sarcomas are complex mesenchymal neoplasms with a poor *** clinical management is highly challenging due to their heterogeneity and insensitivity to current *** there have been advances in understanding specific genomic alterations and genetic mutations driving sarcomagenesis,the underlying molecular mechanisms,which are likely to be unique for each sarcoma subtype,are not fully *** is in part due to a lack of consensus on the cells of origin,but there is now mounting evidence that they originate from mesenchymal stromal/stem cells(MsCs).To identify novel treatment strategies for sarcomas,research in recent years has adopted a mechanism-based search for molecular markers for targeted therapy which has included recapitulating sarcomagenesis using in vitro and in vivo MsC *** review provides a comprehensive up to date overview of the molecular mechanisms that underpin sarcomagenesis,the contribution of MsCs to modelling sarcomagenesis in vivo,as well as novel topics such as the role of epithelial-to-mesenchymal-transition(EMT)/mesenchymal-to-epithelial-transition(MET)plasticity,exosomes,and microRNAs in *** also reviews current therapeutic options including ongoing pre-clinical and clinical studies for targeted sarcoma therapy and discusses new therapeutic avenues such as targeting recently identified molecular pathways and key transcription factors.

Metal halide perovskite solar cells are emerging candidates for nextgeneration thin-film photovoltaic devices with the potential for extremely low fabrication cost and high power conversion *** solar cells have demonstrated a rapid development in device performance over the last decade,from an initial 3.81%to a most recently certified 24.2%,though the challenges of long-term stability and lead toxicity still *** materials,ranging from zero-dimensional carbon quantum dots to threedimensional carbon black materials,are promising candidates for the enhancement of both efficiency and stability of perovskite solar cells,offering unique advantages for incorporation into various device *** this review article,we present a concise overview of important and exciting advancements of perovskite solar cells that incorporate different dimensions of carbon material in their device architectures in an effort to simultaneously improve device performance and long-term *** also discuss the major advantages and potential challenges of each technique that has been developed in the most recent ***,we outline the future opportunities toward more efficient and stable perovskite solar cells utilizing carbon materials.

Background: Osteoarthritis (OA) is a disabling disease that can affect 6% to 12% of the adult population and more than a third of people over 65 years of age. Purpose: To assess whether a group of people with hand osteoarthritis (hOA) who received different types of treatment improved their function after two years of follow-up. Method: The entire sample (n = 97) underwent three follow-up assessments regarding anthropometric parameters of the upper limbs and ability to perform functional activities. subsequently, the sample was divided into two groups for the intervention periods, called the First Period (n = 73) and the second Period (n = 24);the First Period kept the same protocol with orientations, and the second Period went to an intervention with orientation strength exercises and use of orthosis. Findings: In the separate analysis of the three questions of the DAsH pain module, no differences were found between the assessment moments for groups of guidelines, treatment, or symptoms. significant effects were observed for F(2, 162) = 3.5, p = 0.033, η2 = 0.04, and interaction for moments and intervention F(2, 162) = 4.3, p = 0.016, η2 = 0.05. Implications: It can be concluded that only guidance treatment does not benefit patients with hand osteoarthritis. In contrast, guidance, exercise, and orthosis treatment can significantly improve the disease.

Endoscopic ultrasonography (EUs) has gained wide acceptance as an important, minimally invasive diagnostic tool in gastroenterology, pulmonology, visceral surgery and oncology. This review focuses on data regarding risks and complications of non-interventional diagnostic EUs and EUs-guided fine-needle biopsy (EUs-FNB). Measures to improve the safety of EUs und EUs-FNB will be discussed. Due to the specific mechanical properties of echoendoscopes in EUs, there is a low but noteworthy risk of perforation. To minimize this risk, endoscopists should be familiar with the specific features of their equipment and their patients' specific anatomical situations (e.g., tumor stenosis, diverticula). Most diagnostic EUs complications occur during EUs-FNB. Pain, acute pancreatitis, infection and bleeding are the primary adverse effects, occurring in 1% to 2% ofpatients. Only a few cases of needle tract seeding and peritoneal dissemination have been reported. The mortality associated with EUs and EUs-FNB is 0.02%. The risks associated with EUs-FNB are affected by endoscopist experience and target lesion. EUs-FNB of cystic lesions is associated with an increased risk of infection and hemorrhage. Peri-interventional antibiotics are recommended to prevent cyst infection. Adequate education and training, as well consideration of contraindications, are essential to minimize the risks of EUs and EUs-FNB. Restricting EUs-FNB only to patients in whom the cytopathological results may be expected to change the course of management is the best way of reducing the number of complications.

Predictors of loss of hepatitis B surface antigen in HIV-infected patients

AIM:To study factors associated with loss of hepatitis B surface antigen (HBsAg) in patients co-infected with human immunodeficiency virus (HIV) and hepatitis B virus (HBV).METHODs:We retrospectively reviewed the medical records of 5681 patients followed up at two New York City HIV clinics from january 1999 to May *** and laboratory parameters including baseline and follow-up HIV viral loads,CD4 cell counts,alanine transaminase levels,demographics,presence of hepatitis C infection,and treatment with highly active antiretroviral therapy dually active against both HIV and HBV infection,were analyzed to determine factors associated with loss of ***:Three hundred and fifty five patients (355/5681,6.84%) were co-infected with HIV and HBV and were *** these,226 patients with more than 12 mo follow-up were included in further analysis to determine factors associated with loss of HBsAg in the long-term *** the univariate analysis,baseline CD4 cell count was associated with loss of HBsAg (P=0.052).Cox regression analysis revealed that loss of HBsAg was associated with baseline CD4 cell count > 500 cells/mm3 (P=0.016,odds ratio:76.174,95% confidence interval:2.233-2598.481).CONCLUsION:Our study showed an interesting association of loss of HBsAg in HIV-HBV co-infected patients with higher CD4 cell count,suggesting that T-cell cytolytic activity against HBV may still be effective in clearing HBV infection.
