-convergence and ：∮＊-convergence for double sequences on probabilistic normed spaces （Pn spaces for short）. We give some relations related to these notions and find condition on the ideal ∮ for which both the notions coincide. We also define ∮-Cauchy and ：∮＊- Cauchy double sequences on Pn spaces and show that ∮-convergent double sequences are ∮-Cauchy on these spaces. We establish example which shows that our method of convergence for double sequences on Pn spaces is more general.
