Monogenic features of urolithiasis: A comprehensive review

objective: Urolithiasis formation has been attributed to environmental and dietary factors. However, evidence is accumulating that genetic background can contribute to urolithiasis formation. Advancements in the identification of monogenic causes using high-throughput sequencing technologies have shown that urolithiasis has a strong heritable ***: This review describes monogenic factors implicated in a genetic predisposition to urolithiasis. Peer-reviewed journals were evaluated by a PubMed search until July 2023 to summarize disorders associated with monogenic traits, and discuss clinical implications of identification of patients genetically susceptible to urolithiasis ***: Given that more than 80% of urolithiases cases are associated with calcium accumulation, studies have focused mainly on monogenetic contributors to hypercalciuric urolithiases, leading to the identification of receptors, channels, and transporters involved in the regulation of calcium renal tubular reabsorption. Nevertheless, available candidate genes and linkage methods have a low resolution for evaluation of the effects of genetic components versus those of environmental, dietary, and hormonal factors, and genotypes remain undetermined in the majority of urolithiasis ***: The pathophysiology underlying urolithiasis formation is complex and multifactorial, but evidence strongly suggests the existence of numerous monogenic causes of urolithiasis in humans.

There is a recent and growing interest in assessing differential responders to resistance training(RT)for diverse ***,the individual ability to respond to an intervention for a specific measurement,called responsiveness,remains to be better ***,the current study aimed to summarize the available information about the effects of RT on functional performance and muscle strength,power,and size in healthy adults,through the prevalence rate in different responsiveness classifications models.A systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)and was registered at the International Prospective Register of Systematic Reviews(PRoSPERo,CRD42021265378).PubMed/MEDLINE,Scopus,and Embase databases were systematically searched in october 2023.A total of 13 studies were included,totaling 921 *** two studies presented a low risk of *** the effectiveness of RT,the prevalence rate for non-responders ranged from 0%to 44%for muscle strength,from 0%to 84%for muscle size,and from 0%to 42%for functional performance,while for muscle power,the only study found showed a responsiveness rate of 37%.In conclusion,a wide range of differential responders is described for all variables ***,the evidence summarized in this systematic review suggested some caution while interpreting the findings,since the body of evidence found seems to be incipient,and widely heterogeneous in methodological and statistical aspects.

The Burden of Undiagnosed Hypertension and Associated Risk factors among Adults in a Rural Community in Imo Sate, South-East Nigeria

Background: Cardiovascular diseases are the leading cause of death globally and hypertension is a major contributor to this burden. Many people with hypertension have poorly controlled blood pressure and up to half of the adults with hypertension are unaware of their hypertensive status due to factors that bother on poor management and poor screening approaches. The implication is that people who have poor access to healthcare especially those in the rural communities are at increased risk of cardiovascular complications and all-cause mortality. Unfortunately, not much has been done to ascertain the burden of undiagnosed hypertension and associated risk factors in rural communities in Nigeria. Methods: We conducted a community-based cross-sectional study in a rural community in Imo State, Nigeria, on burden of undiagnosed hypertension with participants recruited via a multi-stage sampling method. An interviewer-administered questionnaire was used, and standardized instruments were applied to obtain, process and analyze the data. Tests of association between the independent variables and outcome were conducted using logistic regression. P-value of Results: A total of 380 adults participated in the study. The mean age was 44.2 years. The prevalence of undiagnosed hypertension was 35.8%. Logistic regression revealed that age, with the respondents in the age groups 26 - 35 years (oR = 10.647, 1.910 - 59.345, p-value = 0.007), 36 - 45 (oR = 3.680, 1.263 - 10.723, p-value = 0.017), 46 - 55 years (oR = 2.737, 1.114 - 6.727, p-value = 0.039), 56 - 65 years old (oR = 3.384, 1.610 - 7.115, p-value = 0.001);and being married (oR = 3.846, 1.118 - 13.233, p-value = 0.033), were independent risk factors for undiagnosed hypertension. Conclusion: The prevalence of undiagnosed hypertension in the rural population of South-East Nigeria is high. Younger age (26 - 35 years) had the highest odds of risk for occurrence of hypertension. The main purpose of the paper is to introduce the concept of the Global TransPark (GTP). It is a critical time to develop air logistics infrastructure in the New Economy. The paper also presents a brief review of the air cargo industry in the Asian-Pacific region and China,in particular China’s development in the industry as its economy has grown rapidly and globalized in the past two decades. The paper argues that China needs to develop a GTP to support its future growth,and explains why the Pearl River Delta-Hong Kong-Macao city-region or Extended Metropolitan Region (EMR) is the best location. finally,it proposes that Zhuhai Airport,one of the five airports in the EMR,is the best site for the GTP. New policies from Chinese government as well as close cooperation between local governments of Guangdong and the two Special Administrative Regions of Hong Kong and Macao will be needed to make the establishment of the GTP possible.

This research work relates the surface of a square and the area circumscribed by a circle, resulting in a value called Nikola Tesla constant. This constant starts with the calculation of the areas of the square and the inscribed circle, giving a ratio of 9/40 and from which a residual area of the area proportions of the geometric figures described is obtained. Plotting smooth curves, particularly those in round shapes, can be represented efficiently with the use of Nikola Tesla constant, reducing complex mathematical calculus.

objective: This case report aimed to demonstrate a possible neuromuscular effect of Latarjet nerves transection or truncal vagotomy, in association with sleeve gastrectomy plus antrojejunostomy, in order to reproduce a Roux-en-Y gastric bypass (RYGB) mechanistic principles, in patients with previous Sleeve Gastrectomy (SG) who had had weight regain, with or without concomitant gastroesophageal reflux disease (GERD). Background: Sleeve gastrectomy (SG) is one of the most frequently performed bariatric operations worldwide. Nevertheless, weight regain and gastroesophageal reflux disease (GERD) have been consistently demonstrated, in association with this technique, which may require a revisional procedure. RYGB is an option in such a situation but, implies in gastrointestinal exclusions, which represents a shortcoming of this revision. Surpassing this inconvenient would be of great value for the patients. Methods: We describe herein two cases of SG revision for weight regain and GERD, with a follow-up of one year. Gastroesophageal reflux disease was evaluated by validated questionnaire, upper endoscopy, seriography, high resolution manometry (HRM) and impedance pHmetry (I-pHmetry), in the pre and postoperative periods. A re-Sleeve Gastrectomy with antrojejunal anastomosis was done in both cases, after informed consents. The Latarjet nerves were transected in one case, due to a bleeding in the left gastric vessels and a truncal vagotomy was required in the other, to appropriately treat an associated hiatal hernia. Results: In the postoperative evaluation it was observed a pyloric spasm in both cases, during seriography and endoscopy, kept until the one-year follow-up. There was unidirectional contrast flow to the gastrointestinal anastomosis, filling the jejunal limb, in radiologic contrast study. No contrast passed through the pylorus. Nonetheless, the duodenum was kept endoscopically accessible. In the one-year evaluation, weight loss was adequate and GERD resolution
