The effect of the quality of vital sign recording on clinical decision making in a regional acute care trauma ward

Purpose： Recording vital signs is important in the hospital setting and the quality of this documentation influences clinical decision making. The modified Early Warning Score （mEWS） uses vital signs to categorise the severity of a patient＇s physiological derangement and illustrates the clinical impact of vital signs in detecting patient deterioration and making management decisions. This descriptive study measured the quality of vital sign recordings in an acute care trauma setting, and used the mEWS to determine the impact the documentation quality had on the detection of physiological derangements and thus, clinical decision making. methods： Vital signs recorded by the nursing staff of all trauma patients in the acute care trauma wards at a regional hospital in South Africa were collected from January 2013 to February 2013. Investigator- measured values taken within 2 hours of the routine observations and baseline patient information were also recorded. A mEWS for each patient was calculated from the routine and investigator-measured observations. Basic descriptive statistics were performed using EXCEL Results： The details of lgl newly admitted patients were collected. Completion of recordings was 81% for heart rate, 88~; for respiratory rate, 98~; for blood pressure, 92% for temperature and 41~ for GCS. The recorded heart rate was positively correlated with the investigator＇s measurement （Pearson＇s correlation coefficient of 0.76）; while the respiratory rate did not correlate （Pearson＇s correlation coefficient of 0.02）. In 59~ of patients the recorded respiratory rate （RR） was exactly 20 breaths per minute and 27~ had a recorded RR of exactly 15. Seven percent of patients had aberrant Glasgow Coma Scale readings above the maximum value of 15. The average mEWS was 2 for both the recorded （mEWS（R）） and investigator （mEWS（1）） vitals, with the range of mEWS（R） 0-7 and mEWS（1） 0-9. Background:The characterization of surface and canopy fuel loadings in fire-prone pine ecosystems is critical for understanding fire behavior and anticipating the most harmful ecological effects of ***,the joint consideration of both overstory and understory strata in burn severity assessments is often *** aim of this work was to assess the role of total,overstory and understory pre-fire aboveground biomass(AGB),estimated by means of airborne Light Detection and Ranging(LiDAR)and Landsat data,as drivers of burn severity in a megafire occurred in a pine ecosystem dominated by Pinus pinaster *** the western mediterranean ***:Total and overstory AGB were more accurately estimated(R^(2) equal to 0.72 and 0.68,respectively)from LiDAR and spectral data than understory AGB(R^(2)=0.26).Density and height percentile LiDAR metrics for several strata were found to be important predictors of *** severity responded markedly and non-linearly to total(R^(2)=0.60)and overstory(R ^(2)=0.53)AGB,whereas the relationship with understory AGB was weaker(R ^(2)=0.21).Nevertheless,the overstory plus understory AGB contribution led to the highest ability to predict burn severity(RmSE=122.46 in dNBR scale),instead of the joint consideration as total AGB(RmSE=158.41).Conclusions:This study novelty evaluated the potential of pre-fire AGB,as a vegetation biophysical property derived from LiDAR,spectral and field plot inventory data,for predicting burn severity,separating the contri-bution of the fuel loads in the understory and overstory strata in Pinus pinaster *** evidenced relationships between burn severity and pre-fire AGB distribution in Pinus pinaster stands would allow the implementation of threshold criteria to support decision making in fuel treatments designed to minimize crown fire hazard.

Subcarrier Intensity modulated Optical Wireless Communications:A Survey from Communication Theory Perspective

Subcarrier intensity modulation with direet detection is a modulatiou/detection technique tbr optical wireless communication systems, where a pre-modulated and properly biased radio frequency signal is modulated on the intensity of the optical carrier. The most important benefits of subcarrier intensity modulation are as follows： 1） it does not provide irreducible error floor like the conventional on-off keying intensity modulation with a fixed detection threshold; 2） it provides improved spectral efficiency and supports higher order modulation schemes; and 3） it has much less implementation complexity compared to coherent optical wireless communications with heterodyne or homodyne detection. In this paper, we present an up-to-date review of subcarrier intensity modulated optical wireless communication systems. We survey the error rate and outage performance of subcarrier intensity modulations in the atmospheric turbulence channels considering different modulation and coding schemes. We also explore different contemporary atmospheric turbulence fading mitigation solutions that can be employed for subcarrier intensity modulation. These solutions include diversity combining, adaptive transmission, relay assisted transmission, multiple-subcarrier intensity modulations, and optical orthogonal frequency division multiplexing. moreover, we review the performance of subcarrier intensity modulations due to the pointing error and synchronization error.

＜正＞Recent advanced techniques in molecular biology have facilitated the dissection of many genetic traits in animals. One of the most important tools of these techniques is genetic linkage maps based on DNA markers such as microsatellite markers. The genetic markers mapped on a linkage map can be used to analyze the quantitative trait loci （QTL） responsible for economically important traits such as disease resistance, growth, and fecundity. In addition, genetic linkage maps also facilitate the positional cloning of functional genes contributing to the quantitative traits. mapping and identifying the genetic markers linked to economically important traits is critical for marker-assisted breeding. marker-assisted breeding such as marker-assisted selection （mAS） and marker-assisted gene introgression （mAI） are highly expected to improve the selective breeding programs of aquaculture species. In this paper, we introduce a comprehensive strategy for selective breeding by integrating molecular markers, genetic linkage maps, and QTL data that have been reported in aquaculture species.

俄罗斯科学院西伯利亚分院克拉斯诺亚尔斯克科学中心特殊技术设计局Sibgatulin于2013年5月7日11时55分,将天然电磁脉冲异常信息及分析意见通报给文章第一作者曾佐勋,指出在武汉的东、西两个方向上,具有发生mS6级左右地震的可能性。2013年5月7日22时20分,山东平原县职业中专宋松将其与宋科夫根据震兆日晕结合＂七式、六形还原测震法＂做出的地震预测意见通报了曾佐勋。意见中指出,2013年5月7—20日左右（甚至会在7天内）,在新疆、青海、与西藏交界附近,将发生mS5.5~6.2的地震。在Sibgatulin和宋松等人的预测基础上,利用卫星热红外排气点观测法,曾佐勋于2013年5月8日0时54分,向宋松、王杰、潘黎黎发出预测意见。预测10日内有可能在西藏的排气点（31.5°N,89.0°E）发生mS6.0级左右地震,并于5月8日8时34分,向3人补充了另一排气点（31°N,86°E）的观测情况。2013年5月15日18：54：30,西藏那曲尼玛县（31.6°N,86.5°E）发生mS5.2级中强震,震源深度为10km。此次地震的发震时间落入宋松和曾佐勋的预测窗口;震中落入曾佐勋根据第二个排气点给出的预测范围;震级基本上落入Sibgatulin、宋松、宋科夫以及曾佐勋的预测范围。尼玛地震临震预测成功的意义在于这是一次两国三方合作的范例,也进一步验证了＂地震预测需要多方法的综合分析＂的认识。对该地区地壳结构剖面和活动断裂研究表明,此次地震的发震断层为来多—措迈北北西向右行走滑正断层;地震能量的积聚和存储与中地壳高导低速层关系密切;西藏地区断层活动受南北向挤压、东西向拉伸的现今构造应力场控制。

Exacerbated VEGF up-regulation accompanies diabetes-aggravated hemorrhage in mice after experimental cerebral ischemia and delayed reperfusion

Reperfusion therapy is the preferred treatment for ischemic stroke,but is hindered by its short treatment window,especially in patients with diabetes whose reperfusion after prolonged ischemia is often accompanied by exacerbated *** mechanisms underlying exacerbated hemorrhage are not fully *** study aimed to identify this mechanism by inducing prolonged 2-hour transient intraluminal middle cerebral artery occlusion in diabetic Ins2Akita/+mice to mimic patients with diabetes undergoing delayed mechanical *** results showed that at as early as 2 hours after reperfusion,Ins2Akita/+mice exhibited rapid development of neurological deficits,increased infarct and hemorrhagic transformation,together with exacerbated down-regulation of tight-junction protein ZO-1 and upregulation of blood-brain barrier-disrupting matrix metallopeptidase 2 and matrix metallopeptidase 9 when compared with normoglycemic Ins2+/+*** indicated that diabetes led to the rapid compromise of vessel integrity immediately after reperfusion,and consequently earlier death and further aggravation of hemorrhagic transformation 22 hours after *** observation was associated with earlier and stronger up-regulation of pro-angiogenic vascular endothelial growth factor(VEGF)and its downstream phospho-Erk1/2 at 2 hours after reperfusion,which was suggestive of premature angiogenesis induced by early VEGF up-regulation,resulting in rapid vessel disintegration in diabetic *** reticulum stress-related pro-apoptotic C/EBP homologous protein was overexpressed in challenged Ins2Akita/+mice,which suggests that the exacerbated VEGF up-regulation may be caused by overwhelming endoplasmic reticulum stress under diabetic *** conclusion,the results mimicked complications in patients with diabetes undergoing delayed mechanical thrombectomy,and diabetes-induced accelerated VEGF up-regulation is likely to underlie exacerbated hemorrhagic transformat

Bronchial inflammatory profile in interferon-gamma-mediated immune response in asthma patients during airway response to cold stimulus

Objective:To evaluate the inflammatory pattern and the interferon(IFN)-γin the bronchial secretion of asthma patients in response to acute cold *** and methods:We enrolled 42 patients with *** assessed asthma by Asthma Control Test,the lung function by spirometry before and after the bronchodilator test,followed by collecting induced *** next day,we collected exhaled breath condensate(EBC)and conducted a 3-minute isocapnic hyperventilation with cold air(IHCA),followed by collecting spontaneously produced ***:Group 1 included 20 patients with cold airway hyperresponsiveness(CAHR),and group 2 included 22 patients without *** both groups,a high level of neutrophils in bronchial secretion was observed before and after *** response to IHCA,the number of epitheliocytes in the sputum decreased to a greater extent in patients of group *** baseline epitheliocytes and the concentration of IFN-γafter IHCA had an inverse relationship(r=-0.60;P=0.017).The baseline IFN-γin EBC before and after IHCA was lower in group *** response to cold exposure directly correlated with IFN-γlevels after IHCA(Rs=0.42;P=0.014).Conclusion:In asthma patients with CAHR,there is a relationship between the persistence of mixed inflammation and the level of IFN-γin the ***-γin response to IHCA is decreased with increased cytokine utilization during cold bronchospasm,which is accompanied by the mobilization of neutrophils and the shift in the cytokine spectrum of the respiratory tract towards the T helper cells(Th)1 immune response.

The Impacts of Topography on Spatial and Temporal Rainfall Distribution over Rwanda Based on WRF model

The impact of topography on heavy rainfall during two rain seasons was investigated in order to explain their mechanisms on rainfall distribution over Rwanda. Weather Research and Forecasting (WRF-ARW) model was used to study two historical cases of heavy rainfall which took place over Rwanda during two rain seasons, march to may (mAm) and September to December (SOND), from April 7 to 9, 2012 (for mAm) and from October 29 to 31, 2012 (during SOND). The control experiment was done with actual topography, whereas sensitivity experiment was carried out with topography reduced by half. Results show that rainfall distribution over Rwanda significantly changes when topography is reduced. The reduction in topography leads to a decrease in rainfall amounts in both mAm and SOND seasons, with varying magnitudes. This reveals the importance of orography in determining rainfall amounts and distribution over the region. The accumulated rainfall amount from WRF underestimate or overestimate rain gauge stations data by region and by season, but there is good agreement especially in altitude below 1490 m and above 1554 m during April and October respectively. The results may motivate modelling carters to further improve parameterization schemes in the mountainous regions.

Experimental investigation on Pm2.5 particle emission during polishing of granite

Particle emission during manufacturing processes, whether chemical, physical or mechanical can represent a serious danger for environment and for occupational safety. Especially machining processes, particle emission could have an important impact on shop floor air quality and might jeopardise workers’ health. It is therefore important to find ways of reducing the particle emission at the source of manufacturing processes. To do so, there is a need to know the size, the quantity and the distribution of particles produced by processes currently used in industry. In this study, investigations are done to compare the particle emission (Pm2.5) when polishing two granites (black and white). The black granite contained low Si concentration (about 10% Si) and the white granite contained high Si concentration (about 50% Si). Particle emission was monitored using the DustTrak II equipment with 2.5 μm impactor. The particle grain size was evaluated using X-ray diffraction techniques. machining conditions leading to the generation of finer particles were identified.

One of the most important parameters in meteorology is the mean wind proﬁle in the tropical cyclone boundary *** vertical proﬁle of wind speed and wind direction were measured during the period of the Nisarga cyclone from may 31st,2020,to June 5th,2020,using the newly installed Phased Array Doppler Sodar system at the Center for Space and Atmospheric Science(CSAS),Sanjay Ghodawat University,Kolhapur(16.74◦N,74.37◦E;near India's western coast).Our analysis revealed that the maximum mean wind speed was 17 m/s on June 3,2020,at 10:00 *** also shows the change in wind direction from southwest to southeast on June 2 and 3,*** high-resolution reanalysis data in the domain,0–25◦N,65–110◦E,during the period from may 31st to June 5th,2020,revealed the variation of the atmospheric pressure of the Nisarga cyclone from 1000 to 1008 hPa,sea surface temperature(SST)between 30◦C and 31◦C,outgoing longwave radiation(OLR)between 100 and 240 Wm-2,wind speed between 3 and 15 m/s,and low values of vertical wind shear(VWS)were observed to the north of Nisarga *** observations may provide more insights for the study of boundary layer turbulence during cyclonic activities.
