Cardioembolic events are one of the most feared complications in patients with non-valvular atrial fibrillation(nVAF) and a formal contraindication to oral anticoagulation(OAC).The present case report describes a case of massive peripheral embolism after an implantable cardiac defibrillator(ICD) shock in a patient with nVAF and a formal contraindication to OAC due to previous intracranial *** order to reduce the risk of future cardioembolic events,the patient underwent percutaneous left atrial appendage(LAA) occlusion.A 25 mm AmplatzerTM Amulet was implanted and the patient was discharged the following day without *** potential risk of thrombus dislodgement after an electrical shock in patients with nVAF and no anticoagulation constitutes a particular scenario that might be associated with an additional *** LAA occlusion is a relatively new technique,its usage is rapidly expanding worldwide and constitutes a very valid alternative for patients with nVAF and a formal contraindication to OAC.

AIM: To evaluate the impact of thrombus aspiration(TA) on procedural outcomes in a real-world ST-segment elevation myocardial infarction(STEMI) ***: From May 2006 to August 2008, 542 consecutive STEMI patients referred for primary or rescue percutaneous coronary intervention were enrolled and the angiographic results and stent implantation characteristics were compared according to the performance of manual ***: A total of 456 patients were analyzable and categorized in TA group(156 patients; 34.2%) and non-TA(nTA) group(300 patients; 65.8%). Patientstreated with TA had less prevalence of multivessel disease(39.7% vs 54.7%, P = 0.003) and higher prevalence of initial thrombolysis in myocardial infarction flow n nTA group. There was a higher rate of direct stenting(58.7% vs 45.5%, P = 0.009), with shorter(24.1 ± 11.8 mm vs 26.9 ± 15.7 mm, P = 0.038) and larger stents(3.17 ± 0.43 mm vs 2.93 ± 0.44 mm, P n the TA group as compared to nTA group. The number of implanted stents(1.3 ± 0.67 vs 1.5 ± 0.84, P = 0.009) was also lower in TA group. COnCLUSIOn: In an “all-comers” STEMI population, the use of TA resulted in more efficient procedure leading to the implantation of less number of stents per lesion of shorter lengths and larger sizes.

Transcatheter closure of the left atrial appendage with the Amplatzer cardiac plug device and double antiplatelet treatment for 3 mo has become an alternative treatment for patients with atrial fibrillation at high embolism risk and contraindications for chronic oral *** inadequate implantation of the left atrial appendage closure device and the discontinuation of double antiplatelet therapy are well-known as factors related to device ***,device thrombosis after adequate implantation requiring surgical treatment or restarting chronic oral anticoagulation has been reported and can reach 15% of *** connector pin thrombosis of the Amplatzer cardiac plug,despite a good adherence to antiplatelet treatment,has been recently described as a potential mechanism for device *** clinical case reports the management of this condition for the first time,showing that the early detection of thrombotic complications by transesophageal echocardiography permits solving this serious complication with medical treatment only.

The deployment of hydrogen as an energy carrier is found to be a vital alternative fuel for the future. It is expected that water electrolysis, powered by renewable energy sources, be able to scale-up hydrogen production. However, the reaction kinetic of oxygen evolution reaction(OER) is a sluggish process, which predominantly limits the efficiency of water electrolysis. This review recapitulates the recent progress and efforts made in the design and development of two selected earth-abundant bimetallic electrocatalysts(ni Co and Co Fe) for alkaline OER. Each bimetal electrocatalyst is thoroughly outlined and discussed in five sub-sections, including bimetal(oxy) hydroxides, Layered double hydroxides(LDHs) structures,oxides, composites, alloy and nanostructured electrocatalysts, and assembled with ***, a brief introduction to an in situ/operando characterization techniques and advantages for monitoring the structure of the electrocatalysts is provided. Finally, a summary outlining the challenges and conceivable approaches to advance OER performance is highlighted and discussed.

Malignant transformation of hepatocellular adenoma in a young female patient after ovulation induction fertility treatment:A case report

BACKGROUnD Hepatocellular adenoma(HCA) is a rare benign liver tumor usually affecting young women with a history of prolonged use of hormonal *** the majority is asymptomatic, a low proportion may have significant complications such as bleeding or malignancy. Despite responding to the hormonal stimulus, the desire for pregnancy in patients with small HCA is not contraindicated. However, through this work we demonstrate that intensive hormonal therapies such as those used in the treatment of infertility can trigger serious complications CASE SUMMARY A 33-year-old female with a 10-year history of oral contraceptive use was diagnosed with a hepatic tumor as an incidental finding in an abdominal ultrasound. The patient showed no symptoms and physical examination was unremarkable. Laboratory functional tests were within normal limits and tests for serum tumor markers were negative. An abdominal magnetic resonance imaging(MRI) was performed, showing a 30 mm × 29 mm focal lesion in segment VI of the liver compatible with HCA or Focal nodular Hyperplasia with atypical behavior. After a total of six years of follow-up, the patient underwent ovulation induction treatment for infertility. On a following MRI, a suspected malignancy was warned and hence, surgery was decided. The surgical specimen revealed malignant transformation of HCA towards trabecular hepatocarcinoma with dedifferentiated areas. There was non-evidence of tumor recurrence after three years of clinical and imaging follow-up.,COnCLUSIOn HCAs can be malignant regardless its size and low-risk appearance on MRI when an ovultation induction therapy is indicated.

AIM: To investigate the impact of dual antiplatelet therapy (DAT) in patients on antivitamin K (AVK) regimen requiring percutaneous coronary intervention (PCI).METHODS: Between February 2006 and February 2008, 138 consecutive patients under chronic AVK treatment were enrolled in this registry. Of them, 122 received bare metal stent implantation and 16 received drug elutingstent implantation. The duration of DAT, on top of AVK treatment, was decided at the discretion of the clinician. Adequate duration of DAT was def ined according to type of stent implanted and to its clinical indication. RESULTS: The baseline clinical characteristics of patients reflect their high risk, with high incidence of comorbid conditions (Charlson score ≥ 3 in 89% of the patients). At a mean follow-up of 17 ± 11 mo, 22.9% of patients developed a major adverse cardiac event (MACE): 12.6% died from cardiovascular disease and almost 6% had an acute myocardial infarction. Major hemorrhagic events were observed in 7.4%. Adequate DAT was obtained in only 44% of patients. In the multivariate analysis, no adequate DAT and Charlson score were the only independent predictors of MACE (both P = 0.02). COnCLUSIOn: Patients on chronic AVK therapy represent a high risk population and suffer from a high MACE rate after PCI. An adequate DAT regimen and absence of comorbid conditions are strongly associated with better clinical outcomes.

The development and implementation of sustainable and environmentally friendly agricultural practices are indispensable as alternatives to pesticide use and to keep populations of soil-borne plant pathogens at levels that do not affect crop *** present research evaluates the incidence of soil-borne phytopathogens on the pineapple variety MD-2,which was subjected to different treatments:Incorporation of Crotalaria juncea into the soil(organic amendment),application of dolomitic lime to soil(inorganic amendment),and the use of plastic mulch covering the *** the crop cycle(15 months),the following variables were evaluated:plant height(cm),fruit weight(kg·plant^(−1)),crop yield(ton·ha^(−1)),the bud root disease incidence caused by Phytophthora nicotianae,number of soil phytoparasitic nematodes and colony-forming-units(CFUs)of soil fungi and *** results indicate that Crotalaria juncea treatment reduced the pathogen population(nematode and oomycetes)at levels that did not affect crop development,so that yield increased(18–20%).The incorporation of *** into the soil as an organic amendment favors the populations of fungi disease suppressors(Trichoderma-Aspergillus).The phytoparasitic nematodes(Meloidogyne sp.,Pratylenchus sp.,and Mesocriconema sp.)and oomycetes(Phytophthora spp.,and Pythium spp.)showed a reduction of their population levels by effects of organic amendment(***).The plastic mulch was also effective,probably due to the maintenance of optimal condition to crop growth and weed ***,the dolomitic lime application had the poorest effect under the conditions of the study area on the variables *** described observations are characteristics of a system-based approach for the potential management of soil-borne pathogens of pineapple MD-2 in Veracruz,México.
