How do we manage post-OLT redundant bile duct?

AIM: To address endoscopic outcomes of post-Orthotopic liver transplantation (OLT) patients diagnosed with a "redundant bile duct" (RBD). METHODS: Medical records of patients who underwent OLT at the Liver Transplant Center, University Texas Health Science Center at San Antonio Texas were retrospectively analyzed. Patients with suspected biliary tract complications (BTC) underwent endoscopic retrograde cholangiopancreatography (ERCP). All ERCP were performed by experienced biliary endoscopist. RBD was defined as a looped, sigmoid-shaped bile duct on cholangiogram with associated cholestatic liver biomarkers. Patients with biliary T-tube placement, biliary anastomotic strictures, bile leaks, bile-duct stonessludge and suspected sphincter of oddi dysfunction were excluded. Therapy included single or multiple biliary stents with or without sphincterotomy. The incidence of RBD, the number of ERCP corrective sessions, and the type of endoscopic interventions were recorded. Successful response to endoscopic therapy was defined as resolution of RBD with normalization of associated cholestasis. Laboratory data and pertinent radiographic imaging noted included the pre-ERCP period and a follow up period of 6-12 mo after the last ERCP intervention. RESULTS: One thousand two hundred and eighty-two patient records who received OLT from 1992 through 2011 were reviewed. Two hundred and twenty-four patients underwent ERCP for suspected BTC. RBD was reported in each of the initial cholangiograms. Twentyone out of 1282 (1.6%) were identified as having RBD. There were 12 men and 9 women, average age of 59.6 years. Primary indication for ERCP was cholestatic pattern of liver associated biomarkers. Nineteen out of 21 patients underwent endoscopic therapy and 2/21 required immediate surgical intervention. Ecological succession of benthic organisms on niche-type artificial reefs

As part of efforts to restore fishery resources and recover damaged coastal ecosystems,artificial reefs are often anchored on the seafloor in coastal zones,to provide new habitats for marine *** aim of the study was to describe the structure of a community of benthic invertebrates colonising a niche-type artificial reef(AATN in Spanish).Nine structures were anchored at depths of 16±1.5 m for 99 weeks(22 months)in the Area of Benthic Resource Management and Exploitation(AMERB)in coastal waters of the Region of Bio Bio,*** results showed that,at 3 months from submersion of the NTAR,much of the artificial substrate remained bare and there were only low levels of specimens of Balanus ***,showing mean coverage of 11.26%,and even lower proportions of Rhodophyta,with mean coverage of 0.34%.At 6 months,the presence of hydrozoans was seen,decreasing the coverage of the barnacles,which was aided further at 8 months with the arrival of barnacle predatory *** 8 months,new benthic invertebrates appeared and competed for the *** included sponges and *** 99 weeks,the hydrozoans dominated the substrate,followed by barnacles and Rhodophyta,the first colonising organisms,leading to colonisation by motile macro-invertebrates,mainly consisting of crustaceans,echinoderms and *** AATN artificial system provides an ideal substrate for the development of early ecological succession and the use of this technology should be feasible in the recovery process of habitat damaged by anthropogenic actions and climate change.

Influence of biomass burning on ozone levels in the Megalopolis of Central Mexico during the COVID-19 lockdown

The massive reductions in anthropogenic emissions resulting from the COVID-19 lockdown provided a unique opportunity to evaluate the effect of mitigation measures aiming to abate air *** Mexico,the total lockdown period took place during the dry-hot season when biomass burning activity is ***,we investigate the role of biomass burning emissions on regional ozone levels in the Megalopolis of Central *** studied period covers the lockdown phases 2 and 3,and the first month of the New *** applied a factor separation technique and process analysis to estimate the pure and synergistic contributions of emission reductions under lockdown and that from biomass burning to daily ozone maximum concentrations in 7 metropolitan areas of different states in the *** results revealed that biomass burning plumes likely masked the effect of massive reductions from mobile emissions,impacted the PBL development during phase 3 and favored transition and mixed NO_(x)-limited and VOC-limited regional *** contributed to increased ozone production in the middle to lower PBL by changing the regional background levels which potentially could bias high ozone production efficiency *** the Megalopolis contribution to economic and societal development at national scale,our study suggests that ozone mitigation measures during the dry-hot season targeting mainly mobile emissions will likely be offset by biomass burning plumes.A regional and synergic policy aiming to control biomass burning would help to reduce the occurrence of high ozone levels in Central Mexico with the co-benefit of tackling short-lived climate pollutants.

Environmental Efficiency in Water Use in Offshore Platforms in Campos Basin-RJ-Brazil

In drilling and production of oil at sea, potable water is most commonly transported to oil platforms using offshore supply vessels (OSV). This transport generates a series of environmental impacts, such as the emission of greenhouse gases and risks to human health. Furthermore, transporting high volumes of water requires large vessels and consequently, very high fuel consumption. To minimise the environmental impacts, water supply options were studied. The desalination of seawater and the reusing of streams from grey water (light and dark) and black water were evaluated and compared with the characteristics of the current supply via OSV. The amount of fuel consumed was collected in the logistics department of the oil company that used the OSVs. Air emissions were quantified using the methodology that the IPCC recommended. The information on the types of sewage treatment was found in the technical manuals from the manufacturers. It was concluded that the reused water could achieve up to 70% reduction in atmospheric emissions, the use of chemicals in the water treatment plant, sludge generation and the consumption of marine diesel fuel for the transport of water. On the other hand, the same reduction can be observed both from the effluent disposal at sea and in the chlorine of the sewage treatment plant. Therefore, the source of water used and the reduction of air emissions can be maintained without impairment of the normal water use in offshore platforms in the Campos Basin. The objective of this study was to diagnose problems related to the current form of water supply, to quantify them and to propose measures for increasing environmental efficiency.

Agave salmiana ash and poultry eggshell powder as CaO sources were used for obtaining nanostructured hydro-xyapatite(HAP).The synthesis was carried out by the Green Chemistry Hydrothermal Biosynthesis at 180℃ with a pH of 5,by reacting CaO from Agave Salmiana ash and Eggshell powder,with dibasic caldium phosphate(CaH-PO_(4)+2H_(2)O)in an aqueous solution,with Aloe barbadensis *** product was characterized by X-ray dif-fraction,Fourier transform infrared spectroscopy(FT-IR),Scanning Electron Microscopy(SEM),and Transmission Electron Microscopy(TEM).The size and shape of the hydroxyapatite particles changed dramati-cally in the presence of Aloe *** crystals of Hydroxyapatite were observed when Eggshell powder and Agave salmiana ash were used as raw materials in the presence of the Aloe barbadensis *** with shapes of ribbons and plates from 1 micrometers to 8 micrometers were observed when using the eggshell powder in the presence of Aloe bar badensis and,in the case of Agave salmiana ash in the presence of Aloe bar-badensis,crystals with shapes of quadra ngular prisms and hexagonal(polyhedra)with sizes from 2 micrometers to 20 micrometers were *** was therefore successfully biosynthesized by a green and sus-tainable method that reduces the environmental impact.

Conservative management of perforated duodenal diverticulum: A case report and review of the literature

Duodenal diverticula are a relatively common condition. They are asymptomatic, unless they become complicated, with perforation being the rarest but most severe complication. Surgical treatment is the most frequently performed approach. We report the case of a patient with a perforated duodenal diverticulum, which was diagnosed early and treated conservatively with antibiotics and percutaneous drainage of secondary retroperitoneal abscesses. We suggest this method could be an acceptable option for the management of similar cases, provided that the patient is in good general condition and without septic signs.

Distinct Subcellular Localization of GSK-3<i>β</i>in Melanocytic Nevi: Implications in Melanocyte Senescence

Melanocytic nevi are a transient in vivo proliferation of melanocytes that after time undergo cellular senescence. Most nevi harbor B-Raf mutations, which appear to activate cellular mechanisms of senescence in melanocytes. Glycogen synthase kinase 3β (GSK-3β), a critical downstream effector of the AKT signaling pathway, is involved in the development of melanoma and has been associated with senescence in melanocytes. Our immunohistochemical and immunofluorescence studies revealed distinct, perinuclear, dot-like reactivity of GSK-3β in melanocytic nevi. Furthermore, our tissue microarray analysis demonstrated significant perinuclear dot-like sublocalization of GSK-3β in melanocytic nevi compared with the amount of GSK-3β observed in melanoma (P β in human nevi may contribute to senescence in melanocytes.
