background Although the Head Positioning in acute Stroke Trial(HeadPoST)showed no effect of the flat head position(FP;vs sitting up head position(SUP))on functional outcome,we hypothesised that it could still offer benefits if commenced early in those with acute ischaemic stroke(AIS)of at least moderate *** Subgroup analysis of HeadPoST in participants with National Institutes of Health Stroke Scale(NIHSS)scores≥7,≥10 and≥14,randomised to FP or SUPvascular serious adverse *** regression analyses were undertaken adjusted for study design and baseline risk *** There was no significant differential treatment effect in patient subgroups defined by increasing baseline NIHSS scores:adjusted OR and 95%CI for ordinal shift and binary(3-6)mRS scores:for NIHSS≥7(n=867)0.92(0.67 to 1.25)and 0.74(0.52 to 1.04);NIHSS≥10(n=606)0.80(0.58 to 1.10)and 0.77(0.49 to 1.19);NIHSS≥14(n=378)0.82(0.54 to 1.24)and 1.22(0.69 to 2.14).Conclusions Early FP had no significant effect in patients with moderate-severe AIS.

Asiatic Callosciurus squirrels as seed dispersers of exotic plants in the Pampas

Seed dispersal by exotic mammals exemplifies mutualistic interactions that can modify the habitat by facilitating the establishment of certain species. We examined the potential for endozoochoric dispersal of exotic plants by Callosciurus erythraeus introduced in the Pampas Region of Argentina. We identified and characterized entire and damaged seeds found in squirrel faeces and evaluated the germination capacity and viability of entire seeds in laboratory assays. We collected 120 samples of squirrel faeces that contained 883 pellets in seasonal surveys conducted between July 2011 and June 2012 at 3 study sites within the main invasion focus of C. erythraeus in Argentina. We found 226 entire seeds in 21% of the samples belonging to 4 species of exotic trees and shrubs. Germination in laboratory assays was recorded for Morus alba and Casuarina sp.; however, germination percentage and rate was higher for seeds obtained from the fruits than for seeds obtained from the faeces. The largest size of entire seeds found in the faeces was 4.2 ~ 4.0 ram, whereas the damaged seeds had at least 1 dimension ≥ 4.7 ram. Our results indi- cated that C. erythraeus can disperse viable seeds of at least 2 species of exotic trees. C. erythraeus predated seeds of other naturalized species in the region. The morphometric description suggested a restriction on the maximum size for the passage of entire seeds through the digestive tract of squirrels, which provides useful information to predict its role as a potential disperser or predator of other species in other invaded communities.

Novel technique for anastomotic salvage using transanal minimally invasive surgery:A case report

BACKGROUND Anastomotic leak(AL) after low anterior resection(LAR) can be a highly morbid *** incidence of AL ranges from 5% to 20% depending on patient characteristics and the distance of the anastomosis from the anal *** anastomoses and leaks pose technical challenges for endoscopic *** aim of this report was to describe the use of a commercially available laparoscopic energy device through a transanal minimally invasive surgery(TAMIS) port for the management of a symptomatic leak not requiring relaparotomy(grade B) after a LAR with diverting loop *** SUMMARY A TAMIS GelPOINT Path port was inserted into the anus to access the distal *** was achieved with AirSeal insufflation and a 30 degree laparoscope was introduced through a trocar.A LigaSure TM Retractable L-Hook device was then used to perform a septotomy of the chronic sinus tract identified posterior to the *** procedure was then repeated twice in three weeks intervals with ultimate resolution of the chronic leak *** months after serial TAMIS septotomies,barium enema demonstrated a patent anastomosis with no evidence of persistent leak or *** patient subsequently underwent ileostomy reversal and has had no significant postoperative *** TAMIS septotomy with the LigaSure TM Retractable L-Hook is a feasible andeffective,minimally invasive salvage technique for the treatment of grade B *** studies are needed to assess the generalizability and long-term results of this technique.

Diversity of Arbuscular Mycorrhizal Fungi Associated with Six Rice Cultivars in Italian Agricultural Ecosystem Managed with Alternate Wetting and Drying

Alternate wetting and drying(AWD)system,in which water has been reduced by approximately 35%with an increased occurrence of beneficial arbuscular mycorrhizal(AM)symbiosis and no negative impact on rice yield,was proposed to utilize water and nutrients more *** this study,we selected six rice cultivars(Centauro,Loto,Selenio,vialone nano,JSendra and Puntal)grown under AWD conditions,and investigated their responsiveness to AM colonization and how they select diverse AM *** order to investigate root-associated AM fungus communities,molecular cloning-Sanger sequencing on small subunit rDNA data were obtained from five out of the six rice cultivars and compared with Next Generation Sequencing(NGS)data,which were previously obtained in vialone *** results showed that all the cultivars were responsive to AM colonization with the development of AM symbiotic structures,even if with differences in the colonization and arbuscule abundance in the root *** identified 16 virtual taxa(vT)in the soil compartment and 7 vT in the root *** emphasized that the NGS analysis gives additional value to the results thanks to a more in-depth reading of the less represented AM fungus taxa.

In the past 20-30 years, developmental biologists have made tremendous progress in identifying genes required for the specifica-tion of individual cell types of an organ and in describing how they interact in genetic networks. In comparison, very little is known about the mechanisms that regulate tissue polarity and overall organ patterning. Gynoecia and fruits from members of the Brassicaceae family of flowering plants provide excellent model systems to study organ patterning and tissue specification because they become partitioned into distinct domains whose formation is determined by polarity establishment both at a cellular and whole tissue level. Interactions among key regulators of Arabidopsis gynoecium and fruit development have revealed a network of upstream transcription factor activities required for such tissue differentiation. Regulation of the plant hormone auxin is emerging as both an immediate downstream output and input of these activities, and here we aim to provide an overview of the current knowledge regarding the link between auxin and female reproductive development in plants. In this review, we will also demonstrate how available data can be exploited in a mathematical modeling approach to reveal and understand the feedback regulatory circuits that underpin the polarity establishment, necessary to guide auxin flows.

Infliximab for the treatment of pouchitis

Pouchitis is not a rare complication that develops after an ileal-pouch anastomosis, performed after colectomy in patients refractory to treatment or with complicated ulcerative colitis. This condition may become chronic and unresponsive to medical therapies, including corticosteroids, antibiotics and probiotics. The advent of biological therapies(tumor necrosis factor-α inhibitors) has changed the course of these complications. In particular, in these cases, infliximab(IFX) may represent a safe and effective therapy in order to avoid the subsequent operation for a permanent ileostomy. This article reviews the therapeutic effects of one of the most widely used anti-tumor necrosis factor-α molecules, IFX, for the treatment of complicated pouchitis(refractory to conventional treatment and/or fistulizing).

Facial nerve paralysis in children

Facial nerve palsy is a condition with several implications, particularly when occurring in childhood. It represents a serious clinical problem as it causes significant concerns in doctors because of its etiology, its treatment options and its outcome, as well as in little patients and their parents, because of functional and aesthetic outcomes. There are several described causes of facial nerve paralysis in children, as it can be congenital(due to delivery traumas and genetic or malformative diseases) or acquired(due to infective,inflammatory, neoplastic, traumatic or iatrogenic causes). Nonetheless, in approximately 40%-75% of the cases, the cause of unilateral facial paralysis still remains idiopathic. A careful diagnostic workout and differential diagnosis are particularly recommended in case of pediatric facial nerve palsy, in order to establish the most appropriate treatment, as the therapeutic approach differs in relation to the etiology.
