Children with celiac disease and high tTGA are genetically and phenotypically different

AIm:To investigate whether celiac disease(CD)patients with tissue-transglutaminase antibody(tTGA)≥100 U/mL are different from patients with lower tTGA ***:Biopsy-proven(marshⅢ)pediatric CD patients(n=116)were prospectively included between march 2009 and October *** biopsies were evaluated by a single pathologist who was blinded to all of the patients’clinical *** patients were distributed into 2 groups according to their tTGA level,which was measured using enzyme-linked immunoassay:tTGA≥100 U/mL and Ttgamptoms,human leukocyte antigen(HLA)genotype and degree of histological involvement were compared between the 2 ***:A total of 34(29.3%)children had tTGA valuesmL and 82(70.7%)tTGA levels of≥100 U/*** with high tTGA levels had lower average body weight-for-height standard deviation scores(SDS)than did patients with tTGAmL(-0.20±1.19 SDS vs 0.23±1.03 SDS,P=0.025).In the low tTGA group,gastrointestinal symptoms were more common(97.1%vs 75.6%,P=0.006).more specifically,abdominal pain(76.5%vs 51.2%;P=0.012)and nausea(17.6%vs 3.7%,P=0.018)were more frequent among patients with low *** contrast,patients with solely extraintestinal manifestations were only present in the high tTGA group(18.3%,P=0.005).These patients more commonly presented with aphthous stomatitis(15.9%vs 0.0%,P=0.010)and anemia(32.9%vs 11.8%,P=0.019).In addition,when evaluating the number of CD-associated HLA-DQ heterodimers(HLA-DQ2.5,HLA-DQ2.2 and HLA-DQ8),patients with low tTGA levels more commonly had only1 disease-associated heterodimer(61.8%vs 31.7%,P=0.005),while patients with high tTGA more commonly had multiple ***,patients with tTGA≥100 U/mL more often had a marshⅢc lesion(73.2%vs 20.6%,P≤0.001)while in patients with low tTGA patchy lesions were more common(42.4%vs6.8%,P≤0.001).CONCLUSION:Patients with tTGA≥100 U/mL show several signs of more advanced *** also carry a larger

Immunohistochemical CD3 staining detects additional patients with celiac disease

AIm: To investigate whether performing immuno-histochemical CD3 staining, in order to improve the detection of intra-epithelial lymphocytosis, has an additional value in the histological diagnosis of celiac ***: Biopsies obtained from 159 children were stained by hematoxylin and eosin(HE) and evaluated using the marsh classification. CD3 staining was subsequently evaluated separately and ***: Differences in evaluation between the routine HE sections and CD3 staining were present in 20(12.6%) cases. In 10(6.3%) patients the diagnosis of celiac disease(marsh Ⅱ and Ⅲ) changed on examination of CD3 staining: in 9 cases, celiac disease had initially been missed on the HE sections, while 1 patient had been over-diagnosed on the routine sections. In all patients, the final diagnosis based on CD3 staining, was concordant with serological results, which was not found previously. In the other 10(12.3%) patients, the detection of sole intra-epithelial lymphocytosis(marsh Ⅰ) improved. Nine patients were found to have marsh Ⅰ on CD3 sections, which had been missed on routine sections. Interestingly, the only patient with negative serology had Giardiasis. Finally, in 1 patient with negative serology, in whom marsh Ⅰ was suspected on HE sections, this diagnosis was withdrawn after evaluation of the CD3 ***: Staining for CD3 has an additional value in the histological detection of celiac disease lesions, and CD3 staining should be performed when there is a discrepancy between serology and the diagnosis made on HE sections.

AIm:To investigate whether a tissue-transglutaminase antibody(tTGA) level ≥ 100 U/mL is sufficient for the diagnosis of celiac disease(CD).mETHODS:Children suspected of having CD were prospectively included in our study between march 2009 and September *** patients with immune globulin A deficiency and all patients on a gluten-free diet were excluded from the ***-endomysium antibodies(EmA) were detected by means of immunofluorescence using sections of distal monkey esophagus(EUROImmUN,Luebeck,Germany).Serum anti-tTGA were measured by means of enzyme-linked immunosorbent assay using human recombinant tissue transglutaminase(ELiA Celikey IgA kit Phadia AB,Uppsala,Sweden).The histological slides were graded by a single experienced pathologist using the marsh classification as modified by *** Ⅱ and Ⅲ lesions were considered to be diagnostic for the *** positive predictive values(PPVs),negative predictive values(NPVs),sensitivity and specificity of EmA and tTGA along with their 95% CI(for the cut off values > 10 and ≥ 100 U/mL) were calculated using histology as the gold standard for ***:A total of 183 children were included in the study.A total of 70(38.3%) were male,while 113(61.7%) were *** age range was between 1.0 and 17.6 years,and the mean age was 6.2 *** hundred twenty(65.6%) patients had a small intestinal biopsy diagnostic for the disease;3 patients had a marsh Ⅱ lesion,and 117 patients had a marsh Ⅲ *** the patients without CD,only 4 patients had a marshⅠ*** the 183 patients,136 patients were positive for EmA,of whom 20 did not have CD,yielding a PPV for EmA of 85%(95% CI:78%-90%) and a corresponding specificity of 68%(95% CI:55%-79%).The NPV and specificity for EmA were 91%(95% CI:79%-97%) and 97%(95% CI:91%-99%),*** levels of tTGA were found in 130 patients,although only 116 patients truly had histological evidence of the *** PPV for tTGA was 89%(95% CI:82%-94%),and th

Situation of Water, Hygiene and Sanitation in a Peri-Urban Area in Benin, West Africa: The Case of Sèmè-Podji

This study aimed to analyze the environmental factors related to health vulnerability in the municipality of Sèmè-Podji. For this study, 216 households were surveyed using questionnaires. Twelve key informants were enrolled for depth interviews and 18 water samples were collected from different sources used by the populations. It came out of our investigations that water from traditional sources used at Sèmè-Podji was contaminated by various microorganisms such as Escherichia coli and fecal coliforms. 66.38% of households reject domestic waste and 47.41% of them evacuate sewage in the yard. In addition, 93.05% drop stagnant water for natural infiltration. This municipality faces lack of sanitation. The main health problems experienced by people were malaria (46.35%), diarrheal diseases (15.88%). The study stated some results which could improve the help about health management in the area.

Cholera is re-emerging as an infectious disease with an increase in its overall incidence in Africa since 2008. To analyze the epidemiological aspects of the cholera outbreak in endemic areas in Benin. The study is a retrospective investigation on 2560 data of cholera patients received in health facilities from 2012 to 2016 in Benin. Besides, a prospective study was conducted. This prospective study includes 513 person (health professionals, community workers and people exposed to cholera) of five different areas of the country. It was conducted from April 10 to may 10, 2017. A survey was conducted over 513 in order to collect data on cholera symptoms, hygiene guidelines suggested by the respondents, excreta disposal, drinking water, household waste management and direct observation. Software R 3.4.0 was used for simple correspondence factor analysis (CFA). From 2012 to 2016, 16.48% of samples were taken and 48.34% were positive for Vibrio cholera O1, El Tor biotype, Ogawa serotypes. The Littoral area is leading in the number of cases (33.83%) followed by Atlantic (23.75%), Ouémé (16.79%) and Colline (8.91%). The survey revealed that the exposed populations are unaware of the symptoms of cholera and do not perceive the quality of drinking water as a strong guidance in the prevention of cholera. Nevertheless, the majority of respondents (86%) are aware for the best times to wash their hands and know that they need to go to hospital in case of symptoms. Our study highlights the persistence of vulnerability factors to cholera in the targeted population, despite a good knowledge of prevention rules among staff and the population.

Background:No previous correlation between phenotype at diagnosis of Crohn’ disease(CD)and mortality has been *** assessed the predictive value of phenotype at diagnosis on overall and disease related mortality in a European cohort of CD ***:Overall and disease related mortality were recorded 10 years after diagnosis in a prospectively assembled,uniformly diagnosed European population based inception cohort of 380 CD patients diagnosed between 1991 and *** mortality ratios(SmRs)were calculated for geographic and phenotypic subgroups at ***:Thirty seven deaths were observed in the entire cohort whereas 21.5 deaths were expected(SmR 1.85(95%Cl 1.30-2.55)).mortality risk was significantly increased in both females(SmR 1.93(95%Cl 1.10-3.14))and males(SmR 1.79(95%Cl 1.11-2.73)).Patients from northern European centres had a significant overall increased mortality risk(SmR 2.04(95%Cl 1.32-3.01))whereas a tendency towards increased overall mortality risk was also observed in the south(SmR 1.55(95%Cl 0.80-2.70)).mortality risk was increased in patients with colonic disease location and with inflammatory disease behaviour at *** risk was also increased in the age group above 40 years at diagnosis for both total and CD related *** mortality was mainly due to gastrointestinal causes that were related to ***:This European multinational population based study revealed an increased overall mortality risk in CD patients 10 years after diagnosis,and age above 40 years at diagnosis was found to be the sole factor associated with increased mortality risk.

＜正＞One of the sub-systems of the Trigger/DAQ system of the future ATLAS experiment is the Online Software system. It encompasses the functionality needed to configure, control and monitor the DAQ. Its architecture is based on a component structure described in the ATLAS Trigger/DAQ technical proposal. Regular integration tests ensure its smooth operation in test beam setups during its evolutionary development towards the final ATLAS online system. Feedback is received and returned into the development process. Studies of the system behavior have been performed on a set of up to 111 PCs on a configuration which is getting closer to the final size. Large scale and performance tests of the integrated system were performed on this setup with emphasis on investigating the aspects of the inter-dependence of the components and the performance of the communication software. Of particular interest were the run control state transitions in various configurations of the run control hierarchy. For the purpose of the tests, the software from other Trigger/DAQ sub-systems has been emulated. This paper presents a brief overview of the online system structure, its components and the large scale integration tests and their results.
