The"weekend effect"does not impact on outcome of trauma laparotomy–Experience from a level 1 trauma centre in New Zealand

Purpose:Trauma centres have been proven to provide better outcomes in developed countries for overall trauma,but there is limited literature on the systematic factors that describe any discrepancies in outcomes for trauma laparotomies in these *** study was conducted to examine and interrogate the effect of systematic factors on patients undergoing a trauma laparotomy in a developed country,intending to identify potential discrepancies in the ***:This was a retrospective study of all laparotomies performed for trauma at a level 1 trauma centre in New *** adult patients who had undergone an index laparotomy for trauma between February 2012 and November 2020 were identified and laparotomies for both blunt and penetrating trauma were *** laparotomies and trauma laparotomies in children were *** primary clinical outcomes reviewed included morbidity,length of hospital stay,and *** statistical analysis was performed using R ***:During the 9-year study period,204 trauma laparotomies were performed at Waikato *** majority(83.3%)were performed during office hours(170/204),and the remaining 16.7%were performed after hours(34/204).And 61.3%were performed on a weekday(125/204),whilst 38.7%were performed on the weekend/public holiday(79/204).Most of the parameters in office hours and after hours groups had no statistically significant difference,except lactate(p=0.026).Most of the variables in weekday and weekend groups had no statistically significant difference,except pH,lactate,length of stay,and gastrointestinal complications(p=0.012,p<0.001,p=0.003,p=0.020,respectively).Conclusion:The current trauma system at Waikato hospital is capable of delivering care for trauma laparotomy patients with the same outcome regardless of working hours or after hours,weekday or *** confirms the importance of a robust trauma system capable of responding to the sudden demands placed on it.

The effect of the quality of vital sign recording on clinical decision making in a regional acute care trauma ward

Purpose： Recording vital signs is important in the hospital setting and the quality of this documentation influences clinical decision making. The Modified Early Warning Score （MEWS） uses vital signs to categorise the severity of a patient＇s physiological derangement and illustrates the clinical impact of vital signs in detecting patient deterioration and making management decisions. This descriptive study measured the quality of vital sign recordings in an acute care trauma setting, and used the MEWS to determine the impact the documentation quality had on the detection of physiological derangements and thus, clinical decision making. Methods： Vital signs recorded by the nursing staff of all trauma patients in the acute care trauma wards at a regional hospital in South Africa were collected from January 2013 to February 2013. Investigator- measured values taken within 2 hours of the routine observations and baseline patient information were also recorded. A MEWS for each patient was calculated from the routine and investigator-measured observations. Basic descriptive statistics were performed using EXCEL Results： The details of lgl newly admitted patients were collected. Completion of recordings was 81% for heart rate, 88~; for respiratory rate, 98~; for blood pressure, 92% for temperature and 41~ for GCS. The recorded heart rate was positively correlated with the investigator＇s measurement （Pearson＇s correlation coefficient of 0.76）; while the respiratory rate did not correlate （Pearson＇s correlation coefficient of 0.02）. In 59~ of patients the recorded respiratory rate （RR） was exactly 20 breaths per minute and 27~ had a recorded RR of exactly 15. Seven percent of patients had aberrant Glasgow Coma Scale readings above the maximum value of 15. The average MEWS was 2 for both the recorded （MEWS（R）） and investigator （MEWS（1）） vitals, with the range of MEWS（R） 0-7 and MEWS（1） 0-9. Isolated free intra-abdominal fluid on CT in blunt trauma： The continued diagnostic dilemma

Purpose： The clinical significance of isolated flee fluid （FF） without solid organ injury on computed to- mography （CT） continues to pose significant dilemma in the management of patients with blunt abdominal trauma （BAT）. Methods： We reviewed the incidence of FF and the clinical outcome amongst patients with blunt abdominal trauma in a metropolitan trauma service in South Africa. We performed a retrospective study of 121 consecutive CT scans over a period of 12 months to determine the incidence of isolated FF and the clinical outcome of patients managed in a large metropolitan trauma service, Results： Of the 121 CTs, FF was identified in 36 patients （30%）. Seven patients （6%） had isolated FF. Of the 29 patients who had free fluid and associated organ injuries, 33 organ injuries were identified. 86% （25/ 29） of all 29 patients had a single organ injury and 14% had multiple organ injuries. There were 26 solid organ injuries and 7 hollow organ injuries. The 33 organs injured were： spleen, 12; liver, 8; kidney, 5; pancreas, 2; small bowel, 4; duodenum, 1. Six （21%） patients required operative management for small bowel perforations in 4 cases and pancreatic tail injury in 2 cases. All 7 patients with isolated FF were initially observed, and 3 （43%） were eventually subjected to operative intervention. They were found to have an intra-peritoneal bladder rupture in 1 case, a non-expanding zone 3 haematoma in I case, and a negative laparotomy in 1 case. Four （5？%） patients were successfully managed without surgical interventions. Conclusions： Isolated FF is uncommon and the clinical significance remains unclear. Provided that reli- able serial physical examination can be performed by experienced surgeons, an initial non-operative approach should be considered.

FORGING ASIA′S GLOBAL TRANSPARK IN THE PEARL RIVER DELTA AT ZHUHAI

The main purpose of the paper is to introduce the concept of the Global TransPark (GTP). It is a critical time to develop air logistics infrastructure in the New Economy. The paper also presents a brief review of the air cargo industry in the Asian-Pacific region and China,in particular China’s development in the industry as its economy has grown rapidly and globalized in the past two decades. The paper argues that China needs to develop a GTP to support its future growth,and explains why the Pearl River Delta-Hong kong-Macao city-region or Extended Metropolitan Region (EMR) is the best location. Finally,it proposes that Zhuhai Airport,one of the five airports in the EMR,is the best site for the GTP. New policies from Chinese government as well as close cooperation between local governments of Guangdong and the two Special Administrative Regions of Hong kong and Macao will be needed to make the establishment of the GTP possible.

A Fast Local Level Set Method for Inverse Gravimetry

We propose a fast local level set method for the inverse problem of *** theoretical foundation for our approach is based on the following uniqueness result:if an open set D is star-shaped or x3-convex with respect to its center of gravity,then its exterior potential uniquely determines the open set *** achieve this purpose constructively,the first challenge is how to parametrize this open set D as its boundary may have a variety of possible *** describe those different shapes we propose to use a level-set function to parametrize the unknown boundary of this open *** second challenge is how to deal with the issue of partial data as gravimetric measurements are only made on a part of a given reference domainΩ.To overcome this difficulty,we propose a linear numerical continuation approach based on the single layer representation to find potentials on the boundary of some artificial domain containing the unknown set *** third challenge is how to speed up the level set inversion *** on some features of the underlying inverse gravimetry problem such as the potential density being constant inside the unknown domain,we propose a novel numerical approach which is able to take advantage of these features so that the computational speed is accelerated by an order of *** carry out numerical experiments for both two-and three-dimensional cases to demonstrate the effectiveness of the new algorithm.

Network coding-aware cache replacement policy in on-demand broadcast environments

Network coding has been proved to be an effective technique in improving the performance of data broadcast systems because clients requesting different data items can be served simultaneously in one broadcast. Previous studies showed that its efficiency is highly related to the content of clients' cache. However, existing data broadcast systems do not take network coding information into account when making cache replacement decisions. In this paper, we propose two networks coding-aware cache replacement policies called DLRU and DLRU-CP to supplement network coding assisted data broadcast in on-demand broadcast environments. In DLRU, both data access and decoding contribution are taken into account to make replacement decisions. DLRU-CP is based on DLRU but allows clients to retrieve decodable data items that have not been requested yet. The performance gain of our proposed cache replacement policies over traditional cache replacement policy is shown in the simulation results, which demonstrate conclusively that the proposed policies can effectively reduce the overall response time.

The insulin-like growth factor(IGF)axis plays important roles in cancer development and *** type 1 IGF receptor(IGF-1R)is a key member in the IGF axis and has long been recognized for its oncogenic role in multiple cancer *** we review the occurrence of IGF-1R aberrations and activation mechanisms in cancers,which justify the development of anti-IGF-1R *** describe the therapeutic agents available for IGF-1R inhibition,with focuses on the recent or ongoing pre-clinical and clinical *** include antisense oligonucleotide,tyrosine kinase inhibitors and monoclonal antibodies which may be conjugated with cytotoxic ***,simultaneous targeting of IGF-1R and several other oncogenic vulnerabilities has shown early promise,highlighting the potential benefits of combination ***,we discuss the challenges in targeting IGF-1R so far and new concepts to improve therapeutic efficacy such as blockage of the nuclear translocationofIGF-1R.

Digital monitoring of rotary-percussive drilling with down-the-hole hammer for profiling weathered granitic ground

Rock and geotechnical engineering investigations involve drilling holes in ground with or without retrieving soil and rock samples to construct the subsurface ground *** the basis of an actual soil nailing drilling for a slope stability project in Hong kong,this paper further develops the drilling process monitoring(DPM)method for digitally profiling the subsurface geomaterials of weathered granitic rocks using a compressed airflow driven percussive-rotary drilling machine with down-the-hole(DTH)*** transducers are installed on the drilling machine and record the chuck displacement,DTH rotational speed,and five pressures from five compressed airflows in real-time *** mechanism and operations of the drilling machine are elaborated in detail,which is essential for understanding and evaluating the drilling data.A MATLAB program is developed to automatically filter the recorded drilling data in time series and classify them into different drilling processes in sub-time *** processes include penetration,push-in with or without rod,pull-back with or without rod,rod-tightening and *** drilling data are further reconstructed to plot the curve of drill-bit depth versus the net drilling time along each of the six *** curve is found to contain multiple linear segments with a constant penetration rate,which implies a zone of homogenous geomaterial with different weathering *** effect from fluctuation of the applied pressures is evaluated *** analyses are presented for accurately assess and verify the underground profiling and strength in weathered granitic rock,which provided the basis of using DPM method to confidently assess drilling measurements to interpret the subsurface profile in real time.

Traditional centralized token exchange(CEX)has been suffering from hacking due to the centralized management of users’*** contrast,decentralized token exchange(DEX)maintains users’assets by smart contracts in a decentralized manner,but introduces additional overhead in terms of gas fee and transaction confirmation *** decentralized token exchange(HEX)has been proposed to combine the benefits of CEX and ***,existing HEX is criticized for two ***,trading transactions are time-consuming and expensive for frequent token ***,excessive simultaneous transactions might cause the pending transaction congestion in the Ethereum *** this paper,we propose a payment channel based HEX,which extends existing solutions by adding a new payment channel layer to benefit frequent traders and alleviate the pending transaction ***,we propose the very first gas-price ***-latency function to guide Ethereum transaction issuers to choose an optimal gas price that minimizes the overall *** simulations are conducted to compare the cost in the proposed HEX with that in the conventional *** results demonstrate the effectiveness of our proposed mechanism in terms of reducing gas fees and transaction confirmation latency for frequent traders as well as the pending transaction congestion in Ethereum.

Introduction： Interfaces in Qing Frontier History

Inspired by the vibrant state of the last several decades of scholarship concerning the Qing frontier, the three authors of this introduction organized the “International Workshop on Defining the Jecen： The Evolution of the Qing Frontier, 1644-1918,” which took place on May 25-26, 2012, at the sponsoring institutions-Hong kong Baptist University and the Hong kong Institute for the Humanities and Social Sciences at the University of Hong kong. The primary objective of the workshop was to foster dialogue among colleagues from several Asian and North American institutions who currently are researching the Qing frontier. All participants were encouraged to contribute insights into long-standing questions based on the methodologies derived from their own scholarly backgrounds. Apart from aiming to break down unnecessary barriers of language, geography, and theoretical orientation, the conference also promoted two approaches to the topic that will simultaneously further the progress of contemporary pioneers and open fresh paths for future innovation.
