Background and aim Severe acute respiratory syndrome coronavirus 2(SARS-CoV-2),the virus responsible for the current pandemic,can have multi-organ impact.Recent studies show that liver injury could be a manifestation of the disease,and that liver disease could also be related to a worse prognosis.Our aim was to compare the characteristics of patients with severe coronavirus disease 2019(COVID-19)due to SARS-CoV-2 who required intubation versus stable hospitalized patients to identify the early biochemical predictive factors of a severe course of COVID-19 and subsequent requirement for intubation,specifically in Mexican.Methods This was an observational case-control study nested in a cohort study.Complete medical records of patients admitted for confirmed COVID-19 at a tertiary level center in Mexico City were reviewed.Clinical and biochemical data were collected,and the characteristics of patients who required invasive mechanical ventilation(IMV)(cases)were compared with stable hospitalized patients without ventilation(controls).Results We evaluated 166 patients with COVID-19 due to SARS-CoV-2 infection;114(68.7%)were men,the mean age was 50.6±13.3 years,and 27(16.3%)required IMV.The comparative analysis between cases and controls showed(respectively)significantly lower blood oxygen saturation(SpO_(2))(73.5±12.0%vs.83.0±6.8%,Phydrogenase(LDh)(764.6±401.9 IU/L vs.461.0±185.6 IU/L,P=0.001),and D-dimer(3463(524–34,227)ng/mL vs.829(152–41,923)ng/mL,P=0.003)concentrations.Patients in the cases group were older(58.6±12.7 years vs.49.1±12.8 years,P=0.001).Multivariate analysis showed that important factors at admission predicting the requirement for IMV during hospitalization for COVID-19 were AST≥250 IU/L(odds ratio(OR)=64.8,95%confidence interval(CI)7.5–560.3,P<0.0001)and D-dimer≥3500 ng/mL(OR=4.1,95%CI 1.2–13.7,P=0.02).Conclusions Our study confi

Monogenic features of urolithiasis: A comprehensive review

Objective: Urolithiasis formation has been attributed to environmental and dietary factors. however, evidence is accumulating that genetic background can contribute to urolithiasis formation. Advancements in the identification of monogenic causes using high-throughput sequencing technologies have shown that urolithiasis has a strong heritable component.Methods: This review describes monogenic factors implicated in a genetic predisposition to urolithiasis. Peer-reviewed journals were evaluated by a PubMed search until July 2023 to summarize disorders associated with monogenic traits, and discuss clinical implications of identification of patients genetically susceptible to urolithiasis formation.Results: Given that more than 80% of urolithiases cases are associated with calcium accumulation, studies have focused mainly on monogenetic contributors to hypercalciuric urolithiases, leading to the identification of receptors, channels, and transporters involved in the regulation of calcium renal tubular reabsorption. Nevertheless, available candidate genes and linkage methods have a low resolution for evaluation of the effects of genetic components versus those of environmental, dietary, and hormonal factors, and genotypes remain undetermined in the majority of urolithiasis formers.Conclusion: The pathophysiology underlying urolithiasis formation is complex and multifactorial, but evidence strongly suggests the existence of numerous monogenic causes of urolithiasis in humans.

Appraising the potential of calcium sulfoaluminate cement-based grouts in simulated permafrost environments

The aim of this study is to appraise the potential of calcium sulfoaluminate(CSA)cement-based grouts in simulated permafrost environments.The hydration and performance of CSA cement-based grouts cured in cold environments(10,0,and−10℃)are investigated using a combination of tests,including temperature recording,X-ray diffraction(XRD)tests,thermogravimetric analysis(TGA),and unconfined compressive strength(UCS)tests.The recorded temperature shows a rapid increase in temperature at the early stage in all the samples.Meanwhile,results of the TGA and XRD tests show the generation of a significant quantity of hydration products,which indicates the rapid hydration of CSA cement-based grouts at the early stage at low temperatures.Consequently,the CSA cement-based grouts exhibit remarkably high early strength.The UCS values of the samples cured for 2 h at−10,0,and 10℃ are 6.5,12.0,and 12.3 MPa,respectively.The UCS of the grouts cured at−10,0,and 10℃ increases continuously with age and ultimately reached 14.9,19.0,and 30.6 MPa at 28 d,respectively.The findings show that the strength of grouts fabricated using CSA cement can develop rapidly in cold environments,thus rendering them promising for permafrost applications.

Innate and adaptive immune responses in male and female reproductive tracts in homeostasis and following hIV infection

The male and female reproductive tracts are complex microenvironments that have diverse functional demands. The immune system in the reproductive tract has the demanding task of providing a protective environment for a fetal allograft while simultaneously conferring protection against potential pathogens. As such, it has evolved a unique set of adaptations, primarily under the influence of sex hormones, which make it distinct from other mucosal sites. here, we discuss the various components of the immune system that are present in both the male and female reproductive tracts, including innate soluble factors and cells and humoral and cell-mediated adaptive immunity under homeostatic conditions. We review the evidence showing unique phenotypic and functional characteristics of immune cells and responses in the male and female reproductive tracts that exhibit compartmentalization from systemic immunity and discuss how these features are influenced by sex hormones. We also examine the interactions among the reproductive tract, sex hormones and immune responses following hIV-1 infection. An improved understanding of the unique characteristics of the male and female reproductive tracts will provide insights into improving clinical treatments of the immunological causes of infertility and the design of prophylactic interventions for the prevention of sexually transmitted infections.

Seamless morphing trailing edge flaps for UAS-S45 using high-fidelity aerodynamic optimization

The seamless trailing edge morphing flap is investigated using a high-fidelity steady-state aerodynamic shape optimization to determine its optimum configuration for different flight conditions,including climb,cruise,and gliding descent.A comparative study is also conducted between a wing equipped with morphing flap and a wing with conventional hinged flap.The optimization is performed by specifying a certain objective function and the flight performance goal for each flight condition.Increasing the climb rate,extending the flight range and endurance in cruise,and decreasing the descend rate,are the flight performance goals covered in this study.Various optimum configurations were found for the morphing wing by determining the optimum morphing flap deflection for each flight condition,based on its objective function,each of which performed better than that of the baseline wing.It was shown that by using optimum configuration for the morphing wing in climb condition,the required power could be reduced by up to 3.8%and climb rate increases by 6.13%.The comparative study also revealed that the morphing wing enhances aerodynamic efficiency by up to 17.8%and extends the laminar flow.Finally,the optimum configuration for the gliding descent brought about a 43%reduction in the descent rate.
