Efficacy and safety of tadalafil taken as needed for the treatment of erectile dysfunction in Asian men： results of an integrated analysis

We evaluated the efficacy and safety of as-needed tadalafil in a diverse clinical population （with varying patient demographics, disease severity, and comorbid medical conditions） of Asian men with erectile dysfunction （ED）. An integrated analysis of five double-blind, placebo-controlled trials （N = 1 046） was performed. Patients were randomly assigned to receive 10 mg tadalafil （N = 185）, 20 mg tadalafil （N = 510）, or placebo （N = 351）. Efficacy assessments included the International Index of Erectile Function （IIEF）, Sexual Encounter Profile （SEP） diary and Global Assessment Question （GAQ）. Patients receiving 10 mg or 20 mg tadalafil showed significant improvement from baseline-to-end point on the Erectile Function domain of the International Index of Erectile Function （IIEF-EF） domain score in all clinical sub-populations analyzed, compared with patients receiving placebo （P 〈 0.001）. The 10-mg and 20-mg tadalafil groups showed a mean success rate of 64.1% and 70.5% for sexual intercourse attempts （SEP3, successful intercourse）, respectively, compared with 33.4% in the placebo group （P 〈 0.001）, and 85.5% and 85.4% reported improved erections at end point GAQ, respectively, versus 43.5% in the placebo group （P 〈 0.001）. Tadalafil was well tolerated across all groups studied. Headache and back pain were the most frequently reported adverse events. Overall, tadalafil was effective and well tolerated across a diverse clinical spectrum of Asian men with ED.

Coronavirus disease 2019(COVID-19)is a global pandemic where several comorbidities have been shown to have a significant effect on *** with diabetes mellitus(DM)have a higher mortality rate than non-DM patients if they get *** studies have indicated that patients with a history of diabetes can increase the risk of severe acute respiratory syndrome coronavirus 2 ***,patients without any history of diabetes can acquire newonset DM when infected with ***,there is a need to explore the bidirectional link between these two conditions,confirming the vicious loop between“DM/COVID-19”.This narrative review presents(1)the bidirectional association between the DM and COVID-19,(2)the manifestations of the DM/COVID-19 loop leading to cardiovascular disease,(3)an understanding of primary and secondary factors that influence mortality due to the DM/COVID-19 loop,(4)the role of vitamin-D in DM patients during COVID-19,and finally,(5)the monitoring tools for tracking atherosclerosis burden in DM patients during COVID-19 and“COVID-triggered DM”*** conclude that the bidirectional nature of DM/COVID-19 causes acceleration towards cardiovascular *** to this alarming condition,early monitoring of atherosclerotic burden is required in“Diabetes patients during COVID-19”or“new-onset Diabetes triggered by COVID-19 in Non-Diabetes patients”.

1999年,国际糖尿病足工作组(IWGDF)根据循证医学证据发表了采用减压治疗促进糖尿病足溃疡愈合的预防和管理指南,这一版指南是在此基础上的更新。我们遵循GRADE方法学,采用患者-干预-比较-结果(PICO)模式设计临床问题和关键的重要结局,对医学科学文献进行系统评价分析后撰写了建议并阐述其理由。这些建议是基于系统评价、专家意见(尚无有效证据时)的基础上建立,并在患者依从性、可行性、实用性以及干预的成本等方面进行了利弊权衡。针对糖尿病患者前足或中足底的溃疡愈合,我们建议不可拆卸齐膝高减压装置作为减压治疗的首选。若出现禁忌或患者不耐受不可拆卸减压装置,则可拆卸齐膝高和齐踝高减压装置可作为二线或三线减压治疗方法。合适的鞋袜结合泡沫毡垫可被视为四线减压治疗选择。如果非外科手术减压失败,我们建议通过外科减压干预以促进跖骨头和足趾溃疡愈合。我们还增加了采用减压治疗促进合并感染、缺血的复杂足溃疡和足跟溃疡愈合的新的推荐意见。减压是促进糖尿病神经性足底溃疡愈合诸多干预措施中最重要的手段。遵循这些建议将帮助医务人员和团队,为那些具有感染、住院和截肢风险的糖尿病足溃疡患者提供更好的治疗。

Background:Economic dimensions of implementing quality improvement for diabetes care are understudied *** describe the economic evaluation protocol within a randomised controlled trial that tested a multicomponent quality improvement(QI)strategy for individuals with poorly-controlled type 2 diabetes in South ***/design:This economic evaluation of the Centre for Cardiometabolic Risk Reduction in South Asia(CARRS)randomised trial involved 1146 people with poorly-controlled type 2 diabetes receiving care at 10 diverse diabetes clinics across India and *** economic evaluation comprises both a within-trial cost-effectiveness analysis(mean 2.5 years follow up)and a microsimulation model-based cost-utility analysis(life-time horizon).Effectiveness measures include multiple risk factor control(achieving HbA1c<7%and blood pressure<130/80 mmHg and/or LDL-cholesterol<100 mg/dl),and patient reported outcomes including quality adjusted life years(QALYs)measured by EQ-5D-3 L,hospitalizations,and diabetes related complications at the trial *** measures include direct medical and non-medical costs relevant to outpatient care(consultation fee,medicines,laboratory tests,supplies,food,and escort/accompanying person costs,transport)and inpatient care(hospitalization,transport,and accompanying person costs)of the intervention compared to usual diabetes ***,healthcare system,and societal perspectives will be applied for *** cost and health effects will be discounted at 3%per year for within trial cost-effectiveness analysis over 2.5 years and decision modelling analysis over a lifetime *** will be reported as the incremental cost-effectiveness ratios(ICER)to achieve multiple risk factor control,avoid diabetes-related complications,or QALYs gained against varying levels of willingness to pay threshold *** analyses will be performed to assess uncertainties around ICER estimates by varying costs(95%CIs)across publ

减轻组织的机械性压力在治愈糖尿病相关的足溃疡所需的多种干预措施中最为重要。本文是2023年国际糖尿病足工作组(IWGDF)发布的减压治疗促进糖尿病相关的足溃疡愈合的循证指南,也是对2019年IWGDF指南的更新。我们遵循推荐、评估、发展和评价分级(GRADE)方法学,以患者-干预-比较-结局(patient-intervention-control-outcome,PICO)模式设计临床问题和重要结果,进行系统评价和Meta分析,制定评判表汇总,为每个问题编写推荐和理由。每项推荐都是基于系统综述中发现的证据,在无证据情况下则采用专家意见,以及对判断项目的GRADE等级总结进行仔细权衡,包括可预期和不可预期的影响、证据的确定性、患者价值、所需资源、成本效益、公平性、可行性和可接受性。为了治愈糖尿病患者神经性前足底或中足底溃疡,要使用不可拆卸的齐膝高减压装置作为首选减压干预措施。如果患者存在对不可拆卸减压装置的禁忌证或不能耐受,考虑使用可拆卸齐膝(或踝)高减压装置作为减压干预措施的第二选择。如果无可用的减压装置,考虑使用合适的鞋具结合毡制泡沫作为减压干预措施的第三选择。如果这种非手术减压治疗不能治愈前足底溃疡,可以考虑跟腱延长术、跖骨头切除术、关节置换术或跖骨截骨术。为了治疗继发于屈趾畸形的神经性足底或小趾远端溃疡,可行趾屈肌腱切开术。为了促进后足、非足底或并发感染或缺血的溃疡的愈合,提出了进一步的推荐。所有推荐都已在减压临床路径中进行了总结,以帮助促进本指南在临床实践中的实施。这些减压推荐将帮助医务人员为糖尿病相关的足溃疡患者提供最佳的治疗和预后,降低患者感染、住院和截肢的风险。

This paper presents the effect on compressive strength and self-healing capability of bacterial concrete with the addition of calcium *** to normal concrete,bacterial concrete possesses higher durability and engineering concrete *** production of calcium carbonate in bacterial concrete is limited to the calcium content in *** calcium lactate is externally added to be an additional source of calcium in the *** influence of this addition on compressive strength,self-healing capability of cracks is highlighted in this *** bacterium used in the study is bacillus subtilis and was added to both spore powder form and culture form to the *** subtilis spore powder of 2 million cfu/g concentration with 0.5%cement was mixed to *** lactates with concentrations of 0.5%,1.0%,1.5%,2.0%,and 2.5%of cement,was added to the concrete mixes to test the effect on properties of *** other samples,cultured bacillus subtilis with a concentration of 1×10^cells/mL was mixed with concrete,to study the effect of bacteria in the cultured form on the properties of *** of 100 mm×100 mm×100 mm were used for the *** cubes were tested after a curing period of 7,14,and 28 d.A maximum of 12%increase in compressive strength was observed with the addition of 0.5%of calcium lactate in *** electron microscope and energy dispersive X-ray spectroscopy examination showed the formation of ettringite in pores;calcium silicate hydrates and calcite which made the concrete denser.A statistical technique was applied to analyze the experimental data of the compressive strengths of cementations *** surface methodology was adopted for optimizing the experimental *** regression equation was yielded by the application of response surface methodology relating response variables to input *** method aids in predicting the experimental results accurately with an acceptable range

Processing and design methodologies for advanced and novel thermal barrier coatings for engineering applications

Thermal barrier coating is a crucial thermal insulation technology that enables the underlying substrate to operate near or above its melting temperature. Such coatings bolster engineers＇ perpetual desire to increase the power and efficiency of gas turbine engines through increasing the turbine inlet tempera- ture. Advances in recent years have made them suitable for wider engineering and defense applications, and hence they are currently attracting considerable attention. A thermal barrier coating system is itself dynamic; its components undergo recurrent changes in their composition, microstructure and crystalline phases during its service life. Nevertheless, the performance of multi-layered and multi-material sys- tems tailored for high temperature applications is closely linked to the deposition process. The process improvements achieved so far are the outcome of increased understanding of the relationship between the coating morphology and the operating service conditions, as well as developments in characterization techniques. This article presents a comprehensive review of various processing techniques and design methodologies for thermal barrier coatings. The emphasis of this review is on the particle technology; the interrelationship between particle preparation, modification and the resulting properties, to assist developments in advanced and novel thermal barrier coatings for engineering applications.
