Surgical complications of oncological treatments: A narrative review

Acute kidney injury due to bilateral ureteral obstruction in children

Bilateral ureteral obstruction in children is a rare condition arising from several medical or surgical *** needs to be promptly suspected in order to attempt a quick renal function *** this paper we concentrated on uncommon causes of obstruction,with the aim of giving a summary of such multiple,rare and heterogeneous conditions joint together by the common denominator of sudden bilateral ureteral obstruction,difficult to be suspected at ***,typical and well-known diseases have been just run *** considered pediatric cases of ureteral obstruction presenting as bilateral,along with some cases which truly appeared as single-sided,because of their potential bilateral *** performed a review of the literature by a search on Pub Med,Cross Ref Metadata Search,internet and reference lists of single articles updated to May 2014,with no time limits in the *** that we deal with rare conditions,we decided to include also papers in non-English languages,published with an English *** the sake of clearness,we divided our research results into 8 categories:(1) urolithiasis;(2) congenital urinary tract malformations;(3) immuno-rheumatologic causes of ureteral obstruction;(4) ureteral localization of infections;(5) other systemic infective causes of ureteral obstructions;(6) neoplastic intrinsic ureteral obstructions;(7) extrinsic ureteralobstructions; and(8) iatrogenic trigonal obstruction or *** course,different pathogenic mechanisms underlay those clinical pictures,partly wellknown and partly not completely understood.

Graded and pan-neural disease phenotypes of Rett Syndrome linked with dosage of functional MeCP2

Rett syndrome(RTT)is a progressive neurodevelop-mental disorder,mainly caused by mutations in MeCP2 and currently with no *** report here that neurons from R106W MeCP2 RTT human iPSCs as well as human embryonic stem cells after MeCP2 knockdown exhibit consistent and long-lasting impairment in maturation as indicated by impaired action potentials and passive membrane properties as well as reduced soma size and spine ***,RTT-inherent defects in neuronal maturation could be pan-neuronal and occurred in neurons with both dorsal and ventral forebrain *** of MeCP2 led to more severe neuronal deficits as compared to RTT iPSC-derived neurons,which appeared to retain partial ***,consistent deficits in nuclear size,dendritic complexity and circuitry-dependent spontaneous postsynaptic currents could only be observed in MeCP2 knockdown neurons but not RTT iPSC-derived *** neuron-intrinsic and circuitry-dependent deficits of MeCP2-deficient neurons could be fully or partially rescued by re-expression of wild type or T158M MeCP2,strengthening the dosage dependency of MeCP2 on disease phenotypes and also the partial function of the *** findings thus reveal stable neuronal maturation deficits and unexpectedly,graded sensitivities of neuron-inherent and neural transmission phenotypes towards the extent of MeCP2 deficiency,which is informative for future therapeutic development.

Ubiquitin C gene: Structure, function, and transcriptional regulation

Ubiquitin C (UbC) is one of the four genes encoding for ubiquitin in the mammalian genome. It has been described as the most responsive gene to cellular treats such as UV irradiation, heat shock, oxidative stress and translational impairment;it was also reported to contribute to maintaining ubiquitin steady state levels under physiological conditions. Despite the bulk of knowledge concerning its function, little is known about the molecular mechanisms modulating UbC expression. Here we review the state of the art of UbC structure, function and transcriptional regulation. Starting from the first evidences which circumscribed the genomic region, pointing out both basic promoter marks (such as transcription start site and TATA-like element), and transcript structure (exonintron boundaries) we go through more detailed molecular studies performed by Marinovic in 2002 and by bianchi et al. in 2009 and 2013. Herein, the key players orchestrating UbC gene basal activity are underlined.

Clinical Analysis of the Pharmacological Interactions of Clopidogrel and Gender Differences: A Case-Control Study

Background: Clopidogrel is a prodrug metabolized by cytochrome P450-2C19. Drugs inhibiting this enzyme might reduce its antiplatelet activity. In order to reduce gastrointestinal bleedings, proton-pump inhibitors are usually prescribed in association with clopidogrel. The study aims at assessing the clinical importance of interactions between clopidogrel and inhibitors of CYP2C19. It also aims to evaluate any possible factors that may reduce the therapeutic efficacy of the drug. Particular attention was devoted to possible gender differences in responsiveness to treatment with clopidogrel or clopidogrel plus proton-pump inhibitors. This analysis is a retrospective case-control observational study carried out by the University Hospital of Ferrara. Methods: Subjects were patients who had received clopidogrel from 01-01-2008 to 31-12-2008. For them, we analysed hospital admissions and data of drug prescriptions relative to dispensing of drugs cytochrome P-450-2C19 inhibitors. Patients were subdivided into case and control groups based on the occurrence or not of cardiovascular or cerebrovascular secondary events during therapy with clopidogrel. Results: The study focused on 781 patients, 20.1% of which (n.157) experienced secondary effects. The mean age is 70 years old. Men (67% of the analyzed population) experienced secondary events more than women (OR 1.54;CI 95% 1.04 - 2.28;p < 0.03). 70% of patients took PPIs and we noticed that the risk of secondary events increased by 2.2% with respect to the remaining patients (20.77% vs 18.57%;OR 1.15;CI 0.78 - 1.70;p = NS). Among PPIs, lansoprazole is the most used. For this subgroup the risk is 5.2% higher (risk in those exposed of 23.75% vs 18.57% in those not exposed;or 1.37, 95% CI 0.92 - 2.03;p = NS). The interaction with PPIs is particularly interesting only among women, with a risk 6.3% higher (17.46% exposed, 11.11% non exposed). The risk remains the same among men. Genetic distinctions between autoimmune hepatitis in Italy and North America

AIM: Our goals were to analyze the known genetic predispositions for autoimmune hepatitis (AIH) in AIH Italian population and to compare them with North American counterparts. METHODS: Human leukocyte antigens (HLA) B8, C7, DR3, DR4, DR7, DR11, DR13, DQ2 and the B8-DR3-DQ2 phenotype were determined by microlymphocytotoxicity and polymerase chain reaction in 74 Italian patients (57 with type 1 and 17 with type 2 AIH) and 149 North American patients with type 1 AIH, and in adequate controls. RESULTS: B8-DR3-DQ2 occurred more frequently in Italian patients with type 1 AIH than in Italian controls (30% vs 7%, P<0.0001), but less frequently than in North American counterparts (30% vs 48%, P= 0.02). DR4 occurred less frequently in Italian patients with type 1 AIH (23% vs 43%, P= 0.01) and in controls (16% vs 34%, P= 0.0003) than in North American counterparts. No differences were found in alleles' frequency between type 1 and type 2 Italian AIH patients. DR11 had a frequency lower in type 1 Italian AIH patients than controls (17% vs 35%, P= 0.01). CONCLUSION: HLA DR4 is not associated with AIH in Italy. The known HLA risk factors for AIH occur similarly in Italian patients with type 1 and type 2 AIH, and they are less frequent than in North American patients. B8-DR3-DQ2 is the predominant phenotype of type 1 AIH also in Italy, and HLA DR11 may be a regionally distinctive protective factor against type 1 AIH.

An Assessment of the Within-Year Seasonality in the Loan Loss Provisioning Behavior of European Banks

This study aims at assessing whether a significant within-year seasonality exists in the loan loss provisioning behavior of European listed banks observed in the period from 2004 to 2013. Since the accuracy of auditing processes and the level of disclosure requirements in financial reports differ among quarters, during the year, banks may have a leeway to underestimate and postpone the complete provisioning of loan losses in the less regulated and less audited quarters. We hypothesize that those differences are relevant factors which determine non-lower or significantly higher average levels of loan loss provisions in the half-yearly and especially in the annual financial reports than in the interim management statements disclosed in the first and the third quarters of the year. We also investigate the impact of the recent financial crisis and develop a special analysis for the ltalian banks＇ case. The empirical results support our hypotheses, suggesting that, in some cases, a convergence among quarterly levels of auditing processes and disclosure requirements may be needed. Our work contributes to the existing literature by providing additional evidences and considerations on the within-year seasonality in the loan loss provisioning behavior of European listed banks observed in the last decade.

Orthogonal design for optimization of pigment extraction from surface sediments of the Changjiang River Estuary

Using a suitable solvent for extracting pigments from sediment for high performance liquid chromatography（HPLC） analysis is critical for obtaining qualitative and quantitative estimates of phytoplanktonic and benthic algal biomass,as well as community composition. Five methodological factors（sample dehydration,extraction solvent,extraction duration,number of extractions,and ratio of solvent volume：sample weight） were studied using an L 9（3 4 ） orthogonal design in a sedimentary pigment extraction experiment on samples collected from the Changjiang large-river delta-front estuary（LDE）,using HPLC analysis. The results show that the optimal extraction method for sedimentary pigments should include freeze-drying samples prior to extraction. The effects of different factors on sedimentary pigment extraction were separated by the L 9（3 4 ） orthogonal design experiments and showed that the extraction solvent was the most important,with extraction duration the second most important,and numbers of extraction and ratio of solvent volume：sample weight was the least important. The mixed solvent treatment comprised of acetone,methanol and water（80：15：5,by volume） was best for polar pigment extraction,with 100% acetone better for apolar pigments. For most pigments employed in this study（i.e.,peridinin,fucoxanthin,alloxanthin,diatoxanthin,zeaxanthin,pheophytin-a and β-carotene） ,3 h was found to be enough time for extraction from these deltaic sediments. However,for chlorophyll-a,the most important pigment used for estimating algal biomass,12 h was needed. A small amount of solvent（3 ml） with duplicate extractions obtained the greatest amount and diversity of pigments. Unfortunately,no extraction method was found to be suitable for all pigments in sediments. The choice of extraction procedure should be made in accordance with the objective of each study,taking into consideration the properties of sediments and pigments in question.
